    Fine, I\u27m A Terrible Person, is a funny, heart wrenching, mother daughter story. 73-year-old, Aurora Hmans Feldenburg, a hapless, perpetually broke, eccentric, divorcee living in the wealthy enclave of Marin County, is wakened by a phone call informing her that her father\u27s widow, has died. Possibly her last chance at solvency she decides to drive to Los Angeles to see if there is a will and how she might benefit. Aurora is always ready for the next get rich scheme. Aurora\u27s high-strung daughter, 43-year-old Leyla Feldenburg Rothstein, is a hypersensitive, insecure, perfectionist, insomniac, emotionally damaged from her father\u27s lifelong abuse and rejection. She is married to a wealthy, Jewish prince charming, investment banker who specializes in the legal Cannabis industry. Leyla spends her days keeping up with the other Marin uber mom\u27s, manically pursuing perfection in herself, her home, and her children. When she overhears a conversation at parent\u27s night at her children\u27s elite private school, and suspects her husband is embarking on an affair, she decides to sneak into a Cannabis business conference he is attending, also in LA, to spy on him and put her suspicions to rest. While at the conference she accidentally ingests 40 mg of THC packed granola bite samples. Aurora and Leyla\u27s separate quests simultaneously intersect and enmesh over the course of a weekend, where they both end up staying with LA cousins, quirky, endearing, Sephardic Jews who speak Ladino, mostly in proverbs and cook prodigious quantities of delicacies from the old country. When Aurora\u27s meager inheritance is stolen by a once trusted cousin, she drags Leyla into a ludicrous chase. Unable to resist the pull of the trauma bond she shares with her infuriating mother; Leyla fails to adhere to her boundaries-even after years of therapy-and risks losing everything to another one of Aurora\u27s harebrained schemes

    Unveiling New Realities: An In-Depth Analysis of Virtual Reality\u27s Impact on Postoperative Adolescents

    The traditional approach to postoperative pain management heavily relies on opioids, but there’s been a surge in their misuse and adverse effects, leading to a significant public health concern. This has led healthcare providers to incorporate a multimodal approach aimed at reducing opioid dependence. This proposal introduces an immersive component, virtual reality (VR), not as a replacement for opioids but as a complementary tool offering distinct advantages for enhancing postoperative pain management. By immersing patients in a virtual environment, VR effectively distracts them from pain and discomfort, allowing for reduced opioid dosages and mitigating the risk of opioid misuse. Additionally, virtual reality provides a non-invasive and drug-free alternative, making it a safer and more patient-centered option for improving the overall postoperative experience. Through a comprehensive literature review, research findings were analyzed to highlight the necessity and effectiveness of VR in the postoperative period and among pediatric patients. Given that adolescents constitute a unique and potentially more responsive demographic, particularly in the postoperative setting where limited studies exist, this proposal advocates for a quasi-experimental study design to investigate the significant reductions in pain intensity and opioid usage achievable through immersive virtual reality among postoperative adolescents

    Although cervical cancer is preventable, it continues to pose a significant health concern in the Philippines as evidenced by high cervical cancer diagnosis rates. The purpose of this quantitative study is to investigate low cervical cancer screening rates and the effectiveness of educational interventions in increasing awareness and screening rates. A literature review will examine topics such as low cervical cancer screening rates, cervical cancer screening methods, and barriers. This research proposal involves a quasi-experimental study that investigates education as an intervention for increasing cervical cancer awareness and cervical cancer screening rates. Fifty Filipino women will be randomly assigned to an experimental group or a control group. Inferential and descriptive statistics will be used to evaluate the outcomes of the study

    The Effects of Music Therapy on Pediatric Patients with Congenital Heart Defects in the Pre and Postoperative Setting

    Congenital heart defects is a prominent medical occurrence especially in pediatrics. These defects often require surgeries and extensive treatment plans. These treatment plans often include invasive surgeries and extensive treatment plans which can require long recovery times along with potential surgical complications. This study aims to evaluate the effectiveness of music therapy on pediatric patients with congenital heart defects in the pre and postoperative setting and how music therapy can be implemented in treatment plans to yield effective recovery results for these patients. This research is significant as elevated vital signs and preoperative anxiety are associated with an increased need for anesthesia and analgesia requirements which can correlate to a risk for surgical complications. Elevated postoperative pain can result in an increased need for analgesic medications which can contribute to medication toxicity in a pediatric patient which can cause further complications. This study would consist of 80 pediatric patients on a cardiac unit aged 1 day to 10 years old diagnosed with a congenital heart defect who is undergoing cardiac surgery. A quasi-experimental randomized control trial will be conducted to compare the results of these cardiac patients in both preoperative and postoperative procedures. In these settings nurses would be responsible for gathering pre and postoperative vital signs, assessing preoperative anxiety, and assessing postoperative pain levels. With this study it is expected that patients who received music therapy during both pre and post operative procedures experienced a stabilization of vital signs, reduced preoperative anxiety, and lower postoperative pain levels

    Unlocking VR Magic: Duration of Anxiety Relief for Pediatric Needle Procedures

    There have been various studies that show the effectiveness of virtual reality (VR) being used as a non-pharmacologic and distraction method on school-aged children ranging from 6-12 years old. Despite this research, there has not been a follow-up with the use of VR and its duration of effect between each patient visit within this certain population. This study focuses on measuring the participants’ anxiety and pain levels during the needle-related procedure. The assessment tools that will be used to measure these important variables are the Children’s Fear Scale and Wong-Baker’s FACES scale. The participants of this study will be placed into two groups: VR intevention and standard care (SC) group. The SC group will be followed for the first month and after their trial, the VR group will also be going through the same process for the second. Statistical analysis will also be used to see correlation coefficients. The goal of this study is to evaluate the duration of effectiveness for VR being used in an outpatient setting. Ethical considerations are highly important for the safety and privacy of the participants in the study

    Breastfeeding Support in Your Pocket: Evaluating the Impact of a Mobile App on Maternal Success

    Background: Breastfeeding has well-known benefits for both the nursing infant and the birthing parent. It bolsters the newborn immune system, and reduces the risk of future health conditions in both parties. Due to these advantages, several health organizations internationally recommend exclusive breastfeeding until 6 months of age. However, only a fraction of infants in the United States meet this goal each year. Improved rates and duration of exclusive breastfeeding will lead to better long-term health outcomes for patients. The use of smartphone applications for breastfeeding education and support has not been thoroughly researched. Objective: This thesis explores the feasibility of smartphone apps for breastfeeding education, as well as their effect on parental self-efficacy level, and actual breastfeeding outcomes. The goal of the proposed intervention is to increase exclusive breastfeeding rates and duration through 6 months postpartum. Methods: A literature review will be conducted with current articles that research the use of app-based education and support in breastfeeding. Then, a proposal for further study will detail a randomized controlled trial with an evidence-based educational intervention. The control group will receive standard perinatal care and education, while the experimental group will receive access to a breastfeeding educational app, in addition to standard care and education. The proposed app will include both written and video content, as well as a breastfeeding tracking feature. Data will be collected on infant feeding plans, breastfeeding self-efficacy, and actual breastfeeding outcomes at baseline, 36 weeks gestation, and at 2 days, 3 months, and 6 months postpartum. Statistical analyses will be used to compare the two groups and determine if the study results are statistically significant

    Aim: The goal of this quantitative study is to investigate if educating nurses about the significance of KMC will increase its utilization and promote weight gain in low birthweight and preterm infants. Additionally, it aims to find the optimal duration of KMC required to maximize health outcomes for the infant. This research proposal aims to provide nursing professionals with valuable insights, allowing them to effectively educate parents on practices that promote maximal weight gain in neonates. Background: This is a cost-effective and natural way to enhance health outcomes such as temperature regulation, weight gain, respiratory function, pain mitigation, and facilitation of breastfeeding for the neonate. While it is evident KMC yields significant benefits for infants, there is a lack of extensive research regarding the optimal duration of this intervention. Due to the fact that the optimal duration of KMC is not yet established, parents are unable to decipher if they are practicing it for an adequate length of time. In addition, nursing staff may have limited knowledge regarding KMC which creates challenges in effectively educating parents Methods: The literature critique focused on KMC regarding its importance, duration, and nurse education and how each enriched the health of infants. Further, quantitative research regarding nursing education about KMC and its effects on usage as well as optimal duration of KMC required to boost weight gain in infants will be proposed. Results: It is assumed that educating nurses about the importance of KMC directly causes an increase in the weight gain observed in neonates due to its increased utilization and longer duration

    Promoting Parental Comfort to Facilitate Conversations on Adolescent Sexual Health and Behaviors

    This thesis proposal will investigate the impact of a comprehensive digital sexual health education program on the comfort levels of parents and teens when discussing sensitive topics such as pregnancy and sexually transmitted infections (STIs). The research is guided by Orem’s Self-Care Deficit Nursing Theory, which emphasizes the need to assess and address individuals’ self-care deficits, including the ability to engage in safe and informed sexual behaviors. The research design is a quantitative, quasi-experimental study with a one-group pre and post-test design. The sample consists of 50 parents and 50 teens from Marin County, who will participate in an online intervention using the Media Aware Parent program. The outcomes will be measured by pre and post-intervention surveys and the Parent-Adolescent Communication Scale. The expected results are that the intervention will increase the knowledge, confidence, and comfort levels of parents and teens regarding sexual health issues, and improve the quality of communication between them. The findings could potentially contribute to the development of more effective sexual health education programs and communication strategies for families

    Understanding the Importance of Administrative Mental Health Support for Frontline Nurses Post COVID-19

    During the peak of the COVID-19 pandemic, intensive care units and critical care units were overwhelmed with the number of patients that were on the floors. It was during this time that nurses were experiencing the most mental health decline. Several factors can be associated with the rapid decline of nurses’ mental health. However, these issues could ultimately stem from the lack of action taken by healthcare administration. This includes increased workload and the lack of PPE, resources institutional support. Because these units played such a vital role during the pandemic, it is imperative that healthcare administrators do what they can to identify and root out institutional problems that could further decline nurses’ mental health. Proving that these factors are part of the main problem will lead to changes that support the mental well-being of nurses and the rest of healthcare providers lest another global health crisis happens again. In this article, a literature review and proposal for further study is included. A mixed-methods approach will be employed, with quantitative data collected through surveys to gauge the prevalence and severity of mental health challenges, as well as qualitative data that will be obtained through open-ended interviews to gain insight into their experiences and the effectiveness of current support systems. This study aims to shed light on the magnitude of mental health challenges among not only frontline nurses, but all nurses and healthcare workers, regardless of unit, post COVID-19, and the lack of efficient support systems. This research will provide valuable insights for healthcare administrators, policymakers, and nurse leaders to refine mental health support programs, ensuring that nurses receive the essential support they deserve

    A Pulse for Progress – Examining the Efficacy of Best Nursing Developmental Care Practices for Children with Congenital Heart Disease at Risk for Developmental Delay in the PCICU: A Prospective Quasi-Experimental Cohort Study

    Emerging advances in pediatric cardiovascular surgery have significantly decreased the mortality rates in infants born with critical congenital heart disease (CHD), in turn, leading to increasing prevalence in adolescence and adulthood. At the same time, developmental delays (DD) are on the rise among children with CHD. Recent research suggests that prolonged exposure to the pediatric cardiac intensive care unit (PCICU), with its environmental stressors, negatively implicates neurodevelopment raising the question, and risk, of DD. In January 2023, the American Heart Association (AHA) issued a scientific advisory calling hospitals to implement developmental care practices for hospitalized infants and young children with complex CHD. Recent exploratory research confirms clinicians\u27 acceptability of a developmental care bundle known as the Cardiac Inpatient Neurodevelopmental Care Optimization (CINCO) program. However, there is limited evidence regarding its efficacy to prevent DD. In order to address this evidence gap, this paper proposes a prospective quasi-experimental cohort study aiming to answer the following research question: What is the efficacy of CINCO, in addition to appropriate medical/nursing orders and modifications to the ICU environment, in minimizing DD among children with CHD? By measuring child performance on various age-appropriate tasks using the Denver Developmental Screening Test-2 (DDST-2) at different timepoints (i.e., hospital admission, at 2 weeks length of stay (LOS), at 1 month LOS, upon discharge, and 2 weeks post-discharge) and comparing the results between children who received care that included CINCO with children who received care without CINCO, the efficacy of CINCO is evaluated. The odds ratio is used to express the treatment effect of CINCO. Chi-square tests will be employed for hypothesis testing
