Acoustic Performance Study For The Conference Hall of Ministry of Environment and Urbanizaton Service
Solar air collectors (SAC) are usually used for space heating and drying of agricultural products. In this study, acoustic design analyzes and evaluations were carried out in order to improve acoustical conditions of the Ministry of Environment and Urbanization Service Building Conference Hall* for speech function. In the light of the analysis on the room acoustics, recommendations have been developed for the conference hall to provide acoustic comfort conditions in accordance with the regulations in force in our country and international standards. As a result of these suggestions, the acoustic conditions for conference function recommended in national/international legislation have been provided in the Conference Hall of the Ministry of Environment and Urbanization Service Building
Proteinuria in preeclampsia: is it important?
Objectives: Our aim is to evaluate the laboratory results arid proteinuria levels of preeclamptic women arid their relationships to maternal and fetal outcomes.
Material and methods: One hundred preeclamptic pregnant women who gave birth in our clinic between 2013 and 2015 were included in our study retrospectively. The data collected from the patients included gestational week, age, gravidity, parity, abortus history, blood pressure, biochemical parameters, delivery method, maternal hospitalization time, cesarean indication, complications, blood products required, plasmapheresis use and dialysis need. The details about the newborns were recorded retrospectively. The relationships between preeclampsia signs and maternal and neonatal outcomes were analyzed. The protein amounts were analyzed via 24-hour collected urine analyses and spot urine analyses.
Results: A statistically significant positive correlation was observed between neonatal intensive care unit needs and proteinuria levels. Fetal growth restriction, respiratory distress syndrome and sepsis were observed as the level of proteinuria increased, but the result was not statistically significant. Eclampsia was observed only in patients with massive proteinuria, and it was statistically significant. An increase in cesarean sections, placental abruptions, antihypertensive drug needs and blood product replacement rates was observed as the amount of proteinuria increased in preeclamptic women, but the results were not statistically significant.
Conclusions: The severity of preeclampsia cannot be determined by the level of proteinuria. However, when massive proteinuria is detected, the clinician should be more cautious about maternal and fetal complications
Evaluation of Prostatic Artery Embolization Efficiency in Benign Prostatic Hyperplasia Patients with High Comorbidity
Objective: The purpose of this study was to evaluate efficacy outcomes following prostate artery embolization (PAE) for the treatment of benign prostatic hyperplasia (BPH) patients with high comorbidity.
Materials and Methods: This retrospective study included 22 patients treated with PAE from May 2015 to June 2017. Patients with Charlson comorbidity index >= 2, International Prostate Symptom Score (IPSS) >12, prostate specific antigen (PSA) levels 90 cm(3) were included. Total PSA, maximum flow rate (Q(max)), TPV, IPSS, post-voiding residual (PVR) values were recorded in all patients in the urology clinic before PAE and at 3 and 6 months after PAE.
Results: The average patient age was 73.86 +/- 6.25 years and operative time was 80 minutes (range, 60-120 min). Pre-PAE and 6-month post-PAE values were: IPSS: 25.18 +/- 6.75 an d 11.27 +/- 3.29 (p<0.05), Q(max): 8.31 +/- 3.12 and 17.22 +/- 3.23 (p<0.05), PVR: 87.9 +/- 19.25 and 25.86 +/- 7.72 (p<0.05), TPV: 134.45 +/- 57.56 and 86 +/- 15.4 (p<0.05), and PSA: 3.89 +/- 1.26 and 2.11 +/- 1.06 (p<0.05). Embolization was performed unilaterally due to atherosclerosis and strictures in the internal iliac artery branches in 2 patients. After the procedure, 2 patients experienced transient hematuria which did not require bladder irrigation, 1 patient had acute urinary retention due to dysuria, and 1 patient had transient hematospermia.
Conclusion: PAE may be an alternative treatment method in BPH patients with high comorbidity
The effect of combined preconditioning strategies on isokinetic strength in well trained kickboxers
Purpose: The use of preconditioning strategies (PconSt) alone has a positive impact on performance. However, it is thought that there will be more impact on the performance of the combined preconditioning strategies, and this is the first work to prove it. This study was to examine the effect of combined preconditioning strategies on isokinetic strength in well-trained kickboxers.
Material: The following preconditioning strategies were combined: morning resistance exercise (MRE), ischemic preconditioning (IP), active warm-up (AW-U), hormonal preconditioning (HP) and post-activation potentiation (PAP). Fifteen well-trained male kickboxers (age: 22.38 +/- 4.01years, height: 182.23 +/- 1.05cm, body mass: 77.67 +/- 8.01kg) volunteered for this study. Peak isokinetic knee extension and flexion moment were determined at 60 degrees/s, 180 degrees/s and 240 degrees/s bilaterally. All participants performed the pre-test and then were randomly divided into Sham and PconSt groups on separate days. For the PconSt group, MRE, IP, AW-U, HP and PAP were implemented sequentially before test.
Results: The findings indicated significant increase in strength after combined training in the PconSt groups for right leg flexion at 180 degrees/s flexion (p < 0.05). The average percentage strength difference between sham and PconSt groups was 4.12 +/- 8.95%.
Conclusions: This study showed that a combined preconditioning strategy increased isokinetic strength
Assessment of the Platelet Parameters in Children With Type 1 Diabetes Mellitus
Background: The purpose of this study was to assess platelet morphological parameters in children and adolescents with type 1 diabetes mellitus (T1DM) and to determine the factors affecting those parameters.
Methods: A total of 80 children with type 1 diabetes mellitus and 80 healthy control subjects were included in this study. All subjects' platelet parameters including platelet count (PLT), mean platelet volume (MPV), platelet distribution width (PDW) and plateletcrit (PCT) levels were measured. Age at diagnosis, duration of diabetes, and hemoglobin A1c (HbA1c) levels of patients with T1DM were also recorded.
Results: There was no significant difference in PLT, MPV, PDW and PCT values between patients and controls (P > 0.05). PCT and PLT values were higher in patients with poor metabolic control compared to the other cases (P = 0.002, P = 0.005). Positive correlation was found between HbA1c and PCT (r = 0.28, P = 0.01). There was no correlation between HbA1c and MPV, PDW and PLT.
Conclusions: Indices of platelet morphology such as PCT, PLT, PDW and MPV were similar in children with T1DM and in healthy controls. However, an increase in PCT values in poor glycemic control group and a significant positive correlation between PCT and HbA1c were observed, suggesting that PCT levels may be an early marker of vascular complication
Role of plasma asymmetric dimethylarginine levels in diagnosis and follow-up of acute pulmonary thromboembolism
Purpose: Nitric oxide is an important mediator that plays a major role in platelet adhesion/aggregation. Asymmetric dimethylarginine (ADMA) is a competitive inhibitor of endothelial nitric oxide synthase. In this study, we aimed to examine the ADMA levels in diagnosis and follow-up of acute pulmonary thromboembolism.
Materials and Methods: The study was designed as a prospective study. Thirty consecutive patients with acute PTE and 20 healthy volunteers were enrolled into the study. Patients were followed 1 month. Plasma ADMA levels were measured on admission and following the first month.
Results: Ona dmision there was statistically significant difference between two groups. When we compared serum ADMA levels difference between patients group with resolution and without resolution, there was not any statistically significant difference. There was a significant difference between serum ADMA levels of massive group and the other 2 groups.
Conclusion: Plasma ADMA levels elevated in patients with pulmonary thromboembolism. This finding supports the idea ADMA levels correlated with thrombus burden
Effects of pre-operative individualized education on anxiety and pain severity in patients following open-heart surgery
Objectives: Individualized education is important for preparing patients for the operation both physically and psychologically. This study investigated the effects of pre-operative individualized education for open-heart surgery patients on post-operative anxiety and pain severity.
Methods: This quasi-experimental study was conducted in a university hospital between January and October 2014 and involved 109 patients. Data were collected through a form developed by the authors, the State-Trait Anxiety Inventory (STAI), and the visual analog scale (VAS). Patients underwent STAI 1 day before the operation to identify their sources of anxiety and educational needs, and then individualized education was given accordingly. 1 day after the operation, STAI was used to measure patients' state anxiety level, and VAS was used to measure their pain level. The effect of demographic variables on differences in anxiety and pain was investigated.
Results: The average age of the participants was 59.62; 69.7% were male, 92.7% were married, 49.5% graduated from primary school, and the majority (71.6%) had coronary heart disease. The most common sources of anxiety reported the participants included lack of knowledge, being away from family, risk of death, and pain. An analysis of the participants' pre-operative mean scores for state anxiety displayed statistically significant differences (P < 0.05) according to age groups and gender. No significant difference was detected between mean pre- and post-operative state anxiety scores. There was a statistically significant relationship between mean pre- and post-operative state anxiety scores and mean pain scores.
Conclusions: The individualized education is given to patients before surgery was found to have potential effects on their post-operative pain levels
Effect of seasonal changes on nutritional status and biochemical parameters in Turkish older adults
BACKGROUND/OBJECTIVES: Available data suggest that seasonal changes may influence the nutritional status and overall health of elderly individuals. Therefore, this study was conducted to investigate the effects of seasonal changes and related factors on energy and nutrient intake of older adults.
SUBJECTS/METHODS: Individuals aged 65 years or over were prospectively enrolled in this single-center study (male: 11, female: 20). Data were collected between May 2013 and February 2014 during winter, spring, summer and autumn. Food consumption and biochemical parameters were taken during each season to assess the seasonal nutrition status of the elderly. Upon analysis of biochemical parameters (retinol, vitamin D and vitamin C), an high-performance liquid chromatography device was utilized whereas an Immulite 2000 device was utilized during analysis of serum folic acid and parathyroid hormone.
RESULTS: Fruit, fat, egg and bread consumption varied seasonally in males and females (P < 0.05). During winter, daily energy intake was found to be greater than in other seasons in males (557 kcal) and females (330 kcal) (P < 0.05). Additionally, carbohydrates, vegetable protein, n-3 fatty acid and sodium intake increased in winter, while the n-6/n-3 ratio increased in summer among males (P < 0.05). Dietary fiber and sodium intake in winter, vitamin C, iron and zinc intake in spring, and cholesterol, retinol, vitamin D and niacin intake in autumn were found to be higher in females when compared to other seasons (P < 0.05). Serum parathyroid hormone level was higher in winter, and vitamin D level was higher in autumn in both genders (P < 0.05). In males, blood folic acid level was higher in winter, while vitamin C level was higher in females, and there was no seasonal variation in retinol concentration (P < 0.05).
CONCLUSION: Food consumption and biochemical parameters showed significant seasonal variations in older adults. It is not clear if nutrition plans in older adults will benefit from consideration of seasonal changes in eating habits
Long-term outcomes in patients who underwent surgical correction for atrioventricular septal defect
Objective: The follow-up results of patients operated for atrioventricular septal defect (AVSD) during 1996-2016 at Baskent University are presented.
Methods: Data obtained from hospital records consists of preoperative echocardiographic and angiographic details, age and weight at surgery, operative details, Down syndrome presence, postoperative care details, early postoperative and latest echocardiographic findings and hospitalization for reintervention.
Results: A total of 496 patient-files were reviewed including 314 patients (63.4%) with complete and 181 (36.6%) with partial AVSD (48.4% of all patients had Down syndrome). Atrioventricular (AV) valve morphology was Rastelli type A in 92.2%, B in 6.5%, and C in 1.3% of patients. The operative technique used was single-patch in 21.6% (108), double-patch in 25.8% (128), and modified single-patch (Wilcox) in 52.5% (260) of patients. The follow-up time was 37.79 +/- 46.70 (range, 0-198) months. A total of 64 patients (12.9%) had a rrhythmias while in the intensive care unit; pacemaker was implanted in 12 patients. A total of 78 patients (15.7%) were treated for pulmonary hypertensive crisis. The early morbidity and mortality in the postoperative first month were calculated as 38% and 10%, and the late morbidity and mortality (>1 month) were calculated as 13.1% and 1.9%, respectively. The rate of reoperation in our cohort was 8.9%.
Conclusion: Although the early morbidity and mortality are low in AVSD operations, the rate of reoperations for left AV valve insufficiency are still high. Although Down syndrome is not a risk factor for early mortality, the co-morbid factors, such as longer postoperative mechanical ventilator or inotropic support, lead to higher risk for morbidity. The frequency of pulmonary hypertension and consequent complications are also high
Diagnostic value of acustic radiation force impulse imaging in the assessment of salivary gland involvement in primary Sjogren's sydrome
Aims: The aim of this study is to investigate the diagnostic value of Acoustic Radiation Force Impulse (ARFI) imaging in the assessment of salivary gland involvement in primary Sjogren's syndrome (pSS). Material and methods: Twenty five patients with pSS and 25 healthy volunteers were included. First, echostructures and the thickness of the submandibular and parotid glands were evaluated by B-mode ultrasonography. Then, ARFI imaging with Virtual Touch Quantification (R) was performed. Ten independent shear wave velocity measurements were taken from each gland. Finally, the mean shear wave velocity (SWV) values were calculated, and used for further analysis. Results: The mean SWV values of parotid and submandibular glands were significantly higher in the pSS patients than in the healthy control group (p 0.005). Conclusions: Our findings indicate that ARFI imaging may provide a non-invasive, simple and fast means of assessment of glandular impairment as an alternative test when other salivary gland tests are inconclusive or cannot be performed. ARFI may be a valuable adjunct for the clinical diagnosis of pSS
