Effect of desensitizing tooth pastes hn dsfferent restorative materials' bond strength to coronal and root dentin
Aim: The aim of the study was to evaluate the effects of two desensitizing tooth-pastes on the shear bond strength of resin composite and glass ionomer restorative materials to coronal and root dentin. Materials and Methods: 108 human third molars' distal or mesial sides were ground flat with a silicon-carbide paper. The samples were randomly divided into 3-major groups (N=36) to be brushed with: GroupA: Sensodyne Rapid Relief; GroupB: Colgate Sensitive Pro-Relief; GroupC: Colgate Total 12 toothpastes. Brushing was performed with tooth brushing simulator 2 times/day for 10 days with 10 strokes/brushing. They were further divided into three sub-groups (n=12) to receive the following adhesion protocols to coronal/root dentin. Group1: Clearfil SE Bond + Filtek Z250 Composite; Group2: Adper Single Bond2 + Filtek Z250Composite; Group3: Riva Glass Ionomer Cement. Samples were kept in distilled water for 24 hours. Shear bond strength test was performed with Universal Test Machine (cross head speed: 0.5 mm/sec). The data were evaluated statistically. Results: For all restorative techniques evaluated, the least shear bond strength values were obtained after Sensodyne Rapid Relief usage.Riva Glass Ionomer Cement demonstrated statistically significant inferior shear bond strength values than resin composite. Conclusion: Self-etch systems may be the choice for adhesion after desensitizing toothpaste usage. © 2018 Cumhuriyet University Faculty of Dentistry
Effect of different prophylactic polishing procedures on the surface roughness of microhybrid and nanohybrid resin composites
Objectives: The aim of the present study was to evaluate effects of different professional dental prophylaxis procedures on the surface roughness of a microhybrid (Charisma, Heraeus-Kulzer) and a nanohybrid (Ice, SDI Dental) resin composites. Materials and Methods: 45 disc shaped (2 mm thick /8 mm in diameter) specimens of each restorative materials (totally 90 specimens) were prepared using plexyglass mold. Specimens were polished with medium, fine and ultra-fine Sof-Lex (3M-ESPE) discs and 45 specimens of each composite were randomly divided into five groups (n=9). The first group received no prophylaxis treatment and served as control. The other groups received prophlaxis procedures including Detartrine paste (Septodont) with rotating rubber cup, Gelato paste (Keystone Industries) with rotating rubber cup, pumice-water slurry with rotating rubber cup and pumice-water slurry with rotating brush. After prophlaxis procedures surface roughness values of all specimens were measured using an optical prophylometer (Zygo New View 7200, Ametek). Data was statistically analyzed using Mann Whitney U and Kruskal-Wallis Tests at the 0.01 and 0.025 level of significance respectively. Results: The smoothest surfaces were observed in control groups for both resin composites (p < 0.001). Control groups' surface roughness values of resin composites tested were significantly different (p < 0.001). However there were no statistically significant differences between surface roughness values of microhybrid and nanohybrid resin composites that received same prophylaxis treatment. Gelato paste, pumice-water slurry with rotating rubber cup and pumice-water slurry with rotating brush caused significantly rougher surfaces than control group but surface roughness values obtained with Detartrine were not significantly different from the control group in both composites. Conclusions: All prophylaxis procedures tested in the study increased the roughness values of both composites to a level that would be retantive to bacteria. Because of that dental professional must be careful during prophylaxis procedures otherwise repolishing or rarely repair of the composite restoration would be necessary after prophylaxis. © 2018 Cumhuriyet University Faculty of Dentistry
Sparsity-driven weighted ensemble classifier
In this study, a novel sparsity-driven weighted ensemble classifier (SDWEC) that improves classification accuracy and minimizes the number of classifiers is proposed. Using pre-trained classifiers, an ensemble in which base classifiers votes according to assigned weights is formed. These assigned weights directly affect classifier accuracy. In the proposed method, ensemble weights finding problem is modeled as a cost function with the following terms: (a) a data fidelity term aiming to decrease misclassification rate, (b) a sparsity term aiming to decrease the number of classifiers, and (c) a non-negativity constraint on the weights of the classifiers. As the proposed cost function is non-convex thus hard to solve, convex relaxation techniques and novel approximations are employed to obtain a numerically efficient solution. Sparsity term of cost function allows trade-off between accuracy and testing time when needed. The efficiency of SDWEC was tested on 11 datasets and compared with the state-of-the art classifier ensemble methods. The results show that SDWEC provides better or similar accuracy levels using fewer classifiers and reduces testing time for ensemble
Consensus report for workshop on ⇜early diagnosis and intervention in cerebral Palsy” Turkey’s approach to babies at risk and suggestions 1. National congress on babies at risk (1-4 March 2018, the Ankara Hotel)
The workshop on “Early Diagnosis and Intervention in Cerebral Palsy” aimed to reflect the current situation of and the challenges encountered in the early diagnosis and intervention in cerebral palsy in babies at risk, in order to raise awareness in the corresponding community by putting forth suggestions for providing solutions to these problems, and also, to provide a model suggestion suitable for the whole country. In the direction of these purposes, problems encountered in the prenatal, natal, and postnatal periods, and the corresponding suggestions were discussed, and vital topics concerning numerous disciplines, especially for the postnatal period, were emphasized in the workshop. With the aim of providing a solution to the current problems in the country regarding the matter, by employing an interdisciplinary approach, the establishment of Monitoring Centers for the Babies at Risk was proposed, and a model for these centers was created. Implications derived from this workshop will establish a foundation for the creation of a special strategic plan for the early diagnosis and intervention in cerebral palsy for the babies at risk. Angiopoietin-1, Angiopoietin-2, and Periostin Levels in Children with Recurrent Wheeze
Background: Recurrent wheeze (RW) is frequent in preschool children. Wheezing phenotypes, asthma predictive index (API), and modified API (mAPI) have been described for clinical purposes. Our aim was to examine whether inflammatory markers including serum angiopoietin (Ang)-1, Ang-2, and periostin levels differ according to wheezing phenotypes and mAPI. Materials and Methods: Ninety-eight children who were <4 years of age with history of at least 4 episodes of wheezing during the past 12 months and 51 age-matched healthy controls were included in the study. Children with RW were classified according to wheezing phenotypes as episodic viral wheeze or multitrigger wheeze, and positive or negative mAPI. Blood for Ang-1, Ang-2, and periostin levels was drawn during wheezing episode-free periods. Results: Atopic children with RW (31.4 +/- 34.4 ng/mL) demonstrated higher serum Ang-1 levels than nonatopic children (16.5 +/- 13.8 ng/mL) with RW (P = 0.03). When we compared children according to wheezing phenotypes, we could not find any difference in serum Ang-1, Ang-2, and periostin levels between groups. Children with positive mAPI showed similar Ang-1, Ang-2, and periostin levels with children having negative API and healthy children. Conclusions: We have found higher serum Ang-1 levels in atopic children with RW, and this result might be explained by increased inflammation. The evidence was not strong enough to associate serum Ang-1, Ang-2, or periostin and asthma in preschool children with RW. However, Ang-1 can be a candidate for investigating its role in predicting atopic children and diagnosing atopic childhood asthma
The comparison of physical activity, disability, and quality of life of pregnant women in different trimesters
Objective: The purpose of this study was to compare the physical activity, disability, and quality of life in pregnant women in different trimesters. Material and Methods: This was a retrospective cohort study and a total of 91 pregnant women (first trimester n=20, second trimester n= 34, third trimester n=37) were enrolled. The physical activity levels were evaluated with the International Physical Activity Questionnaire; the disability was evaluated with the Oswestry Low Back Pain Questionnaire, and the quality of life levels was determined with the Short Form-36 questionnaire. Results: The demographic features of the pregnant women were similar in different trimesters (p>.05). The comparison of their physical activity levels in terms of trimesters, the level of moderate and vigorous physical activities, and the activity level of walking showed no difference (p>.05). The total physical activity level in the second trimester was higher than in the first trimester (p=.048); the level of disability due to low back pain was significantly higher in the third trimester than other trimesters (p=.003); and the physical function parameter of the quality of life scale was significantly lower in the third trimester compared to the first and the second ones (p=.001). However, there were no significant differences in the other parameters of the quality of life (p>.05). Conclusion: The levels of physical activity, physical functioning and disability varied in different trimesters. It can be stated that the progress of the pregnancy decreases physical function and triggers disability due to low back pain; however, the change in the level of physical activity is related to the adaptation to pregnancy and not to gestational age. Effect of mometasone furoate nasal spray on the DNA of nasal mucosal cells
Background/aim: Allergic rhinitis (AR) is a respiratory disease caused by inflammation of the nasal mucosa. Intranasal corticosteroids (ICs) are an effective treatment for AR; however, their use has been associated with atrophy in nasal mucosae. Because DNA damage has been linked to several chronic diseases, we hypothesize that use of ICs could cause DNA damage in nasal mucosa cells, leading to mucosal atrophy and septal perforation.
Materials and methods: Sixty patients with moderate or severe AR were divided randomly into two groups. Mometasone furoate (MF) and antihistamine tablets (desloratadine) were given to the study (IC) group. Physiologic saline and desloratadine were given to the control ((serum physiologic (SP)) group. Nasal irrigation fluid was taken from patients before study commencement and after 4 weeks of treatment. The comet assay was applied to detect DNA damage in nasal mucosa cells.
Results: Nineteen patients were excluded, leaving a study population of 41 patients (IC group: 17 patients; SP group: 24 patients). Genotoxic damage was evaluated by comet assay.
Conclusion: Treatment with MF spray for 4 weeks does not cause DNA breaks within cells in the nasal mucosa. These results could form the basis of clinical trials involving treatment with different ICs over longer treatment periods
Review of Clinical Recommendations on Prolactinoma and Pregnancy
Prolactinomas are the most common hormone-secreting pituitary adenomas. Prolactinomas account for nearly 30-40 percent of all the pituitary adenomas. Although it affects individuals over a wide age range, it is more common in 20-40-year-old female patients, who are in their reproductive age. Prolactinomas may cause hypogonadism, menstrual cycle dysfunction (oligomenorrhea or amenorrhea) and infertility (luteal phase abnormalities or anovulation) in premenopausal women. When pregnancy is excluded, hyperprolactinemia in approximately 10 to 20 percent of the patients results in amenorrhea. Women with untreated pro-lactinomas are generally unable to achieve pregnancy, as the hyperprolactinemia affects the pulsatility of gonadotropin-releasing hormone (GnRH) and diminishes follicle-stimulating hormone (FSH) as well as luteinizing hormone (LH) secretion. The sum of these effects induces amenorrhea, infertility, and hypogonadism, thereby posing difficulties in fertility. Therefore, in most women prolactinoma is diagnosed prior to conception. However, ovulation and fertility usually improve after proper diagnosis and treatment of prolactinoma. Therefore, during the surveillance of these patients, the onset of pregnancy is a common phenomenon. Management of these pregnancies may sometimes be challenging and require a multidisciplinary approach involving an endocrinologist, a gynecologist, a radiologist and an experienced neurosurgeon in order to achieve the best outcomes both for the patient as well the infant. Chronic Subdural Hematoma Associated with Arachnoid Cyst of the Middle Fossa: Surgical Treatment and Mid-Term Results in Fifteen Patients
AIM: To report neurological and radiological features, surgical management, and mid-term outcomes of patients with chronic subdural hematoma (CSDH) associated with ipsilateral arachnoid cyst (AC) of the middle fossa.
MATERIAL and METHODS: A total of 453 patients with CSDH were treated at our clinic between August 2004 and August 2012. Of these patients, 15 had ipsilateral AC in the middle fossa. A single burr hole craniostomy was performed to drain the hematoma. The AC was left intact in 14 patients, and one patient had no surgical intervention. The follow-up period ranged from 13 to 88 months (mean 43.07 +/- 23.23 months).
RESULTS: The patients with AC associated CSDH were found to be younger than the patients with CSDH alone, and the mean age of 58 patients was 13.15 +/- 13.17 years, while it was 11 +/- 14.22 years in the other patients. Eleven patients had experienced head trauma at 21-50 days before admission. Hematoma evacuation through a single burr hole with closed-system subdural drainage performed at 2-4 days after surgery improved the symptoms in all patients. Two patients developed subdural fluid collection, which was treated by subduroperitoneal shunt placement.
CONCLUSION: Greater prevalence of ACs in patients with CSDHs has been reported in the literature. We recommend the drainage of the hematoma via a single craniostomy and to leave the AC intact as the first choice of treatment if the associated AC is a Galassi type I or II. Additional subduroperitoneal shunting may be performed in patients with Galassi type III cyst
The effect of smartphone usage on the median nerve
Purpose: The aim of this study was to examine the effects of smartphone use on the median nerve.
Materials and Methods: Participants were classified into three groups according to their smartphone use frequencies; high smartphone users (with Smartphone addiction scale (SAS) score of higher than 71), low smartphone users (SAS score less than 71) and classical type mobile phones users. The Visual analogue scale (VAS), quick-disabilities of arm, shoulder, hand (qDASH) and median nerve conductions of the participants were assessed. Groups were compared to each other as well as dominant and non-dominant hand nerve measurements of participants were also compared.
Results: VAS and qDASH values were not significantly different between groups. It was observed that the rare use of smartphones affected the median nerve conductions in a lesser manner compared to the classical mobile phone use. However, using smart phone in an increasing rate influenced the median nerves adversely.
Conclusion: Excessive use of smartphone can lead to carpal tunnel syndrome via affecting the median nerve. In recent decades, there has been a widespread use of smartphones even at the level of addiction. This should be noted while designing the smartphones and voice applications should be used more often. Furthermore, it can be beneficial to follow up the median nerve conductions of patients who are addicted to smartphones with the help of electrophysiological studies
