    Environmental pollution and cost efficent equipment selection in cement factories

    In this study, it is aimed to be able to select the technological equipment to minimize the dust particles and various harmful gases released during the cement production process. "TOPSIS Method" is used as a method to carry out the work. The data used in the method are taken from the dust removal system reports of 6 different cement factories active in Turkey. The clinker production capacities of the factories are identical to each other; but the dust removal systems that they use are technically different. There are two types of dust elimination systems that are commonly used. These; electro static and dust (bag) filter. The working principles of both filters are different from dust holding capacities, environmentally harmful gas emissions and investment costs. With the result of the topsis method used in this study, the cement factory aimed to reach investors in terms of cost and environmental sensitivity while making technological equipment selection. Bu çalışmada, çimento üretim sürecinde etrafa salınan toz partikülleri ve çeşitli zararlı gazları minimize edecek teknolojik ekipman seçimi yapılabilmesi amaçlanmıştır. Çalışmayı sonuca taşıyacak yöntem olarak da “TOPSIS Metodu” kullanılmuştır. Metotta kullanılan veriler, Türkiye’de aktif üretim yapan 6 ayrı çimento fabrikasının tozsuzlaştırma sistem raporlarından alınmıştır. Veri alınan fabrikaların klinker üretim kapasiteleri birbirleriyle aynıdır; ancak kullandıkları tozsuzlaştırma sistemleri farklıdır. Yaygın olarak kullanılan iki tür tozsuzlaştırma sistemi vardır. Bunlar; elektro ve torbalı filtredir. Her iki filtrenin çalışma prensipleri toz tutma kapasiteleri, çevreye zararlı gaz salınımları ve yatırım maliyetleri birbirlerinden farklıdır. Bu çalışmada kullanılan topsis yönteminden çıkan sonuçla, çimento fabrikası yatırımcılarını teknolojik ekipman seçimi yaparken maliyet ve çevreye duyarlılık açısından optimumum sonuca ulaştırmayı amaçlamıştır

    Pulmonary complications in adult patients with sickle cell disease

    Sickle cell disease (SCD) is an inherited hemoglobin disorder, associated with recurrent painful episodes, ongoing hemolytic anemia and progressive multi-organ failure. While survival beyond the fourth decade of life for a patient with SCD was previously considered unusual; nowadays, in countries with developed health care systems, patients with SCD could reach into adulthood. However, irreversible organ damages have become more prevalent along with the patients' reaching adulthood. The lungs as well are among the organs involved in SCD. Pulmonary complications are significant causes of morbidity and mortality and are substantially responsible for deaths. Although our understanding of these conditions has improved over the past 10-15 years, there remains no specific treatment of pulmonary complications of SCD. Limited data on health care issues, particularly in older adults with SCD, poses multiple challenges to patients, their families and health care providers. The incidence and severity of pulmonary complications require the physicians' further interest in this subject. The present review comprises the following pulmonary complications of SCD: i) acute chest syndrome, ii) venous thromboembolus, iii) asthma, iv) respiratory dysfunction, v) respiratory disorders during sleeping, vi) pulmonary hypertension

    Publication Rates and Citation Analysis of Oral and Poster Presentations at the First Congress of the Society of Urological Surgery in Turkiye

    Objective: Publications of congress presentations (oral/poster) contribute to the literature by creating new information. In this study, we examined the publication rate of poster and oral presentations at the 1st Congress of the Society of Urological Surgery in Turkiye held in 2012. Materials and Methods: We investigated the congress booklet belonging to the 1st Congress of the Society of Urological Surgery in Turkiye held in 3-7 October 2012. We investigated the publication rates of 176 poster and 103 oral presentations under the headings of uro-oncology, andrology, pediatric urology, endourology, transplantation, continence and other by searching the PubMed and Science Citation Index Expanded (SCI-E) databases. Results: Of the 176 poster and 103 oral presentations, 79 (28.3%) were subsequently published. The publication rate in journals included in the SCI-E was 24.3%. While 33% of oral presentations were published, 25.5% of poster presentations became publications and this difference was not found to be statistically significant (p= 0.48). The mean duration from presentation to publication was 21.1 months. Conclusion: The publication rate of presentations at the 1st Congress of the Society of Urological Surgery in Turkiye was higher than in most national congresses in different disciplines, but lower than many international congresses in the field of urology. Future studies investigating congress data from several consecutive years will provide clearer results

    Mediterranean diet as the diet of choice for patients with chronic kidney disease

    Traditional dietary management of chronic kidney disease (CKD) focuses on the quantity within the diet of energy and protein, and the restriction of single micronutrients, with little mention of dietary quality. Dietary patterns that are more plant-based, lower in meat (including processed meat), sodium and refined sugar, and have a higher content of grains and fibres are now included in multiple clinical guidelines for chronic disease prevention. The Mediterranean diet (MD) has been associated with reduced cardiovascular disease incidence in both observational and interventional studies. A wealth of evidence links MD with other beneficial effects on chronic diseases such as diabetes, obesity or cognitive health. This review examines each constituent of the classical MD and evaluates their suitability for the management of patients with CKD. We also evaluate the potential hyperkalaemia risk of increasing fruit and vegetable intake. Overall, a decrease in net endogenous acid production and increase in fibre may lead to a better control of metabolic acidosis. This, together with other putative favourable effects of MD on endothelial function, inflammation, lipid profile and blood pressure, provide mechanistic pathways to explain the observed reduced renal function decline and improved survival in CKD patients adhering to an MD

    The approach to community-acquired pneumonia: A survey study

    INTRODUCTION AND AIM: Community-acquired pneumonia (CAP), which is often seen in daily practice, is a lower respiratory tract and pulmonary parenchyma infection which develops in society and daily life with community-acquired pathogens in individuals with no known immune failure. Delay in the treatment of pneumonia is known to increase morbidity and mortality. Various scoring systems are currently used in the identification of treatment groups in pneumonia. With the aim of evaluating the approach to CAP cases, the infection. MATERIALS AND METHODS: Working Group of the Turkish Respiratory Research Association (TUSAD) prepared a 22-item questionnaire. RESULTS: The survey was published on the TUSAD official website between July 2013 and June 2016. A total of 78 individuals responded to the questionnaire on the website. CONCLUSION: The responses to the questionnaire could indicate the way forward for new guidelines for physicians in respect of the approach to CAP

    Introduction: The aim of this study was to screen patients aged >= 65 years admitted to the Baskent University Hospital Neurology Polyclinic between 2011 and 2013 and classify them according to the International Classification of Headache Disorders, 3rd edition. Materials and Method: In total, 175 patients (126 females and 49 males) were screened. First, they were separated into primary and secondary headache groups, and the subgroup headache types were then defined within these groups according to the International Classification of Headache Disorders, 3rd edition; then, the concurrence of accompanying chronic diseases were examined. Results: In the primary headache group, 21.1% of the patients had migraine, 39.4% had tension-type headache, and 51% had trigeminal neuralgia; in the secondary headache group, 24.2% of the patients had headache attributed to hypertension, 18.2% had headache attributed to temporal arteritis, 18.2% had headache attributed to cervical pathology, and 12.1% had headache attributed to intracranial mass. Conclusion: This study was conducted to detect the most commonly seen headache types in geriatric population, and the results of this study are important in terms of increasing awareness of the evaluation of both primary and secondary headaches in geriatric patients. Because the study was conducted with a less patient population, it is important in terms of more accurately demonstrating the factors constituting the etiology of headache in this patient group

    Prognostic factors for maximally or optimally cytoreduced stage III nonserous epithelial ovarian carcinoma treated with carboplatin/paclitaxel chemotherapy

    ObjectiveTo identify factors predictive of poor prognosis in women with stage III nonserous epithelial ovarian cancer (EOC) who had undergone maximal or optimal primary cytoreductive surgery (CRS) followed by six cycles of intravenous carboplatin/paclitaxel chemotherapy. MethodsA multicenter, retrospective department database review was performed to identify patients with stage III nonserous EOC who had undergone maximal or optimal primary CRS followed by six cycles of carboplatin/paclitaxel chemotherapy at seven gynecological oncology centers in Turkey. Demographic, clinicopathological and survival data were collected. ResultsA total of 218 women met the inclusion criteria. Of these, 64 (29.4%) patients had endometrioid, 61 (28%) had mucinous, 54 (24.8%) had clear-cell and 39 (17.9%) had mixed epithelial tumors. Fifty-five (25.2%) patients underwent maximal CRS, whereas 163 (74.8%) had optimal debulking. With a median follow-up of 31.5 months, the 5-year progression-free survival (PFS) and overall survival (OS) rates were 34.8% and 44.2%, respectively. Bilaterality (hazard ratio [HR] 1.44, 95% CI 1.01-2.056; P = 0.04), age (HR 2.25, 95% CI 1.176-4.323; P = 0.014) and maximal cytoreduction (HR 0.34, 95% CI 0.202-0.58; P < 0.001) were found to be independent prognostic factors for PFS. However, age (HR 2.6, 95% CI 1.215-5.591; P = 0.014) and maximal cytoreduction (HR 0.31, 95% CI 0.166-0.615; P < 0.001) were defined as independent prognostic factors for OS. ConclusionThe extent of CRS seems to be the only modifiable prognostic factor associated with stage III nonserous EOC. Complete cytoreduction to no gross residual disease should be the main goal of management in these women

    The Effect of Sumatriptan in lschemic Conditions in the Rat Heart

    Objectives: The aim of this study was to investigate the effect of SUM on IR-induced injury in rat heart and its effect on IPC-induced protection. Materials and Methods: The rats were randomly divided into four groups: IR, SUM-IR, IPC, and SUM-IPC. The mean arterial blood pressure and heart rate were recorded to calculate PRP. Standard limb lead 2 ECG were recorded to evaluate arrhythmia parameters. Results: The PRP values in the SUM-IPC group were significantly lower than in the SUM-IR group at the beginning of reperfusion (p<0.05). The incidence of VT in the IPC, SUM-IR, and SUM-IPC groups was significantly lower than in the IR group (p<0.05). VF was only observed in the IR group. Conclusion: SUM protects the heart against IR injury but is not as protective as IPC alone. Although SUM diminishes IPC-induced protection against VT, the preventive effect of SUM against VF may be predictive for cardioprotection in ischemic conditions

    Investigating the readiness of students in primary teacher education program through verbal, model, and mathematical representations of number sets

    The aim of this study is to determine the readiness of the first year students in Primary Teacher Education Department about the number sets. In line with this aim, students were asked to express counting numbers, natural numbers, whole numbers, rational numbers, irrational numbers, real numbers, and complex numbers with verbal, model, and mathematical representations. This study was designed as a survey study which is a descriptive model. A total of 61 students at Primary Teacher Education Program at Faculty of Education in a private university in the province of Ankara, Turkey participated in the study on a voluntary basis. Content analysis method was used to analyze the data. Results showed that Primary Teacher Education students have low level of readiness in the number sets. It was determined that participants had difficulty in expressing number sets with models; they tended to define natural numbers and counting numbers verbally and they defined irrational and real numbers associating them with other number sets. Moreover, it was ascertained that they used verbal, model, and mathematical expressions interchangeably when expressing the number sets. © Karadeniz Technical University. All rights reserved
