    Évaluation par cross match de la compatibilité sanguine chez le cygne tuberculé (Cygnus olor)

    Blood transfusion in birds remains a rare practice due to technical limitations and a lack of data on erythrocyte compatibility both within and between species. This thesis presents the first assessment of transfusion compatibility in the mute swan (Cygnus olor), a species not previously studied in this context. Given their large size, high blood volume, and relative accessibility, swans could serve as potential blood donors for other avian species, particularly domestic birds. The study involved performing major and minor cross-matches between six adult swans admitted to the CHUV-FS at the National Veterinary School of Alfort. The protocol followed established veterinary methods, with adjustments made to suit the unique characteristics of avian species. Testing was conducted within 40 days post-sampling, a period chosen based on available veterinary literature, though no specific recommendations exist for this species. The results generally showed good compatibility among the tested swans, with two incompatibilities observed in the same individual. These were indicated by microscopic agglutination during minor cross-matching, suggesting the presence of natural alloantibodies. While this is a single case, it raises the possibility of previously unidentified antigenic systems in mute swans. No hemolysis was detected during testing. The study's limitations include a small sample size, the subjectivity involved in interpreting weak agglutination reactions, the lack of transfusion trials, and technical challenges during sample handling. These factors underscore the need to standardize protocols and develop diagnostic tools tailored for avian species.This thesis opens several avenues for future research. It provides an initial experimental framework for assessing blood compatibility in mute swans and supports the feasibility of further investigations into avian xenotransfusion. Future studies could explore in vitro compatibility between mute swans and other avian species using the cross-matching technique. This study contributes to establishing the foundations of avian transfusion medicine based on rigorous and reproducible immunological criteria and encourages broader comparative studies across avian species.Le recours à la transfusion sanguine chez les oiseaux demeure rare, en raison de contraintes techniques et du manque de données sur la compatibilité érythrocytaire inter et intra-espèce. Cette thèse propose une première évaluation de la compatibilité transfusionnelle chez le cygne tuberculé (Cygnus olor), une espèce jusqu'à présent non étudiée sous cet angle. En raison de leur taille, de leur volume sanguin élevé et de leur accessibilité relative, les cygnes pourraient représenter des donneurs potentiels pour d'autres oiseaux, notamment domestiques. L'étude repose sur la réalisation de cross-matchs majeurs et mineurs entre six cygnes adultes recueillis au CHUV-FS de l'école vétérinaire d'Alfort. Le protocole employé suit les méthodes décrites en médecine vétérinaire, avec adaptation aux caractéristiques propres de l'espèce aviaire. Les tests ont été réalisés, dans un délai maximal de 40 jours après le prélèvement, durée choisie selon les données disponibles en littérature vétérinaire, bien qu'aucune recommandation spécifique n'existe pour cette espèce. Les résultats ont mis en évidence une compatibilité généralement bonne entre les individus testés. Deux incompatibilités ont été observées, traduite par une réaction d'agglutination microscopique lors d'un cross-match mineur, toutes deux sur le même cygne, suggérant la possible présence d'anticorps érythrocytaires naturels. Ce constat ponctuel interroge sur l'existence de systèmes antigéniques encore non décrits chez le cygne. Par ailleurs, aucune hémolyse n'a été constatée au cours des tests. Les limites de l'étude tiennent notamment à la taille réduite de l'échantillon, à la subjectivité inhérente à l'interprétation des réactions faibles, à l'absence de transfusion réelle, ainsi qu'aux difficultés techniques rencontrées lors de la manipulation. Ces éléments soulignent la nécessité de standardiser les protocoles et de développer des outils diagnostiques spécifiques aux espèces aviaires. Les perspectives ouvertes par cette thèse sont multiples. Elle fournit une première base expérimentale à l'évaluation de la compatibilité du sang chez le cygne et soutient la faisabilité de recherches futures sur la xénotransfusion aviaire. En particulier, le cygne pourrait faire l'objet d'évaluations in vitro de compatibilité avec d'autres espèces aviaires, grâce à la technique de cross-match. Cette étude contribue à poser les fondements d'une médecine transfusionnelle aviaire basée sur des critères immunologiques rigoureux et reproductibles, en appelant à des études comparatives plus larges à l'échelle des oiseaux

    Impedance spectroscopy and thermal stability study of the Ba2TiSi2O8–CaTiO3 composites for radio frequency applications

    International audienceAbstract This work presents the study of the electrical properties of Ba 2 TiSi 2 O 8 (BTS) ceramic with additions of 1% up to 9% in bulk of CaTiO 3 (CTO) in the radio frequency region. The structural analysis was performed by X-ray diffraction, and the refinement confirmed the structures of BTS and CTO with no evidence of spurious phases. The electrical properties were analyzed by Impedance Spectroscopy, whereas it was observed that the dielectric permittivity ( ε ′ r ) of the composites was lower than that of the pure BTS sample. The conductivity of the samples followed the universal power law, and the charge carriers demonstrated sudden translational jumps (0 < n < 1). The imaginary part of the impedance ( Z ′′ ) showed peaks that shifted to the high-frequency region with increasing temperature. The Cole–Cole diagram was used to analyze the behavior of grains and grain boundaries; the samples presented non-Debye relaxation processes. Finally, the temperature capacitance coefficient was calculated, and the values indicate that the additions of CTO promoted an improvement in thermal stability compared to the BTS matrix

    Étude des facteurs de rémission du diabète sucré chez le chat : étude prospective de 30 cas vus au CHUVA entre 2021 et 2024

    Diabetes mellitus is a common endocrinopathy in cats, for which clinical remission, defined as the maintenance of euglycemia without insulin therapy for at least four weeks, represents a major therapeutic goal. However, published studies show highly variable definitions, protocols, and sample sizes, making it difficult to reliably identify factors associated with remission at the time of diagnosis. This study aimed to describe the clinical, biological, and therapeutic characteristics of cats newly diagnosed with diabetes mellitus, prospectively followed between 2021 and 2024 at the National Veterinary School of Alfort (EnvA - CHUVA), and to identify potential factors associated with the occurrence of remission. Included cats met strict criteria: recent diagnosis of diabetes mellitus, initial treatment with either ProZinc® or Lantus® insulin, and a minimum follow-up period of six months. Data collected at admission included signalment, history, complete clinical examination, biochemical results, and imaging and urinalysis findings. Follow-up consisted of standardized visits up to six months post inclusion, with treatment adjustments and remission assessment. Statistical analysis was performed using the Kaplan-Meier method, and group comparisons were conducted using the Log-rank test. A total of 30 cats were enrolled. Most were male (80%), neutered, of domestic shorthair breed (87%), with a median age of 10.5 years. The most frequent clinical signs were weight loss (83%), polydipsia (77%), and polyuria (73%). Biochemical abnormalities commonly observed included hypercholesterolemia (67%), marked elevated fructosamine levels (67 %), and moderate to marked ketonemia (53%). The main comorbidities were pancreatitis, ketosis, and chronic kidney disease (each affecting around 30% of cats). Five out of thirty cats achieved remission during the follow-up period, within 41 to 172 days after diagnosis. At 6 months post-diagnosis, the remission rate estimated by the Kaplan-Meier curve was 35%. Exposures significantly associated with a faster remission included the presence of an abdominal abnormality on palpation (p = 0.002), urine specific gravity < 1.035 (p = 0.008), mild glucosuria (p = 0.012), and ketosis (p = 0.014). These findings represent a first step toward a larger-scale study and emphasize the need for standardized protocols to refine the prognostic assessment of this condition.Le diabète sucré est une endocrinopathie fréquente chez le chat, dont la rémission clinique, définie par le maintien d'une euglycémie sans insulinothérapie pendant au moins quatre semaines, représente un objectif thérapeutique majeur. Cependant, les facteurs associés à la rémission identifiés au moment du diagnostic sont difficiles à interpréter en raison d'une grande hétérogénéité des études publiées. Ce travail s'inscrit dans ce contexte et avait pour objectif de décrire les caractéristiques cliniques, biologiques et thérapeutiques des chats nouvellement diagnostiqués de diabète sucré, suivis prospectivement entre 2021 et 2024 au Centre Hospitalier Universitaire Vétérinaire d'Alfort (CHUVA), et d'identifier les éventuels facteurs associés à la survenue d'une rémission. Les chats inclus répondaient à des critères précis : diagnostic récent de diabète sucré, traitement initial par insuline ProZinc® ou Lantus®, et suivi d'au moins six mois. Les données recueillies à l'admission comprenaient le signalement, les commémoratifs, l'examen clinique complet, les résultats biologiques, ainsi que les examens d'imagerie et urinaires. Le suivi comportait plusieurs visites standardisées jusqu'à six mois après l'inclusion, avec ajustement du traitement et évaluation de la rémission. L'analyse statistique a été réalisée selon la méthode de Kaplan-Meier, et les comparaisons entre groupes effectuées à l'aide du test du Logrank. Trente chats ont été inclus. Ils étaient majoritairement mâles (80 %), stérilisés, de race Européenne (87 %), et âgés en médiane de 10,5 ans. Les signes cliniques les plus fréquents étaient l'amaigrissement (83 %), la polydipsie (77 %) et la polyurie (73 %). Sur le plan biologique, une hypercholestérolémie (67 %), une hyperfructosaminémie marquée (67 %) et une cétonémie modérée à marquée (53 %) étaient fréquemment observées. Les comorbidités principales incluaient pancréatite, cétose et maladie rénale (environ 30 % chacune). Cinq chats sur trente ont atteint la rémission au cours du suivi, dans un délai de 41 à 172 jours post diagnostic. À 6 mois post-diagnostic, le taux de rémission estimé par la courbe de Kaplan-Meier était de 35 %. Les expositions significativement associées à une rémission plus rapide étaient la présence d'une anomalie à la palpation abdominale (p = 0,002), une densité urinaire < 1,035 (p = 0,008), une glucosurie discrète (p = 0,012) et la présence d'une cétose (p = 0,014). Ces résultats sont une première étape vers une étude sur un effectif plus important nécessitant une standardisation des protocoles pour affiner l'évaluation pronostique de cette affection

    Bone Augmentation of Atrophic Alveolar Ridges Using a Synthetic Bone Substitute With Mesenchymal Stem Cells: A Randomized, Controlled Clinical Trial

    International audienceObjectives To assess the efficacy and safety of a cell‐based therapy for 3D bone augmentation of severe alveolar bone defects prior to dental implant placement. Materials and Methods A Phase 2 randomized controlled clinical trial evaluated the safety and efficacy of a cell therapy using expanded autologous iliac crest‐derived mesenchymal cells seeded on a synthetic bioabsorbable bone substitute covered with a non‐resorbable membrane. The control group received an autogenous bone block graft. After 5 months, CBCT scans were compared to measure the bone volume changes achieved after the regenerative surgery. Subsequently, dental implants were placed in the regenerated areas. Results A total of 48 patients were included and randomized (36 patients in the test group and 12 in the control group). However, seven patients did not reach the minimum required number of expanded MSCs and were therefore unable to be treated. The tested intervention demonstrated significantly greater gains in bone volume, with a mean difference of 480.01 mm 3 ( p = 0.032). Similarly, the mean change in bone crest volume from baseline to 5 months was notably higher in the test group (1066.91 mm 3 ) compared to the control group (586.9 mm 3 ). Adverse reactions and patient morbidity were minor in both groups. Implants were placed on the regenerated bone, and all were integrated successfully in both groups. Conclusions The cell‐based therapy resulted in significant changes in bone volume compared to the control treatment, enabling dental implants in all patients. The procedure was associated with minimal adverse effects and patient morbidity

    Efficiency of health systems in developing countries: the case of West African countries

    International audienceDeveloping countries are faced with numerous health challenges such as lack of funding, increasing frequency and magnitude of epidemic risks, organizational and socio-cultural difficulties. In this context, we developed this study to assess for the first time the efficiency of health systems in the countries of the West African sub-region, firstly, to identify the systems that best adapt to these challenges and secondly, to highlight the factors that influence the health production process. To achieve this, we used the World Bank’s, Worldwide Development Indicators and Worldwide Governance Indicators databases, from 2000 to 2022. We applied the stochastic fixed-effect frontier method of Kumbhakar et al. (2014) to account for unobservable heterogeneity in the estimates. We used a novel multiple imputation approach to deal with missing data, while determining the fractions of missing information in the estimates. The results show that the average relative efficiency for all countries in the sub-region is 89.7%. Countries in the West African sub-region could theoretically increase life expectancy at birth by an average of 10.3%, which represents 6.5 years, with the same level of resources used. The results also show that health systems in these countries have higher permanent inefficiency than temporal inefficiency, suggesting that they mainly face structural challenges. Per capita health expenditure, gross domestic product per capita, out-of-pocket expenditure per capita, literacy rate, poverty gap and voice and accountability were positively associated with the efficiency of their health systems

    L'examen échocardiographique transthoracique chez l'éléphant d'Asie (Elephas maximus) : mise au point de la technique (Étude pilote)

    The Asian elephant (Elephas maximus) is an endangered species in which cardiovascular diseases are common but most often diagnosed post-mortem. Transthoracic echocardiography is a non-invasive cardiac imaging method commonly used in domestic animals. In contrast, for elephants, the few rare mentions in the literature do not provide any standardized protocol, and the application of this imaging tool remains, to date, unestablished in the species. After a first section devoted to the elephant and its morphological and physiological particularities, as well as the main cardiovascular diseases reported in the species, this thesis presents a pilot study whose objective was to assess the feasibility of transthoracic echocardiography in the Asian elephant (Elephas maximus), to develop a standardized procedure to optimize image quality, and to evaluate the variability of the measurements obtained. To this end, preliminary trials were carried out on ten elephants: seven in the wild in Cambodia and three captive in France. These trials made it possible to develop a standardized echocardiographic protocol based on medical training and positive reinforcement. The main features of this protocol included the exclusive use of a left parasternal approach, prior moistening of the thoracic skin with warm water before applying contact gel, and extension of the left forelimb using a training ball to optimize access to the acoustic window. Each examination included two views of the left ventricle (LV): a long-axis view for M-mode measurements and a short-axis view for two-dimensional (2D) analysis. In total, 72 examinations were performed over 4 days on the three captive elephants, with delayed analysis of eight 2D and M-mode variables, including two indices of LV systolic function (fractional shortening determined in M-mode and left ventricular fractional shortening calculated in 2D mode). A general linear model was used to determine intra-day and inter-day coefficients of variation. All examinations were successfully performed without anesthesia. Intra-day and inter-day variability was low for all parameters (coefficients of variation <10%), while inter-individual variability was higher (5% to 19.6%), which is explained by the large morphological differences between elephants. In conclusion, LV dimensions and systolic function can be assessed in awake Asian elephants with excellent repeatability and reproducibility. This pilot study paves the way for the non-invasive ante-mortem diagnosis of cardiopathies in elephants and for the longitudinal monitoring of cardiac function in zoological parks.L'éléphant d'Asie (Elephas maximus) est une espèce menacée, chez laquelle les maladies cardiovasculaires sont fréquentes mais le plus souvent diagnostiquées post-mortem. L'échocardiographie transthoracique est une méthode d'imagerie cardiaque non invasive, couramment utilisée chez les animaux domestiques. À l'inverse, chez l'éléphant, les quelques rares mentions présentes dans la littérature ne proposent aucun protocole standardisé, et l'application de cet outil d'imagerie reste à ce jour non établie dans l'espèce. Après une première partie consacrée à l'éléphant et à ses particularités morphologiques, physiologiques ainsi qu'aux principales affections cardiovasculaires recensées dans l'espèce, cette thèse présente une étude pilote dont l'objectif a été d'évaluer la faisabilité de l'échocardiographie transthoracique chez l'éléphant d'Asie (Elephas maximus), de développer une procédure standardisée pour optimiser la qualité des images, et d'évaluer la variabilité des mesures obtenues. Pour ce faire, des essais préliminaires ont été réalisés chez dix éléphants : sept en milieu naturel au Cambodge et trois captifs en France. Ces essais ont permis d'élaborer un protocole échocardiographique standardisé reposant sur du medical training et du renforcement positif. Les principales caractéristiques de ce protocole ont inclus l'utilisation exclusive d'un abord parasternal gauche, l'humidification préalable de la peau thoracique avec de l'eau tiède avant l'application du gel de contact, et l'extension du membre antérieur gauche grâce à l'usage d'une balle d'entraînement afin d'optimiser l'accès à la fenêtre acoustique. Chaque examen comprenait deux vues du ventricule gauche (VG) : une vue grand axe pour les mesures en mode temps-mouvement (TM) et une vue petit axe pour l'analyse bidimensionnelle (2D). Au total, 72 examens ont été réalisés sur 4 jours chez les trois éléphants captifs, avec analyse différée de huit variables 2D et TM, incluant deux indices de fonction systolique du VG (fraction de raccourcissement déterminée en mode TM et fraction de raccourcissement du ventricule gauche calculée en mode 2D). Un modèle linéaire général a été utilisé pour déterminer les coefficients de variation intra-jour et inter-jours. Tous les examens ont pu être réalisés avec succès sans anesthésie. La variabilité intra-jour et inter-jour était faible pour tous les paramètres (coefficients de variation <10 %), tandis que la variabilité interindividuelle était plus élevée (5 % à 19,6 %), ceci s'expliquant par la grande différence morphologique entre les éléphants. En conclusion, les dimensions du VG et sa fonction systolique peuvent être évaluées chez l'éléphant d'Asie éveillé avec une excellente répétabilité et reproductibilité. Cette étude pilote ouvre la voie à la réalisation non invasive de diagnostics ante-mortem de cardiopathies chez l'éléphant et au suivi longitudinal de la fonction cardiaque en parc zoologique

    International audienceIntelligence artificielle au service du développement durable des territoires ? Usages et imaginaires dans le contexte du Grand ParisL’essor de l’Intelligence artificielle (IA) depuis les années 2010 se traduit par l’intégration croissante de dispositifs intelligents dans les activités humaines dont la gestion urbaine et territoriale (Languillon-Aussel, 2024). Dans les discours des acteurs privés et, dans une moindre mesure, des acteurs publics, l’IA apparait comme une innovation pouvant contribuer à rendre la ville et le territoire plus durable, résilient et intelligent (Cugurollo, et al., 2024 ; Monstadt & Coutard, 2019). Il émerge ainsi une offre de dispositifs intelligents traitant de problématiques localisées et précises des villes et des territoires telles que l’optimisation de la gestion de l’énergie, la prédiction des mobilités, la détection de fuites d’eau et l’amélioration du trafic (Picaud, 2023).Cependant, le développement de ces dispositifs soulève un paradoxe important : si l’IA est mobilisée pour répondre aux enjeux de durabilité, elle a aussi des coûts à la fois sociaux (e.g. discrimination, automatisation) et environnementaux (e.g. extraction de ressources, consommation énergétique) (Crawford, 2022). Dans cette contribution, nous nous intéressons aux usages effectifs des dispositifs intelligents pour la gestion du territoire, à l’imaginaire des acteurs qui les portent et à la manière dont sont considérés les enjeux autour de l’IA. Nous rapportons les premiers résultats d’une enquête auprès d’acteurs du territoire de la Métropole du Grand Paris (MGP) qui constitue un terrain d’observation privilégié compte tenu de son écosystème dense d’acteurs du numérique. Quatorze entretiens semi-directifs ont été menés auprès d'entreprises, collectivités, élus locaux et experts sur la période de mai à juillet 2025, complétés par un questionnaire en ligne adressé aux habitants de la MGP. La contribution présentera trois axes principaux. Premièrement, nous analyserons les imaginaires technologiques portés par les acteurs interrogés, qui s’inscrivent dans la continuité des approches solutionnistes du numérique. Les enjeux sociaux et environnementaux tendent à être perçus comme des problèmes d’information (e.g. données incomplètes, analyses imprécises) que l’IA peut aider à résoudre. Par exemple, l’amélioration de la consommation d’eau d’un bâtiment passera par sa mise en données puis leur analyse via l’IA pour en extraire des informations et les transmettre aux usagers. Deuxièmement, nous examinerons les usages effectifs de l’IA et les tensions dans leur mise en œuvre territoriale. Nous montrerons comment l'IA est utilisée principalement sous la forme d’un capteur virtuel afin de mesurer un phénomène à partir de l’analyse de données hétérogènes provenant de capteurs physiques déjà existants. Il s’agira aussi de mettre en évidence les contradictions d’usage entre marketing et efficacité, les difficultés de collaboration entre entreprises et collectivités, ainsi qu'un traitement secondaire des considérations éthiques et écologiques. Enfin, nous aborderons la question des fragilités numériques et sociales à travers les enjeux d’appropriation de l’IA, d’inégalités de compétences et d’obstacles à l’acculturation ainsi que des limites de l’évaluation environnementale des dispositifs

    Effectiveness and safety of third-line advanced therapies in patients with ulcerative colitis: A multicentre retrospective cohort study

    International audienceBackground : The effectiveness of advanced therapies beyond second-line therapies has been poorly described in patients with ulcerative colitis (UC).Aim :To describe the outcomes of third-line advanced therapy in patients with UC in a real-world setting.Methods :We conducted a multicentre retrospective study in patients with UC who received third-line advanced therapy after the failure of a first-line anti-TNF agent and second-line vedolizumab. The primary endpoints were steroid-free clinical remission at weeks 14 and 54.Results :We analysed 237 therapeutic sequences in 150 patients (55 with an anti-TNF agent, 80 with tofacitinib, and 102 with ustekinumab), accounting for 245.3 patient-years. Steroid-free clinical remission at week 14 was achieved in 14 (25.5 %) patients treated with an anti-TNF agent, 40 (50.0 %) with tofacitinib, and 54 (52.9 %) with ustekinumab (RR = 1.96 [1.19–3.25] for tofacitinib and RR = 2.08 [1.28–3.39] for ustekinumab, compared with an anti-TNF agent, respectively). Steroid-free clinical remission at week 54 was achieved in 16 (29.1 %) patients in the anti-TNF group, 37 (46.2 %) in the tofacitinib group and 56 (55.4 %) in the ustekinumab group (RR = 3.07 [0.98–9.60] for tofacitinib and RR = 3.09 [1.01–9.43] for ustekinumab, compared with anti-TNF, respectively). In total, 13.7 % of the patients underwent colectomy. Adverse events occurred in 94 (39.7 %) patients and were less frequent with ustekinumab.Conclusion : In this retrospective study, third-line advanced therapies resulted in a high rate of steroid-free clinical remission at weeks 14 and 54, especially in patients treated with ustekinumab and tofacitinib
