A case report of small bowel obstruction secondary to congenital peritoneal band in adult
AbstractIntroductionSmall bowel obstruction (SBO) is common in adult surgical procedures, mainly due to postoperative adhesions. Acute SBO in adults without history of abdominal surgery, trauma or clinical hernia is less common and has various etiologies. Congenital band is an extremely rare cause.Presentation of caseA 56-year-old man was admitted to our hospital with a two-day history of abdominal pain and bilious vomiting. He had no history of abdominal surgery or any other medical problems. A contrast-enhanced CT of the abdomen showed a distention of small bowel loops with transition point in the right hypochondrium. Distended loops of small bowel were located in the left side of the abdomen, whereas collapsed loops was located in the right side. The normal bowel wall enhancement was preserved. After initial treatment with intravenous fluid and nasogastric suction, he was operated. At laparoscopy a band obstructing the ileum was clearly observed. This anomalous band extending from gallbladder to transverse mesocolon caused a small window leading to internal herniation of the small bowel and obstruction. The band was coagulated and divided. Postoperative outcome was uneventful and the patient was discharged on the second postoperative day. There was no recurrence of symptoms on subsequent follow-up.DiscussionCongenital peritoneal bands are not frequently encountered in surgical practice and these bands are often difficult to classify and define. Diagnosis of acute intestinal obstruction due to CPB must be included in the differential diagnosis in any patient with no history of abdominal surgery, trauma, clinical hernia, inflammatory bowel disease or peritoneal tuberculosis.ConclusionDespite technological advances in radiology preoperative diagnosis remains difficult, however the diagnosis of SBO due to CPB must be considered in any patient with no history of abdominal surgery, Trauma or clinical hernia consulting for occlusive syndrome. The laparoscopic approach should be intended initially for its feasibility and benefits
Faba bean drought responsive gene identification and validation
AbstractThis study was carried out to identify drought-responsive genes in a drought tolerant faba bean variety (Hassawi 2) using a suppressive subtraction hybridization approach (SSH). A total of 913 differentially expressed clones were sequenced from a differential cDNA library that resulted in a total of 225 differentially expressed ESTs. The genes of mitochondrial and chloroplast origin were removed, and the remaining 137 EST sequences were submitted to the gene bank EST database (LIBEST_028448). A sequence analysis identified 35 potentially drought stress-related ESTs that regulate ion channels, kinases, and energy production and utilization and transcription factors. Quantitative PCR on Hassawi 2 genotype confirmed that more than 65% of selected drought-responsive genes were drought-related. Among these induced genes, the expression levels of eight highly up-regulated unigenes were further analyzed across 38 selected faba bean genotypes that differ in their drought tolerance levels. These unigenes included ribulose 1,5-bisphosphate carboxylase (rbcL) gene, non-LTR retroelement reverse related, probable cyclic nucleotide-gated ion channel, polyubiquitin, potassium channel, calcium-dependent protein kinase and putative respiratory burst oxidase-like protein C and a novel unigene. The expression patterns of these unigenes were variable across 38 genotypes however, it was found to be very high in tolerant genotype. The up-regulation of these unigenes in majority of tolerant genotypes suggests their possible role in drought tolerance. The identification of possible drought responsive candidate genes in Vicia faba reported here is an important step toward the development of drought-tolerant genotypes that can cope with arid environments
Mesomechanical aspects of computational modeling for non-homogeneous materials joined by forming
AbstractThe metal forming and joining industries are constantly evolving to adapt to new materials and alloys and to meet customers’ needs. However, the size of the companies in this segment is typically small. Therefore, the progress of the mesomechanical modeling on non-homogeneous materials is relatively slow. This article aims to contribute to the advancement of such computational idea. The industries and academics alike may benefit the mesomechanical aspects of computational modeling for materials joined by forming using the established theories known in the field of mechanics. It may be used as an introductory for the society of joining by forming. This article reviews the basic concept of the finite element method, the fundamental mechanics and highlights some common problems in joining by forming, namely contact problems, local variations and interfacial cracking. This article aims to disseminate the meso/micro-mechanics approach to the field of joining by forming, which currently is dominated by experimental works and macroscopic modeling. Using the simple approaches discussed in this article, researchers with little or no background in computational mechanics can implement the concepts
Greater Sage-Grouse Resource Selection Drives Reproductive Fitness Under a Conifer Removal Strategy
AbstractThe link between individual variation in resource selection (e.g., functional response) and fitness creates a foundation for understanding wildlife-habitat relationships. Although many anthropogenic activities adversely affect these relationships, it is largely unknown whether projects implemented to benefit wildlife populations actually achieve this outcome. For sagebrush (Artemisia spp.) obligate species such as the greater sage-grouse (Centrocercus urophasianus; sage-grouse), expansion of juniper (Juniperus spp.) and pinyon-pine (Pinus spp.; conifers) woodlands into sagebrush ecosystems has been identified as a conservation threat. This threat is intensified when a sagebrush ecosystem is bounded by naturally unsuitable habitats. As such, federal, state, and private land managers have implemented landscape-level management to remove conifers on thousands of hectares of sagebrush habitat across the western United States. Despite the scale of contemporary conifer treatments, little was previously known whether sage-grouse will occupy these manipulated landscapes and whether occupancy has consequences on fitness components. To address these questions, we monitored nest and brood success rates for 96 radio-marked sage-grouse from 2012-2015 that inhabited conifer-dominated landscapes in the Box Elder Sage-grouse Management Area in Utah where mechanical conifer removal projects were completed. We then linked sage-grouse resource selection to individual nest (n = 95) and brood (n = 56) success by incorporating random-slope Resource Selection Functions as explanatory predictors in a logistic brood success model. Using the novel approach of random slope covariates, we demonstrated that sage-grouse selected for nest and brooding sites closer to conifer removal areas and that the probability of individual nest and brood success declined (β = −0.10 and β = −0.74, respectively) as sage-grouse females selected sites farther from conifer removal areas. Our research provided the first evidence that mechanical conifer removal treatments can increase suitable available breeding habitats for female sage-grouse and that individuals who occupied these areas experienced enhanced nest and brood success
Predictive values of some atherogenic risk factors in young workers occupationally exposed to vinyl chloride and heavy metals
AbstractTraditional risk factors do not explain all of the risk for incident coronary heart disease (CHD) events. Human susceptibility to atherosclerosis and consequently coronary heart disease is maximally exhibited when the environment is unfavorable, especially in workplace. Thus, the present work was undertaken to study the relation of lipoprotein (a) to the other atherogenic risk factors in young workers occupationally exposed to vinyl chloride or some heavy metals by studying the effect of exposure to these agents on the lipid profiles, immunological parameters and the antioxidant defense enzyme system. The results of this study revealed that, in metalists, the cluster features of dyslipidemia, impairment in antioxidant defense mechanism and high levels of Lp (a), CICs, C3 and C4 represent unfortunate events on their cardiovascular system. In VCW, vinyl chloride metabolites caused severe oxidative stress reflected by impairment in the antioxidant defense accompanied by propagation of lipid peroxidation. Additionally, the elevated levels of Lp (a), CICs, C3 and C4 may point out to their role as atherogenic risk factors in those workers. In conclusion, young workers occupationally exposed to VC may be at high risk of developing cardiovascular disease in spite of having normolipidemia
Planning for green infrastructure: The spatial effects of parks, forests, and fields on Helsinki's apartment prices
AbstractAs the importance of urban green spaces is increasingly recognised, so does the need for their systematic placement in a broader array of socioeconomic objectives. From an urban planning and economics perspective, this represents a spatial task: if more land is allocated to various types of green, how do the economic effects propagate throughout urban space? This paper focuses on the spatial marginal effects of forests, parks, and fields and estimates spatial hedonic models on a sample of apartment transactions in Helsinki, Finland. The results indicate that the capitalization of urban green in apartment prices depends on the type of green, but also interacts with distance to the city centre. Additionally, the effects contain variable pure and spatial spillover impacts, also conditional on type and location, the separation of which highlights aspects not commonly accounted for. The planning of green infrastructure will therefore benefit from parameterizing interventions according to location, green type, and character of spatial impacts
Is willingness to pay higher for cancer prevention and treatment?
AbstractIt is often assumed by health economists that the principal objective of health care is to maximise population health. However, people may be willing to sacrifice overall health in order to direct resources towards high priority disease areas, such as cancer. This paper examines whether society is willing to pay more for cancer prevention and treatment than for other types of health care. The policy context in the UK, where special assessment criteria and funding arrangements are currently in place for certain cancer drugs, is described. Selected empirical studies that have examined the extent of public support for a ‘cancer premium’ are then summarised and discussed. The evidence available is not sufficiently strong to conclude whether or not willingness to pay is higher for cancer prevention and treatment
A new approach for crude oil price prediction based on stream learning
AbstractCrude oil is the world's leading fuel, and its prices have a big impact on the global environment, economy as well as oil exploration and exploitation activities. Oil price forecasts are very useful to industries, governments and individuals. Although many methods have been developed for predicting oil prices, it remains one of the most challenging forecasting problems due to the high volatility of oil prices. In this paper, we propose a novel approach for crude oil price prediction based on a new machine learning paradigm called stream learning. The main advantage of our stream learning approach is that the prediction model can capture the changing pattern of oil prices since the model is continuously updated whenever new oil price data are available, with very small constant overhead. To evaluate the forecasting ability of our stream learning model, we compare it with three other popular oil price prediction models. The experiment results show that our stream learning model achieves the highest accuracy in terms of both mean squared prediction error and directional accuracy ratio over a variety of forecast time horizons

AbstractHighly efficient and eco-friendly, one pot synthesis of 1,2,4,5-tetra substituted imidazoles and 2,4,5-trisubstituted imidazoles was reported under solvent free conditions using nanocrystalline silica supported tin oxide (SiO2:SnO2) as a catalyst with excellent yield. The present methodology offers several advantages such as mild reaction conditions, short reaction time, good yield, high purity of product, recyclable catalyst without a noticeable decrease in catalytic activity and can be used for large scale synthesis. The synthesized SiO2:SnO2 nanocrystalline catalyst was characterized by XRD, BET surface area and TEM techniques
Interdialytic Weight Gain: Trends, Predictors, and Associated Outcomes in the International Dialysis Outcomes and Practice Patterns Study (DOPPS)
BackgroundHigh interdialytic weight gain (IDWG) is associated with adverse outcomes in hemodialysis (HD) patients. We identified temporal and regional trends in IDWG, predictors of IDWG, and associations of IDWG with clinical outcomes.Study DesignAnalysis 1: sequential cross-sections to identify facility- and patient-level predictors of IDWG and their temporal trends. Analysis 2: prospective cohort study to assess associations between IDWG and mortality and hospitalization risk.Setting & Participants21,919 participants on HD therapy for 1 year or longer in the Dialysis Outcomes and Practice Patterns Study (DOPPS) phases 2 to 5 (2002-2014).PredictorsAnalysis 1: study phase, patient demographics and comorbid conditions, HD facility practices. Analysis 2: relative IDWG, expressed as percentage of post-HD weight (<0%, 0%-0.99%, 1%-2.49%, 2.5%-3.99% [reference], 4%-5.69%, and ≥5.7%).OutcomesAnalysis 1: relative IDWG as a continuous variable using linear mixed models; analysis 2: mortality; all-cause and cause-specific hospitalization using Cox regression, adjusting for potential confounders.ResultsFrom phase 2 to 5, IDWG declined in the United States (−0.29kg; −0.5% of post-HD weight), Canada (−0.25kg; −0.8%), and Europe (−0.22kg; −0.5%), with more modest declines in Japan and Australia/New Zealand. Among modifiable factors associated with IDWG, the most notable was facility mean dialysate sodium concentration: every 1-mEq/L greater dialysate sodium concentration was associated with 0.13 (95% CI, 0.11-0.16) greater relative IDWG. Compared to relative IDWG of 2.5% to 3.99%, there was elevated risk for mortality with relative IDWG≥5.7% (adjusted HR, 1.23; 95% CI, 1.08-1.40) and elevated risk for fluid-overload hospitalization with relative IDWG≥4% (HRs of 1.28 [95% CI, 1.09-1.49] and 1.64 [95% CI, 1.27-2.13] for relative IDWGs of 4%-5.69% and ≥5.7%, respectively).LimitationsPossible residual confounding. No dietary salt intake data.ConclusionsReductions in IDWG during the past decade were partially explained by reductions in dialysate sodium concentration. Focusing quality improvement strategies on reducing occurrences of high IDWG may improve outcomes in HD patients
