How do geriatricians evaluate decision-making ability for older adults with cognitive impairment? Results from an European survey
International audienceContext: The assessment of decision-making ability of older adults with cognitive impairment is a complex challenge that geriatricians often face in relation to risk-taking situations (driving, aging in place, financial decisions, etc.). However, there are no clear and consensual practice guidelines. An overview of current practices and needs seemed necessary. Methods: We co-created and conducted an online survey to describe practice and knowledge, among European geriatricians. The survey was structured in 3 parts: a description of the professional’s practice regarding cognitive impairment, a specific questionnaire about everyday risky decision-making evaluation and an investigation of the clinician’s knowledge about relevant ethical and legal recommendations. Each part consisted of both multiple choice and open questions, analyzed through descriptive statistics and qualitative analysis methods. Results: Based on the responses of 123 geriatricians across Europe, our survey showed that clinical interview is the cornerstone of geriatric assessment of decision-making ability of patients with mild to moderate dementia. When faced with risk-taking dilemma situations, geriatricians tend to favor a context of safety above autonomy, but they can support risky decision-making if it is consistent with the patient’s previous lifestyle, depending on the degree of risk to self and others, on the decision-making ability assessed, and if there is some form of shared decision-making. Conclusion: Assessing decision-making ability is challenging for geriatricians, who in our study relied more on their clinical interview and global cognitive tests than more in-depth evaluations. Supporting independent decision-making, when associated with risk-taking, requires better detection and anticipation shared with the patient environme
Shutterstoc
Deinstitutionalisation, community-based care and coercion: Did France learn from international experiences?
International audienceFor the past 50 years, many countries have been moving psychiatric support from a hospital-based model to community-based care. This process, known as deinstitutionalisation, has been defined with three main components: the movement of individuals from hospitals into the community; their diversion from hospital admission; and the development of alternative community services (Introd.
A control banding method for chemical risk assessment in occupational settings in France
International audienceBackground: This study describes a method whose aim is to help companies assess the chemical occupational risks related to labeled products and industrial chemical emissions. The method is intended to be used by industrial hygienists at the scale of one company. Both inhalation and cutaneous exposure routes are taken into account. Methods: The method relies on a control-banding scheme. A work situation is described by exposure parameters such as the process or the local exhaust ventilation and by the hazard of the product. Each possible value of the parameters is associated with a “band,” which is associated with an integer value. The multiplication of these values results in a score, which represents a priority for intervention. The higher the score, the more the situation warrants investigation for implementing prevention measures, such as chemical substitution and the addition of local exhaust ventilation. To simplify communication, the priority is associated with a colored priority band: red for “very high priority,” orange for “high priority,” and green for “moderate priority.” The priority bands are computed for all work situations performed in a company. Results: An example of the use of this method is described in a French façade insulation company. Conclusion: A tool named Seirich was developed to implement this method and promote good practices for helping industrial hygienists in the prioritization of interventions for reducing chemical risk in France

Romantic love and involvement in bullying and cyberbullying in 15-year-old adolescents from eight European countries and regions
International audienceSexual minority youth are at greater risk for bullying victimization than their heterosexual peers but data on perpetration and cybervictimization is limited. Using representative data from seven European countries and one region (N = 14,545), this study compared traditional bullying victimization and perpetration, and cyberbullying victimization among 15-year-old adolescents who reported ever being in love with same- or both- gender peers (sexual minority) versus opposite-gender peers (non-minority). Adolescents who have never been in love and non-respondents were also included. Analyses were stratified by gender and adjusted for country/region and family affluence. Compared to those attracted to opposite-gender peers, traditional bullying perpetration was more likely to be reported by both-gender attracted girls, while bullying victimization was more likely to be reported by both-gender attracted girls and both- and same-gender attracted boys. All sexual minority youth were more likely to report cybervictimization compared to their non-minority peers. Adolescents who have never been in love reported lower levels of bullying involvement than all other youth. Sexual minority stigma may contribute to higher risk of bullying involvement among adolescents. Interventions need to specifically address bullying involvement and associated health risks of sexual minority youth. Available evidence shows that explicit school policies and interventions tailored to local settings are particularly effective
