    Modeling the impact of national and regional lockdowns on the 2020 spring wave of COVID-19 in France

    International audienceSeveral countries have implemented lockdowns to control their COVID-19 epidemic. However, questions like “where” and “when” still require answers. We assessed the impact of national and regional lockdowns considering the French first epidemic wave of COVID-19 as a case study. In a regional lockdown scenario aimed at preventing intensive care units (ICU) saturation, almost all French regions would have had to implement a lockdown within 10 days and 96% of ICU capacities would have been used. For slowly growing epidemics, with a lower reproduction number, the expected delays between regional lockdowns increase. However, the public health costs associated with these delays tend to grow with time. In a quickly growing pandemic wave, defining the timing of lockdowns at a regional rather than national level delays by a few days the implementation of a nationwide lockdown but leads to substantially higher morbidity, mortality, and stress on the healthcare system

    Infant neurodevelopment and behavior in Guadeloupe after lead exposure and Zika maternal infection during pregnancy

    International audienceBACKGROUND: Prenatal lead exposure is known to have neurotoxic effects on the developing fetus, while some viral infections may have a tropism for the central nervous system. Our objective was to study whether the effects of prenatal lead exposure on infant development and behaviors at 18 months of age are modified by the occurrence of a maternal infection to Zika virus (ZIKV) during pregnancy. METHODS: During the ZIKV epidemic in Guadeloupe in 2016 a cohort of pregnant women was set up. Blood samples (pregnancy, childbirth and cord) (n = 297) enabled us to measure blood lead levels aimed to determine prenatal lead exposure and the likelihood of maternal infection during pregnancy (ZIKV status + vs -). The 18 months "Ages and Stages Questionnaire" (ASQ) was used to generate scores for global development, fine and gross motor skills, communication, problem solving, and personal-social skills. The questions from a longitudinal cohort study conducted in Canada (Québec) were used to generate hyperactivity, opposition, inattention and physical aggression scores. Associations were tested by multivariate linear regressions. RESULTS: Prenatal lead exposure was associated with delays in neurodevelopment at 18 months, reflected by lower scores in ASQ totals, and in the fine motor and problem-solving domains. Some of these associations appeared to be sex-specific, observed almost exclusively in boys (ASQ total, fine motor and personal-social scores). Prenatal lead exposure was not associated with behavioral scores. ZIKV infection during pregnancy was associated with a lower fine motor ASQ score, and higher scores for hyperactivity, opposition and physical aggression. Significant interaction between prenatal lead exposure and ZIKV status was observed with a lower personal-social score in ZIKV (-) only, and for hyperactivity and inattention scores, though some of these interactions (ASQ personal-social score, inattention score) were no longer significant when children with microcephaly were excluded from the analyses. DISCUSSION/CONCLUSION: Our study confirms previous findings of associations between prenatal exposure to lead at low levels and adverse neurodevelopmental outcomes during infancy and the particular vulnerability of boys. It suggests associations between ZIKV infection during pregnancy and adverse effects on a number of neurodevelopmental functions (fine motor function) and behaviors (opposition, hyperactivity), that need to be confirmed at later age. There is no strong evidence of interaction between ZIKV infection and lead exposure but both prenatal risk factors may affect fine motor function

    Low socioeconomic status was associated with a higher mortality risk in multiple sclerosis

    International audienceBackground - The relationship between socioeconomic status (SES) and mortality among persons with multiple sclerosis (PwMS) is poorly understood. Objective - To investigate the association between SES and mortality risk in PwMS. Methods - From health-administrative data, we identified 12,126 incident MS cases with a first demyelinating event (MS 'onset') occurring between 1994 and 2017. Cox proportional hazard model assessed the association between socioeconomic status quintiles (SES-Qs) at MS onset and all-cause mortality. Results - Lower SES-Qs were associated with higher mortality risk; adjusted hazard ratios: SES-Q1 (most deprived) =1.61 (95% confidence interval (CI) = 1.36-1.91); SES-Q2 = 1.26 (95% CI = 1.05-1.50); SES-Q3 = 1.22 (95% CI = 1.02-1.46); SES-Q4 = 1.13 (95% CI = 0.94-1.35) versus SES-Q5 (least deprived). Conclusion - A lower SES was associated with higher mortality risk in PwMS

    Bioaerosols deteriorate indoor air quality (IAQ) and can affect human health and well-being. Sampling of microbial aerosols inindoor air is critical but quantification/identification of microorganisms at low concentrations is difficult. Filters used in air handling units (AHUs) can be considered as a long-term sampler accumulating microbial contamination to assess IAQ. This PhD work aimed to develop a methodology to analyse exposure to microbial pollutants in indoor air in offices using AHU filters. A sampling methodology using filter media coupons on AHU filters was applied over 13 months. Preliminary tests showed that the presence of the coupons on the filter surface did not increase the pressure drop and that the collection efficiency of submicrometric particles was about 50% by mass. Colony forming unit (CFU) counts and analyses by qPCR, RT-qPCR and 16S/ITS sequencing were conducted. Microbial concentrations on the filter remained almost stable over the 13 months (between 102 and 103 CFU/cm²). The qPCR results revealed higher concentrations compared to the cultural methods. The fungal and bacterial genera identified corresponded to the occupancy of the building (Micrococcus) but also to the outdoor air. Two respiratory viruses were identified on the filter (Adenovirus and Coronavirus). AHU filters can be used to analyse the microbiological contamination of indoor air. However, other studies will have to optimize this methodology, especially the extraction yield of nucleic acids;Les bioaérosols dégradent la qualité de l’air intérieur (QAI) et peuvent affecter la santé et le bien-être des personnes. L’échantillonnage d’aérosols microbiens dans l’air intérieur est essentiel mais la quantification/identification des microorganismes en faibles concentrations est difficile. Les filtres installés dans les centrales de traitement d'air (CTA) peuvent être considérés comme un échantillonneur à long terme accumulant la contamination microbienne pour évaluer la QAI. Cestravaux de thèse ont permis de développer une méthodologie pour analyser l'exposition aux contaminants microbiens de l'air intérieur dans des bureaux en utilisant les filtres de CTA. La méthodologie de prélèvement par coupons de média filtrant déposés sur les filtres d'extraction des CTA a été appliquée pendant 13 mois sur un bâtiment basse consommation. Des tests préliminaires ont montré que la présence des coupons à la surface du filtre n'augmentait pas la perte de charge et que l'efficacité de collecte des particules submicrométriques était de l'ordre de 50% en masse. Les comptages en unités formant colonies (UFC) et analyses par qPCR, RT-qPCR et séquençages 16S/ITS ont été menés. Les concentrations microbiennes sur le filtre sont restées pratiquement stables au cours des 13 mois (entre 102 et 103 UFC/cm²). Les résultats par qPCR ont révélé des concentrations plus élevéescomparé aux méthodes culturales. Les genres fongiques et bactériens identifiés correspondent à l'occupation du bâtiment(Micrococcus) mais aussi de l'air extérieur. Deux virus respiratoires ont été identifiés sur le filtre (Adenovirus etCoronavirus). Les filtres de CTA peuvent permettre d'analyser la contamination microbiologique de l'air intérieur.Cependant, d'autres études devront être menées pour optimiser cette méthodologie, notamment le rendement d’extraction des acides nucléiques

Since October 7, students have been vocal, engaging in debates, openly opposing one another, and organizing demonstrations.This polarization has brought the Israeli-Palestinian issue back to the forefront, placing university leaders in a challenging position: they must constantly balance the protection of free speech with campus safety, while maintaining good relations with donors, who represent a major source of funding for higher education institutions in the United States.La violence de l'attaque inédite du Hamas contre Israël, puis celle de la riposte de Tsahal dans la bande de Gaza ont déclenché des réactions intenses au sein de la plupart des campus universitaires américains.     COVID-19 Infection despite Previous Vaccination in Cancer Patients and Healthcare Workers: Results from a French Prospective Multicenter Cohort (PAPESCO-19)

    International audienceIn a multicenter prospective cohort of cancer patients (CP; n = 840) and healthcare workers (HCWs; n = 935) vaccinated against COVID-19, we noticed the following: i/after vaccination, 4.4% of HCWs and 5.8% of CP were infected; ii/no characteristic was associated with post-vaccine COVID-19 infections among HCWs; iii/CP who developed infections were younger, more frequently women (NS), more frequently had gastrointestinal, gynecological, or breast cancer and a localized cancer stage; iv/CP vaccinated while receiving chemotherapy or targeted therapy had (NS) more breakthrough infections after vaccination than those vaccinated after these treatments; the opposite was noted with radiotherapy, immunotherapy, or hormonotherapy; v/most COVID-19 infections occurred either during the Alpha wave (11/41 HCW, 20/49 CP), early after the first vaccination campaign started, or during the Omicron wave (21/41 HCW, 20/49 CP), more than 3 months after the second dose; vi/risk of infection was not associated with values of antibody titers; vii/the outcome of these COVID-19 infections after vaccination was not severe in all cases. To conclude, around 5% of our CPs or HCWs developed a COVID-19 infection despite previous vaccination. The outcome of these infections was not severe

    La psychiatrie : domaine d’exception au principe de consentement des personnes ?

    National audienceLa loi reconnaît que le patient ne peut être soigné sans son consentement, et qu’il peut refuser des soins. La psychiatrie fait exception : des soins peuvent être imposés sous contrainte au patient, au motif de son incapacité à consentir, liée aux troubles psychiques. Toutefois depuis 2016, les différentes formes de contrainte et de restriction des libertés en psychiatrie font l’objet d’une vigilance renforcée des formes de contrainte intra‑hospitalière que sont l’enfermement dans une chambre (isolement) ou l’usage de sangles (contention). Ce contrôle accru ne semble pas, à lui seul, suffire à diminuer le recours à la contrainte ni à assurer le respect des droits des personnes. La persistance de formes de disqualifications et d’une stigmatisation contribuent à des formes de tolérance envers un traitement différencié des personnes avec un trouble psychique. L’on constate cependant un nouvel accent mis sur l’importance de tenir compte de ce que les personnes peuvent exprimer pour elles, tant sur le plan du respect de leurs droits fondamentaux, que dans la perspective de trouver un mieux‑être et une vie qui a du sens

    La médiation en santé : un levier relationnel de lutte contre les inégalités sociales de santé à consolider

    La médiation en santé constitue un processus relationnel qui concourt à la résorption d'inégalités sociales de santé grâce à sa fonction d'interface entre publics vulnérables et acteurs de santé. D'initiative associative et historiquement ciblée sur des publics marginalisés (VIH, prostitution, gens du voyage…), la médiation en santé s'adresse désormais à des publics en situation de vulnérabilité pour des motifs variés qu'ils soient endogènes (précarité socioéconomique, crainte de la stigmatisation, situation de handicap, etc.) ou exogènes (complexité du système de soins, difficultés et délais d'accès, etc.). Malgré l'absence d'étude médico-économique en France ou à l'étranger, de nombreuses initiatives territoriales ont permis d'éprouver la pertinence et l'utilité de la médiation en santé dans la lutte contre les inégalités sociales de santé, conduisant à sa reconnaissance institutionnelle progressive notamment au travers la loi de modernisation du système de santé de 2016..
