Identification des dimensions de maturité des dispositifs de consultation dédiée à la prévention : une étude qualitative multi-cas
Introduction: In response to the political will to strengthen health prevention, several structured preventive consultation initiatives have emerged in France. This study aims to identify the dimensions that characterize the organizational maturity of such initiatives.Methods: A qualitative three-part study was conducted on three initiatives: “centres d’examen de la santé”, “mon bilan prevention”, and MedPrev. The analysis combined document review, comparison of self-administered questionnaires, and grounded theory analysis of semi-structured interviews conducted with project designers and coordinators.Results: Twelve maturity dimensions were identified: innovation, vision, market approach, deployment, empowerment, evaluation, economic model, digital tools, professional posture, governance, change strategy, and networking. Each dimension was structured into a four-level scale. The positioning of the initiatives on this model reveals unequal levels of maturity, across highly distinct dimensions depending on the initiative.Discussion: The model provides a structured and differentiated reading of how preventive initiatives are developing. It avoids prescriptive judgment, offering each actor a potential tool for reflecting on their own trajectory.Conclusion: This study enhances understanding of the organizational dynamics involved in the deployment of preventive health initiatives.Introduction : Face à la volonté politique de renforcer la prévention en santé, plusieurs dispositifs structurés de consultations préventives ont émergé en France. Cette étude vise à identifier les dimensions caractérisant la maturité organisationnelle de ces dispositifs.Méthode : Une étude qualitative en trois volets a été conduite sur trois dispositifs : les centres d’examens de la santé, mon bilan prévention et MedPrev. L’analyse repose sur une approche croisée combinant revue documentaire, analyse comparative des auto-questionnaires, et analyse par théorisation ancrée des entretiens semi-dirigés menés auprès de concepteurs et gestionnaires.Résultats : Douze dimensions de maturité ont été identifiées : innovation, vision, approche du marché, déploiement, pouvoir d’agir, évaluation, modèle économique, outils numériques, posture professionnelle, gouvernance, stratégie de changement et mise en réseau. Chaque dimension est structurée selon une échelle de 4 niveaux. Le positionnement des dispositifs sur ce modèle révèle des niveaux de maturité inégaux, et ce, sur des dimensions très distinctes selon les dispositifs étudiés.Discussion : Ce modèle offre une lecture structurée et différenciée du développement des dispositifs de prévention. Il permet d'apprécier leur degré de structuration sans porter de jugement normatif, laissant à chaque acteur la liberté d’en faire un outil de réflexion sur sa propre dynamique.Conclusion : Ce travail contribue à la compréhension de la diversité des logiques organisationnelles à l’œuvre dans le déploiement de dispositifs de prévention

Intra-familial transmission of Hepatitis B virus in a peri-urban community from the Democratic Republic of the Congo
International audienceBackgroundDespite global elimination targets set for 2030, Hepatitis B virus (HBV) infection remains a major public health challenge in low-income countries, including the Democratic Republic of Congo (DRC). Limited evidence on the regional transmission pathways precludes progress towards HBV elimination. This study aimed to assess the prevalence, molecular characteristics, and transmission dynamics of HBV in the Lukelenge health district, a peri-urban area in central DRC.MethodsWe employed a two-tiered recruitment strategy: community member volunteers were enrolled during the first phase, and upon notification of HBV positivity in an index case, family contacts were subsequently recruited in the second phase. Participants were screened for hepatitis B surface antigen (HBsAg), followed by PCR amplification of HBV DNA and sequencing. Genotyping and phylogenetic analysis of preS/S sequences were performed to explore regional HBV diversity and transmission patterns.ResultsA total of 751 participants from 677 households were included. The overall HBsAg prevalence was 3.8% [95% CI 2.6–5.7], with the highest rate (10.1% [95% CI 4.9–18.2]) found in children aged 5 years and younger. All 42 HBV isolates belonged to genotype E, with 97.6% sharing the ayw4 serotype. Mutations with relevancy to immune escape were detected in 9.5% of strains, while those possibly linked to antiviral resistance were found in 4.7%. Maximum likelihood phylogenetic analysis showed intra-familial clustering of preS/S sequences, suggesting that parent-to-child transmission was the most frequent mode of HBV spread in the study population.ConclusionsHBV in Lukelenge shows intermediate endemicity, especially affecting young children. Intra-familial transmission is revealed to be predominant, likely involving both vertical and horizontal pathways. Family-targeted interventions, including maternal screening and universal birth-dose vaccination, should be prioritized to eliminate HBV in this region
Potential role of water-percolates pH and sand size on the transfer/ retention of Escherichia coli cells through sandy soil
International audienceUnder natural conditions, water contaminants can be retained during infiltration through sandy soil or a sand column. The interaction between water pH and sand size may influence this process. This study aims to assess the combined effects of these two parameters on the transfer or retention of E. coli cells (used as a model) during the percolation of bacteria-contaminated water through a sand column. Experiments were conducted from August to December 2024, using sand with three different size ranges (0.1-0.3 mm, 0.3-0.5 mm, and 0.5-0.8 mm) poured into PVC pipes. Water at various pH levels (5, 6, 7, 8, and 9) containing E. coli (7×107 CFU/ml) was prepared in Erlenmeyer flasks with a tap connected. Percolation tests of the contaminated water were then carried out through the sand columns. Bacteriological analysis of the percolated water was performed on the first drops and after 2, 4, 6, and 8 hours. The concentrations of cells in the percolated water fluctuated between 0.036×104 and 29.5×104 CFU/ml. The lowest concentration was observed at 4 hours of percolation with water at pH 5, while the highest was noted at 8 hours with water at pH 9. The lowest percentage of retained cells in 1 ml of percolated water was 99.71% for sand size 0.1-0.3 mm, 99.70% for 0.3-0.5 mm, and 99.57% for 0.5-0.8 mm. E. coli cells in acidic percolating water were retained more effectively in the sand column than those in alkaline water. This retention process appears to be more efficient with smaller sand grain sizes. The percentages of retained cells varied over time. Water treatment through sand filtration is a complex process that depends on several factors for its effectiveness. This knowledge could help predict the fate of bacterial contaminants in the environment. Continued field and laboratory experiments are necessary to clarify the individual roles of various microbiological, physicochemical, and hydrological factors involved in the complexities of interactions during the transfer of bacteria-contaminated water through sand columns at the household scale, as well as through sandy soil or subsoil in natural environments
Experimental Study of Magnetic Near-Field Microstrip Electronic Probe for PCB EMC Emission Measurement
International audienceAn experimental study on magnetic near-field (NF) scanning of printed circuit board (PCB) emission radiation is developed in this paper. The design and installation of the electromagnetic (EM) NF scanner is introduced. The test bed of magnetic NF emission in the microwave frequency range is described. The methodology of the microstrip magnetic NF probe is discussed. The probe calibration process was performed following the IEC 61967-1 NF scanning standard. The NF scanner functioning is tested with passive microstrip circuit square loop probe and device under test (DUT) PCB radiation in the test plan positioned at 1-mm above the ground plane. Based on the standard test with I-shape 50-Ω transmission line (TL), the calibration process of radiated magnetic field was validated by comparison between HFSS® simulation and experimentation in very wideband frequency from 0.1-GHz to 3-GHz. Then, a nonstandard TL based DUT was experimented. Accordingly, the cartographies of scanned magnetic NF at two different test frequencies, 2 GHz and 3 GHz, are discussed. The NF scanner is under development for targeting the EMC radiated emission of PCB dedicated to operate in 6G wireless communication.</div
Evolution of Lifestyles and Adaptation Strategies to Social, Cultural, Economic and Rainfall Changes in Makary, Yagoua, Koza and Mozogo (Sudano Sahelian Zone of Cameroon)
International audienceRecent rainfall fluctuations across the Sudano-Sahelian zones of Cameroon reveal a marked increase in interannual variability and the frequency of extreme hydroclimatic events. This study investigates how these rainfall shifts reshape livelihoods and adaptive responses in two contrasting ecological settings: the humid Sudano-Sahelian plains (Makary and Yagoua) and the drier mountainous areas (Koza and Mozogo). Anchored in a socio-ecological vulnerability and resilience framework, the research combines household surveys, semi-structured interviews, time-series analysis of rainfall data (1985–2023), and participatory workshops with local stakeholders. Findings indicate a moderate rise in the number of annual heavy rainfall days (>20 mm), more pronounced in lowland sites, together with greater interannual variability in the mountainous areas (CV ≈ 30–35%). This pattern corresponds to regional trends in the Sudano-Sahel, characterized by intensified rainfall extremes and prolonged dry spells. In the plains, recurrent flooding events lead to severe crop losses, seed depletion, and soil degradation, whereas in mountainous areas, reduced and erratic rainfall induces localized droughts, water stress, and depletion of grazing resources. Local communities exhibit differentiated adaptation strategies. In the plains, households rely on economic diversification, temporary migration, fish farming, and irrigation intensification. In the mountains, coping mechanisms include delayed sowing, soil and water conservation structures, and controlled transhumance. Female participation in income-generating activities (39–47%) and water collection (mean 5.1 h/day in dry zones) reflects a progressive reshaping of gender roles under climatic constraints. However, higher “knowledge erosion” indices in mountainous sites (41–47%) indicate weakened intergenerational transmission of traditional agro-ecological knowledge. Collective adaptation mechanisms such as cooperatives, water management committees, and mutual aid networks are more active in the plains, while mountainous areas face weaker local governance (perceived inefficiency 41–62%) and growing resource-use conflicts (27–33%). Limited access to early warning systems (alert coverage <22%) and low household financial capital (USD 45–120 per household) severely constrain anticipatory and investment capacities. Overall, the study highlights context-dependent adaptive trajectories: managing hydric excess in the humid plains versus coping with scarcity in dry mountainous landscapes. Strengthening local adaptive capacities, integrating gender-sensitive approaches, and developing effective early-warning and institutional support mechanisms are essential to enhancing socio-ecological resilience and sustainability in these climate-stressed Sudano-Sahelian environments

Quelles inégalités scolaires au prisme de l’Indice de Position Sociale (IPS) à Mayotte et La Réunion ?
International audienceMalgré leurs différences et leurs spécificités, La Réunion et Mayotte sont deux territoires français de l’océan Indien qui présentent des défis majeurs sur le plan éducatif. Le présent chapitre vise à mesurer et analyser les inégalités scolaires dans une approche à la fois comparative et contrastive permettant de mieux appréhender les fractures socio-spatiales à l’échelle de chacun des deux départements et à l’échelle plus large des territoires ultramarins. Pour aborder ces inégalités, l’indicateur qui a été choisi est l’Indice de position sociale (IPS). Le niveau privilégié est celui du collège où les élèves n’ont pas encore opté pour une filière déterminée. L’étude, de nature quantitative, se situe dans la perspective d’une approche sociogéographique visant à mettre en visibilité et à interroger les déterminants spatiaux de la ségrégation scolaire à différentes échelles.La Réunion et Mayotte sont deux territoires ultramarins qui partagent des points communs sur le plan historique et géographique. Anciennes colonies françaises devenues Départements d’outre-mer en 1946 pour La Réunion et en 2011 pour Mayotte, ces deux îles de l’Océan Indien sont considérées aujourd’hui comme régions ultrapériphériques au sein de l’Union européenne. Malgré des différences en termes de niveau socio-économique, de structure par âge ou de composition ethnique et linguistique, les deux territoires présentent certains points communs. Leur système éducatif est confronté à des défis majeurs. Les jeunes de moins de 20 ans atteignent 30% de la population totale à La Réunion et 55% à Mayotte (pour 23% dans l’hexagone). En 2021, la population en dessous du seuil de pauvreté monétaire était de 36% à La Réunion et 59% à Mayotte qui reste le département le plus inégalitaire de France (Roinsard, 2022). La part des enfants issus de familles pauvres y atteint respectivement 60% et 80%, en ce qui concerne ces deux Départements et régions d’outre-mer. A l’échelle des DROM, le taux de décrochage scolaire est deux fois plus élevé que dans l'hexagone .Si les besoins éducatifs sont tout particulièrement urgents dans l’académie de Mayotte - a fortiori après le passage du cyclone Chido qui a lourdement impacté le département en décembre 2024, l’académie de La Réunion accuse aussi des retards – bien que moins importants - dans le domaine de l’éducation. Dans les deux cas, les inégalités scolaires y sont très marquées à l’instar des autres départements d’outre-mer (Guadeloupe, Martinique et Guyane), qui témoignent de décalages importants par rapport aux autres départements français.Il convient toutefois de se garder de tout comparatisme et de tenir compte des spécificités de ces territoires qui ont chacun leurs héritages et leurs trajectoires. Le présent chapitre vise à mesurer et analyser les inégalités scolaires dans une approche à la fois comparative et contrastive permettant de mieux appréhender les fractures socio-spatiales à l’échelle de chacun des deux départements et à l’échelle plus large des territoires ultramarins. Pour aborder ces inégalités, l’indicateur qui a été choisi est l’Indice de position sociale (IPS), rendu public depuis octobre 2022 par le Ministère de l’Éducation Nationale. Nous présentons d’abord cet indicateur afin d’en évaluer la pertinence et les limites pour mesurer les inégalités scolaires. Nous mettons ensuite en évidence les disparités socio-spatiales à plusieurs échelles, tout d’abord à l’échelle des Départements et régions d’outre-mer (DROM), puis à l’échelle plus spécifique de La Réunion et de Mayotte, en montrant l’intérêt de mettre en visibilité la ségrégation socio-spatiale à un niveau plus détaillé, celui des communes et des quartiers
