How to Overcome Bone Resorption in Complete Removable Prosthesis?
The existence of alveolar processes and the integrity of the maxillae outside of a specific pathological process are linked to the presence of teeth. The aging process is most often accompanied, at the buccal level, by a narrowing of the prosthetic corridor in relation to the importance of bone resorption and the invasion of peripheral elements. Thus, we are and will be confronted more and more with complete edentulous patients, presenting very strong resorption of the alveolar processes, even of the osseous bases, which will increase proportionally the difficulties of obtaining a good retention and good stability of the prostheses.Our work aims to review the various means available to us to prevent bone resorption, throughout the stages of prosthetic and post-prosthetic realization
Sustainable Post Covid19 Lockdown Strategy Through Evidence-Based Policy: Analysis of Covid19 Fatalities Across Europe
The Covid19 epidemic is having much larger fatalities in western Europe than everywhere else in the world. However, the greater peak daily fatalities have been 10 to 20 times less than the models’ predictions and similarly occurred after half the time predicted by the models. Similar patterns of the outbreak have been achieved regardless of diversity in the containment measures. Thanks to a strict, generalized, lockdown, the United Kingdom, or Belgium, had after about the same number of days, much larger peak daily fatalities per million than countries adopting more sustainable approaches such as the Netherlands or Sweden. In the Netherlands, social distancing is suggested but not forced through an “intelligent lockdown”. In Sweden, there is simply no lockdown. It is now evident that Covid19 is much less contagious and lethal than what was thought, and if care is taken of the risk categories, but basic freedom is not removed restricting every movement of the healthy population damaging democracy and economy, it is possible to achieve better results. With fatalities uniformly declining across Europe, it is therefore advocated to return the sooner the better as before the lockdown period, with full compliance with preventive health instructions and social divergence, and care to protect the highest-risk groups from infection, especially for the elderly and those with chronic and respiratory diseases
The Increasing Number of Infected in Victoria, Australia since June 15, 2020, is the Result of Over-Testing and Over-Controlling without Safety
On June 15, the Victorian government was rocketed by a corruption scandal, and the government started the search for more Covid19 cases, by increasing the number of tests per day, and by focusing the testing on the more disadvantaged suburbs of Melbourne, where more cases were expected. By over-testing and then over-controlling, the percentage of positive cases over-tested that was initially about constant at everything but worrying values of 0.1 to 0.2%, i.e. 1 to 2 over 1,000, has dramatically increased, by a factor of 10, now approaching 2%. Apart from mass testing knocking doors after doors of single-family homes, one potentially infected family after the other, with entire public buildings looked down, and all the occupants prevented from leaving the building and mass tested, a sample wrong indication is for people feeling sick to reach a hospital possibly moving by public transport within a 5 million people city, to get tested after having queued and then returning home. We predicted since the early days after June 15 that this strategy could have brought to the first real dangerous outbreak of Covid19 in Australia, and this is exactly what they have achieved.
Stem Cell Concept in Thyroid Cancer: Stem Cell
The most frequently diagnosed endocrine cancer, which causes more deaths than any other endocrine cancer, is thyroid cancer. Cancer stem cells are rare cells found in tumors that can regenerate themselves, phenotypically leads to various tumor cell populations, and trigger tumorigenesis. Cancer stem cells have been identified in many cancers, including thyroid cancer. Having an understanding of the molecular mechanisms which control the biology of cancer stem cells and the disease processes will help us in designing more rational targeted therapies for aggressive thyroid cancers. A Rare Case Report of Pedunculated Fibrolipoma in Thigh with Review of Literature
Fibrolipoma is one of the rare variants of the lipoma, and very few cases have been reported in the thigh. These lesions are generally painless, but can grow to large size due to malignant transformation to liposarcoma. Surgical excision is the treatment of choice. The prognosis is generally good as recurrence rate is very less if adequate excision is done. In old age patient, risk of malignant transformation is very high leading to early excision. A Rare Cause of Diabetes Insipidus in Children: Langerhansian Histiocytosis
Diabetes insipidus is a disease in which large volumes of dilute urine (polyuria) are excreted, We report the observation of a 2-year-old child who presents with stagnation in weight with polyuria polydipsia of diabetes insipidus, the cause of which is Langerhansian histiocytosis. Through this work, we highlight the clinical, paraclinical, evolutionary and therapeutic aspects of this affection.
 
Retention Practices Among Orthodontists in Morocco: A Cross-sectional study
Introduction: Retention is the orthodontic treatment phase which ensures that the teeth are maintained in their corrected positions. There are currently various protocols of retention commonly adopted by orthodontists. The objective of this study is to evaluate the retention procedures which are used by orthodontists in the private practice.Materials and methods: This was a cross-sectional descriptive analytical study which targeted orthodontists in the city of Rabat- Morocco. 67 questionnaires were distribute to the orthodontists of private practice. The questionnaire was organized into seven sections representing specific information about the socio-demographic status of the respondents, selection of a retention system, details of commonly used fixed and removable retainers, the duration of the retention period, and follow up appointments, instructions for patients, finally the clinical problems encountered during the retainer period. Results: All orthodontists participating in the survey confirmed the indispensable use of an orthodontic retainer after active treatment. The fixed retainer was the preferred choice of the majority of our orthodontists in the mandibular arch for the different clinical situations whereas, at the maxillary arch, more than 30% of orthodontists use both types of retainer (fixed and removable). 62.5% of orthodontists choose a vacuum-formed retainer as a removable retainer. For fixed retainer, the bonded canine to canine was most preferred by the majority of orthodontists in the upper and lower arches. 50% of orthodontists recommend lifetime orthodontic retainer with follow-up appointments of 2 to 3 times/year. Conclusions: This survey provides an overview of orthodontic retention procedures adopted by orthodontists in the region. A multitude of responses let us conclude that guidelines are desired to find a common protocol
Some Doubt the Covid19 Containment Measures on the Generally Healthy Population Made any Difference for Italy: Covid19 Fatalities Much Larger in Europe, United States and Canada than elsewhere
There is a clear indication from the number of fatalities in similar countries that adopted different lockdown strategies, for example, Belgium, Spain, the United Kingdom or Italy, and others, that the more strict measures applied to the healthy population did not pay. There is also a clear indication that the compartmental model predictions of March were wrong, overrating peak fatalities of more than one order of magnitude. It is an unfortunate circumstance that the mainstream media in the west, as well as the “high impact” peer review, ignore the real-world pattern of Covid19 infection. Evaluation of Chemical Properties of Polymethylmethacrylate in Two Different Media by Using Ultraviolet-Visible Spectroscopy
Aim: The aim of this study was to evaluate the effect of concentration of chlorine on the polymethylmethacrylate and the influence of chemical properties of polymethylmethacrylate using ultraviolet-visible spectroscopy.Materials and methods: A total of 30 acrylic blocks of dimensions 65X 13X 3 mm as per the specification of ASTM [American standard for testing materials] were made in heat cure denture base resin to evaluate the effect of chlorine on polymethylmethacrylate. 30 samples were divided into 3 groups of 10 samples in each. Group, I was stored in artificial saliva which acts as a control group, Group II was stored in a 3ppm concentration of chlorine, and Group III was stored in a 100ppm concentration of chlorine.Results: Ultraviolet rays are passed into the sample, the results are obtained on a graph with a wavelength in nanometer on the X-axis and the percentage of reflectance on the Y-axis. If an organic compound absorbs ultraviolet radiation, it means that the compound contains carbonyl or conjugated double bonds3. Since polymethylmethacrylate absorbs ultraviolet rays, the existence of these bonds is confirmed in the graph.Conclusion: Within the limitations of the study it was concluded that the effect of chlorine on polymethylmethacrylate can react over the conjugated bond or carbonyl group at higher concentrations, whereas the action of chlorine is negligible at a lower concentrations
Hormone Replacement Therapy: Risks and Benefits
The climacteric is a physiological period of the female reproductive cycle characterized by a decrease in the levels of sex hormones, mainly estrogen. It consists of several phases, with amenorrhea as the initial clinical manifestation followed by long-term changes, such as vaginal atrophy. In this context, hormone replacement therapy (HRT) aims to improve the symptoms of female reproductive senescence, seeking to provide a comfortable physical and psychological condition during this period. However, as with any drug treatment, HRT has its risks and benefits, and it is up to the medical practitioner, in agreement with the patient, looking for the best therapeutic option, minimizing the risks and consequences resulting from this clinical approach
