    Facial Clues and Vertical Dimension of Occlusion: Diagnostic Study in a Moroccan Population

    Goal of the study: To detect the prevalence, association, and correlation that may exist between the vertical dimension of occlusion and certain facial clues in a toothed population in search of a simplified method for the determination of DVO in complete edentulous people.Introduction: The methods for recording the vertical dimension of occlusion, rest, or phonetic vertical are very numerous, attesting to the absence of a universal scientific method for its determination. The aesthetic criterion of respecting morphological proportions constitutes for many authors a preferential reference guide. Determining the vertical occlusion dimension (DVO) remains a complex and imprecise step using several complementary methods. The exploitation of clues Facials seems a simple and promising means but a subject still to controversies. Material and method: We conducted a diagnostic study, spread over a period of one and a half years within the faculty of dentistry of Rabat. The facial indices studied were as follows: the inter-pupillary distance, the distance between the inner canthus and the labial commissure, the distance between the center of the pupil and the labial commissure, the internal canthus distance from the eyes × 2, the external canthus distance from the ear, the distance between external canthus and internal canthus (opposite eye), the width of eyes × 2 and distance between the external wing of nose and eyebrow. The measurements (DVO and different facial clues) were taken by the same operator directly on the toothed subjects, using a caliper.Results and discussion: On the 103 subjects, our sample presented a predominantly female with an average age of 21 years ± 3.9. The average vertical occlusion dimension was 50.49 mm ± 6.45. We noted a concordance between the DVO and two facial indices namely the internal canthus-labial commissure distance (51.16 mm ± 6.11) and the center of the pupil-labial commissure distance (52.18 mm ± 6.66). The reliability analysis was in favor of a positive, strong and very significant correlation between the DVO and the following measures: the inter-pupillary distance, the internal canthus distance-labial commissure, and the center distance of the pupil-labial commissure (p <0.001). By adjusting the inter-pupillary distance, the internal canthus-labial commissure distance, the center distance of the pupil-labial commissure, the internal canthus distance from the eyes × 2, the external canthus-ear distance, the width of the eyes × 2, the distance between the external wing of the nose and the eyebrow, only the distance between the internal canthus and the labial commissure, the distance between the external canthus and the ear and the width of the eyes × 2 were explanatory variables for the DVO.Conclusion: The inter-pupillary distance, the internal canthus-labial commissance distance, and the center distance of the pupil-labial commissure approximate the vertical dimension of occlusion in the dentate subject. The use of these facial indices seems interesting incomplete removable prosthesis because of its simplicity and speed

    Effect of Health Information Websites on Healthcare Facility Visits in the Eastern Province of Saudi Arabia

    Introduction: The internet has been widely available with 18 million users in Saudi Arabia alone. The rapid growth of internet use has proven to affect healthcare. The main objective was to determine a correlation between health-related information (HRI) website use & healthcare facility visits in the Eastern Province of Saudi Arabia.Methodology: This study was designed as a survey-based cross-sectional study involving the population of the Eastern province of Saudi Arabia. Data was collected by distributing a validated survey via an online survey application over a 4-month period. It consisted of demographic data and questions concerning internet use for HRI. Logistic regression was used to establish a correlation between internet use for HRI & healthcare facility visits and to see influencing factors.Results: From the 1095 replies, 788 fit our inclusion criteria. 90% of our subjects have used the internet for HRI using mainly General medical websites 47.9%, Social Media 20.17%, Forums 19.16%, and government websites 12.69%. We found 52.27% have used HRI websites to diagnose & treat their own medical conditions without professional medical advice. Visiting healthcare facilities after reading online HRI occurred 62.2% of the time, while the remaining did not due to being reassured of their condition. Outpatient clinics were most visited at 57.63% followed by ER & Pharmacy visits at 21.11% each. Those who have used the internet for HRI were more likely to visit a healthcare facility than those who haven't (OR(95%CI)) 2.05(1.24-3.36) (p-value<0.05).Conclusions: There seems to be a correlation between HRI website use & healthcare facility visits, which is influenced by the level of education & occupation status

    Bar Attachement Retained Mandibular Complete Denture: A Case Report

    The mandibular complete denture is a challenge for the dentist. Unfavorable anatomical and physiological conditions such low bearing surface and a significant bone resorption, often compromise the prosthetic retention.The exploitation of root stumps is a reliable alternative that leads to a better prosthetic integration by increasing the retention, if the total prosthesis is connected to these root stumps via axial ball or bar attachments.In addition, this prosthetic alternative considerably improves the patient's comfort and masticatory coefficient and favors the preservation of the bone capital, provided that the requirements of root preservation as well as clinical and laboratory techniques are respected.We would like to illustrate this by presenting a clinical case in which we used the connecting bar in the mandibular arch

    Management of an Angulated Maxillary Central Incisor Impacted: A Case Report

    The inclusion of the maxillary incisors, although rare, poses even more difficulty to the practitioner who diagnoses it. This difficulty is increased in front of an angulated incisor in a labial position with the “policeman's sign”, the prognosis of orthodontic therapy being reserved.This article aims to describe a clinical case of retention of two maxillary central incisors with radicular angulation and in a vestibular position in an 11-year-old adolescent, referred in the Rabat dental consultation and treatment center. clinically, we can note the absence of the two maxillary central incisors on the dental arch, confirmed by the X-ray with the presence of obstacles on the path of the incisors which are in a very high position with angulated roots. The challenge was to put on the arch of the teeth and especially the aesthetic challenge. The satisfactory result obtained is only the result of the motivation of the patient as well as the dexterity of the practitioner. Despite the current treatment, we are comforted in our choice to treat. it is obvious for us that the only absolute contraindication to traction in the case of an impacted incisor remains dilaceratio

    Urinary diversion is often indicated after radical cystectomy for bladder cancer. It can either be non-contingent or continent. Ileal conduit and orthotopic urinary diversions (neobladder) are by far the most commonly used diversions. The choice of the urinary diversion to be carried out is done on several levels in relation to the underlying disease, the state of the patient, and the surgeon's preference. It is inappropriate to make direct comparisons between enterocystoplasty and ileal conduit because of the differences in the choice of patients for each technique making a prospective randomized trial unlikely.The choice of the technique must be made after clearly informing and explaining to the patient to enable him accept and adapt to his urinary diversion

    The management of elderly diabetic patients has become a real public health problem due to the increase in the number of patients. In this population, the complications of diabetes are more serious and are associated with manifestations more specifically gerontological. The standardized geriatric assessment is essential to define the situation of each patient according to his degree of autonomy and undernutrition, his cognitive functions, and his social environment. The therapeutic objectives must be individualized according to the clinical status of the patient. Insulin therapy is often unavoidable and requires a good education of the patient or his family. The management of diabetes in elderly patients must also avoid too much laxity in a person who is remained healthy but also unreasonable activism in the fragile subjects due to the hypoglycemic risk

    Neglected Giant Schwannoma of the Sciatic Nerve – A Case Report

    Background: Schwannomas originating from the sciatic nerve are extremely rare and usually present as a pathological mass in palpable examination or pain located in the thigh. Motor and sensory deficits are observed more often when the size of the tumor is more than 40 mm.Case Description: A 19-years- old female patient was referred to Suez Canal University Hospital after 6 years of pain and 2 years of improper management. Neurological examination and MRI of the right thigh were done and revealed a huge mass attached to the right sciatic nerve. Surgical excision was undertaken, carefully dissecting the lesion from the sciatic nerve. Histopathological examination revealed the tumor to be a schwannoma. The patient had marked postoperative recovery with marked pain reduction and improved neurological deficits.Conclusions: Schwannomas of the sciatic nerve are rare tumors eccentrically located on the nerve. Although rare, schwannoma of the sciatic nerve should be systematically suspected if thigh mass or persistent sciatica is reported. Surgical excision has a good prognosis
