The Maternal and Perinatal Death Surveillance and Response (MPDSR) is a system to identify, report and create mechanisms for reducing preventable maternal and neonatal deaths, and stillbirth. MPDSR has been implemented in Bangladesh in the last decade. This study was conducted to identify the factors including the barriers and enablers influencing the implementation of MPDSR within the existing health system in Bangladesh. We conducted a scoping review for literature published between 1999 to 2019. Two reviewers selected the articles based on a scoping review technique usingthe Prisma flow checklist and diagram. The articles were collected from search enginessuch as PubMed and Google scholar, and also from websites of public andprivate organizations, and United Nations (UN) Agencies. We synthesized the available evidence on barriers and facilitators of the current MPDSR process in Bangladesh. The search process identified 890 journal articles, 100 different guidelines and reports; 11 of them met the inclusion criteria for enablers and barriers of MPDSR implementation in Bangladesh. The enablers included interdisciplinary teamwork with good communication, capacity development, evidence from MPDSR review meetings leading to improvements in health services, coordination, and organized supervision and monitoring. The barriers included difficultiesincollecting data from hard-to-reach areas, ensuring the quality of data, limitationsinrecord keeping, social barriers, lack of effective coordination and planning for a timely response, limited human resources capacity, lack ofmotivation among the staff, varying levels of training and competence to identify complications, lack of supervision and poor implementation of guideline. The findings from this scoping review can guide policy makers in addressing and overcoming the barriers of MPDSR implementation in Bangladesh

Wound Healing Process as the Best Strategy to Save Cancer Patients

    Get PDF
The objective of this study is to develop effective strategies that can save cancer patients. Cancer therapy got to a bad start to rely on toxic chemicals to kill cancer cells (CCs) and to set up a rule on the reduction of tumor size to evaluate cancer drugs. These were mistakes committed by cancer establishments at a time we did not have whole knowledge of cancer to result in horrendous cancer fatality, which is still on the way to increase. In 2022, President Biden urged health profession in to come up solutions to bring down cancer mortality by 50% in the following 25 years. Currently available cancer drugs, which are mostly based on the killing of CCs, are causing cancer mortality to increase by 5% annually. Health profession must get serious to develop strategies that can bring down cancer fatality. The best strategy to save cancer patients is to follow wound healing process, since cancer is caused due to wound unhealing

Multidisciplinary Management of Intracranial Complications of Sinusitis: Case Series

    Get PDF
Intracranial complications of rhinosinusitis in children are rare. Symptoms are nonspecific, and diagnosis relies on a high index of suspicion and early imaging evaluation. Treatment should be multidisciplinary and may involve antibiotics alone or in combination with surgery to drain the sinus focus and intracranial abscess. We describe four cases of intracranial complications in pediatric patients who were successfully managed with a multidisciplinary approach

Neurocognitive Effects of Alcohol in the Elderly with Dementia

    Get PDF
In Brazil, it is estimated that 2 million people live with dementia syndromes, with a tendency to triple by 2050, according to the Global Burden of Disease. At the same time, Vigitel found that 20.8% of the population will consume alcohol excessively in 2023. In view of this, it is important to elucidate the relationship between alcohol and dementia in the elderly. This study aims to assess the relationship between alcohol consumption throughout life and the impact on neurocognition in the elderly. Using the descriptors "aged", "dementia" and "ethanol" with the operator AND in the PubMed, VHL and Cochrane databases, associated with the inclusion criteria of publication in the last 20 years, availability in full and English, Portuguese or Spanish language, 9 studies were selected, after excluding duplicates and analyzing titles and abstracts. Of the studies reviewed, 4 found an association between light to moderate alcohol consumption and a lower risk of developing and progressing dementia, while 2 had inconclusive findings. With regard to high consumption, 3 studies found no statistically significant relationship with dementia, and 4 associated this amount of alcohol with a higher risk of developing and progressing the condition. This review suggests that light to moderate alcohol consumption may be associated with a lower likelihood of developing or progressing to dementia, while findings on the effects of high consumption are divergent, yet most recent evidence points to the pathological role of this amount of alcohol consumption on cognition. The need for more research is highlighted, especially with samples from the Brazilian population, since none of the studies found were Brazilian

The article published earlier discusses space and time, the existence within space, and human existence. If science proves the correctness of zero-dimensional space, the contradictions in modern physics can be resolved. Material philosophy is based on thinking about the material universe. Cosmogony is thinking based on space and existence in space. Based on our ancestors handed down the basic idea of the origin of the universe, in-depth study of the existence of space in the scientific philosophy thinking

Today\u27s scientific information presents a moment for investigating the true cosmos. The contradictions within modern physics necessitate an examination of the fundamental issue of space, a question that has been deferred for over 2500 years. During the era of Aristotle, understanding of the cosmos was insufficient, leading Western philosophical thought to pivot entirely toward the philosophy of cosmic material, abandoning the philosophy of cosmic origin and the existence of divinity within space, relegating these inquiries to theology. The debate between Newton and Leibniz over absolute space left physics with inadequate information, subject to skepticism and lacking conclusion. Today, modern physics, systems science, physical cosmology, and other scientific disciplines provide information on time, mechanics, the composition of cosmic spaceboth material and immaterial. Based on the philosophical thoughts of Daoism, Buddhism, and dialectical materialism, there may now exist an opportunity to investigate the existence of divinity within space and to study the cosmic origin. It is hoped that scientists will reconsider and investigate the issue of immaterial existence within space, and reevaluate absolute space (Liu,0-Dimensional Universe - Absolute space test, liu (2021)?liu?2020?). This may be the key to resolving the contradictions between quantum mechanics and relativity, and I request your Consider publishing the letter

Theoretical Prerequisites for Optimizing the Spray Pattern Angles of an Adaptive Sprayer for a Field Sprayer

    Get PDF
Vibrations of the sprayer field boom in the transverse-vertical plane result in a decrease in the quality of the technological operation. This is especially true in relation to the operation of small-sized single-support wheelbarrow-type sprayers. One of the possible solutions to compensate for the effect of transverse vibrations of the boom on the quality of spraying is the use of adaptive sprayers with a variable root angle of the spray torch, responding to the position occupied by the sprayer in relation to the surface being treated. The purpose of the work is to optimize the current values of the frontal spray angles of the nozzles of the adaptive distribution system of a small boom sprayer. The novelty of the study lies in the fact that, unlike industrial agricultural machinery, insufficient attention is paid to the implementation of technologies using small-scale mechanization tools. A prototype of a single-support boom motor sprayer was used as the object of study. The field experiment was carried out at the experimental sites of the Oryol State Agrarian University. Registration of data on the deviation of the sprayer from the vertical axis was carried out using a specially developed mathematical processing of the decrypted experimental data was carried out by a spreadsheet processor in the Microsoft Excel environment. The study of the obtained analytical dependencies was carried out in the environment of the mathematical calculation system Mathcad 14.0. The work used the method of nomogramming functions of several variables. It has been experimentally established that during operation the maximum deviations of the sprayer from the vertical can be up to 30. At the same time, the average amplitude of the transverse operating vibrations of the boom of a single-support barrow-type boom sprayer ranges from +11 to -18. The amplitude of lateral vibrations of a single-support sprayer depends on the operators preparedness and the speed of movement of the unit. Balancing the sprayer is important, aking into account the moment of forces caused by the weight of the one- sided field boom. A formula has been derived for calculating the spray width of one sprayer, taking into account the geometric parameters of a single-support sprayer, as well as its angle of inclination in the transverse-vertical plane. An analytical relationship has been obtained that makes it possible to calculate the required limits of the root angle of the spray pattern of an adaptive sprayer, taking into account the installation distance of the sprayer, relative to the vertical plane passing through the support point of the sprayer. The values of the root spray angles were found for the deflectors of adaptive sprayers, with the amplitude of oscillations of the sprayer from -18 to +11 in the transverse-vertical plane. A nomogram has been developed to determine the optimal values of the frontal angles of the spray pattern of sprayer nozzles. An analytical technique for optimizing the current values of frontal spray angles has been proposed, taking into account the position of the distribution system and performed for the first time. The practical value of the study lies in the possibility of using a formula to determine the limits of change in root spray angles when designing and developing adaptive sprayers. The technique can be used to develop software for the control complex of a sprayer equipped with an adaptive system for distributing working fluid in changing conditions
