Addressing the Mental Health Needs of Youth in Times of Crisis: A Study on Job and Financial Insecurity Post-COVID-19
The COVID-19 pandemic has had a significant impact on various aspects of social and economic life. One area that has been particularly affected is the mental health of young people. This study aims to investigate the factors that contribute to mental illness in youth in the post-COVID-19 pandemic period. A sample of 300 youths participated in the study by completing a questionnaire that was developed based on the YoungMinds Malaysia initiative. The questionnaire explored the perceptions of young people on various factors that can contribute to mental illness, including job and financial concerns. The study findings shed light on the unique mental health perceptions of youth in comparison to other age groups. The study highlights the need for targeted intervention strategies to address the mental health issues faced by youth, particularly in the areas of job and financial insecurity. The findings have significant implications for policymakers and mental health professionals in Malaysia and beyond
Enhancing Sustainability through Efficient Public Spending: Polish Procurement Perspectives
Sustainability is a leading criterion in sustainable financing decisions, both for financial markets and private actors as well as for public finance. State and local government actors play a key role in implementing and supporting the concept of sustainable finance, but also recognise the special role of non-financial risks (so-called ESG risks - Environmental, Social, Governance), within which environmental, social and governance risks are distinguished.
The integration of sustainability and inequality into the public procurement system has brought with it the need for changes in the hierarchy of values affecting the public spending process. One of the demonstrated goals of using sustainable public procurement is to increase the efficiency of public spending. In order to demonstrate and research efficiency gains in the indicated area, it becomes necessary to analyse the role of axiology in the public procurement law system while recognising risks and threats.�
In 2015, the United Nations (UN) adopted the 2030 Agenda for Sustainable Development. This created a new pathway to improve the lives of countries and societies. The Agenda includes a set of goals that include, among others, eradicating poverty, reducing inequality, improving health and well-being, protecting the planet and ensuring human well-being . All the goals contained in the 2013 Agenda have been grouped into 17 Sustainable Development Goals (SDGs), with the overall goal of creating a better world not only for present but also for future generations .�
The public procurement procedure has evolved from a typical economic tool to a complex tool that embodies and implements several dimensions of sustainable development, being now a "multi-purpose instrument" . Changes in the sustainability paradigm, the impact of non-financial factors, changes in the perception of the role of finance in financing sustainable development, the need to preserve the financial stability of the public sector, the need to integrate the public financial system with the safety net and, above all, changes in the perception of the environment and the human position in it imply changes in the public finance and procurement paradigm.
The analysis underpinning the conclusions is based on the use of quantitative research methodology. In order to collect information and data, the research method of document analysis and analysis of literature was used, which includes in particular reports, regulations of European law, including EU law, and Polish law. The analysis of the quantitative data, in particular the legal analysis, was carried out in order to draw final conclusions and determine the results of the research

Bullying es a Cause of Funtional Neurological Disorders (Conversion Disorder): a Case Report
The conversion disorder, today called functional neurological disorder is a highly disabling condition both physically and mentally, which affects children simulating various diseases of the nervous system in its motor, sensitive or sensory components, difficult to diagnose, due to different causes with a prolonged evolution and generally causes social isolation in children; the neurofunctional picture requires for diagnosis using numerous medical tests with a high cost and burden for the health system that was previously done by ruling out. Bullying or school bullying is characterized by violence between peers at school, in which there is one or more bullies against the victim, using various types of violence, whether physical, emotional, psychological, and more recently cyberbullying, causing various traumas in the victim, sometimes with serious consequences for their health. In the present case, we describe a girl who was hospitalized due to highly disabling pain in the spine and lower limbs, which prevented her from sitting and walking, to which paresthesias were later added, which began after a traumatic accident. After several studies that ruled out an organic cause of the symptoms and their improvement with a placebo, a diagnosis of functional neurological disorder was made, which was later proven to have been caused by the stress of the bullying of whisch she was a victim
Assessing Health-Related Quality of Life in Bolivian Hemodialysis Patients: Validation of the KDQOL-36™ Questionnaire
Background: Chronic kidney disease (CKD) is a global health concern, affecting 2% of patients who may progress to end-stage renal disease (ESRD). Hemodialysis (HD) is the primary therapy, but it poses challenges, impacting patients\u27 quality of life. In Bolivia, CKD prevalence is 40%, making it the fourth leading cause of death. Risk factors encompass restricted access to safe water, sanitation, and exposure to toxic chemicals, presenting notable public health challenges in Bolivia.. The aim of the study is to assess Health-Related Quality of Life (HRQOL) in Bolivian HD patients through the application and validation of the cross-culturally adapted Latin-Spanish version of the KDQOL-36™ questionnaire.
Methods: The cross-sectional study, approved by the Ethical Committee, involved 724 CKD patients undergoing HD in Bolivia\u27s La Paz department. The sampling technique used proportional allocation based on registered patients in 23 HD units across three cities. Inclusion criteria comprised patients over 18 with at least two months of HD, capable of responding to the questionnaire. The cross-culturally adapted KDQOL-36TM survey, administered by trained healthcare staff, was assessed using SPSS for reliability, exploratory, and confirmatory factor analysis, ensuring validity and appropriateness for Bolivian patients. Data was collected between October and December 2023.
Results: The study included predominantly female participants (51.5%) with a mean age of 55.9 years and diverse educational backgrounds. HD was mainly received from the private health sector (65.7%), and the average duration was 31.4 months. Descriptive analysis of KDQOL-36™ scores showed variations across subscales, with symptoms and problems scoring highest (67.13, Min 10.42 Max 100) and the burden of kidney disease scoring lowest (24.61, Min 0 Max 100). Validity estimates, including McDonald\u27s Omega (0.92), as well as exploratory and confirmatory factor analyses, were conducted.
Conclusions: The Latin-Spanish and cross- cultural adaptation version of the KDQOL-36™ questionnaire seems valid and reliable for assessing HRQOL in Bolivian patients with kidney disease undergoing HD. The sociocultural characteristics in the La Paz department may differ from those in other departments. Therefore, it is recommended to conduct this study in various Bolivian contexts. The utilization of this instrument is recommended for clinical research in Bolivia
Study of Appointment Scheduling Pattern of MRI Department and Analysis to Optimize Turnaround Time in a Tertiary Care Hospital
Introduction: As healthcare sectors are complex, it\u27s difficult to handle the process involved in the management. The cost of healthcare is increasing so it is important to increase efficiency as well. Assessing appointment schedules and Turnaround time will help to analyze the workflow of the MRI department and optimize the processes.
Methods: The methods used were cross-sectional, prospective, descriptive, quantitative analysis, and qualitative methods. The patients for the study were selected through random sampling techniques and in-person data gathering was performed.
Results: Data was collected, and charted on an Excel tracker sheet, and in-depth analysis was performed. The tracker sheet included various study parameters like Registration Time, Waiting before changing, Changing Time, Waiting before Case history, Case History, Waiting before the scan, and MRI Scan Time. The data analysis included the average turnaround time for MRI i.e. from registration to scan was 1 hour 14 minutes.
Discussion: The reasons that were causing the delay in the MRI department were enumerated. Reasons like carrying any metal object in the department, first preference for emergency patients, nature of the examination, and movement of the patient during the scan were studied in depth.
Application: This study provides the pattern that was supposed to be followed for scheduling patient\u27s appointments in the MRI department and to optimize the turnaround time for the patient. The possible reasons causing a delay in the process of the MRI department by observing the current practices in the MRI department including the infrastructure, and staffing
Oral Squamous Cell Carcinoma: Challenges in Treatment for Low- and Middle-Income Countries: Fetching towards the far Ahead Horizon
This �perspective article discusses the origin of tobacco, its spread and the changes it produces in human body, most prevalent of which it is oral squamous cell carcinoma. Especially in low-and middle -income countries, where the use of tobacco is already trend setting and concomitant cases are also increasing, so some simpler measures should be needed to implement which can reduce the size of the lesion and the lymph node shrink from the original size so that it prevents not only the load of reconstruction, but might also lead to improved mortality outcomes
The Effect of Myofascial Release and Cervical Traction on Pain, Range of Motion and the Neck Disability Index in Patients with Chronic Neck Pain: A Randomized Controlled Trial
Purpose: This study examined the effects of the myofascial release and cervical traction after applying conservative physical therapy to patients chronic neck pain. Methods: Patients were randomly divided into two groups, namely myofascial release (7 subjects) and cervical traction (7 subjects). Each group performed their therapy 45 minutes per day, two times a week, for four weeks. Pain intensity was measured using the visual analog scale (VAS). Function was measured with the neck disability index (NDI). The cervical range of motion (CROM) was measured with a cervical range of motion (CROM) goniometer. Results: After four weeks of therapy, the VAS (p < .05) and NDI (p < .05) significantly decreased, and ROM significantly increased in both groups (p < .05). There were also significant differences between the two groups for these three measures, except for neck flexion and neck extension(p<.05). Conclusion: Myofascial release and cervical traction are more effective than cervical traction alone for reducing VAS and NDI and increasing ROM in patients with chronic neck pain
Evaluating the Impact of Large Language Model AI on Acute Pancreatitis Management: A Chatgpt-Based Investigation
Background: Evidence-based management of acute pancreatitis (AP) is important for patient outcomes. The present study evaluated suggestions by artificial intelligence (AI) chatbot system, ChatGPT, for the management of acute pancreatitis, its alignment with clinical guidelines, and assistance in clinical decision-making.
Methods: Six questions on pancreatitis management were curated by experienced RACS-qualified general surgeons and were put forth to ChatGPT. The chatbot was also asked to provide five high-level evidence references to support each of its responses. Each response was analyzed for its accuracy and comprehensiveness with respect to current internationally recognized guidelines and by two Board-Certified General Surgeons for acute pancreatitis management, as well as for its spelling, grammar, and reference quality. A five-point Likert Scale was utilized to analyze ChatGPT\u27s responses, with scores ranging from 1 (strongly disagree) to 5 (strongly agree). Ten questions were designed to assess accuracy, consistency, informativeness, reliability, and coherence. These were independently rated by three junior doctors and two General Surgeons, with any scoring discrepancies resolved through consensus.
Results: ChatGPT successfully adhered to clinical guidelines when generating recommendations for the management of acute pancreatitis. The depth of information remained general and non-specific but was presented in an academic manner with appropriate grammar, spelling and sentence structure. ChatGPT missed pertinent references, with some being totally fabricated or erroneous.
Conclusion: ChatGPT holds promise for delivering prompt and accessible medical information to non-experts, which may benefit in situations where medical professionals and resources may be scarce or patients are reluctant to seek such services. The inclusion of aberrant or fabricated references is a challenge for researchers and clinicians and breaches academic integrity. Ethically, it is imperative for researchers to exercise prudence when utilizing ChatGPT for research purposes
Effectiveness of Wobenzym in Combined Therapy of Chronic Bacterial Prostatitis. Results of Systematic Review and Meta-Analysis
A systematic review and meta-analysis of research data on the effectiveness of the drug Wobenzym in the treatment of chronic bacterial prostatitis was performed. The aim of this study to evaluate the effectiveness of Wobenzym in the complex therapy of chronic bacterial prostatitis. The analysis included randomized and non-randomized controlled studies of the effectiveness of Wobenzim in the complex therapy of chronic bacterial prostatitis. The search was carried out in the databases CENTRAL, PubMed, ICTRP, eLibrary, ClinicalTrials.gov., Google Scholar, CyberLeninka, search engines. The meta-analysis was conducted using the recommendations of the Cochrane Handbook for Systematic Reviews of Interventions. Statistical heterogeneity was assessed using the Cochran test and visually when constructing forest plots. A random effects model and a fixed effect model were used. Works published over the entire period were analyzed, 712 publications were identified, of which 9 fully met the stated criteria, all studies were non-randomized controlled. The final analysis included the results of 1292 patients. Meta-analysis demonstrated the superiority of complex therapy including Wobenzym compared to therapy without Wobenzym in all studied parameters; eradication of the pathogen, decreased quality of life (QoL) scores, an increase in the linear speed of blood flow in the veins and peak systolic blood flow velocity in the arteries of the prostate gland, maximum urine flow rate, reduction in the number of points of the NIH-CPSI "Pain" subscale (difference 5 points) and NIH-CPSI "Quality of Life" subscale. The use of therapy including Wobenzym in patients with chronic prostatitis leads to a greater increase in the number of CDA+, CD8+ lymphocytes, phagocytic activity of lymphocytes, the level of complement CH-100 and immunoglobulins M, G, A. A systematic review/meta-analysis revealed an evident, significant, positive effect of the drug Wobenzym in the complex therapy of patients with chronic bacterial prostatitis, which is associated with its pathogenetic orientation in relation to this group of patients
