    Cognitive deficits in treatment-resistant depression : protocol for a systematic review and meta-analysis

    Introduction Major depressive disorder (MDD) is a major global healthcare challenge. This is, in part, due to the lack of treatment response and chronic course of MDD. Such a course of illness is often termed treatment-resistant depression (TRD) and is seen in over one-third of people with MDD. Reasons for treatment resistance are not well established, nor is the definition of TRD. Duration and severity of depression, however, are associated with TRD and are also associated with cognitive deficits. Thus, TRD could be particularly prone to cognitive deficits and at heightened risk for neuroprogression. While the cognitive profile of MDD has been investigated in several systematic reviews, no systematic review of cognition in TRD exists to date. The present study will fill this gap in the literature. It is expected that TRD will show more severe cognitive deficits than generally reported in MDD and deficits in all cognitive functions are expected. Methods and Analysis A systematic review following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines will be performed of the databases Embase, Pubmed/MEDLINE, PsychINFO and Cochrane including peer-reviewed studies on humans using standardised cognitive tests. Pilot searching was performed in January 2025 and the full search will be commenced in June 2025, with additional searches following completion. Where sufficient data are reported, a meta-analysis comparing deficits in TRD with MDD and healthy control participants will be performed; alternatively, effects based on norms will be calculated. Meta-regression, subgroup and sensitivity analyses will be conducted to explore moderators that are sufficiently reported in the literature.     Einfluss der lokalen HIFU-Behandlung auf das Überleben bei fortgeschrittenem Pankreaskarzinom

    Hintergrund: Das Pankreaskarzinom ist eine hochaggressive maligne Erkrankung, die aufgrund eines häufig lange asymptomatischen Verlaufs meist erst in einem fortgeschrittenen, inoperablen Stadium diagnostiziert wird. Mit einer 5-Jahres-Überlebensrate von etwa 11 % zählt es zu den führenden krebsbedingten Todesursachen in Deutschland. Trotz moderner Chemotherapieregime konnte die Prognose bislang nur begrenzt verbessert werden. Ziel dieser Studie war es, den Einfluss einer zusätzlichen lokalen HIFU-Behandlung (hoch-intensiver fokussierter Ultraschall) während der palliativen Erstlinien-Chemotherapie (1L-HIFU) auf das Gesamtüberleben zu untersuchen. Methoden: In einer retrospektiven Vergleichsanalyse wurden 207 Patienten mit fortgeschrittenem Pankreasadenokarzinom eingeschlossen, die zwischen 2014 und 2022 am Universitätsklinikum Bonn behandelt wurden. 83 Patienten erhielten während der Erstlinien-Chemotherapie eine zusätzliche ultraschallgesteuerte HIFU-Behandlung. 124 Patienten wurden ausschließlich systemisch behandelt. Das Gesamtüberleben wurde mittels Kaplan-Meier-Analyse ausgewertet; Subgruppenanalysen erfolgten nach UICC-Stadium und Chemotherapieregime. Ergebnisse: Das mediane Gesamtüberleben war in der 1L-HIFU-Kohorte signifikant (p = 0,0011) verlängert (15,8 Monate) im Vergleich zur Non-HIFU-Kohorte (9,8 Monate). Auch in den Subgruppen zeigte sich ein konsistenter Überlebensvorteil zugunsten der 1L-HIFU-Behandlung: Im UICC-Stadium III betrug das mediane Überleben 18,5 Monate versus 10,9 Monate (p = 0,0018), im Stadium IV 13,1 Monate versus 9,3 Monate (p = 0,006). Zudem war das Überleben sowohl unter FOLFIRINOX-Therapie (18,3 Monate vs. 10,3 Monate) als auch unter Gemcitabin-basierter Chemotherapie (12,4 Monate vs. 7,7 Monate) signifikant verlängert. Die HIFU-Therapie war insgesamt mit einer sehr geringen Nebenwirkungsrate assoziiert. Schlussfolgerung: Diese Studie konnte zeigen, dass die Ultraschall-gesteuerte HIFU-Therapie einen positiven Einfluss auf das Überleben von Patienten mit fortgeschrittenem Pankreasadenokarzinom hat – und dies bei insgesamt sehr geringer Nebenwirkungsrate. Neben dem bisher beobachteten klinischen Nutzen – insbesondere der Linderung der tumorassoziierten Symptomatik und der Verbesserung der Lebensqualität – zeigten sich zudem relevante prognostische Vorteile. Um die klinische Relevanz dieser Ergebnisse zu bestätigen und eine mögliche Integration in zukünftige Therapieempfehlungen zu ermöglichen, sind nun größere randomisierte kontrollierte Studien erforderlich

    Wenn ein Schuldner nicht all seine Gläubiger befriedigen kann oder muss, kollidieren ihre Forderungen. Lässt die Rechtsordnung die Gläubiger konkurrieren, weil sie hofft, dass alles gut gehen wird, oder weil sie einen Eingriff als illegitim oder unpraktikabel ansieht? Ordnet sie gleichmäßige Befriedigung an oder spricht sie bestimmten Forderungen Vorrang zu? Während die Insolvenz viel Aufmerksamkeit auf sich zieht, fehlen Prinzipien für das allgemeine Problem. Dabei stellt es sich in verschiedensten Kontexten, etwa bei Leistungsstörungen, Gefährdungshaftung, Haftpflichtversicherungen, Nachlassverbindlichkeiten, der Haftung des Kommanditisten und nach Unternehmensspaltungen. Auf der Grundlage einer historischen Betrachtung entwirft Gregor Albers ein Modell dafür, wann welches Prinzip Anwendung findet.     Zusammenhang von Eosinophilie und Atherosklerose bei Patienten mit Asthma bronchiale

    Hintergrund und Zielsetzung Asthma ist eine entzündliche Erkrankung, die mit Atherosklerose in Verbindung steht. Die gemeinsamen Mediatoren dieser Entzündung sind jedoch bislang nicht identifiziert worden. Eosinophile spielen möglicherweise sowohl bei asthmatischer als auch bei atherosklerotischer Entzündung eine Rolle. Daher zielt diese Studie darauf ab, die potenzielle Rolle von Eosinophilen bei atherosklerotischen Veränderungen von Asthmapatienten zu untersuchen. Methoden Wir untersuchten die Strain-Werte der Arteriae carotides communes (ACC) mittels vaskulärem Speckle-Tracking. Die Ergebnisse wurden zwischen Asthmapatienten mit niedriger (<300/&micro;l) und hoher (&ge;300/&micro;l) Bluteosinophilenzahl (BEC) verglichen. Ergebnisse Wir rekrutierten konsekutiv 100 Patienten aus der pneumologischen Ambulanz ein, 42 mit einer BEC <300 und 58 mit einer BEC &ge;300 n/&micro;l. Patienten mit hoher BEC wiesen eine ausgeprägtere Erkrankung auf, mit einer höheren Rate akuter Exazerbationen (1,3 &plusmn; 2,1 vs. 2,6 &plusmn; 2,4 n/Jahr, p = .005), einem niedrigeren forcierten exspiratorischen Volumen (FEV1 % des Sollwerts: 77,7 &plusmn; 19,6 vs. 65,5 &plusmn; 18,9 %, p = .002) und einer höheren Rate an Anti-IL-5-gerichteter Therapie (2,4 % vs. 50,0 %, p < .001). Die Gruppe mit niedriger BEC bestand häufiger aus Frauen (76,2 % vs. 51,7 %). In Bezug auf andere typische kardiovaskuläre Risikofaktoren waren beide Gruppen ähnlich. Die Gruppe mit hoher BEC zeigte eine erhöhte arterielle Steifigkeit, was sich in niedrigeren radialen Strain-Werten äußerte (mittlerer radialer Strain der rechten ACC: 2,7 &plusmn; 1,4 % für BEC &ge;300 n/&micro;l vs. 3,5 &plusmn; 1,7 % für BEC 2 = 0.131, b = –0.001, p = 0.001) und die linke ACC (R2 = 0.086, b = –0.001, p = 0.015). Schlussfolgerung Diese Studie ist die erste, die eine erhöhte Gefäßsteifigkeit als Zeichen einer frühen Atherosklerose bei Asthmapatienten mit einer BEC &ge;300 n/&micro;l aufzeigt. Wir fanden eine Korrelation zwischen Bluteosinophilen und Gefäßsteifigkeit, was die Hypothese unterstützt, dass Eosinophile an der Pathogenese der Atherosklerose beteiligt sind. Die zugrunde liegenden Mechanismen dieser Korrelation scheinen vielschichtig zu sein und bedürfen umfassender Studien, um vollständig verstanden zu werden.Background and Objective Asthma is an inflammatory disease associated with atherosclerosis.     The impact of mechanical stretch on the stability of desmosomes in epithelia

    The mechanoresponse of epithelia is a well-known and essential process. Both cytoskeleton and cell adhesion sites react to mechanical stimuli, e.g. mechanical stretch. Cell adhesions like desmosomes act as connection sites in epithelia, they mediate cell-cell adhesion by providing mechanical strength and promoting an intercellular network by connecting the intermediate filaments between adjacent cells. Desmosomes are highly dynamic structures, which need to assemble and disassemble in order to adapt to an everchanging mechanical environment. This dynamic is put into excecution by protein exchange between the desmosomal structure and a pool of freely diffusing molecules in the cytoplasma. All in all, desmosomes are important sites of epithelial mechanoresponse, however, the research on desmosomes is predominantly based on experiments performed on static conditions. Therefore, little is known about the behaviour of desmosomal proteins, including their exchange kinetics upon mechanical stretch. In order to address this knowledge gap, an experimental approach to analyse the responses of several desmosomal proteins to mechanical stretch was carried out within the scope of this thesis. A monolayered epithelial system of MDCK (Madin-Darby Canine Kidney) cell lines stably expressing a fluorescently tagged desmosomal fusion protein was probed with FRAP (Fluorescence Recovery After Photobleaching) to analyse protein exchange kinetics of the desmosomal proteins DP (desmoplakin), PG (plakoglobin) and Dsc (desmocollin). An experimental set-up with sealable silicone chambers was developped for cell stretching. The application of mechanical stretch was performed with a computer-controlled apparatus, the cell stretcher. Uniaxial, cyclic stretch with an 80 mHz (Millihertz) frequency was applied for a duration of 2 hours (h) to the analysed samples in live cell conditions. The exchange kinetics of the desmosomal proteins DP, PG and Dsc were analysed at different time points after stretch in order to test the impact of mechanical stretch on the stability of desmosomes. The recovery curves displayed a biphasic ascent, indicating the presence of two distinct kinetic processes, free diffusion and exchange kinetics at the desmosomal sites. To decide whether an exponential fit could be applied to the recovery curves, a straight was drawn between the first and the last point of the exponential fit while excluding the offset. If the maximum distance between this straight and the exponential fit was exceeding the standard deviation of the noise from the recovery curves this recovery curve was fitted with an exponential fit, otherwise, it was fitted with a linear fit. This analysis has shown that, in the absence of mechanical stretch, the analysed desmosomal proteins are stable components of the desmosomal structure. Upon exposure to uniaxial, cyclic stretch PG and Dsc exhibit a reversible mechanoresponse, which expresses as an increase of their exchanging fraction.     Die Entstehung der alt-katholischen Kirche im Großherzogtum Hessen-Darmstadt (1870-1895/6)

    Die Arbeit untersucht die Entstehung der alt-katholischen Kirche im Großherzogtum Hessen-Darmstadt von der Vorgeschichte über den Protest gegen die Dogmen des 1. Vatikanischen Konzils bis zum Ende des Episkopats von Joseph Hubert Reinkens (gest. Anfang 1896). Bereits vor dem 1. Vatikanischen Konzil gab es im Großherzogtum Hessen-Darmstadt einzelne anti-ultramontane Bestrebungen. In Reaktion auf die Dogmatisierung von Jurisdiktionsprimat und Unfehlbarkeit des Papstes 1870 wurde der Protest dagegen an einigen Orten institutionell eigenständig. Im Großherzogtum Hessen-Darmstadt entstanden als Reaktion darauf unterschiedlich große alt-katholische Gemeinden, die sich auch als unterschiedlich langlebig erwiesen. Offenbach war die größte Gemeinde mit eigenem Pfarrer und stabilem Gemeindeleben. Gießen hatte eine lange Vorgeschichte: Bereits 1870, fast unmittelbar nach ihrer Verkündung, protestierten Professoren der Universität gegen die neuen Dogmen. Dort kam es jedoch nie zu einem kontinuierlichen Gemeindeleben, und de facto starb die Gemeinde 1894 aus. Im ländlichen Heßloch gelang es (national-)liberalen Winzern in ihrem Protest gegen den ultramontanen Ortspfarrer und seine Anhänger 1874/75, eine altkatholische Gemeinde zu gründen, die ein stabiles Gemeindeleben mit Gottesdiensten im zweiwöchigen Rhythmus entwickelte und sogar einen eigenständigen alt-katholischen Religionsunterricht einrichten konnte. Nach dem Bau einer eigenen Kirche im Jahr 1890 erhielt die Gemeinde 1893 einen eigenen Geistlichen. Die kleine Mainzer Gemeinde war publizistisch tätig und kritisierte polemisch ultramontane Positionen. Sie kümmerte sich zunächst um den Religionsunterricht und konnte dank ihrer Nähe zu Wiesbaden alle zwei Wochen Gottesdienst feiern. Die Wormser Gemeinde wurde 1875 gegründet, konnte sich aber nicht dauerhaft etablieren. In Mölsheim (Rheinhessen) scheiterte der Versuch einer Gemeindegründung noch früher. In Darmstadt wurde 1895/96 eine neue Gemeinde gegründet, die trotz ihrer geringen Mitgliederzahl ein Gemeindeleben entwickelte. Dort ist insbesondere die Beteiligung von Frauen an Wahlen bemerkenswert. An anderen Orten in Hessen lebten Altkatholiken in der Diaspora. Das dauerhafte Bestehen einer Gemeinde war von verschiedenen Faktoren abhängig: Zum Teil wurde ein bereits bestehender liberaler Protest gegen den Ultramontanismus nun in der alt-katholischen Kirche fortgesetzt. Die Größe einer Gemeinde war entscheidend. Unterhalb einer Zahl von 100 Personen war ein sehr hohes Maß an Engagement aller Gemeindemitglieder erforderlich, um das Gottesdienstleben zu ermöglichen. Die Verfügbarkeit eines Geistlichen und sein Engagement für die alt-katholische Sache waren ebenfalls entscheidend. Darüber hinaus spielten die finanziellen Möglichkeiten, ein Kirchengebäude und die Weitergabe der Konfessionszugehörigkeit an die nächste Generation eine wichtige Rolle. Im Großherzogtum Hessen-Darmstadt wurde die alt-katholische Kirche als eigenständige Konfession anerkannt und unterschied sich damit in Bezug auf das Staatskirchenrecht von allen anderen Gebieten des Deutschen Reiches. Sie wurde also weder besonders gefördert noch benachteiligt.The Emergence of the Old Catholic Church in the Grand Duchy of Hesse-Darmstadt (1870-1895/6) The dissertation examines the emergence of the Old Catholic Church in the Grand Duchy of Hesse-Darmstadt from prehistory to the protest against the dogmas of the 1st Vatican Council to the end of the episcopate of Joseph Hubert Reinkens (d. 1896). Even before the First Vatican Council, there were some anti-ultramontane activities in the Grand Duchy of Hesse-Darmstadt. In response to the dogmatization of the primacy of jurisdiction and papal infallibility 1870, protests against this became institutionally independent in some places. In the Grand Duchy of Hesse-Darmstadt arose Old Catholic parishes of different size, time of origin and durability: Offenbach was the largest parish with its own pastor and stable parish life. Giessen had a long history, professors of the university protested against the new dogmas as early as 1870. There, however, a continuous congregational life never came about, and de facto the congregation died out in 1894. In rural Heßloch, (national) liberal winegrowers, in their protest against the ultramontane local priest and his followers, were able to found an Old Catholic congregation in 1874/75, which had a stable congregational life with biweekly services and religious instruction. After building its own church in 1890, the congregation received its own clergyman in 1893. The small Mainz congregation was very active in publishing and polemically criticized ultramontane positions. It initially took care of religious instruction and, thanks to its proximity to Wiesbaden, was able to establish a regular congregational life on a biweekly basis. The Worms congregation was founded in 1875, but was unable to gain a lasting presence. In Mölsheim in Rhine Hesse, the attempt to found a congregation failed even earlier. In Darmstadt, a new congregation was founded in 1895/96, which developed a congregational life despite its small number of members. There, the participation of women in elections is particularly noteworthy. In other places in Hesse, Old Catholics lived in the diaspora. The long-term existence of a community depended on various factors: In part, an already existing liberal protest against ultramontanism was continued in the Old Catholic Church. The size of a congregation was crucial. Below a number of 100 people, a very high level of commitment was needed from all parishioners to make worship life possible. The availability of a clergyman and his commitment to the Old Catholic cause were also crucial. In addition, the financial possibilities, a place of worship, and the passing on of denominational affiliation to the next generation played an important role. In the Grand Duchy of Hesse-Darmstadt, the Old Catholic Church was recognized as an independent denomination, thus differing from all other areas in the German Empire in terms of state-church law. Thus, it was neither particularly promoted nor disadvantaged

    Peritonsillar abscess in children : A retrospective analysis on surgical and antimicrobial approaches

    Background Peritonsillar abscess (PTA) is a prevalent infection for specialists in otorhinolaryngology and pediatric primary care providers, that has the potential to cause severe complications. The aim of this study is to investigate the surgical treatment of pediatric peritonsillar abscesses and to compare the risk profiles of bilateral surgery versus surgery on the affected side alone. In addition, the evaluation of the microbiological smears obtained intraoperatively should provide information on whether the calculated antibiotic therapy adequately covers the microbial spectrum. Methods We conducted a retrospective analysis of pediatric patients (n = 150), who were treated for PTA between 2009 and 2024 by unilateral tonsillectomy (UTE) or bilateral tonsillectomy (BTE). Patient charts were analyzed regarding risk of bleeding, occurrence of other complications, recurrence rates in case of UTE as well as microbiological flora and antibiotic treatment. Results Postoperative bleeding did not differ significantly between both groups. In 4.4% of the patients treated by UTE a recurrent PTA was found. No other severe complications after surgical treatment were found. Antibiotic treatment mainly relied on Cefuroxime and Ampicillin-Sulbactam, which is in accordance with the detected microbiological flora. Conclusions No relevant differences were found with regard to the complication rate between UTE und BTE in pediatric patients. Broad-spectrum antibiotics were used in accordance with the detected microbiological flora. Since 2019, calculated antibiotic therapy with Ampicillin-Sulbactam has been the treatment of choice for pediatric PTA

    Imaging Biomarkers in Spinocerebellar Ataxias

    Spinocerebellar ataxia type 3 (SCA3) is a progressive neurodegenerative disorder with multisystem involvement and no disease-modifying treatment to date. As targeted therapies move into clinical testing, the need for sensitive imaging biomarkers is critical. This thesis presents a multimodal framework for understanding the structural and microstructural progression of SCA3 and for developing magnetic resonance imaging (MRI) based biomarkers applicable across disease stages. Drawing on cross-sectional and longitudinal data from the European SCA3/Machado– Joseph Disease Initiative (ESMI), this work demonstrates that infratentorial white matter structures—particularly the pons, medulla oblongata, and cerebellar peduncles—showed early and consistent degeneration. Among these, pons volume showed early decline already in pre-symptomatic mutation carriers and showed the highest sensitivity to change, emerging as a robust imaging biomarker across disease stages. Regional atrophy trajectories were modulated by disease severity and duration in distinct, structure-specific patterns. Diffusion tensor imaging showed cerebellar white matter degeneration in cross-sectional comparisons, but high intra-individual variability between visits limited its utility for detecting longitudinal change. Neurofilament light chain (NfL) levels were abnormal decades before clinical onset, but plateaued thereafter and showed limited responsiveness to progression. In contrast, mutant ataxin-3 (ATXN3) concentrations remained relatively constant across the disease course and did not show relevant dynamics. The Scale for the Assessment and Rating of Ataxia (SARA), while widely used clinically, showed lower sensitivity to early progression and exhibited delayed changes relative to imaging markers. To support future biomarker development, a novel method for myelin water estimation was incorporated. While not yet applied in SCA3, it holds great promise for detecting subtle myelin alterations, particularly given the early and pronounced white matter involvement and emerging evidence of oligodendrocyte dysfunction. Together, the findings of this thesis highlight the utility of multimodal imaging for refining disease staging, patient stratification, and identification of suitable imaging biomarkers for clinical trials

    Consciousness and Human Brain Organoids : A Conceptual Mapping of Ethical and Philosophical Literature

    Human brain organoids (HBOs) are three-dimensional structures derived from human pluripotent stem cells that model aspects of fetal brain development. As HBO models grow more complex, ethical concerns arise, particularly around the potential for consciousness. Defining and detecting consciousness in HBOs remains unresolved, with existing theories offering conflicting predictions. This systematic review examines how consciousness is conceptualized in the ethical and philosophical literature concerning HBOs. We selected peer-reviewed publications written in English from 2013 onward that directly address consciousness regarding HBOs. After screening 51 sources, 24 were analysed in themes: Consciousness Terminology, Biological Limitations, Theories of Consciousness, Detecting Consciousness, Comparisons with Conscious Entities, and Special Entities. Uncertainty about consciousness in general complicates the conversation around HBOs. Clear communication is essential to avoid misconceptions, and future research may benefit from focusing on organoid intelligence as a more tractable concept
