    Impact of CAKUT syndrome and UTI on pediatric BMI

    Background: Several recent studies suggest that continuous low-dose antibiotics prophylaxis used in VUR may affect the growth of children. Guidos P.J. et al. (2018) found a significant increase in BMI in children who had prior antibiotic usage. CAKUT syndrome involves both the kidney and urinary tract and generally is associated with antibiotic therapy among pediatric patients and metabolism of waste products. Aim: We aimed to evaluate the impact of congenital genitourinary defects (CAKUT syndrome) on BMI in pediatric patients compared to patients without structural pathology. Materials and methods: We prospectively enrolled 49 pediatric patients admitted to the surgical and nephrological departments from June 1 to November 31, 2023. All patients were divided into two groups: study group (n=27) – CAKUT syndrome (VUR, PKD, horseshoe kidney, renal agenesis/hypoplasia, etc.) and prior/current episodes of CAP; control group (n=22) – UTI, without CAKUT (proved by US, VCUG or CT). All patients underwent complete work-up including clinical examination, evaluation of BMI according to WHO standards(underweight, healthy weight, overweight, obese, renal function tests (creatinine, urine, GFR), and imaging studies (US and/or VCUG/CT). Written informed consent was obtained from the parents. P<0,05 was considered significant. Results: In the study group, 7.4% of patients were obese; 14.8% - overweight; 51.8% -healthy weight; 26% -underweight. BMI distribution in the control group: 4.5% -obese; 4.5% -overweight, 72.7% -healthy weight; 18.3% -underweight. We observed no significant risk of obesity (OR=1.68; 95% CI 0.1422 to 19.8541; P=0.06805), being overweight (OR=3.6522; 95% CI 0.3774 to 35.3435; P=0.2633) or underweight (OR=1.575; 95% CI =0.3948 to 6.2839; P=0.5199) among children with CAKUT syndrome comparing to their peers without congenital defects of urinary system. Conclusion: CAKUT syndrome does not affect pediatric BMI. There is the same risk of being obese, overweight, or underweight among children with and without congenital defects of urinary system and UTI

    The global prevalence of metabolic syndrome in Ukraine and the world gives reason to consider it one of the most dangerous diseases of the modern world. One of the promising directions in the pharmacotherapy of pathological conditions is the use of the drug in the form of bioidentical implants that contain a medicinal substance. Experimental metabolic syndrome in animals was modeled with the help of a Cafeteria diet, which imitated the problematic eating patterns of modern humans. Animal body weight, blood glucose, lactate and insulin content, HOMA index, and lipid metabolism indicators were evaluated as markers of metabolic syndrome and criterion signs of treatment effectiveness. Results showed that a 6-week Cafeteria diet causes a metabolic imbalance and is accompanied by signs characteristic of metabolic syndrome. Probable signs of insulin resistance accompanied these changes. Both the tablet and pellet form of the drug positively affected the indicators of carbohydrate metabolism and reduced the manifestations of insulin resistance.     Determining the state of peripheral hemodynamic indicators is one of the modern challenges, which can be a barrier for athletes of a particular type of sport of a certain somatotype in relation to individual constitutional features and prevent achieving sports results. The aim of the work was to study the relationship between the parameters of the rheovasogram of the hip with anthropometric dimensions and components of body weight and somatotype in track and field athletes with a high level of mastery of the ecto-mesomorphic somatotype. We conducted a study of high-level sprinters using the method of P.P. Shaparenka where 50 anthropometric body sizes were determined. The component composition of body weight was carried out using the Matejko method, and the Heath-Carter method was used for score assessment of the somatotype components. It was determined that 28 track and field athletes belonged to the ecto-mesomorphic constitutional type. Rheovasographic indicators of the thigh were measured on a computer diagnostic multifunctional complex by using tetrapolar rheography. Correlation analysis was performed using Spearman’s non-parametric statistical method in the STATISTICA 5.5 program. It was established that in sprinters who belong to the ecto-mesomorphic somatotype, there are numerically reliable correlations between the amplitude parameters of the hip rheovasogram with anthropometric dimensions and somatotypological indicators. The greatest strength and number of relationships was seen between between longitudinal, girth, transverse, body dimensions, the thickness of skin and fat folds, components of somatotype and body weight and basic impedance, amplitudes of the systolic wave and rapid blood filling, where the average strength of the connections prevail. The characteristics of the relationship between the amplitude indicators of the rheovasogram of the thigh and the indicators of the external structure of the body, which we determined, confirm the conditioning of the indicators of peripheral hemodynamics by constitutional features

    Abstract: The full-scale invasion of the russian federation into Ukraine actualized the problem of the use of chemical weaponsof destruction, in particular with a pronounced irritant effect. Today, there is a need to analyze cases of the use of such weapons duringmilitary operations in Ukraine and to characterize the consequences of their use, to develop tactics for preserving the lives and healthof both employees of the Armed Forces of Ukraine, employees of the security and defense sector, and the civilian population. Thepurpose of the article is to highlight the problem of using chemical weapons of irritant action in the conditions of a full-scale invasionof the russian federation into Ukraine and the social and medical consequences of its use. We used the following methods andtechniques: empirical analysis (observation, historical analogy, comparison), complex research (structural analysis and synthesis,generalization, argumentation). The basis for our research was the sources of scientometric databases Web of Science, Scopus, PubMed, Index Copernicus and our own experience. Our research makes it possible to develop a complex of medical and othermeasures aimed at preventing or as much as possible weakening the impressive and destructive effect of weapons of mass destructionin order to preserve the life, health, fighting capacity and working capacity of the personnel of the troops and the population, as well asthe preservation of military, civilian, natural objects, animals, material values and development of tactics for protection in case of itsapplication.     Monitoring is a major component of asthma management in children. Regular monitoring allows for diagnosis confirmation, treatment optimization, and natural historyreview. Numerous factors that may affect disease activity and patient well-beingneed to be monitored: response and adherence to treatment, disease control, disease progression, comorbidities, quality of life, medication side-effects, allergen andirritant exposures, diet and more. However, the prioritization of such factors and theselection of relevant assessment tools is an unmet need. Furthermore, rapidly developing technologies promise new opportunities for closer, or even “real-time,” monitoring between visits. Following an approach that included needs assessment, evidenceappraisal, and Delphi consensus, the PeARL Think Tank, in collaboration with majorinternational professional and patient organizations, has developed a set of 24 recommendations on pediatric asthma monitoring, to support healthcare professionals indecision-making and care pathway design
