    Increased drug-seeking and vulnerability to relapse after escalation of nicotine intake in male and female rats

    Abstract Nicotine addiction is characterized by escalated drug use, craving and a high relapse rate after abstinence. However, because of difficulties in demonstrating escalation of nicotine use in rats, its relation to other addiction-related phenomena is currently unknown. Recently, we showed that, compared to rats with a fixed moderate dose of nicotine, rats with access to increasing high doses of nicotine for self-administration progressively escalated their nicotine intake. Whether these animals with escalating patterns of nicotine self-administration also develop other behavioral signs of addiction remains to be investigated. Here we report that after escalation of nicotine intake, animals have a greater difficulty of abstaining from seeking the drug, a greater responsiveness to nicotine-induced craving-like behavior, and an increased vulnerability to re-escalate nicotine intake post-extinction than rats with stable patterns of nicotine intake. No substantial sex differences in the development of these different addiction-related phenomena were observed. Finally, after escalation, nicotine intake also became primarily dependent on nicotine reinforcement and less so on the nicotine-paired cue. Overall, this study shows that most of the post-escalation behavioral changes previously seen with other drugs of abuse are generalizable to nicotine intake escalation

    Asteroids Fail to Retain Cometary Impact Signatures

    International audienceAbstract A bombardment of comets is thought to have occurred in the inner solar system as a result of a dynamical instability among the giant planets after gas disk dispersal. Vesta, the second largest asteroid in the main asteroid belt, likely differentiated before gas disk dispersal, implying its crust witnessed the cometary bombardment. The composition of howardite, eucrite, and diogenite (HED) meteorites, which represent fragments of Vesta’s crust, could therefore have been altered by cometary impacts. Here we combine noble gas mass spectrometry measurements, N -body simulations, collision rate calculations, and impact simulations to estimate the cometary contribution to Vesta. While our dynamical simulations indicate that Vesta likely underwent a significant number of collisions with large comets, we find no xenon cometary signature in HED meteorites. This apparent contradiction arises due to the fact that cometary impacts were at high speeds and Vesta’s weak gravitational attraction made it incapable of retaining cometary material. Smaller asteroids are even less likely to retain such material. Therefore, if a cometary xenon signature is ever detected in an asteroid belt object, it must have been acquired during formation, within the same source region as comet 67P/Churyumov-Gerasimenko, and have been implanted later into the asteroid belt

    Introduction: non-aneurysmal subarachnoid hemorrhages (naSAH) account for 10–15% of spontaneous subarachnoid hemorrhage (SAH) and are usually less severe than aneurysmal forms. However, uncertainties remain regarding the timing of severe complications and the ability to identify high-risk patients at admission. We investigated the time-dependent incidence of complications in non- severe naSAH and compared perimesencephalic and diffuse patterns. Methods: we conducted a retrospective, single-center study including all adult patients admitted to the Bordeaux university hospital neuro-ICU between january 2018 and december 2023 for non-severe naSAH (world federation of neurological surgeons (WFNS) I–II). Patients were classified as perimesencephalic SAH (pmSAH) or diffuse SAH (dSAH) according to standardized radiological criteria. Complications were recorded with their time of occurrence to assess time-dependent incidence and compare both groups. Results: among 1112 spontaneous SAH admissions between 2018 and 2023, 149 patients met the inclusion criteria (121 pmSAH, 28 dSAH). The 14-day cumulative incidence of severe complications (limited to delayed cerebral ischemia and acute hydrocephalus) was 11%. The median time to onset was 2.5 days. Incidences were significantly higher in the dSAH group (36% vs 5%, p < 0.001), resulting in a markedly increased relative risk of severe complications (RR = 7.20, 95% CI 2.85–18.16, p < 0.0001). Incidence curves significantly differed between groups (log-rank p = 0.021). A one-year etiological workup identified a causative lesion in only 8% of cases, with most patients remaining without a definitive diagnosis. Conclusion: this retrospective study shows that severe complications in naSAH occur early and are significantly more frequent in diffuse than in perimesencephalic forms. These findings suggest that pmSAH may require less intensive monitoring, whereas dSAH warrants reinforced surveillance during the acute phase.Introduction : les hémorragies sous-arachnoïdiennes non anévrismales (HSAna) représentent 10 à 15 % des hémorragie sous-arachnoïdienne (HSA) spontanées et sont généralement considérées comme moins sévères que les formes anévrismales. Nous avons étudié l’incidence temporelle des complications dans les HSAna non sévères et comparé leurs occurrences dans les formes périmésencéphaliques et diffuses. Méthodes : nous avons mené une étude rétrospective, monocentrique, incluant tous les adultes admis en neuro-réanimation au centre hospitalier universitaire (CHU) de Bordeaux entre 2018 et 2023 pour HSAna non sévère (fédération mondiale des chirurgiens neurologiques (WFNS) I–II). Les patients étaient classés en formes périmésencéphaliques (HSApm) ou diffuses (HSAd) selon des critères radiologiques standardisés. Les complications étaient recueillies avec leurs dates de survenue afin d’analyser leur incidence temporelle et de comparer ces deux groupes d’HSA. Résultats : parmi 1112 HSA spontanées admises entre 2018 et 2023, 149 patients répondaient aux critères (121 HSApm, 28 HSAd). L’incidence cumulée à 14 jours des complications sévères (comprenant uniquement des ischémies cérébrales retardées et des hydrocéphalies aigues), était de 11%. Leur délai médian d’apparition était de 2.5 jours et leurs incidences étaient significativement plus élevées dans le groupe HSAd (36% vs 5%, p < 0.001) entrainant un risque relatif marqué pour les complications sévères dans le groupe HSAd (RR = 7.20, IC95% 2.85–18.16, p < 0.0001). Les courbes d’incidence différaient significativement entre les deux groupes (log-rank p = 0.021). Un bilan étiologique complet permettait d’identifier une lésion causale dans seulement 8% des cas, laissant la majorité sans diagnostic définitif. Conclusion : cette étude rétrospective souligne que les complications sévères surviennent généralement précocement dans les HSAna, et sont significativement plus fréquentes dans les formes diffuses que dans les périmésencéphaliques. Ces résultats suggèrent que les HSApm suivent une évolution stable justifiant potentiellement d’une surveillance moins intensive, alors que les HSAd non anévrismales nécessitent probablement une surveillance renforcée en phase aiguë

    Dopaminergic modulation of the insular cortex in anxiety-related behaviors and emotional valence

    Anxiety is an adaptive response of individuals exposed to a potentially threatening context. However, anxiety levels can be persistently high independently of the environment and become pathological. Although anxiety disorders represent the most prevalent psychiatric conditions - characterized by chronic high levels of anxiety and an attentional bias towards negative valence - the underlying neurobiology remains poorly understood. Numerous studies in humans and in preclinical models revealed the implication of different neuromodulators including serotonin, norepinephrine, but also dopamine. Imaging studies have shown that the insular cortex (or insula), particularly its anterior region, is hyperactivated in individuals with anxiety disorders in response to salient or negative stimuli. Although dopamine neurotransmission is known to regulate anxiety in humans and animal models, its specific regulatory effects on the anterior insula have remained largely unexplored.This PhD dissertation aims to investigate the role of dopamine transmission in the insular cortex in shaping anxiety and emotional valence in mice. Through a multifaceted approach, this research uncovered how dopamine modulates anterior insula function in anxiety and valence processing at three key levels of analysis. (1) First, we mapped the dopaminergic system of the insular cortex and revealed a high density of neurons expressing type-1 dopamine receptors (D1) in the insula, particularly important in the anterior insula, and seven times greater than the density of neurons expressing type-2 dopamine receptors (D2). Then, we found that pharmacological activation of D1 in the anterior insula is anxiogenic, suggesting a direct link between insular dopamine signaling and anxiety-related behaviors. (2) Using fiber-photometry, we identified that the amplitude of dopamine release onto D1+ neurons in the anterior insula while mice were in anxiogenic spaces or receiving mild foot shocks was both positively correlated with mice level of trait anxiety. (3) Finally, population dynamics and deep-learning analyses of anterior insula single-unit recordings uncovered distinct coding patterns of anxiety-provoking and safe environments, as well as tastants of positive and negative valence. Remarkably, systemic D1 activation, which heightens anxiety-related behaviors, dampens this coding dichotomy by increasing coding variability for protected spaces while increasing the coding specificity for anxiogenic spaces. Interestingly, the coding reliability of anxiogenic areas was positively correlated with mice level of trait anxiety, and we observed a trend towards a positive correlation between the coding reliability of a negative tastants, and mice level of anxiety.Altogether, our findings provide a new model of neural population coding of anxiety and emotional valence and unravel D1-dependent coding mechanisms in the mouse anterior insula.L'anxiété est une réponse adaptative des individus exposés à un contexte potentiellement menaçant. Cependant, des niveaux d'anxiété peuvent rester élevés de manière persistante, indépendamment de l'environnement, et devenir pathologiques. Bien que les troubles anxieux soient les affections psychiatriques les plus répandues – caractérisés par des niveaux chroniques élevés d'anxiété et un biais attentionnel envers les stimuli à valence négative – les mécanismes neurobiologiques sous-jacents restent encore mal compris. De nombreuses études, menées chez l'humain et dans des modèles précliniques, ont mis en évidence l'implication de différents neuromodulateurs, notamment la sérotonine, la noradrénaline, mais aussi la dopamine. Des études d'imagerie fonctionnelle ont montré que le cortex insulaire (ou insula), en particulier sa région antérieure, est hyperactivé chez les individus souffrant de troubles anxieux en réponse à des stimuli saillants ou négatifs. Bien que la neurotransmission dopaminergique soit connue pour réguler l'anxiété chez l'humain et dans les modèles animaux, ses effets spécifiques sur l'insula antérieure restent largement inexplorés.Cette thèse vise à explorer le rôle de la transmission dopaminergique dans le cortex insulaire dans la modulation de l'anxiété et de la valence émotionnelle chez la souris. À travers une approche multifactorielle, cette recherche a permis de révéler comment la dopamine module la fonction de l'insula antérieure dans les comportements liés à l’anxiété et à la valence émotionnelle, selon trois niveaux d’analyse clés. (1) Tout d’abord, nous avons cartographié le système dopaminergique insulaire et identifié une forte densité de neurones exprimant les récepteurs dopaminergiques de type 1 (D1), particulièrement marquée dans l’insula antérieure, et sept fois supérieure à celle des neurones exprimant les récepteurs de type 2 (D2). De plus, l’activation pharmacologique des récepteurs D1 dans l’insula antérieure s’est révélée anxiogène, établissant un lien direct entre la signalisation dopaminergique insulaire et les comportements associés à l’anxiété. (2) Par photométrie en fibre, nous avons montré que l’amplitude de la libération de dopamine sur les neurones D1-positifs de l’insula antérieure augmentait lorsque les souris étaient exposées à des espaces anxiogènes ou à des chocs électriques légers. Cette libération de dopamine était positivement corrélée avec le niveau d'anxiété intrinsèque des souris, renforçant l'idée que la dopamine joue un rôle central dans la modulation des réponses anxieuses. (3) Enfin, grâce à l’analyse par intelligence artificielle des dynamiques de population neuronale et des enregistrements de neurones unitaires dans l’insula antérieure, nous avons identifié des propriétés de codage neuronal distinctes pour les environnements anxiogènes et protégés, ainsi que pour les stimuli gustatifs à valence positive ou négative. De façon intéressante, l’activation systémique des récepteurs D1, qui augmente les comportements de type anxieux, perturbe cette dichotomie de codage en rendant le codage des espaces protégés plus variable et celui des espaces anxiogènes plus spécifique. De plus, le codage des espaces anxiogènes était corrélé positivement au niveau d’anxiété intrinsèque des souris. Nous avons également observé une tendance à une corrélation positive entre la spécificité du codage des stimuli gustatifs négatifs et le niveau d'anxiété des souris.En conclusion, ces résultats mettent en lumière un nouveau modèle de codage neuronal dans l’insula antérieure, en lien avec l’anxiété et la valence émotionnelle. Ils dévoilent également des mécanismes de codage dépendant des récepteurs D1 dans l’insula antérieure de la souris, ouvrant ainsi de nouvelles perspectives pour comprendre et traiter les troubles anxieux

    Integrative and Conjugative Elements (ICEs) are self-transmissible mobile genetic elements that play a key rolein bacterial genome evolution by mediating horizontal gene transfer (HGT). Unlike plasmids, ICEs integrate into the host genome, excise to form circular intermediates, and are then transferred to recipient cells via conjugationbefore reintegrating into the chromosome. These elements significantly contribute to bacterial adaptability by promoting the acquisition of antibiotic resistance genes, virulence factors, and metabolic traits. A novel family ofICEs, referred to as MICE (Mycoplasma Integrative and Conjugative Elements), was identified in severalMollicutes species. In Mycoplasma hominis, a human urogenital pathogen, approximately 45% of clinical isolatescarry ICEs randomly inserted into the chromosome. These elements span 27 to 30 kbp and contain between 25and 30 CDSs.In this thesis, we first aimed to assess the circularization capacities of M. hominis 4788 ICE (ICEHo-4788) underdifferent environmental conditions and investigate the transcription of the ICE CDSs. The second part of our study focused on transfer capacities of M. hominis ICE through mating experiments.Using qPCR, RT-qPCR, and LC-MS/MS, we evaluated ICEHo-4788 circularization, gene transcription, andprotein expression under different conditions: axenic growth phases, exposure to mitomycin C (MMC), to thermal stress (cold and heat shocks), and co-culture with HeLa cells. Under axenic conditions, a peak of circularization was observed at 12 h of growth, with a 9.4-fold increase compared to the baseline. MMC exposure and cold shockled to 3.3- and 3.0-fold increases in circular forms, respectively. In co-culture with HeLa cells, circularization was strongly enhanced, with peaks at 72 h and 7 days post-inoculation, showing 10- and 23-fold increases, respectively.RT-qPCR analysis showed that all ICEHo-4788 CDSs were transcribed. Additionally, RT-PCR targeting allintergenic regions yielded amplicons, suggesting that the element may be transcribed as a single polycistronicm RNA. No major transcriptional changes were observed in axenic culture or under MMC and cold shock conditions. However, in HeLa co-culture, a marked transcriptional upregulation was detected at 7 days postinoculation,with 5- to 23-fold increases depending on the CDS.Proteomic analyses revealed that four ICEHo4788-associated proteins were significantly upregulated at 12 hcompared to 19 h in axenic culture, while no significant changes in protein expression were detected in responseto thermal stress or MMC.Given that co-culture with HeLa cells strongly promoted ICE circularization and transcription, we then used thiscondition to investigate the conjugative properties of ICE in M. hominis. Mating experiments were conducted byco-incubating HeLa cells for 7 days with a donor M. hominis strain carrying a tetracycline resistance gene inserted within an intergenic region of the ICE, and two recipient strains harboring a different antibiotic resistance marker(gentamicin or ofloxacin).Whole genome sequencing and SNP polymorphism analyses revealed two distinct HGT mechanisms. While theICE was transferred from the donor to the recipient strain, a Mycoplasma Chromosomal Transfer (MCT) occurred in the opposite direction, from the recipient to the donor cell, independently of ICE mobilization and involved the exchange of large chromosomal regions.Together, these findings demonstrate that environmental stimuli, particularly host-like conditions, modulate thecircularization and transcriptional activity of M. hominis ICEs. Moreover, our results provide the first experimental evidence of two HGT mechanisms in M. hominis, ICE transfer and MCT, with MCT acting as a potent driver ofextensive genomic recombination. These complementary mechanisms underline the efficiency of HGT inM. hominis, contributing to its genome plasticity and adaptability.Les éléments génétiques intégratifs et conjugatifs (ICEs) sont des éléments mobiles auto-transmissibles jouant un rôle clé dans l’évolution des génomes bactériens via le transfert horizontal de gènes (THG). Contrairement aux plasmides, les ICEs s’intègrent dans le génome de l’hôte, s’excisent sous forme circulaire, se transfèrent par conjugaison vers une cellule receveuse, puis se réintègrent dans le chromosome. Ils contribuent à l’adaptation bactérienne en facilitant l’acquisition de gènes de résistance aux antibiotiques, de facteurs de virulence ou de fonctions métaboliques. Une famille d’ICEs, appelés MICE (Mycoplasma Integrative and Conjugative Elements),a été identifiée chez plusieurs espèces de Mollicutes. Chez Mycoplasma hominis, pathogène humain du tractusurogénital, environ 45 % des isolats cliniques portent des ICEs de 27 à 30 kpb, intégrés de façon aléatoire dans le génome.Cette thèse visait d’abord à évaluer la capacité de circularisation d’un ICE de M. hominis (ICEHo-4788) dans différentes conditions environnementales, et à analyser la transcription et l’expression de ses gènes. Dans un second temps, nous avons exploré son potentiel de conjugaison par des expériences de conjugaison.À l’aide de qPCR, RT-qPCR et LC-MS/MS, nous avons étudié la circularisation, la transcription et l’expression protéique de l’ICEHo-4788 dans diverses conditions : phases de croissance axénique, exposition à la mitomycine C (MMC), stress thermique (froid/chaud), et co-culture avec des cellules HeLa. En culture axénique, un pic de circularisation a été observé à 12 h, avec une augmentation de 9,4 fois par rapport à la référence. L’exposition à la MMC et au froid a entraîné une augmentation de respectivement 3,3 et 3,0 fois. En co-culture avec les cellules HeLa, la circularisation a été fortement induite, avec des pics à 72 h et 7 jours post-inoculation (augmentations respectives de 10 et 23 fois).L’analyse RT-qPCR a révélé que tous les gènes codants de l’ICEHo-4788 étaient transcrits. Les RT-PCR ciblant les régions intergéniques ont toute produit des amplicons, suggérant une transcription polycistronique. Aucune variation notable de transcription n’a été observée en culture axénique ni après exposition à la MMC ou au stress thermique. En revanche, une forte induction transcriptionnelle a été détectée à 7 jours de co-culture avec les cellules HeLa, avec des augmentations allant de 5 à 23 fois selon les gènes. Sur le plan protéique, quatre protéines associées à l’ICE étaient significativement surexprimées à 12 h par rapport à 19 h en culture axénique, tandis qu’aucune variation n’a été détectée après stress thermique ou exposition à la MMC.Étant donné l’effet marqué des conditions mimant l’hôte sur l’activation de l’ICE, nous avons mené des expériences de conjugaison en co-culture avec des cellules HeLa pendant 7 jours. Une souche donneuse de M. hominis portant un gène de résistance à la tétracycline inséré dans l’ICE a été co-incubée avec deux souches receveuses portant chacune un marqueur de résistance différent (gentamicine ou ofloxacine). Les colonies doublement résistantes ont été sélectionnées sur milieu contenant les deux antibiotiques.Le séquençage des génomes complets et l’analyse des polymorphismes nucléotidiques ont révélé deux mécanismes distincts de THG. D’une part, l’ICE a été transféré du donneur vers le récepteur. D’autre part, un transfert chromosomique (MCT) s’est produit en sens inverse, du receveur vers le donneur, indépendamment de l’ICE,impliquant l’échange de larges segments chromosomiques.Ces résultats montrent que les conditions environnementales, notamment celles mimant l’hôte, modulent la circularisation et l’expression des ICEs chez M. hominis. Ce travail apporte les premières preuves expérimentales de deux mécanismes de THG dans cette espèce, soulignant l’efficacité de ces échanges génétiques et leur rôle dans la plasticité du génome et l’adaptation bactérienne, notamment face aux antibiotiques

    Improvement of piezocatalytic performance of Na0.5Bi0.5TiO3 perovskite using K doping for efficient Rhodamine B degradation

    International audiencePiezocatalysis, based on the piezoelectric properties of catalysts, breaks down the barrier between mechanical energy and chemical energy. It describes the use of charges induced by piezoelectricity to assist typical chemical processes while harvesting various forms of mechanical green energy. The performance of piezocatalysis is predominantly governed by the piezoelectric properties of materials. The main aim of this work is to evaluate and analyze the potential of potassium-doped sodium bismuth titanate Na0.5-xKxBi0.5TiO3 abbreviated as NKxBT (x = 0, 0.15, 0.20, and 0.25), as a piezocatalyst in the degradation of the organic dye Rhodamine B (RhB) under ultrasonic vibration. The synthesis of NKxBT nanopowders was conducted using the sol-gel autocombustion method. Coupled structural analysis reveals the presence of an intermediate Morphotropic Phase Boundary (MPB; where two phases coexist) in the optimal NK15BT composition. The piezocatalytic degradation results showed a total piezo-degradation in only 90 min and a rate constant 8 times higher than the undoped NK0BT. The enhanced piezocatalytic activity results from a synergistic effect of MPB presence, reduced particle size, optimal bandgap and high lattice strain. The NK15BT sample also demonstrated good reusability and good mineralization.</p

    Unveiling the key role of rare-earth (La Versus Nd) in Ni+ -based layered nickelates: impact on structures and physical properties

    "ADC - Accord Couperin / American Chemical Society (2024-2026)"International audienceWe report the synthesis of bulk RE1–xAxNiO3 (RE = La, Nd; A = Sr, Ca) perovskite phases under high oxygen pressure (T = 900 °C, PO2 = 250 bar) and identify a solubility limit of x = 0.07 for phase-pure samples. This solubility limit, shared by the various RE and A combination investigated, is likely constrained by the Ni4+ content (t2g6) whose electronic effect limits doping, regardless the size of RE and A atoms. Alkaline earth (hole) doping induces a decrease in the volume of the unit cell of RE1–xAxNiO3, despite the presence of larger alkaline earth atoms, and an increase in the orthorhombic distortion in the case of RE = Nd (Nd1–xAxNiO3). After topotactic reduction, RE1–xSrxNiO2 infinite-layers were obtained by mixing the perovskite phase with 2 mol CaH2 in a tube sealed under secondary vacuum and treated at low temperature (250 °C). Thermogravimetric analysis was used to determine the oxygen stoichiometries of the compounds with accuracy. We find that while the perovskite parent compound RENiO3 displays a high crystalline quality, regardless the nature of the rare-earth (Nd or La), marked stacking faults are present in the reduced NdNiO2 infinite-layer, as determined by X-ray diffraction measurements using synchrotron radiation. The high-pressure diffraction experiment further demonstrates the role of pressure in attenuating the effect of these stacking faults that were also partially simulated and modeled using the FAULTS program. These defects are virtually absent for Sr2+ doping at 7% or when Nd is replaced by La. The occurrence of such stacking faults is confirmed by HRTEM analysis in the case of NdNiO2. These stacking faults are more pronounced in the reduced infinite layer compositions for which the parent perovskite structure is more distorted, as measured by the departure of the Ni–O–Ni angle from 180°, establishing therefore a memory effect between the distortion of the perovskite structure and the occurrence of stacking faults after reduction. The more the perovskite structure is distorted, the greater the stacking fault rate. Finally, we report and discuss the magnetic properties, the electrical resistivity, and the specific heat of La and Nd-based infinite layer compositions with respect to their structural properties. More specifically, we shed light on the contribution of f-electrons of Nd3+ to the specific heat and discuss the possible signature of spin glass state in LaNiO2 through magnetic and specific heat measurements

    Introduction: hypertension is the most common chronic disease worldwide, affecting about one quarter of the French population. Although seemingly simple, its diagnosis can face human and technical challenges. As emergency departments (EDs) often serve as an entry point into the healthcare system, they could play a major role in optimizing hypertension management. The objective of this thesis is to identify predictive factors shared by patients who were started on antihypertensive therapy following an emergency department visit. Methods: this study was conducted at Bordeaux University Hospital. We established a common profile based on age, sex, pain level, triage score, reason for consultation, and etiology of discomfort among patients treated for hypertension, selected from those who attended the emergency department between 2013 and 2021. The Bootstrapping method was used to robustly estimate these predictive factors. Results: we included a total of 208,971 patients, among whom 20,801 initiated antihypertensive therapy within one year following the consultation. A higher proportion of women were hypertensive. Treated patients were more frequently aged over 55 years (47.5% vs. 16%), had higher triage scores (levels 1–3: 66.1% vs. 51.4%), reported less pain, consulted more often for medical reasons (66.8% vs. 60.3%), and presented more frequently with medical syncope or related conditions (8.5% vs. 5.2%). The bootstrapping model strengthened the associations for age and triage score, but not for sex or pain. Discussion: particular attention should be paid to the risk of hypertension in older patients, those with more severeIntroduction : l’hypertension artérielle (HTA) est la pathologie chronique la plus fréquente dans le monde et toucherait environ soit un quart des français. Bien que d’apparence simple, son diagnostic peut se heurter à des problématiques humaines et techniques. Les services d’urgences, en étant pour beaucoup une porte d’entrée dans le système de soins, pourraient être un acteur majeur dans l’optimisation de la prise en charge de l'hypertension. L’objectif de cette thèse est d’identifier des facteurs prédictifs communs aux patients traités contre l’hypertension après une consultation aux urgences. Méthode : nous avons mené ce travail au centre hospitalier universitaire (CHU) de Bordeaux. Nous avons dressé un profil commun en fonction de l’âge, du sexe, de la douleur, du score de triage, du motif de consultation et de l’étiologie du malaise, aux patients traités contre l’HTA parmi des patients ayant consulté entre 2013 et 2021 aux urgences. Nous avons utilisé du Bootstrapping pour estimer de façon robuste ces facteurs. Résultats : nous avons pu inclure 208 971 patients, dont 20 801 se sont vu instaurer un traitement antihypertenseur dans l’année suivant la consultation. Il y a plus de femme qui étaient hypertendu. Les patients traités sont le plus souvent âgés de plus de 55 ans (47,5 % vs 16%), avec des scores de triage élevés entre 1 et 3 (66,1% vs 51,4%, sont moins douloureux, consultent davantage pour des motif médicaux (66,8% vs 60,3%) ou présentent des malaises d’étiologie médicale (8,5% vs 5,2%). Le modèle par Bootstrapping renforce les résultats pour l’âge et le score de triage mais pas le sexe ni la douleur. Discussion : il faut être d’autant plus vigilant sur le risque d’HTA chez les patients âgés, qui sont graves et dont le motif d’admission est médical ou s’il est question de malaise d’étiologie non traumatique, quelque soit le sexe du patient. Le choix des paramètres étudiés et le recueil rétrospectif sont des limites dont on pourra s’affranchir lors de futurs travaux. Nous devons améliorer nos pratiques en tant que soignant. Une filière de l’HTA via les urgences semble nécessaire pour en améliorer sa prise en charge

    Background: the online reputation of general practitioners, an expanding phenomenon, raises ethical, legal, and practical issues in the context of the growing digitization of healthcare services. Google, through its business listings, has become the main platform for such evaluations. Objectives: to examine the nature and determinants of online reviews concerning general practitioners in the Landes department, published on Google in 2024. Secondarily, to analyze sociodemographic, professional, and geographic factors associated with the presence, frequency, tone, and content of these evaluations. Methods: this mixed-methods observational study combined a cross-sectional quantitative analysis of the Google listings of 473 physicians and healthcare structures identified via the national CNAM directory in October 2024, and a qualitative analysis of 3,036 published reviews. Data collected included ratings, reviewer status (particularly "Local Guide" designation), presence of written comments, and physician characteristics (gender, practice type, location, additional activities). A thematic categorization of comments was conducted into 34 categories, grouped into three main domains: doctor–patient relationship, medical competence, and office organization. Statistical analysis was performed using R software. Results: of the physicians and healthcare structures studied, 63% had a Google listing, and 58% had received at least one review. The average rating was 4.2/5, with strong polarization (92.3% of ratings were extreme). Qualitative analysis revealed a predominance of relational criteria: listening (669 mentions) and kindness (638) were the most frequently cited categories. In contrast, organizational aspects (reception, appointment scheduling, punctuality) were less often mentioned and more frequently associated with negative comments. Physicians practicing in rural areas, those with a specific medical focus, or those working alone generally received more positive evaluations. Conversely, healthcare structures, though receiving more reviews, were rated less favorably overall. Practice setting emerged as a major discriminating factor in the nature of comments. Lastly, reviewers identified as “Local Guides” (27.5% of evaluators) gave more moderate ratings, suggesting a more nuanced use of the rating system. Conclusion: the online reputation of general practitioners is generally positive but highly polarized. It is primarily shaped by relational aspects of care, rather than technical or organizational criteria. Google has established itself as the central platform for this digital reputation. These findings offer valuable insights into patient expectations and could serve as a lever for improving practices in general medicine, provided the inherent biases and risks are properly managed.Contexte : l’e-réputation des médecins généralistes, phénomène en expansion, soulève des enjeux éthiques, juridiques et pratiques dans un contexte de numérisation croissante des services de santé. Objectifs : examiner la nature et les déterminants des avis en ligne concernant les médecins généralistes des Landes, publiés sur Google en 2024. Secondairement, analyser les facteurs sociodémographiques, professionnels et géographiques associés à la présence, la fréquence, la tonalité et la nature des évaluations. Méthode : étude observationnelle mixte, combinant une analyse quantitative transversale des fiches Google des 473 médecins et structures de soins identifiés dans l’annuaire de la CNAM des Landes en octobre 2024, et une analyse qualitative des 3036 évaluations publiées. Les données recueillies portaient sur les notes, le statut des évaluateurs (notamment le statut de « Local Guide »), la présence de commentaires écrits, ainsi que sur les caractéristiques des médecins (sexe, type d’exercice, localisation, activité complémentaire). Une catégorisation thématique des commentaires a été réalisée en 34 catégories, regroupées en trois grands axes : relation médecin–patient, compétences médicales et organisation du cabinet. Les variables étudiées incluaient des données démographiques, professionnelles, géographiques ainsi que le statut d’auteur « Local Guide ». L’analyse statistique a été réalisée à l’aide du logiciel R. Résultats : 63 % des médecins et structures disposaient d’une fiche Google, dont 58 % comportaient au moins une évaluation. La note moyenne s’élevait à 4,2/5, avec une forte polarisation (92,3 % de notes extrêmes). L’analyse qualitative a montré une prépondérance des critères relationnels dans les commentaires : l’écoute (669 mentions) et l’amabilité (638) figuraient parmi les catégories les plus fréquemment citées. À l’inverse, les aspects organisationnels (secrétariat, délais, ponctualité) étaient moins souvent abordés et faisaient plus souvent l’objet de commentaires négatifs. Les médecins exerçant en milieu rural, pratiquant un exercice particulier ou seuls, obtenaient des évaluations globalement plus positives. Par ailleurs, les structures de soins, bien que recevant plus d’avis, étaient globalement moins bien notées. Le mode d’exercice apparaissait comme un facteur discriminant majeur dans la nature des commentaires. Enfin, les utilisateurs identifiés comme « Local Guides » (27,5 % des évaluateurs) attribuaient des notes plus modérées, traduisant un usage plus nuancé de la notation en ligne. Conclusion : l’e-réputation des médecins généralistes est majoritairement positive, bien que fortement polarisée. Elle repose principalement sur les dimensions relationnelles de la pratique médicale, au détriment des critères techniques ou organisationnels. Google s’affirme comme une base incontournable de cette e-réputation. Ces données offrent une perspective précieuse sur les attentes des patients et pourraient constituer un levier d’amélioration des pratiques en médecine générale, à condition d’en maîtriser les biais et les dérives
