Challenges and advances in the treatment of radial palsy in Argentina during 2024
In 2024, the management of radial nerve injuries in Argentina was profoundly influenced by structural, economic and health factors. The health system faced difficulties arising from the post-pandemic period, a prolonged economic crisis and unequal access to specialized resources. The high incidence of upper limb injuries was related to the increase in road and work accidents, as well as urban violence. These injuries mainly affected young males from vulnerable sectors.Diagnosis depended, in many cases, on clinical experience, given the limited availability of studies such as electromyography. This reality caused diagnostic delays that reduced the chances of functional recovery. In terms of treatment, tendon transfers continued to be the most widely used technique in public hospitals due to their simplicity and low cost, while nerve transfers began to be implemented in more complex centers.Post-surgical rehabilitation showed marked inequalities between regions. The shortage of specialized physiotherapists, coupled with the lack of continuity in treatments, negatively affected the results. In addition, many patients faced barriers to accessing orthotics, rehabilitation or postoperative check-ups, resorting to NGOs or solidarity campaigns.From the academic sphere, research into surgical treatments was promoted and 3D printing was explored as an alternative for personalized splints. Rehabilitation Nursing Care to a person with Immobility Syndrome due to Intestinal Pseudo-Obstruction: Case Report
Introduction: Intestinal Pseudo-Obstruction is a chronic condition that, when associated with immobility syndrome, can translate into the loss of independence, decreased functional capacity and reduced quality of life. The management of the disease, the consequences and the expectations of the person being cared for are a permanent challenge for Rehabilitation Nursing.Objective: To describe the benefits of implementing a Rehabilitation Nursing program to the person with immobility syndrome due to chronic intestinal pseudo-obstruction.Case Report: Descriptive study, case report type of a person with immobility syndrome due to chronic intestinal pseudo-obstruction. After identifying the nursing diagnoses, a care plan was developed based on Nursing Ontology, and an individualized rehabilitation program was implemented, in a home setting.Conclusion: With the implementation of the program, there were gains in increased muscle strength, improved balance, increased tolerance to activity and independence in activities of daily living. The improvements of the implementation of this rehabilitation program corroborate that the Rehabilitation Nurse should include in their intervention plans exercises focused on re-education to exertion and muscle strengthening to mitigate the complications resulting from prolonged immobility and the underlying disease, motivating and involving the person in care
Causal factors of sudden death in young athletes
It is undeniable that physical activity provides multiple benefits, both for general health and cardiovascular health, but on rare occasions, sudden cardiac death can occur in athletes even without previous symptoms. Regarding the causes and incidences, there are different studies from various countries and regions that present different results, due to the cultural variations in each country and their practiced sports. Moreover, in some countries, like Italy for example, screenings for conditions such as hypertrophic cardiomyopathy are implemented more frequently compared to other countries. Sudden death involves a broad spectrum of associated pathologies, and in athletes or individuals under 35 years old, the most common causes are primary cardiomyopathies, cardiac channelopathies, and congenital anomalies of the coronary arteries.Objectives: Know the main causes of sudden cardiac death in young athletes. Material and methods: This work is a systematic review of the literature, where different studies and articles will be analyzed to compile the most relevant and up-to-date information regarding the main causes of sudden death in young athletes. The search will be conducted through databases such as PubMed, SciELO, and the Argentine Society of Cardiology\u27s website. Results: Based on the results obtained, it was observed that the most common causes of sudden death in young athletes vary depending on the region where the studies were conducted. This variation is mainly due to sociocultural factors that influence how a given society behaves and which sports are practiced. Conclusion: Although it is a rare event with low incidence, sudden cardiac death is a catastrophic event that, in most cases, leads to death. However, it is an event that can be prevented, and with the correct therapeutic intervention in the first few minutes, it can be successfully reversed. But to achieve this, it is necessary to understand the topic and be aware of its main causes to act correctly and efficiently
Severe pneumonia in Young smokers in the province of Buenos Aires
Introduction: Smoking is related to morphological alterations of the bronchial mucosa epithelium. In Argentina, 33.4% of the population between 18 and 64 years of age are smokers, with a predominance in the lower income areas of the country. According to the World Health Organization (WHO), 1.6 million people die of pneumonia every year, with smoking being a major risk factor for its development. Materials and methods: A prospective observational descriptive study was carried out at the Eurnekian Hospital in Ezeiza to describe the evolution of young adult patients with severe pneumonia. Data was collected through anamnesis and review of physical medical records of patients admitted with a diagnosisof pneumonia. Results: Thirty-eight young immunocompetent patients hospitalized with pneumonia were analyzed. The average age was 40 years, with a predominance of males (70.7%). The average number of years of consumption was 20, and 20 patients (55.2%) of the total were minors at the beginning of their smoking habit. With regard to the presence of previous pathologies, 7 patients (18.2%) were diabetic, 9 were hypertensive, 3 had suffered an acute myocardial infarction, 4 had suffered a cerebrovascular accident; 3 had advanced heart failure, 1 had renal failure and 3 had suffered tuberculosis for which they had received full treatment. With regard to oxygen saturation on admission to hospital, 8 patients had saturation levels of between 80-84.9%, 21 between 85-90% and 9 >90%. The average length of stay in hospital was 5.8 days, with only 9 patients being hospitalized for between 10-14 days. Conclusion: The results show that the onset of smoking habit is prior to the age of majority. We believe it is advisable to carry out more campaigns for the prevention of addictions and smoking cessation in adolescents and young adults, with an emphasis on primary health prevention.
Mortality in mechanical ventilation in prone position in patients with acute respiratory distress síndrome
Introduction: Prone ventilation has been shown to improve oxygenation and lung mechanics in patients with acute respiratory distress syndrome, but I consider it necessary to delve deeper into the relationship between the prone position and mortality. Objectives: To evaluate whether the prone position decreases the risk of mortality in adult patients with acute respiratory distress syndrome vs. supine ventilation, in a global and segmented manner, as well as to know the main adverse effects related to it. Material and methods: A meta-analysis of randomized controlled clinical trials comparing patients in the prone vs. supine position was performed with a search in Pubmed, Embase, Cochrane Library and LILACS, and mortality, hospital stay, days of mechanical ventilation and adverse effects were evaluated. Results: Seven randomized controlled clinical trials were included in the analysis. The prone position showed a non-significant tendency to decrease mortality when analyzed globally. When stratified by subgroups, a significant decrease in the risk of mortality was found in patients: 1) ventilated with low tidal volume, 2) prolonged prone position, and 3) established before 48 hours of disease progression in severe hypoxemia. The adverse effects related to prone position were the development of pressure ulcers and orotracheal tube obstruction. Conclusion: Prone position ventilation is a safe strategy and reduces mortality in patients with severe oxygenation impairment. It should be established early, for prolonged periods, and associated with a protective ventilation strategy
Appreciation of intensive care unit staff regarding cardiological and neurological risk factors
Introduction: the number of cardiovascular, neurological and psychological diseases has increased over time due to changes in working conditions in the health system. Objective: determinar los factores de riesgo cardiológicos y neurológicos y sus elementos asociados en pacientes de una UTI.Methods: observational, descriptive study of medical and non-medical personnel working in the intensive care unit of Hospital HIGA Luisa C. de Gandulfo de Lomas de Zamora, in the southern area of Buenos Aires province. To obtain the information we used a survey based on cardiovascular and neurological risk factors. Results: 10 % had hypertension and 60 % were smokers. Sixty percent were of normal weight, 45,5 % were overweight and 21 % obese; in addition, 73 % were physically active. Neurological factors such as fatigue were reported by 82,2 %, and 90 % experienced stress. 82,8 % of the respondents had more than one job.Conclusion: the health of intensive care unit (ICU) staff is compromised by factors such as chronic stress, long working hours and exposure to intense situations. These aspects contribute to an alarming rate of hypertension, dyslipidemia and smoking, as well as neurological problems such as fatigue. It is crucial to implement strategies that address these challenges, including psychological support and policies that promote work-life balance, to improve staff well-being and the quality of patient car
Nurse interventions in the rehabilitation of adults with neurogenic bowel after spinal cord injury: Rapid Review
Introduction: Neurogenic bowel is a complex condition that affects adults with spinal cord injury, requiring multiple interventions to optimize bowel function. Objective: This review aims to identify the main nursing interventions in the rehabilitation of people with spinal cord injury who present a neurogenic bowel.Methods: A rapid literature review based on the Cochrane guidelines was carried out by searching databases on the EBSCO Host and PubMed platforms. Articles in English, published between 2018 and 2024, with free access and addressing nursing interventions for the management of neurogenic bowel were included.Results: Narrative synthesis was carried out using thematic analysis to analyze the 7 articles included in the review. The following nursing interventions were found: digital rectal stimulation, abdominal massage, electrical rectal stimulation, diet management, stimulation of the gastro-colic reflex, muscular exercise and follow-up.Conclusions: Digital and electrical rectal stimulation were the most widely used, while dietary strategies proved important, but without standardized guidelines. Innovative technologies have shown promise, but accessibility can be a challenge. In addition, there is a significant gap in the literature on nursing interventions, as well as great heterogeneity in the studies analyzed. The combination of interventions has been shown to be effective in bowel management in adults with neurogenic bowel, post spinal cord injury. However, the lack of solid scientific evidence reinforces the need for more studies to validate the best nursing practices in this area
Risk related to data display screens in nurses at a hospital: Scoping review
Introduction: Currently, occupational risks are considered a health problem with consequences on work performance and personal well-being. Display screens pose an increased risk of musculoskeletal disorders (MD) such as upper limb disorders. Objective: To map the relationship between the use of display screens and work-related musculoskeletal disorders (WMSD) among nurses, with a focus on identifying specific ergonomic risks and related psychosocial factors by nurses in a hospital setting. Methods: The recommendations of the Joanna Briggs Institute (JBI) for a scoping review were followed. All publicly available sources of information will be considered. Included studies published in English and Portuguese without geographical or cultural restrictions. Duplicates were removed, and two independent reviewers selected abstracts and evaluated the full text of the selected studies based on inclusion criteria. The study selection results will be displayed in a Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) flowchart. Results: 64 articles published between 2016 and 2021 were found, with 17 articles included. This allowed for the identification of MD as originating from shift work. Consequently, work-family conflict arises, with factors associated with risk being chronic occupational fatigue/burnout. The importance of MD prevention programs related to work is revealed, with benefits for nurses\u27 health. Conclusions: Although WMSD is the main reason for high absenteeism rates among nurses, there is no scientific evidence that these injuries result from the use of display screens. Therefore, conducting studies aiming to correlate this usage with nurses\u27 daily interventions is suggested.
Effects of cannabis oil on people with Autism Specter Disorder
Introduction: Autism Spectrum Disorder (ASD) is characterized by deficits in communication and social interaction. Despite lacking a definitive cure, multidisciplinary therapeutic and pharmacological approaches are employed to mitigate their symptoms. However, the potential side effects of conventional treatments drive the search for alternatives such as cannabis derivatives for symptom relief. Objective: to analyze the eficacia of cannabis derivatives in the treatment of ASD. Method: a search for information was carried out in the databases and search engines Pubmed, Embase, Google Scholar and SciELO. The inclusion criteria are oriented towards observational studies and clinical trials analyzing the effects of cannabis derivatives in individuals diagnosed with autism spectrum disorders. Results: the treatment of autism using cannabis oil derivatives has emerged as a contemporary research topic in the academic literature. Despite that several studies, including systematic reviews and double-blind clinical trials, have examined the benefits of cannabidiol (CBD) in individuals diagnosed with ASD. Conclusion: it is feasible to infer, based on the scientific papers reviewed in this study, that treatment of Autism Spectrum Disorder (ASD) with cannabis derivatives could be beneficial. Effects of physical and cognitive exercise on the independence of older adults in the period 2013-2023. Systematic review
The aging of the population is an increasingly relevant reality in modern society, which implies the need to understand how to extend the functionality and independence of older adults, aspects related to quality of life. The best known factors are physical and cognitive exercise. This article aimed to conduct a systematic review on the effects of physical and cognitive exercise on the independence of older adults during the period 2013-2023. The methodology employed was the systematic review of the literature, limited to original studies conducted on older adults as study subjects. It was found that most articles focused on physical exercise, even some used physical exercise as an activity to improve the cognitive status of older adults. The results were based on the analysis of 13 selected articles, most of which were published in Spanish and focused on Spanish and Latin American populations. Physical exercises demonstrated the best results in terms of independence, showing significant improvements in participants\u27 abilities and skills. Similarly, the importance of cognitive stimulation in older adults was recognized. In conclusion, both physical and cognitive exercise have positive effects on the functional independence in older adults
