    Evaluation der modifizierten strukturierten Notrufabfrage bei akutem Thoraxschmerz im Rahmen des Projekts „Telemedizin im Rettungsdienst in Mittelhessen“

    HINTERGRUND Im deutschen Rettungsdienstsystem lässt sich in den vergangenen Jahren ein zunehmender Personalmangel bei steigenden Einsatzzahlen beobachten. Um auch zukünftig ausreichend notärztlich besetzte Rettungsmittel für kritische Not-fälle vorhalten zu können, müssen nicht dringend indizierte Notarzteinsätze re-duziert werden. Um dies zu erreichen, wurde von der „Arbeitsgemeinschaft Tele-medizin im Rettungsdienst Mittelhessen“ eine modifizierte standardisierte Notruf-abfrage „Brustschmerz“ vorgestellt, die das häufige Meldebild „Akutes Koronar-syndrom“ bereits durch die Leitstelle in stabile (ACS 1) und vital bedrohte Pati-ent*innen (ACS 2) einteilen soll. Bei ausreichender Sicherheit dieser Abfrage könnten die stabilen Patient*innen von Rettungsfachpersonal mit telenotärztli-cher Unterstützung versorgt werden und die knappen notärztlichen Ressourcen stünden für kritische Einsätze zur Verfügung. Diese Studie evaluiert die modifi-zierte standardisierte Notrufabfrage „Brustschmerz“ aus der Zentralen Leitstelle des Landkreises Marburg-Biedenkopf hinsichtlich ihrer Präzision in der Einteilung von Patient*innen mit Akutem Koronarsyndrom in stabil vs. kritisch im Vergleich mit dem präklinischen Erscheinungsbild. METHODEN Im Zeitraum von Januar bis August 2021 wurden Daten von Rettungsdienstein-sätzen im Landkreis Marburg-Biedenkopf mit dem Meldebild „Akutes Koronar-syndrom“ erhoben, die zuvor von der Leitstelle anhand der modifizierten standar-disierten Notrufabfrage „Brustschmerz“ in die Gruppen ACS 1 (= stabil) und ACS 2 (= kritisch) klassifiziert wurden. Beide Gruppen wurden hinsichtlich Altersstruk-tur, Geschlechterverteilung und Vitalparametern verglichen. Der präklinische Zu-stand wurde mittels eines für die Studie entwickelten Scoresystems auf einer Skala von 0 - 20 Punkten eingeschätzt und ein Cut-Off-Wert für einen kritischen Zustand auf ≥3 festgelegt. Sensitivität und Spezifität der Notrufabfrage wurden im Vergleich zur Einteilung durch den Score ausgewertet. ERGEBNISSE Von insgesamt 635 Fällen mit dem Meldebild „ACS“ konnten 595 Datensätze ausgewertet werden (93,7%). Die Gruppe ACS 1 war mit n=101 Personen deut-lich kleiner als die Gruppe ACS 2 (n=494). Im Vergleich von Altersstruktur und Geschlechterverteilung zwischen den beiden Gruppen konnten keine statistisch signifikanten Unterschiede festgestellt werden, ebenso in den Durchschnittswer-ten der erhobenen Vitalparameter. In 88% der Fälle wurden vital bedrohte Pati-ent*innen auch als solche erkannt (Sensitivität), die Spezifität der Notrufabfrage lag hingegen bei 18,7%. DISKUSSION In der hier vorgestellten Studie wurde erstmals eine modifizierte standardisierte Notrufabfrage zum Meldebild „Akutes Koronarsyndrom“ evaluiert und hinsichtlich einer Voraussage des präklinischen Zustandes der Patient*innen ausgewertet. Mit 17% der Einsätze, die als stabil (ACS 1) zugeordnet wurden, könnte nach Implementierung eines Telenotarztsystems etwa jeder sechste Notarzteinsatz zu diesem Meldebild zugunsten von kritischeren Einsätzen vermieden werden. Die Sensitivität von 88% und Spezifität von 18,7% sprechen für eine Notrufabfrage, die auf eine möglichst hohe Patient*innensicherheit ausgelegt ist und nicht auf diagnostische Genauigkeit. Limitierend für die Aussagekraft dieser Studie ist das relativ kleine Patient*innenkollektiv und die ausschließliche Anwendung in einem Landkreis. Um die Ergebnisse dieser Studie zu bestätigen, sind weitere Studien mit einem größeren Studienkollektiv und dem Einschluss mehrerer Landkreise mit unterschiedlicher demographischer Struktur notwendig. In der Literatur gibt es bisher nur vereinzelt Auswertungen von standardisierten Notrufabfragen, auch weil diese in deutschen Leitstellen bisher nicht ubiquitär eingesetzt werden. Notrufabfragen, die eine Differenzierung in der medizinischen Notwendigkeit von Notarzteinsätzen ermöglichen, wurden bisher nicht unter-sucht. Die Ergebnisse dieser Arbeit sind ein erster Beitrag in der Neuausrichtung von Rettungsdienstabläufen und einer notwendigen Umstrukturierung in der Ver-teilung von zunehmend knappen notärztlichen Ressourcen. Weitere Studien müssen zeigen, ob sich dieser Ansatz auch auf andere Landkreise und weitere Meldebilder übertragen lässt. Offen bleibt auch, inwiefern die Spezifität und damit die Effektivität einer solchen Abfrage erhöht werden kann, ohne die Patient*in-nensicherheit zu gefährden.Background The German emergency medical services face an increasing physician shortage while the demand for time-critical emergency medical care is constantly rising. In order to guarantee a safe and quick distribution of emergency physicians to criti-cal ill patients, medically unnecessary missions must be reduced to prevent a lack of emergency care. As a first approach, a modified dispatch algorithm for acute chest pain has been put in place in the emergency medical services of the central Hesse region. In a second step, paramedic-staffed ambulances with phy-sician-based telemedical support could be dispatched to stable cases of patients with acute coronary syndrome, if the algorithm proves to be safe in the distinction between stable and critical cases. The goal of this study is to evaluate whether the modified dispatch algorithm for acute chest pain could predict, if the patient would be in a stable or critical condition in prehospital care. Method Between January and August 2021, data of 595 cases of patients with acute cor-onary syndrome has been collected. These cases have been categorized by the medical dispatch centre with the “modified dispatch algorithm for acute chest pain” into the groups ACS 1=stable condition and ACS 2=critical condition. Both groups have been analysed regarding age, sex and average vital parameters. The prehospital condition was evaluated by a score that has been developed for this study, with a scale from 0 to 20 and a cut-off at ≥3 for critical condition. Sen-sitivity and specificity of the modified dispatch algorithm have been calculated in relation to the score. Results Out of 635 cases of dispatched acute coronary syndromes, 595 data records have been analysed (93,7%). The group ACS 1 = stable was significantly smaller (n=101) than the group ACS 2 = critical (n=494). Both groups showed no statis-tically significant differences regarding age, sex or average vital parameters. In 88% of the recorded cases, critical ill patients were recognized as such, while the specificity of the dispatch algorithm was only at 18.7%. Discussion & Conclusion This study evaluated for the first time a modified dispatch algorithm for acute chest pain towards the precision in predicting a critical condition in prehospital care. With 17% of the missions that were safely assigned as stable, almost every fifth emergency physician mission to these kinds of cases could be avoided in favour of more critical missions after the full implementation of a telemedical sys-tem in prehospital emergency medicine. The sensitivity of 88% and specificity of 18,7% verify a dispatch algorithm that was designed for the highest possible pa-tient safety instead of diagnostic accuracy. The relatively small patient collective and the exclusive application in only one administrative district are limiting for the meaningfulness of this study. In order to confirm the results, further studies with a larger study collective and the inclusion of several districts with different demo-graphic structures are necessary. So far, there have only been few evaluations of standardized dispatch algorithms in the literature, mostly because they have not yet been used comprehensively in german dispatch centres. Standardized dispatch algorithms that allow differentiation of the medical necessity of emer-gency medical services have not yet been fully evaluated. The results of this work are a first contribution to the realignment of rescue service processes and a necessary restructuring in the distribution of scarce emergency staff resources. Further studies must show whether this approach can also be transferred to other administrative districts and other emergency cases. It also remains to be seen to what extent the specificity and thus the effectiveness of such a dispatch algorithm can be increased without endangering patient safety

    Development of an ab initio electrochemical cell: Understanding the dielectric properties of interfacial water and Mg dissolution from first principles

    Electrochemical process will play an important role in sustainable energy conversion and storage solutions. Examples are new generations of supercapacitors, metal-air-batteries, transient electronics or new concepts in sustainable metallurgy. In order to realize such disruptive innovations it is imperative to develop accurate modelling techniques and achieve an atomistic scale understanding of electrochemical processes. First principles techniques, which are free of empirical parameters, would be the optimum method. While experiments are routinely performed at constant electrode potential, realizing these conditions in ab initio simulations has remained very challenging. At present, none of the 40+ commonly used DFT codes provides a potentiostat scheme to explore charge transfer and reactions at electrified interfaces via ab initio molecular dynamics. A viable potentiostat technique requires two constituents: (i) a robust method to either apply an electric field or charge the electrode and (ii) an algorithm to treat the electrode charge as a thermodynamic degree of freedom in order to drive the system to the desired electrode potential. In this thesis, I introduce a novel method to include electric fields in electronic structure simulations, the fully solvated electrode (FSE). It allows us to realize applied electric fields of any distribution, shape and magnitude inside an ab initio simulation cell. In the spirit of the Helmholtz layer that is comprised of solvated ions, the FSE is able to place a fully solvated counter charge directly into the liquid phase. This counter charge compensates for the electrode charge, so that the simulation cell is charge-neutral in total, in order to enable a straightforward implementation into existing electronic structure codes. In order to derive a potential control algorithm, the electrode charge has to be treated as a thermodynamic degree of freedom. I discuss the challenges towards achieving potential control in electronic structure simulations and derive a canonical thermopotentiostat algorithm in order to determine the flow of charge into or out of the simulation cell at constant electrode potential conditions. Both the FSE and the thermopotentiostat can be straightforwardly included into standard ab initio DFT packages via a flexible, extendable python-interface which exposes the internal variables of standard DFT implementations to external python routines. The presented computational setup enables flexible user-defined workflows that use the DFT package as a "quantum engine". Moreover, the python interface provides a platform for future developments, such as, e.g., advanced implicit solvent models, QM/MM approaches and pH-control algorithms. In order to highlight the opportunities provided by this new approach and demonstrate its performance, I study the structure and dynamics of interfacial water at electrified surfaces at the atomistic-scale. Recent experiments demonstrated that nano-confined water features dielectric constants that are lowered by up to a factor 40, compared to bulk water. Because interfacial water serves as the environment for electrochemical reactions, understanding its screening properties is essential to study electron-transfer reactions. I reveal the origin of this strong reduction of the dielectric constant of nano-confined water via combined empirical and ab initio molecular dynamics at constant electrode potential: Interfacial water features a dielectric dead layer, within which most of the interfacial potential drop occurs and where the field is essentially unscreened. As a consequence, for sufficiently large fields water will screen the electric field via dissociation into H+ and OH-. In studies of electrochemical reactions with implicit solvents, the water dissociation effect is usually not considered, although it may considerably affect the mechanisms of electrochemical reactions. Combining the FSE and the thermopotentiostat, I studied the dissolution of Magnesium in water under externally applied fields. Contrary to existing models, the simulations demonstrate that water is not only a spectator, but acts as an active reactant. Moreover, the identified mechanism provides new insights towards the origin of the anomalous hydrogen-evolution reaction (HER) on Magnesium under anodic conditions. The developments presented in this thesis provide a ready-to-use simulation package. They open the door towards simulations of electrochemical and electro-catalytic processes from first principles to the broad community of computational scientists.Elektrochemische Reaktionen spielen eine wichtige Rolle bei der Umwandlung und Speicherung von umweltfreundlicher Energie. Wichtige Beispiele sind zum einen neue Generationen von Superkondensatoren, Metall-Luft-Batterien, transiente Elektronik oder neue Konzepte in der nachhaltigen Metallurgie. Um solche bahnbrechenden Innovationen zu realisieren, müssen präzise Modellierungstechniken entwickelt und elektrochemische Prozesse bis auf die atomare Skala verstanden werden. Techniken der "first principles", die ohne empirische Parametrisierung auskommen und allein auf der Lösung der stationären Schrödinger-Gleichung basieren, sind hierfür ideal. Während Experimente üblicherweise bei konstantem Elektrodenpotential durchgeführt werden, ist es eine große Herausforderung diese Bedingungen in ab initio Simulationen nachzubilden. Bis heute bietet keines der über 40 existierenden Dichtefunktionaltheorie (DFT)-Programme die Möglichkeit, einen Potentiostaten zu verwenden, um den Ladungstransfer und die damit verbundene Reaktion an geladenen Grenzflächen mit Hilfe von Molekulardynamik-Simulationen zu verstehen. Ein funktionsfähiger Potentiostat besteht aus zwei Komponenten: (i) einer robusten Methode, um entweder ein elektrisches Feld anzulegen oder eine Elektrode zu laden und (ii) einem Algorithmus um die Elektrodenladung als einen thermodynamischen Freiheitsgrad zu behandeln, so dass das System zum gewünschten Zielpotential geführt werden kann. In dieser Arbeit stelle ich eine neue Methode zum Anlegen elektrischer Felder in Elektronenstruktursimulationen vor, die "fully solvated electrode" (FSE). Sie erlaubt es, elektrische Felder von beliebiger Verteilung, Form und Stärke in einer ab initio Simulationszelle anzulegen. Analog zur Helmholtz Schicht, die aus gelösten Ionen besteht, ist die FSE in der Lage eine vollständig gelöste Gegenladung direkt in die flüssige Phase des Elektrolyten einzubringen. Diese Gegenladung gleicht die Elektrodenladung aus, so dass die Simulationszelle insgesamt ladungsneutral bleibt, um eine einfache Implementierung in existierende Elektronenstrukturprogramme zu ermöglichen. Um einen Potentialkontrollalgorithmus abzuleiten, muss die Elektrodenladung als thermodynamischer Freiheitsgrad behandelt werden. Ich diskutiere die Herausforderungen bei der Implementierung der Potentialkontrolle in Elektronenstrukturprogrammen und entwickle einen kanonischen Thermopotentiostat-Algorithmus um den Fluss von Ladung in und aus der Zelle heraus zu bestimmen. Der Thermopotentiostat und die FSE können beide durch eine erweiterbare Python-Schnittstelle, die die internen Variablen dieser Programme an externe Python-Routinen übergibt, unmittelbar in Standard-DFT-Pakete integriert werden. Das präsentierte computergestützte Setup ermöglicht benutzerdefinierte Routinen, die das DFT-Paket als "Quantum Engine" verwenden. Darüber hinaus bietet die Python-Schnittstelle eine Plattform für zukünftige Entwicklungen z.B. in Form von fortgeschrittenen impliziten Solvatationsmodellen, QM/MM Ansätzen und pH Kontrollalgorithmen. Um die Möglichkeiten dieser neuen Entwicklung aufzuzeigen, habe ich die Struktur und die Dynamik von Grenzflächenwasser an elektrifizierten Oberflächen auf atomarer Skala untersucht. Kürzlich durchgeführte Experimente haben gezeigt, dass auf Nanoskalen eingeschlossenes "nano-confined" Wasser eine um den Faktor 40 verringerte Dielektrizitätskonstante im Vergleich zu Volumenwasser aufweist. Da Grenzflächenwasser die Umgebung elektrochemischer Reaktionen bildet, ist Einsicht in seine Abschirmfähigkeit für das Verständnis von Elektronentransferreaktionen von enormer Bedeutung. Durch empirische und ab initio Molekulardynamik- Simulationen bei konstantem Elektrodenpotential konnte ich den Ursprung der enormen Reduktion der Dielektrizitätskonstante von "nano-confined" Wasser aufdecken: Grenzflächenwasser besitzt eine dielektrisch tote Schicht, in der der größte Teil des Potentials abfällt und in der das Feld im Wesentlichen nicht abgeschirmt wird. In der Konsequenz wird Wasser, für entsprechend große Felder, das elektrische Feld durch Dissoziation abschirmen und dabei H+ und OH- Ionen ausbilden. In Studien über elektrochemische Reaktionen, die implizite Solvatationsmodelle nutzen, wird der Effekt der Wasserdissoziation häufig nicht berücksichtigt, obwohl er einen erheblichen Einfluss auf die Mechanismen von elektrochemischen Reaktionen haben kann. Durch Kombination der FSE und des Thermopotentiostaten, untersuche ich die Auflösungsprozesse von Magnesium in Wasser unter angelegtem Feld. Im Widerspruch zu exisitierenden Modellen demonstrieren die Simulationen, dass Wasser nicht nur als Medium, sondern als aktiver Reaktant an der Reaktion beteiligt ist. Darüber hinaus liefert der identifizierte Mechanismus neue Einblicke in den Ursprung der anomalen Wasserstoff Evolution (HER) auf anodisch polarisiertem Magnesium. Die Entwicklungen, die in dieser Thesis präsentiert werden, stellen ein einsatzfertiges Simulationspaket bereit. Sie eröffnen die Möglichkeit Simulationen von elektrochemischen und elektrokatalytischen Reaktion ausgehend von "first principles" einer breiten Öffentlichkeit von computergestützt arbeitenden Wissenschaftlern zugänglich zu machen

    Optimal Monetary Policy in a Currency Union: Implications of a Country-specific Cost Channel

    There is growing empirical evidence that the strength of the cost channel of monetary policy differs across countries. Using a New Keynesian model of a two-country monetary union, we show how the introduction of a cost channel (differential) alters the optimal monetary responses to union-wide and national shocks. The cost channel makes monetary policy less e¤ective in combating inflation, but it is shown that the optimal response to the decline in effectiveness is a stronger use of the instrument. On the other hand, the larger the cost channel di¤erential, the less aggressive will the optimal monetary policy be. For almost all parameter constellations, our welfare analysis suggests a clear-cut ranking of policy regimes: commitment outperforms the Taylor rule, the Taylor rule outperforms strict inflation targeting, and strict inflation targeting outperforms discretion

    Attenuated psychotic symptoms, substance use and self-reported PTSD in adolescence

    Background: The occurrence of attenuated psychotic symptoms (APS) is a major concern in populations with substance use disorders (SUDs). However, APS also frequently develop in the course of Post-Traumatic Stress Disorder (PTSD). This study explores how the prevalence of APS differs between adolescent patients with only SUD, SUD with a history of traumatic experiences (TEs), and with SUD and self-reported PTSD.Methods: We recruited n = 120 treatment-seeking adolescents at a German outpatient clinic for adolescents with SUD. All participants filled out questionnaires assessing APS (PQ-16, YSR schizoid scale), trauma history, PTSD symptoms (both UCLA PTSD Index), and SUD severity (DUDIT) next to an extensive substance use interview. We performed a multivariate analysis of co-variance with the four PQ-16 scales and the YSR scale as outcomes and PTSD status as predictor. Additionally, we performed five linear regressions predicting each PQ-16 score and YSR score based on tobacco, alcohol, cannabis, ecstasy, amphetamine, and methamphetamine use.Results: Participants with co-occurring SUD and self-reported PTSD showed significantly higher APS prevalence rates (PQ-16 score, p = .00002), more disturbed thought content (p = .000004), more perceptual disturbances (p = .002), more negative symptoms (p = .004) and more thought problems (p = .001) compared to adolescents with SUD and a history of trauma and adolescents with only SUD. Past-year substance use was not predictive for APS prevalence (F(75) = 0.42; p = .86; R2 = .04).Conclusion: Our data suggests that the occurrence of APS in adolescents with SUD is better explained by co-occurring self-reported PTSD than by substance use frequency or substance class. This finding might indicate that APS might be reduced through treating PTSD or focusing on TEs in SUD therapy.Gefördert durch den Open-Access-Publikationsfonds der UB Marburg

    Virtual screening, molecular dynamics simulatons and chemoinformatics in pharmacology

    The Human Genome Project completed in 2003 has uncovered over 90% of the human genome. Since then, more and more omics data have become available. The exponential growth in the amount of biological data prompts progressive developments in computational methods for data analysis to solve various biological and pharmaceutical problems. One of the most revolutionary works is the development of AlphaFold, an open-access database that uses an artificial intelligence system to accurately predict protein structure from amino acid sequences. These structures allow scientists to better study and understand newly identified proteins and their mechanisms which can accelerate the drug discovery process. This thesis focuses on the application of different computational methods in attempts to solve different pharmacological problems. Chapter 1 introduces readers to the contents of this thesis and outlines the work done. The next chapter describes the basic concepts embodied in this thesis, including concepts of molecular dynamics and metadynamics simulations, virtual screenings, and chemoinformatics methods, priming the readers for the subsequent chapters. The first content chapter of the thesis illustrates the application of homology modelling, whole protein docking, molecular dynamics and metadynamics simulations to unravel pharmacological questions associated with β adrenergic receptors, specifically on selectivity, efficacy, and potency of the β receptor ligands. Proteins are dynamic objects that move and change with different ligands, effectors, and conditions. However, most structural data of protein only captured snapshots of the protein. This chapter highlights the importance of the dynamics of protein and shows that molecular dynamics simulation is an especially useful method in studying and predicting the dynamic effects of different ligands and effectors on the receptors. The next content chapter showcases the application of large-scale virtual screening to discover potential therapeutic compounds for factor IX. Structure-based screening of large-scale compound libraries has become a common practice in early drug discovery process to identify lead and probe compounds. This method is especially useful for novel pockets or targets where little to nothing is known about the binder or pocket. Factor IX is a protein necessary for the coagulation of blood and has been suggested as a target for anti-thrombotic therapy. In this part of the thesis, we targeted a novel allosteric pocket of factor IX, that was discovered by our collaborator, using a docking approach, in an attempt to find inhibitors of this protein and optimise the hit compounds as in the drug discovery process. The last content chapter of this thesis is on the generation of a targeted combinatorial virtual library. Virtual compound libraries are frequently used in the process of virtual screening. We generated a virtual di-peptide mimic library that can potentially mimic the action of di-peptides on protein and attempted validation of compounds in our library by docking to WD repeat-containing protein 5 (WDR5). Multiple chemoinformatics approaches were showcased in this chapter, from the comparison of two- and three-dimensional structures of compounds to computing various chemical properties that are important for small-molecule drugs.Im Rahmen des 2003 abgeschlossenen Humangenomprojekts wurden über 90 % des menschlichen Genoms entschlüsselt. Seitdem sind immer mehr "omics"-Daten verfügbar geworden. Das exponentielle Wachstum der biologischen Datenmenge führt zu einer fortschreitenden Entwicklung von Berechnungsmethoden für die Datenanalyse zur Lösung verschiedener biologischer und pharmazeutischer Probleme. Eine der revolutionärsten Arbeiten ist die Entwicklung von AlphaFold, einer frei zugänglichen Datenbank, die ein System der künstlichen Intelligenz zur genauen Vorhersage der Proteinstruktur aus Aminosäuresequenzen verwendet. Diese Strukturen ermöglichen es den Wissenschaftlern, neu identifizierte Proteine und ihre Mechanismen besser zu untersuchen und zu verstehen, was den Prozess der Arzneimittelentdeckung beschleunigen kann. Diese Arbeit befasst sich mit der Anwendung verschiedener Berechnungsmethoden bei der Lösung verschiedener pharmakologischer Probleme. Kapitel 1 führt den Leser in den Inhalt dieser Arbeit ein und gibt einen Überblick über die durchgeführten Arbeiten. Das nächste Kapitel beschreibt die grundlegenden Konzepte dieser Arbeit, einschließlich der Konzepte der Molekulardynamik- und Metadynamik-Simulationen, der virtuellen Screenings und der chemoinformatischen Methoden, um den Leser auf die nachfolgenden Kapitel vorzubereiten. Das erste inhaltliche Kapitel der Arbeit veranschaulicht die Anwendung von Homologiemodellierung, Docking an der gesamten Oberfläche des Proteins, Molekulardynamik- und Metadynamik-Simulationen zur Klärung pharmakologischer Fragen im Zusammenhang mit β-adrenergen Rezeptoren, insbesondere zur Selektivität, Wirksamkeit und Potenz der β-Rezeptor-Liganden. In diesem Kapitel wird die Bedeutung der Dynamik von Proteinen hervorgehoben und gezeigt, dass die Molekulardynamiksimulation eine besonders nützliche Methode zur Untersuchung und Vorhersage der dynamischen Auswirkungen verschiedener Liganden und Effektoren auf die Rezeptoren ist. Das nächste Kapitel befasst sich mit der Anwendung eines groß angelegten virtuellen Screenings zur Entdeckung potenzieller therapeutischer Verbindungen für Faktor IX. Das strukturbasierte Screening großer Substanzbibliotheken ist zu einer gängigen Praxis in der frühen Arzneimittelentdeckung geworden, um Leit- und Prüfsubstanzen zu identifizieren. Diese Methode ist besonders nützlich für neuartige Taschen oder Ziele, bei denen wenig bis gar nichts über potentielle Liganden oder die Tasche bekannt ist. Faktor IX ist ein Protein, das für die Blutgerinnung essenziell ist und als Ziel für eine antithrombotische Therapie vorgeschlagen wurde. In diesem Teil der Arbeit haben wir eine neuartige allosterische Tasche von Faktor IX, die von unserem Kollaborationspartner entdeckt wurde, mit Hilfe eines Docking-Ansatzes ins Visier genommen, um Inhibitoren dieses Proteins zu finden und die gefundenen Verbindungen bezüglich ihrer pharmakologischen Eigenschaften zu optimieren. Das letzte Kapitel dieser Arbeit befasst sich mit der Erstellung einer gezielten kombinatorischen virtuellen Bibliothek. Virtuelle Substanzbibliotheken werden häufig im Prozess des virtuellen Screenings eingesetzt. Wir haben eine virtuelle Dipeptidomimetika-Bibliothek erstellt, die potenziell die Wirkung von Dipeptiden auf Proteine nachahmen kann, und haben versucht, die Verbindungen in unserer Bibliothek durch Docking an das WD repeat-containing Protein 5 (WDR5) zu validieren.     Local Inflation Reconsidering the International Comovement of Inflation

    In this paper we reconsider the degree of international comovement of inflation rates. We use a dynamic hierarchical factor model that is able to decompose Consumer Price Index (CPI) inflation in a panel of countries into (i) a factor common to all inflation series and all countries, (ii) a factor specific to a given sub-section of the CPI, (iii) a country group-factor and (iv) a country-specific component. With its pyramidal structure, the model allows for the possibility that the global factor affects the country-group factor and other subordinated factors but not vice versa. Using quarterly data for industrialized and emerging economies from 1996 to 2011 we find that about two thirds of overall inflation volatility are due to country-specific determinants. For CPI inflation net of food and energy, the global factor and the CPI basketspecific factor account for less than 20% of inflation variation. We argue that "local inflation" rather than "global inflation" (Ciccarelli and Mojon (2010)) is a better description of the evidence.     Combined Minimally Invasive Mitral Valve Surgery and Percutaneous Coronary Intervention: A Hybrid Concept for Patients with Mitral Valve and Coronary Pathologies

    We evaluated the feasibility of hybrid percutaneous coronary intervention (PCI) and minimally invasive mitral valve surgery (MIMVS) in patients with concomitant coronary and mitral disease. Of 534 patients who underwent MIMVS at our institution between 2012 and 2018, those with combined mitral and single vessel coronary pathologies who underwent MIMVS and PCI were included. Patients were excluded if they had endocarditis or required emergency procedures. Preprocedural, procedural, and postprocedural data were retrospectively analyzed. In total, 10 patients (median age, 75 years; 7 males) with a median ejection fraction (EF) of 60% were included. Nine patients underwent PCI before and one after MIMVS. The success rate was 100% in both procedures. There were no postoperative myocardial infarctions or strokes. Two patients developed delirium and one required re-thoracotomy for bleeding. The median stay in intensive care and the hospital was 3 and 8 days, respectively. The 30-day survival rate was 100%. A hybrid PCI and MIMVS approach is feasible in patients with mitral valve and single vessel coronary disease. In combined pathologies, the revascularization strategy should be evaluated independent from the mitral valve pathology in the presence of MIMVS expertise.     Specifying and Assigning “Bundles of Rights” on Data: An Economic Perspective

    This paper shows that with respect to data the legal category of "property" (physical property or intellectual property) is not a suitable model for dealing with the complexity of data governance problems. Although data can be a valuable asset and might need incentives for its generation, the economic and technological conditions regarding data differ significantly from the problems regarding physical goods, innovation, and creative works. Through the much easier excludability and the greater importance of the non-rivalry and context-dependency of the use of data, not the incentive problem for data generation but the problems of access and sharing of data have become the most urgent policy issues. One particularly important reason can be the exclusive de facto control of firms over data, for which no formal property rights exist, but which can lead to market failures with respect to competition and innovation. The "bundle of rights" approach (based upon the economic theory of property rights) with its flexibility and wide range of options, how to specify these rights and to whom these rights can be assigned, offers a framework that allows for finding appropriate data governance solutions that fit to the specific economic and technological conditions of different industries and contexts. The paper also shows that current digital policy discussions can be interpreted as discussions about the proper specification and assignment of bundles of rights on data (as, e.g., with regard to the GDPR, data portability or the Digital Markets Act)

    The role of sports physicians in doping: a note on incentives

    How to ban the fraudulent use of performance-enhancing drugs is an issue in all professional – and increasingly in amateur – sports. The main effort in enforcing a “clean sport” has concentrated on proving an abuse of performance-enhancing drugs and on imposing sanctions on teams and athletes. An investigation started by Freiburg university hospital against two of its employees who had been working as physicians for a professional cycling team has drawn attention to another group of actors: physicians. It reveals a multi-layered contractual relations between sports teams, physicians, hospitals, and sports associations that provided string incentives for the two doctors to support the use performance-enhancing drugs. This paper argues that these misled incentives are not singular but a structural part of modern sports caused by cross effects between the labor market for sports medicine specialists (especially if they are researchers) and for professional athletes

    Diese Dissertation untersucht den spezifischen Charakter des mathematischen Erkennens aus der Perspektive der antiken und modernen Philosophie der Mathematik. Ihre vier Hauptthesen befassen sich mit Parallelen zwischen platonischem Ideenrealismus und aristotelischem Abstraktionsrealismus in Bezug auf Gleichheit als Grundlage des mathematischen Erkenntnisaktes (These 1a), dem erkenntnistheoretischen Spalt zwischen der sinnlichen Welt und mathematischen Objekten, der bereits in der Antike entstand (These 1b), wie moderne Denkschulen und ausgewählte Autoren mit den metaphysischen Grundlagen der Mathematik umgehen (These 2), und wie der epistemische Spalt potenziell überbrückt werden könnte (These 3). Das erste Kapitel untersucht die mathematikphilosophischen Positionen von Platon und Aristoteles. Es wird argumentiert, dass beide zentrale Parallelen im Hinblick auf die Anerkennung der Gleichheit von zunehmend komplexeren Strukturen für den Erkenntnisakt aufweisen, welche auch die Grundlage für das mathematische Erkennen ausmacht. Die spezifischen Modi der sinnlichen und intellektuellen Erkenntnisweisen unterscheiden sich in ihrer Klarheit der Objekterfassung. Es entsteht jedoch eine erkenntnistheoretische Lücke zwischen der sinnlichen Welt und mathematischen Objekten. Das zweite Kapitel betrachtet, wie diese Lücke in der Spätantike behandelt wurde, insbesondere in den Werken von Plotin und Proklos. Proklos versucht insbesondere, die platonische Lehre der Anamnesis mit Aristotelesʼ Konzept der Abstraktion zu integrieren. Durch die Abstraktion mathematischer Inhalte aus sinnlichen Darstellungen kann der diskursive Prozess der Wiedererinnerung ausgelöst werden. Das dritte Kapitel wendet sich der modernen Philosophie der Mathematik zu. Die drei großen Schulen des Logizismus, Intuitionismus und Formalismus werden in Bezug auf ihre Beziehung zu antiken Positionen und ihrem Umgang mit metaphysischen Grundlagen untersucht. Das Kapitel analysiert dann die platonistischen Ansichten der Mathematiker und Physiker Werner Heisenberg, Kurt Gödel, Roger Penrose und Alain Connes. Obwohl sich ihre Ansätze unterscheiden, erkennen sie alle an, dass Mathematik ohne metaphysische Grundlage nicht existieren kann. Das vierte Kapitel synthetisiert die vorhergehenden Analysen, um die vier Hauptthesen zu bewerten. Es wird argumentiert, dass Proklos eine integrierte Darstellung bietet, wie die Anerkennung der Gleichheit von zunehmender Komplexität sowohl bei Platon als auch bei Aristoteles als Grundlage des mathematischen Erkenntnisaktes fungiert. Die in These 1b identifizierte epistemische Lücke bleibt ein Problem für moderne Platonisten. These 2 zeigt, wie die drei modernen Schulen und einzelne Platonisten kritisch mit metaphysischen Annahmen umgehen. Die epistemische Lücke wird insofern überbrückt, als die physische Existenz sinnlicher Objekte und die metaphysische Existenz mathematischer Strukturen teilweise in deren Anwendung auf erstere zusammenfallen. Zusammenfassend zeigt die Dissertation die anhaltende Relevanz und Fruchtbarkeit des transversalen Vergleichs der antiken mathematischen Philosophie mit modernen Positionen, um tiefere Einblicke in den spezifischen Charakter des mathematischen Erkennens und dessen metaphysische Grundlagen zu gewinnen.This dissertation investigates the specific character of mathematical cognition from the perspective of ancient and modern philosophy of mathematics. Its four main theses address parallels between Platonic idealism and Aristotelian abstraction realism with respect to equality as a basis for mathematical cognition (thesis 1a), the epistemic gap between the sensible world and mathematical objects that arose already in antiquity (thesis 1b), how modern schools of thought engage with metaphysical foundations for mathematics (thesis 2), and how the epistemic gap could potentially be bridged (thesis 3). The first chapter examines the philosophies of mathematics of Plato and Aristotle. It argues that a central parallel exists between them in the recognition of equality of gradually increasing complexity, which provides the foundation for mathematical cognition. The specific modes of sensory and intelligible cognition differ in their clarity of apprehending objects. However, an epistemic gap emerges between the sensible world and mathematical objects. The second chapter considers how this gap was dealt with in late antiquity, examining especially works by Plotin and Proclus. Proclus in particular attempts to integrate the Platonic doctrine of anamnesis with Aristotleʼs concept of abstraction. By abstracting mathematical content from sensory representations, the discursive process of recollection can be triggered. The third chapter turns to modern philosophy of mathematics. The three major schools of logicism, intuitionism and formalism are examined in terms of their relation to ancient positions and their engagement with metaphysical foundations. The chapter then analyzes the Platonist views of mathematicians and physicists Werner Heisenberg, Kurt Gödel, Roger Penrose and Alain Connes. While their approaches differ, they all acknowledge mathematics cannot exist without metaphysical grounding. The fourth chapter synthesizes the preceding analyses to evaluate the four main theses. It argues that Proclus provides an integrated account of how the recognition of equality of increasing complexity operates in both Plato and Aristotle as the basis for mathematical cognition. The epistemic gap identified in thesis 1b remains a problem for modern Platonists. Thesis 2 reveals how the three modern schools and individual Platonists critically engage with metaphysical assumptions. Thesis 3 suggests a potential solution to the epistemic gap, drawing on concepts from antiquity and neurobiological approaches like that of Connes. The complexity of mathematical structures allows inferring their metaphysical existence insofar as they cohere with and correspond to the sensible world while only being partially applicable to it. Their complexity, coherence and correspondence provide justification for accepting their metaphysical truth and existence. The epistemic gap is bridged to the extent that the physical existence of sensible objects and metaphysical existence of mathematical structures partially coincide in the latterʼs application to the former. In summary, the dissertation demonstrates the ongoing relevance and fruitfulness of transversally comparing ancient mathematical philosophy with modern positions to gain deeper insight into the specific character of mathematical cognition and its metaphysical foundations
