    Case Report: When Does Puberty Become Traumatic?

    International audiencePuberty provokes physiological upheaval that can be psychologically traumatic and destabilizing for the child. Before the transformations of puberty, the body is a protective vessel that acts as a stable reference for the child. A child's emotional security is derived from a sense of predictability and well-being. However, the nascent sexuality and burgeoning libido experienced during puberty can trigger unsettling changes in the psycho-affective and psycho-dynamic equilibrium of the child as he or she transforms into an adolescent. This article presents puberty as a transformative experience with traumatic impact that needs to be considered in therapy conducted with adolescents. At best, pubescent trauma can cause superficial issues in a child's adaptive abilities; at worse, it can lead to pathological symptoms. This article presents a qualitative study derived from a clinical case of an adolescent girl who expresses her pubescent suffering through social withdrawal and mutism. The study determines several symptomatic and traumatic indicators caused by the sudden physiological transformations of puberty, such as perceived breaches in a child's sense of safety and the child's ability to predict. The study also explores the feelings of helplessness, vulnerability, and aloneness that pubescent adolescents endure, which are then exacerbated by the sensed inability to turn to parents for help or peers for support

    Management of anaphylaxis due to COVID‐19 vaccines in the elderly

    International audienceOlder adults, especially men and/or those with diabetes, hypertension, and/or obesity, are prone to severe COVID-19. In some countries, older adults, particularly those residing in nursing homes, have been prioritized to receive COVID-19 vaccines due to high risk of death. In very rare instances, the COVID-19 vaccines can induce anaphylaxis, and the management of anaphylaxis in older people should be considered carefully. An ARIA-EAACI-EuGMS (Allergic Rhinitis and its Impact on Asthma, European Academy of Allergy and Clinical Immunology, and European Geriatric Medicine Society) Working Group has proposed some recommendations for older adults receiving the COVID-19 vaccines. Anaphylaxis to COVID-19 vaccines is extremely rare (from 1 per 100,000 to 5 per million injections). Symptoms are similar in younger and older adults but they tend to be more severe in the older patients. Adrenaline is the mainstay treatment and should be readily available. A flowchart is proposed to manage anaphylaxis in the older patients

    The Importance of Developing Electrochemical Sensors Based on Molecularly Imprinted Polymers for a Rapid Detection of Antioxidants

    International audienceThis review aims to pin out the importance of developing a technique for rapid detection of antioxidants, based on molecular imprinting techniques. It covers three major areas that have made great progress over the years in the field of research, namely: antioxidants characterization, molecular imprinting and electrochemistry, alone or combined. It also reveals the importance of bringing these three areas together for a good evaluation of antioxidants in a simple or complex medium, based on selectivity and specificity. Although numerous studies have associated antioxidants with molecular imprinting, or antioxidants with electrochemistry, but even electrochemistry with molecular imprinting to valorize different compounds, the growing prominence of antioxidants in the food, medical, and paramedical sectors deserves to combine the three areas, which may lead to innovative industrial applications with satisfactory results for both manufacturers and consumers

    Affective temperaments of Lebanese patients with schizophrenia: comparison by gender and severity of psychosis

    International audienceObjectives: Our aim in this study was to identify affective temperament differences utilizing the TEMPS-A in a large sample size of Lebanese patients with schizophrenia and compare them to healthy controls. Gender differences, demographic factors and degree of psychotic symptoms were also considered. A cross-sectional study was conducted at the Psychiatric Hospital of the Cross (PHC) from March to June 2019. Two-hundred fifty chronic patients with schizophrenia were compared to 250 healthy controls randomly chosen from the general population. Results: Patients with schizophrenia significantly had higher mean depressive, cyclothymic, irritable and anxious temperament scores compared to healthy controls. Healthy controls significantly had a higher mean hyperthymic temperament score compared to patients with schizophrenia. In the group of patients with schizophrenia exclusively, females scored higher in terms of depressive, cyclothymic and anxious temperaments compared to males. In the group of healthy controls, males scored higher in terms of hyperthymic and irritable temperaments compared to females, whereas a higher mean depressive and anxious temperament scores were significantly found in females compared to males. In addition, higher PANSS total scores, as well as higher positive, negative and general subscales scores were significantly associated with higher depressive, cyclothymic, irritable and anxious temperament scores

    An Iterative and Collaborative End-to-End Methodology Applied to Digital Mental Health

    International audienceArtificial intelligence (AI) algorithms together with advances in data storage have recently made it possible to better characterize, predict, prevent, and treat a range of psychiatric illnesses. Amid the rapidly growing number of biological devices and the exponential accumulation of data in the mental health sector, the upcoming years are facing a need to homogenize research and development processes in academia as well as in the private sector and to centralize data into federalizing platforms. This has become even more important in light of the current global pandemic. Here, we propose an end-to-end methodology that optimizes and homogenizes digital research processes. Each step of the process is elaborated from project conception to knowledge extraction, with a focus on data analysis. The methodology is based on iterative processes, thus allowing an adaptation to the rate at which digital technologies evolve. The methodology also advocates for interdisciplinary (from mathematics to psychology) and intersectoral (from academia to the industry) collaborations to merge the gap between fundamental and applied research. We also pinpoint the ethical challenges and technical and human biases (from data recorded to the end user) associated with digital mental health. In conclusion, our work provides guidelines for upcoming digital mental health studies, which will accompany the translation of fundamental mental health research to digital technologies

    Dietary Patterns and Risk Factors of Frailty in Lebanese Older Adults

    International audienceFactors associated with frailty, particularly dietary patterns, are not fully understood in Mediterranean countries. This study aimed to investigate the association of data-driven dietary patterns with frailty prevalence in older Lebanese adults. We conducted a cross-sectional national study that included 352 participants above 60 years of age. Sociodemographic and health-related data were collected. Food frequency questionnaires were used to elaborate dietary patterns via the K-mean cluster analysis method. Frailty that accounted for 15% of the sample was twice as much in women (20%) than men (10%). Identified dietary patterns included a Westernized-type dietary pattern (WDP), a high intake/Mediterranean-type dietary pattern (HI-MEDDP), and a moderate intake/Mediterranean-type dietary pattern (MOD-MEDDP). In the multivariate analysis, age, waist to height ratio, polypharmacy, age-related conditions, and WDP were independently associated with frailty. In comparison to MOD-MEDDP, and after adjusting for covariates, adopting a WDP was strongly associated with a higher frailty prevalence in men (OR = 6.63, 95% (CI) (1.82–24.21) and in women (OR = 11.54, 95% (CI) (2.02–65.85). In conclusion, MOD-MEDDP was associated with the least prevalence of frailty, and WDP had the strongest association with frailty in this sample. In the Mediterranean sample, a diet far from the traditional one appears as the key deleterious determinant of frailt

    Balance, barycentremetry and external shape analysis in idiopathic scoliosis: What can the physician expect from it?

    International audienceObjective: Our objective was to establish a corridor of normality for the external shape 3D parameters and then to assess these variables in adolescent idiopathic scoliosis (AIS).Methods: Adolescent with mild and severe AIS were included prospectively, as well as a control group of asymptomatic subjects. A quasi‐automatic 3D reconstruction of the spine and manual 3D reconstruction of the external envelope was performed from biplanar radiography. The center of mass position, the axial intersegmental moment resulting at the apex and junctional vertebrae, and the coronal trunk balance were automatically computed. A normality corridor of asymptomatic subjects was calculated as the range [5th-95th percentiles] for external shape parameters at each vertebral level.Results: Forty-one asymptomatic subjects (19 females; 22 males; 21 yo, SD=4) and sixty AIS (56 females; 4 males; 13 years old, SD=1.9; 30 mild and 30 severe; 34 thoracic curves and 26 thoraco-lumbar or lumbar curves) were included. All parameters based on the external shape showed differences between AIS and controls, as well as between mild and severe scoliosis. For instance, the intersegmental moment applied to the upper junctional vertebra was above the 95th percentile of controls in 70% of AIS patient. The percentage of severe patients showing parameters higher than the normality corridor was significantly higher than mild patients (p<0.0001).Conclusions: The analysis of center of mass, vertebral intersegmental moment and coronal trunk balance parameters appear to be relevant in characterizing the 3D deformity of adolescent idiopathic scoliosis. The upper junctional intersegmental moment seems to be able to distinguish the different stages of curvature severity

    A generalized method for Sparse Partial Least Squares (Dual-SPLS): theory and applications

    National audienceIn data analysis, high dimensionality is often a delicate obstacle to overcome which can be solved by representing the data in a lower dimensional space using projection methods like the Partial Least Squares regression (PLS) or by resorting to variable selection methods like the lasso approach. The Sparse Partial Least Squares (SPLS) combines the two latter in order to better interpret the results due to the sparsity imposed on the new directions. Several implementations have been proposed. However, problems of accuracy of predictions and correct interpretation of regression coefficients arise in these approaches. Hence we developed the Dual Sparse Partial Least Squares, a flexible method that results in more accurate predictions and better interpre- tation of the coefficients due to their sparsity. In this paper we present the theory behind Dual-SPLS and some applicative results on petroleum data sets

    Electron transport in DNA bases: An extension of the Geant4-DNA Monte Carlo toolkit

    International audienceThe purpose of this work is to extend the Geant4-DNA Monte Carlo toolkit to include electron interactions with the four DNA bases using a set of cross sections recently implemented in Geant-DNA CPA100 models and available for liquid water. Electron interaction cross sections for elastic scattering, ionisation, and electronic excitation were calculated in the four DNA bases adenine, thymine, guanine and cytosine. The electron energy range is extended to include relativistic electrons. Elastic scattering cross sections were calculated using the independent atom model with amplitude derived from ELSEPA code. Relativistic Binary Encounter Bethe Vriens model was used to calculate ionisation cross sections. The electronic excitation cross sections calculations were based on the water cross sections following the same strategy used in CPA100 code. These were implemented within the Geant4-DNA option6 physics constructor to extend its capability of tracking electrons in DNA material in addition to liquid water. Since DNA nucleobases have different molecular structure than water it is important to perform more accurate simulations especially because DNA is considered the most radiosensitive structure in cells. Differential and integrated cross sections calculations were in good agreement with data from the literature for all DNA bases. Stopping power, range and inelastic mean free path calculations in the four DNA bases using this new extension of Geant4-DNA option6 are in good agreement with calculations done by other studies, especially for high energy electrons. Some deviations are shown at the low electron energy range, which could be attributed to the different interaction models. Comparison with water simulations shows obvious difference which emphasizes the need to include DNA bases cross sections in track structure codes for better estimation of radiation effects on biological material
