DMAIC Based Approach for Analyzing and Controllling Defective Product in Rubber Clutch Damper Product: [DMAIC Based Approach for Analyzing and Controllling Defective Product in Rubber Clutch Damper Product]
Rejects represent a major quality issue in the production process. If not effectively mitigated, they can increase production costs, disrupt process flow, and reduce customer satisfaction. This study aimed to minimize product rejection in rubber clutch damper manufacturing by applying the Six Sigma DMAIC (Define, Measure, Analyze, Improve, Control) methodology. In the Define phase, Critical to Quality (CTQ) characteristics were identified to determine key improvement priorities. The Measure phase involved data collection to calculate Defects per Million Opportunities (DPMO), sigma level, and the construction of a Pareto chart to highlight the dominant sources of defects. Subsequently, the Analyze phase employed Failure Mode and Effects Analysis (FMEA) to evaluate the Severity, Occurrence, and Detection of potential failures, leading to the determination of the Risk Priority Number (RPN) as a basis for improvement prioritization. In the Improve phase, corrective actions targeting the highest RPN factors were implemented, followed by the Control phase, which established sustainability through One Point Lessons and Standard Operating Procedures (SOPs). The implementation of the DMAIC framework resulted in a substantial improvement in process capability, indicated by an increase in sigma level from 4.11 to 4.58, and a reduction in the total production rejection rate from 1.51% to 0.31%. A paired t-test confirmed a statistically significant difference between pre- and post-improvement rejection rates (t = 61.63, p < 0.0001), leading to the rejection of the null hypothesis. These findings demonstrate that the applied improvements effectively reduced product defects and achieved a total rejection rate below the company’s management target of 0.40%. The study confirms the effectiveness of the DMAIC approach in enhancing product quality and operational efficiency within rubber component manufacturing.
Bahasa Indonesia Abstract: Permasalahan reject merupakan salah satu isu krusial dalam proses produksi yang apabila tidak segera ditangani, dapat berdampak pada peningkatan biaya produksi, keterlambatan pengiriman antarproses, serta penurunan tingkat kepuasan pelanggan. Penelitian ini bertujuan untuk menurunkan tingkat reject pada produk rubber clutch damper melalui penerapan metodologi Six Sigma dengan pendekatan DMAIC (Define, Measure, Analyze, Improve, Control). Pada tahap Define, dilakukan identifikasi Critical to Quality (CTQ) untuk menentukan karakteristik yang berpengaruh signifikan terhadap kualitas produk. Tahap Measure meliputi pengumpulan data produksi guna menghitung nilai Defects Per Million Opportunities (DPMO), tingkat sigma, serta penyusunan diagram Pareto untuk mengidentifikasi jenis cacat dengan kontribusi terbesar terhadap total reject. Selanjutnya, tahap Analyze dilaksanakan menggunakan metode Failure Mode and Effects Analysis (FMEA) dengan penilaian terhadap parameter Severity, Occurrence, dan Detection, yang dikalikan untuk memperoleh nilai Risk Priority Number (RPN) sebagai dasar prioritas tindakan perbaikan. Tahap Improve difokuskan pada penerapan solusi korektif terhadap faktor penyebab dengan nilai RPN tertinggi, sedangkan tahap Control menitikberatkan pada keberlanjutan hasil perbaikan melalui penyusunan One Point Lessons (OPL) dan Standard Operating Procedures (SOP). Penerapan metodologi DMAIC menghasilkan peningkatan kapabilitas proses dengan kenaikan nilai sigma dari 4,11 menjadi 4,58, serta penurunan rejection rate total produksi dari 1,51% menjadi 0,31%. Hasil uji paired t-test menunjukkan perbedaan yang signifikan antara tingkat rejection rate sebelum dan sesudah perbaikan (t = 61,63; p < 0,0001), sehingga hipotesis nol ditolak. Dengan demikian, penerapan DMAIC terbukti efektif dalam menurunkan tingkat reject secara signifikan hingga di bawah target manajemen sebesar 0,40%, sekaligus meningkatkan kualitas produk dan efisiensi proses produksi
Striking a Balance: Navigating Economic Growth and Sustainability in Indonesia’s Palm Oil Sector
Indonesia’s palm oil plantations have an irreplaceable role in Indonesia’s economy, contributing to job creation, rural poverty reduction, export revenue, and gross domestic product. Although such exploitation of its natural resources is within Indonesia’s permanent sovereignty, developed countries have expressed environmental concerns over Indonesia’s surge in palm oil expansion, which has brought about deforestation, biodiversity loss, and greenhouse gas emissions. This research will analyze whether Indonesia can navigate economic growth and sustainability in its palm oil sector, amidst such conflicting interests. Utilizing a normative legal research methodology with a statute approach, relevant laws, regulations, and practices will be used to identify whether Indonesia can achieve a sustainable palm oil industry that aligns with national development goals while also adhering to global environmental standards, ensuring both economic growth and sustainability
Anemia and Erythropoietin Use Among Hemodialysis Chronic Kidney Disease Patients at Rumah Sakit Umum Siloam
Background: Chronic kidney disease (CKD) affects an estimated 8-16% of the population and is increasing in prevalence. Anemia, a common and significant complication of CKD, is primarily caused by reduced erythropoietin production, which is essential for red blood cell production. Erythropoietin, a kidney-produced hormone, stimulates bone marrow to produce red blood cells. This study examines trends in the use of erythropoiesis-stimulating agents (ESAs) and the management of anemia in dialysis CKD patients before and after the implementation of ESA reimbursemen.Methods: This cohort study was conducted at Rumah Sakit Umum Siloam, Tangerang, Indonesia, from February to July 2017. Patients who received blood transfusions or iron supplements during the study were excluded. Data collected included age, gender, dry weight, history of diabetes mellitus, hypertension, hemodialysis adequacy, and nutritional status. Statistical analysis with a 95% confidence interval (CI) was used to assess the association between hemoglobin levels (Hb) and erythropoietin use.Results: Sixty patients completed the study. The proportion of anemic patients (Hb <10 g/dL) increased from 22 (36.7%) to 28 (46.7%) after erythropoietin administration. A mean dose of 6000 IU/week (CI: 4679 to 7321 IU/week) was effective in achieving target hemoglobin levels, while a dose of 4131 IU/week (CI: 3479 to 4782 IU/week) was sufficient to maintain them. Additionally, a dosage of 103.31 IU/kg/week increased hemoglobin by 1 g/dL in anemic patients.Conclusions: Erythropoietin use should be optimized given the increasing prevalence of anemia. A dosage of 103.31 IU/kg/week is recommended to achieve target hemoglobin levels, while 4131 IU/week is suggested for maintaining hemoglobin within the target range.
Sudden Onset of Diabetic Ketoacidosis in a Patient with Discordant HbA1C: A Unique Case of Fulminant Type 1 Diabetes Mellitus (FT1DM) in a Middle-aged Man
Introduction: Diabetic Ketoacidosis (DKA) is an acute complication of diabetes mellitus (DM) which present as the first manifestation in fulminant type 1 diabetes mellitus (FT1DM) without significant elevation of HbA1C. We present a unique case of a 52-years-old male with DKA without previous history of DM and HbA1c of 6.6.Case Illustration: A 52-years-old male was brought to hospital due to persistent vomiting. He experienced polyuria and unexplained weight loss but denied previous history of DM. He was diagnosed with DKA due to his blood glucose of 1107 mg/dL along with elevated blood ketone and metabolic acidosis. However, his HbA1c was a mere 6.6. His serum lipase was increased, consistent with pancreatic damage. He was then discharged with basal-bolus insulin. Discussion: Although the symptoms of this patient was consistent with hyperglycemic crisis, the presentation of DKA is usually associated with elevated HbA1c, approximately 10.4 - 16.9%. This unusual HbA1c points to the diagnosis of FT1DM. It is caused by sudden beta-cell destruction triggered by viral infections, alongside genetic disposition, leading to sudden depletion of insulin occurred less than a week and presents with ketoacidosis. It is characterized by low HbA1c (< 8.7%) yet extremely high blood glucose (‰¥ 288 mg/dl) and elevated pancreatic enzymes (depicting pancreatic damage). A diagnosis of FT1DM can be ascertained in this patient as his presentations matches its characteristics.Conclusion: FT1DM is a sub-type of T1DM which can suddenly occur in patients and associated with discordant HbA1c. It is important to recognize and treat it accordingly to avoid fatal outcomes
Pap Smear Practices among Female Healthcare Professionals in Indonesia and Their Associated Factors
Introduction : Cervical cancer is the fourth most frequent disease in women worldwide. However, pap smear coverage in Indonesia remains below the target. The purpose of this study was to describe pap smear practice among female healthcare professionals and its associated factors.
Methods : This cross-sectional study was conducted from August 2022 to June 2023 among married female healthcare professionals of reproductive age who worked in Kelapa Dua district, Tangerang, Banten, Indonesia. Three general hospitals, two primary clinics, and one community health center were involved in this study. Data was collected using a self-administered questionnaire and analyzed using a chi-square test.
Results : From 236 respondents, only 80 respondents (33.9%) had pap smear tests. The most common reasons for not doing the test were lack of time and having not experienced any of the symptoms. Those who were doing the pap smear tests were likely to be more than 40 years old (OR 29.900; 95% CI 9.595 – 93.172; p<0.001), married for more than 10 years (OR 28.737; 95% CI 9.955 – 82.955; p<0.001), multiparous (OR 6.941; 95% CI 2.706 – 17.805; p<0.001), and have higher economic income (OR 7.333; 95% CI 1.443 – 37.274; p=0.020). Female healthcare professionals other than medical practitioners, nurses, or midwives were less likely to do the pap smear tests (OR 0.370; 95% CI 0.167 – 0.823; p=0.02).
Conclusion: Pap smear practice among female healthcare professionals in Indonesia was inadequate. Age, marriage duration, parity status, types of healthcare professionals, and socioeconomic status were significantly associated with pap smear practice
Superficial Spreading Cervical Squamous Cell Carcinoma to Endometrium: A Rare Case Report
Introduction: Squamous cell carcinoma (SCC) was the most common variant of cervical cancer with metastases potential to many organs, commonly parametrium and vagina, but in a rare condition can also spread to endometrium. Further evaluations are needed since superficial spreading of cervical SCC are related to poorer outcomes.
Case Presentation: A 62 y.o, post-menstrual, P3A0 women came with complaint of vaginal discharged mixed with blood since 6 months ago. Pap smear result suggestive of malignancy so the patient undergo guided biopsy from endocervix that revealed finding similar to carcinoma intraepithelial (CIN) 3. The patient underwent TAH-BSO operation. Microscopic examination from cervix showed tumor cell with round nuclei, prominent nucleoli, pleomorphic, hyperchromatic, increased nuclear-cytoplasm ratio in whole epithelial layer. Microscopic examination from endometrium showed epithelial squamous cell proliferation in almost 98% endometrial tissue and atypical cell nuclei, consistent with tumor founded in cervix. The final diagnosis was superficial endometrial spreading SCC with moderate differentiation. Immunohistochemistry (IHC) assay with p16 showed positive result in endometrium. Patient then consulted to gynecologist – oncology specialist for further treatment and evaluation.
Conclusion: Superficial spreading of cervical SCC to endometrium was a rare finding. Detection of SCC in endocervix can be considered as predictor for proximal spreading of cervical SCC so further examination like endometrial curretage are suggested to assist the early detection. IHC assay with p16 resulted positive in our patient suggesting that endometrial SCC in this patient was correlated with HPV infection that commonly find in cervical cancer

Bahasa Inggris adalah salah satu bahasa yang paling dikenal di dunia. Sebagai salah satu bahasa yang paling banyak digunakan di dunia, orang-orang mulai belajar bahasa Inggris di semua tingkatan. Namun, beberapa orang tidak beruntung dan tidak mendapatkan kesempatan untuk belajar bahasa Inggris secara resmi, itu sebabnya layanan masyarakat ini dilakukan. Dengan tim yang terdiri dari 4 orang, kami melakukan pengabdian masyarakat dengan mengajarkan Bahasa Inggris yang menyenangkan kepada anak-anak dari Panti Asuhan Vistos Kasih Ikhlas. Anak-anak bersenang-senang ketika belajar bahasa Inggris, sementara mereka juga fokus pada subjek. PRODUKSI BERBASIS GREEN ECONOMY: PENINGKATAN KAPASITAS DAN MANAJEMEN USAHA EKSTRAK JAHE DI KABUPATEN GRESIK
Mitra program ini adalah pengusaha eskstrak jahe di Desa Wates Kecamatan Balongpanggang Kabupaten Gresik. Tujuan program ini adalah meningkatkan kapasitas produksi dan meningkatkan keterampilan manajemen usaha mitra. Permasalahan mitra yaitu; peralatan produksi masih tradisional dan manual, proses dilakukan di lantai, produk berwarna lebih gelap dan rasa yang tidak seragam, proses pemasakan sering tumpah dan membahayakan tenaga kerja, kapasitas produksi masih rendah, tenaga kerja tidak menggunakan APD dan tidak standar; penjualan tidak stabil, kemasan menggunakan plastik tipis, belum memiliki pencatatan keuangan yang baik, dan belum memiliki label produk. Metode pelaksanaan kegiatan, yaitu; pengembangan TTG mesin ekstraksi dengan automasi, panel surya, pengadaan mesin parut, meja, pelatihan dan penampingan; operasi dan maintenance, manajemen usaha, keuangan, dan kemitraan. Hasil kegiatan ini, antara lain; tersedianya satu unit TTG mesin ekstraksi jahe dan mesin parut, pelatihan pengoperasian mesin dan maintenance ringan, pelatihan kesehatan pangan dan penggunaan APD, keuangan usaha, promosi produk dengan perbaikan kemasan, menjalin kemitraan dengan marketplace tokopedia dan shopee, produk mitra juga sudah tersedia di beberapa toko dan kantor pemerintahan di Gresik. Dampak kegiatan ini bagi mitra, antara lain adanya peningkatan omset penjualan sebesar 20%, peningkatan kapasitas produksi sebesar 35%, penambahan tenaga kerja sebanyak 1 orang; dan peningkatan pengetahuan manajemen usaha sebesar 50%.
WORKSHOP ON THE MAKING OF TEA-BASED BURSTING BOBA AT SMA KRISTEN KARUNIA JAKARTA
Tea is one of the beverages widely consumed due to its functional aspects, yet its applications remain limited primarily to brewing. Another product that has gained significant popularity among the community, especially school-age children, is bursting popping boba due to its explosive sensation in the mouth. Bursting popping boba is produced using the spherification technique, a molecular gastronomy component. It is commonly based on sugar or syrup solutions. One approach that can enhance tea product diversification is transforming it into bursting popping boba. However, tea-based bursting popping boba has yet to be widely recognized by the public, exceptionally high school students. Hence, there is a need for a workshop on the production of bursting popping boba. This workshop was held on November 4, 2022, at SMA Kristen Karunia Jakarta. The Community Service Program activity was successfully executed and received positive feedback from the high school students of SMA Kristen Karunia Jakarta, as indicated by the questionnaire results. Specifically, 97.5% of the participants found this activity beneficial and informative. In comparison, 92.5% expressed interest in participating in such activities again.
IMPROVING DATA LITERACY AMONG JOURNALISTS THROUGH WORKSHOP AND CONFERENCE TO ADVANCE THE ANALYTICAL SKILLS IN REPORTING
In an extraordinary situation such as pandemic and disaster, journalists perform a beneficial function in presenting accurate and reliable information. Information is passed on to the public who are in dire need of guidance during an emergency situation especially when the public is overloaded with unreliable information. Therefore, journalists have an obligation to present truth and eliminate disinformation by providing fact-based storytelling to the public as stated in one of the elements of journalism. The practice of data journalism has become popular as we are becoming more familiar with the presence of data as well as the development of technology. Thus, improving data literacy among journalists has become essential. The creation of Data and Computational Journalism Conference and Workshop Indonesia (DCJ-CI) 2022 aims to enhance the data skills of journalists nationwide by presenting a series of hands-on workshops, talk shows, seminars, and networking sessions to journalists, journalism students and lecturers, as well as data enthusiasts in Indonesia. This event was attended by hundreds of participants throughout the country and joined by speakers and experts from countries like U.S., England, India, Singapore, Nepal, and Indonesia. DCJ-CI resulted in data journalism and computational products published in several media in Indonesia.
