THE ROLE OF PROBIOTIC ON AFFECTING ELDERLY’S MOOD WITH PARKINSONS DISEASE
Introduction: Parkinson’s Disease is a progressive disorder which affect the nervous system and cause movement problems in elderly. Parkinson’s disease has non-motor symptoms which include losing sense of smell, cognitive deficits, depression, constipation, and rapid eye movement sleep behaviour disorder. Constipation can also cause stress. Probiotics can help improve constipation in Parkinson’s disease patients. Fermented foods like pickles, yogurt, kimchi, and apple cider vinegar can be sources of probiotics. Moreover, probiotics can be consumed in the form of capsules, powder, and beverages as supplement. Objective: To know the role of probiotic on affecting elderly’s mood with Parkinson disease.Materials and Method : Journals and articles searching through PUBMED, google scholar, and other health information website between the years 2016-2024.Result: Gut brain axis mechanism can be activated by the accumulation of α-synuclein in the main pathway of the vagus nerve which lead to nerve degeneration. Dysfunction of the intestinal nervous system causes an imbalance of neurotransmitter regulation in the intestine, which then disturbs intestinal movement. In the other hand, neurotransmitter regulation imbalance can also cause stress or depression. Lactobacillus spp. and Bifidobacterium spp. can influenced normal gut flora which often used in depression therapy by reducing 5-HT metabolism that can improve cognitive function and other pathways. Conclusion: Probiotic may help constipation and depression in Parkinson’s disease patient through gut brain axis mechanism. The form of probiotic given is fermented milk rich in probiotics (yogurt) with rich-fiber foods such as whole wheat products, lentils, nuts and apricots
EARLY EXPERIENCE WITH TARIC TOTAL ANKLE ARTHROPLASTY, SUBTALAR FUSION, AND MODIFIED BROSTROM IN PATIENTS WITH RHEUMATOID ARTHRITIS
Rheumatoid arthritis (RA), a chronic inflammatory disease, primarily affects synovial joints, leading to progressive joint damage, pain, and physical limitations. (1) RA frequently involves the feet and ankles, where synovitis results in pain, stiffness, and joint destruction, often altering foot mechanics and heightening fall risk, thereby diminishing quality of life. (2) Although clinical exams help in identifying synovitis, their accuracy in RA assessment can be limited.(1) Historically, end-stage rheumatoid ankle arthritis has been managed with ankle arthrodesis to achieve pain relief and joint stability. (2) However, Total Ankle Arthroplasty (TAA) offers a promising alternative by preserving ankle motion and potentially mitigating osteoarthritis in adjacent joints.(3) This case report details a 57-year-old female with right ankle osteoarthritis due to RA, diagnosed eight years prior via rheumatoid factor positivity, successfully managed with a combined approach of TAA, subtalar fusion, and a modified Brostrom-Gould (BG) repair to enhance function and alleviate pain
THE RELATIONSHIP OF THE DEGREE OF DRY EYE SYNDROME WITH THE DEGREE OF DIABETIC NEUROPATHY AT GOTONG ROYONG HOSPITAL SURABAYA
Background: Dry eye syndrome is an uncomfortable condition in the eyesight caused by lubrication in the eyes and lack of moisture. The prevalence of dry eyes in Indonesia is 27.5% with an average age of more than 37 years. One of the risk factors for dry eye syndrome is diabetes mellitus. Dry eye syndrome is common in type 2 diabetes patients who experience polyneuropathy complications. Therefore, it is necessary to carry out further research regarding the relationship between dry eye syndrome and diabetic neuropathy. Objective: To determine the relationship between the degree of dry eye syndrome and the degree of diabetic neuropathy at the Gotong Royong Hospital Surabaya.Method: This research uses a correlation analytical research design with a cross sectional method. Sampling was carried out using a purposive sampling technique by looking for respondents according to predetermined criteria. Primary data was obtained through the results of examinations carried out on DM sufferers (Schirmer test and MNSI and MDNS questionnaires), while secondary data was obtained officially through medical records (HbA1c) from the Gotong Royong Hospital from August to October 2023. Data in the study were analyzed using tests Spearman's rho correlation. Results are significant if p<0.05.Results: The total sample was 71 patients who met the inclusion and exclusion criteria. The average age of patients was 60.4 years. More males (54.9%) than females (45.1%). There were 58 patients with a diagnosis of dry eye syndrome with diabetic neuropathy (72%), 10 patients with a diagnosis of dry eye syndrome without diabetic neuropathy (14.1%), and 13 patients with a diagnosis of diabetic neuropathy without dry eye syndrome ( 18.3%). In the analysis of the Spearman's rho correlation test, the value of p = 0.000 (p<0.05) was obtained. Conclusion: There is a relationship between the degree of dry eye syndrome and the degree of diabetic neuropathy at the Gotong Royong Hospital Surabaya
ASSOCIATION OF LONG-TERM MEDITERRANEAN DIETARY PATTERN AND DEPRESSIVE MOOD IN OLDER ADULTS: A SYSTEMATIC REVIEW OF COHORT STUDIES
Depression is a significant global health concern that occurs frequently among individuals aged 65 and older. In addition to pharmacological therapy and psychotherapy, dietary interventions play a crucial role in the multifactorial, biopsychosocial treatment of psychiatric disorders and could be used as a potential therapeutic target. Increasing evidence is emerging about the important role of the Mediterranean diet (MeDi) in the pathophysiology of mood disorders, particularly depression. This systematic review aims to assess whether long-term adherence to MeDi affects depressive symptoms in the elderly based on published cohort studies. Literature searches are conducted on several databases (PubMed, ProQuest, ScienceDirect, and Google Scholar) with keywords related to “Mediterranean diet”, “depression” and “elderly”. The research was done based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) protocol. Studies were then screened based on the inclusion criteria, with the result of 3 included cohort studies out of 5,123 identified studies. Study characteristics were extracted and the risk of bias was assessed with the Joanna Briggs Institute Critical Appraisal Checklist for each study. Based on the study analysis, the relationship between adherence to the Mediterranean diet and depression in elderly over time is inconclusive due to conflicting results and limited studies
SPONTANEOUS OGILVIE’S SYNDROME AND CHILAIDITI SYNDROME IN AN OCTOAGENARIAN FEMALE: A STUDY OF A RARE CASE WITH LITERATURE REVIEW
Introduction : Ogilvie’s syndrome, also known as acute colonic pseudo-obstruction (ACPO), is a unique type of colonic specificially cecum and ascending colon dilatation without underlying mechanical or anatomical causes.
Case Report : Eightytwo years old female patient was taken to the ER with enlarged stomach over the last two weeks which felt bigger last two days. Yesterday, was the last time defecated and were not other complaints. The patient has a history of hypertension and consuming antidyspeptic drugs. Physical examination showed compos mentis mental status, blood pressure of 140/80 mm of mercury, O2 saturation of 92%, other vital sign was normal. Abdomen examination showed obstruction bowel sign without tenderness, hypokalemia laboratory result, and the patient was treated conservatively. To our best knowledge, this is the first-ever case reported in English scientific literature from Indonesia.
Discussion : A clinical syndrome with symptoms, signs, and a radiological appearance of a major bowel blockage without any mechanical cause is known as acute colonic pseudo-obstruction (ACPO). We disclose a rare instance of Ogilvie's syndrome that was idiopathic, spontaneous, non-traumatic, non-infectious. Senility is the most probable risk factor in this case study, mostly in males. However, our patient was a female, eight decades, with Chilaiditi syndrome.
Conclusion : Timely identification Ogilvie’s syndrome is of crucial importance. Excessive age and metabolic cause seems to be the main risk factor in this case. More research is needed to identify the pathophysiologic pathways connected to aging that predispose to Ogilvie's disease

Results: The result showed that the group treated with the ethanolic extract of Calendula officinalis flower with concentration of 1:1 were more effective than the group treated with the ethanolic extract of Calendula officinalis flower with concentration of 1:2, the control group with Povidone Iodine, and the control group with distilled water. The result of Pairwise Comparison test which compared the group treated with the ethanolic extract of Calendula officinalis with concentration of 1:1 and concentration of 1:2 is in a significan value p=0.000 (p<0.05), which means that both treatment groups had a meaningful differences.
Conclusion: There was a significant effect between the group treated with the ethanolic extract of Calendula officinalis flower with concentration of 1:1 and the group treated with ethanolic extract of Calendulla officinalis flower with concentration of 1:2
COMPARISON OF THE QUALITY OF LIFE OF PATIENTS PRE AND POST SENILE CATARACT SURGERY WITH PHACOEMULSIFICATION TECHNIQUE AT PHC HOSPITAL SURABAYA
Background : Cataract is an eye disease that causes the clear lens to become cloudy. This results in visual impairment and reduces a person's quality of life. Cataracts impair a person's ability to see at a distance, resulting in impediments to mobility related to dependency such as the ability to recognize people, see and avoid obstacles on the road. Cataracts can also impair a person's ability to see at close range. Cataract patients can therefore experience difficulty in performing activities such as dressing, reading and sewing. One way to treat cataracts is by performing cataract surgery using the phacoemulsification technique. This surgery allows the lens to heal naturally without the need for stitches, allowing the patient to resume their daily activities and improve their quality of life.
Objective: To find the difference in quality of life between pre and post senile cataract surgery patients with phacoemulsification technique at PHC Surabaya Hospital.
Methods: Quality of life scores were taken using a quality of life research questionnaire for senile cataract patients containing 25 question points divided into 11 subscales. Data collection will be carried out twice, namely before phacoemulsification cataract surgery and 8 weeks after phacoemulsification cataract surgery.
Results: There is a significant difference between pre and post cataract surgery quality of life with phacoemulsification technique based on the quality of life research questionnaire for senile cataract patients at PHC Surabaya Hospital, with the result (P=0.000).
Conclusion: There is a significant difference between the quality of life pre and post phacoemulsification cataract surgery at PHC Surabaya Hospital based on the cataract patient quality of life research questionnaire
