ASSESSMENT OF READINESS OF GONDOKUSUMAN SUB-DISTRICT IN FILLING INTEGRATED PALLIATIVE CARE FOR OLDER PEOPLE
Background : Every year the number of elderly people in Indonesia continues to increase. In 2018 the highest number of elderly people was in the DI Yogyakarta Province, amounting to 12.37%. Data from Riskesdas showed that 51.28% of the elderly experienced health complaints and 25.99% of the elderly experienced illness. This can reduce the patient's quality of life. Therefore, integrated palliative care is needed for the elderly to improve the quality of life for patients and their families in dealing with health problems. Palliative care already exists in 5 hospitals in 5 regions in Indonesia, namely DKI Jakarta, DI Yogyakarta, Surabaya, Denpasar, and Makassar.Objective : The general objective of this study was to explore the readiness of Gondokusuman District in fulfilling integrated palliative care for the elderly.Method : This study used a thematic analysis research design. The research sample was 46 respondents. The data is taken from previous research entitled "Mapping Community Assets Related to Health and Social Services for the Elderly in Yogyakarta City in 2019".Result : An overview of health-related policies was found, such as the Chronic Disease Management Program (Prolanis), the Health Social Security Administration (BPJS), and Supplementary Food Provision (PMT). However, no policy description regarding palliative care has been found. A description of the availability of drugs was found which included prescribing, distribution and financing. Educational programs and content found in the form of education through counseling and counseling. Resources found are in the form of health infrastructure, community infrastructure, and human resources. However, there is no culture related to integration either vertically or horizontally.Conclusion : There is a description of health-related policies, availability of medicines, educational programs and content, and resources that support palliative care in Gondokusuman District. ARTIFICIAL INTELLIGENCE IN DETECTING ATHEROSCLEROSIS PLAQUES AND ELECTROMAGNETIC THERAPY TO ALTER CHOLESTEROL LEVEL
Introduction: Artificial Intelligence (AI) application in healthcare fields include matching patient symptoms to appropriate physician, helped to diagnose patients, determine patient’s prognosis, drug discovery, translating languages, and organize data. AI technology used in analysis and diagnosing coronary atherosclerotic plaques has increased annually. There are studies suggested that extremely low-frequency electromagnetic fields exposure can affect lipid metabolism.Purpose: To use applied AI in a device that can both identifying atherosclerosis in blood vessel and treat with low-frequency electromagnetic wave.Method: Experiments carried out using a device to detect and treat blockage of blood vessels by emitting electromagnetic signals. This tool is an AI breakthrough that not only includes a diagnostic function, but can also perform electromagnetic therapy.Result: Previous studies mentioned that AI can be applied to identify atherosclerosis. There are some studies that states low-frequency electromagnetic waves can alter HDL and cholesterol levels in blood. Larger studies are needed to combine both function in identify atherosclerosis plaques and electromagnetic therapy.Conclusion: Rapidly developing AI technology can be applied to identify atherosclerosis plaque and treat them. Further studies are required to provide definitive proof of electromagnetic wave effects on the removal of cholesterol from plaques using thermal effects or raising HDL concentrations in blood
ANTIBACTERIAL ACTIVITIES TEST OF RED GINGER (Zingiber officinale var. rubrum) ETHANOL EXTRACT ON STAPHYLOCOCCUS EPIDERMIDIS
Background: Antibiotic is a drug often used against infections caused by bacteria; however, irrational use of antibiotics can cause multidrug resistance (MDR). Staphylococcus epidermidis is a bacteria that has developed an MDR characteristic against antibiotics. Staphylococcus epidermidis is a normal opportunistic flora. Said bacteria can produce a biofilm that enables the bacteria to adhere to prosthetic devices and enter the bloodstream. Red ginger (Zingiber officinale var. rubrum) is a medicinal plant considered antibacterial for its secondary metabolites.Purpose: This study aims to determine the antibacterial effect of red ginger (Zingiber officinale var. rubrum) ethanol extract against Streptococcus epidermidis using a non-equivalent control group design study.Method: Red ginger (Zingiber officinale var. rubrum) ethanol extract was made with the maceration method. This study uses a microdilution test on 96 well plates. The absorbance value was read using a spectrophotometer to determine the Minimum Inhibitory Concentration (MIC) value, and streaking agar was used to find the Minimum Bactericidal Concentration (MBC) value.Result: The result shows that the red ginger (Zingiber officinale var. rubrum) ethanol extract at a concentration of 62,5 μg/mL inhibits 75% of bacteria growth, at 125 μg/mL inhibits 60% of bacteria growth, at 250 μg/mL inhibits 45% of bacteria growth, at 500μg/mL inhibits 65% of bacteria growth and at 1.000μg/mL inhibits 50% of bacteria growth. The streaking agar test result shows bacteria growth on every concentration.Conclusion: Red ginger (Zingiber officinale var. rubrum) ethanol extract in the concentration range of 62,5-1.000 μg/mL have no MIC and MBC value against Staphylococcus epidermidis.
CYANOTIC HEART DISEASE: AN OVERVIEW OF TETRALOGY OF FALLOT
Tetralogy of Fallot (TOF) is the most common cyanotic heart disease with anomalies consist of: ventricular septal defect (VSD), overriding aorta, pulmonary stenosis and hypertrophic of the right ventricular. It also has various anomalies type such as TOF with absent pulmonary valve, TOF with pulmonary atresia, TOF with atrioventricular septal defect, TOF with coronary artery anomalies, etc which will be discussed in detail. Proper diagnosis is required when dealing with the TOF continue with radiological examination. Adequate history taking and correct physical examination are necessary in dealing with congenital heart defects, especially TOF case. In this overview, we also discuss about the comprehensive management, from tet spell management, palliative management of TOF that required shunt with various technique, and surgical total corrective of TOF with many approach such as transannular accompany with right ventriculotomy or transatrial with transpulmonary or beating heart surgery technique to prevent reperfusion injury complication
CORRELATION OF SLEEP TIME WITH QUALITY OF LIFE IN ELDERLY AT MANDING TIMUR VILLAGE, SUMENEP
Background: In the elderly, there are anatomical, physiological, and psychological changes that can result in reduced sleep time in the elderly, and are thought to affect the quality of life of the elderly.Objective: This study aimed to determine the relationship between sleep time and quality of life in the elderly.Methods: This study used an observational analytic method with a cross-sectional approach. The sample in this study was 35 elderly people in East Manding Village. The sampling technique used was purposive sampling by determining the inclusion and exclusion criteria. The tool used is the WHOQOL-BREF test to measure the quality of life of the elderly and uses the sleep time provisions from the Ministry of Health (Ministry of Health) to measure sleep time in the elderly.Results: Data analysis used a contingency correlation test with a significance value of p<0.05. The correlation between sleep time and quality of life in the physical condition domain was positive, moderate, significant (r = 0.508 with p = 0.000); with the domain of psychological condition is positive, moderate strength, significant (r = 0.408 with p = 0.03); with the environmental factor domain being positive, moderate in strength, significant (r = 0.510 with p = 0.002); while in the domain of social relations there is no significant correlation.Conclusion: Sleep time is positively, moderately, and significantly correlated with quality of life in the domains of physical conditions, psychological conditions, and environmental factors. This means that the higher the sleep time, the better the quality of life in physical, psychological, and environmental conditions. These findings contribute to science and health promotion in geriatrics for the welfare of the elderly
LITERATURE REVIEW: Effectiveness of Chatbot to Reduce the Risk of Coronary Heart Disease using Android-based application
Background: An innovative approach in the form of a discussion platform designed to help users deal with health issues related to coronary artery disease. Chatbot platforms allow the collection of users' data, which is analyzed through natural language processing and behavioral analysis, to provide each user with a customized solution based on their current situation. The data collected and analyzed is accessible. The platform is developed using chatbot technology. Users can interact with chatbots to generate personal chat data stored on the platform. Conflicting information and sensitivity to Coronary Heart Disease (CHD) issues hinder effective communication. Recent technological solutions to maintain weight loss are limited. A chatbot would be suitable to support weight loss as it requires no human intervention, is available 24 hours a day, and supports natural communication while maintaining anonymity. The health system needs an effective and low-cost way to provide optimal health outcomes using conversation-enabled Artificial Intelligence (AI). Humans can interact well with this AI in the form of a fully automated and self-contained text-based mobile tutoring service. CHD is a serious health problem worldwide with multiple and interrelated causes. At the same time, chatbots are becoming more popular for interacting with users in mobile health apps.Objective: We evaluated an Android application. Its main goal is to prevent lifestyle-related diseases that are a risk for CHD, which has been considered at risk for multiple coronary artery disease (CAD), with the overarching goal of gaining compassion through mobile health improvements. The insights gained in this preview article will be used to plan future health care systems and to design an AI capable of advanced healthcare, chronic disease prevention, and self-treatment.Results: The Role of Artificial Intelligence in preventing Coronary Heart Disease (CHD) is done through health screenings. The app warns the user to exercise regularly and maintain food intake by reducing foods high in calories and adding foods high in fiber. Conclusion: Usage of AI in healthcare is associated with CHD prevention, which alters healthy lifestyles. It can also encourage a change in attitude, a high level of user concern for health, and obtain complete health information. Research on artificial intelligence and its use in telemedicine needs to be continued, with clinical trials examining the impact on blood pressure, body mass index, smoking, diabetes mellitus, and user engagement and feedback
STUDY CORRELATION BETWEEN BURDEN AND QUALITY OF LIFE OF DEPENDENT ELDERLY CAREGIVERS IN KLITREN, YOGYAKARTA
Introduction: Elderly with disability need caregiver’s assistance in their activities of daily living. The caregiver can experience burden during caring for dependent elderly. The burden can affect caregiver’s quality of life.Objectives: to measure and test the correlation between dependent elderly’s caregiver burden and quality of life in Klitren Kampong, Gondokusuman District, Yogyakarta Municipality. Materials and Methods: The subjects were primary informal caregivers aged ≥ 18 years with good cognitive functions who caring for the elders with ADL score <12. Respondents were selected using consecutive sampling technique. This is a quantitative research with cross-sectional study design. Zarit Burden Interview and Caregiver Reaction Assessment were used to assessing caregiver’s burden. WHO Quality of Life-BREF was used to assess caregiver’s quality of life. Data was analysed using spearman’s rank correlation. Results: There were 21 female study subjects. In each of the CRA and ZBI domains, the caregivers’ burden tend to be low (mean=8.63±10.730–72.42±10.983). Caregivers’ quality of life tend to be low (mean=49.76±11.962–60.76±11.726). There is a strong and statistically significant correlation between the impact on finance (p=0.000, ρ=-0.678) and impact on health (p=0.002, ρ=-0.602) domain on CRA with the environmental domain of WHOQoL-BREF. There is a strong and statistically significant correlation between the impact on schedule domain on CRA with the psychological domain of WHOQoL-BREF (p=0,000, ρ=-0.683). Conclusion: There are significant correlations between caregivers’ burden and quality of life especially on certain domains. Caregivers with higher burden have lower quality of life
DEMOGRAPHIC FACTORS AFFECTS THERAPEUTIC COMPLIANCE OF PATIENTS WITH CHRONIC MYELOCYTIC LEUKEMIA IN SURABAYA
Background: Leukemia is a hematologic malignancy that can affect all ages. In 2018, the death rate of leukemia in Indonesia reached 11,314 people. Imatinib is the first-line treatment for chronic myelocytic leukemia, which works by inhibiting the tyrosine kinase in the oncoprotein BCR-ABL. In Indonesia, the drug is the preferred therapy provided in hospitals and covered by the Social Security Administrator (BPJS). The side effects of these drugs are different from person to person, such as nausea, vomiting, headaches, indigestion, and joint pain. Compliance with taking medication is a determinant in the success of therapy, but several factors that influence compliance include age, gender, education level, and total income each monthPurpose: To compare the demographic factors that affect patients with chronic myelocytic leukemia in taking drugs in the Gist and Leukemia Community Surabaya.Method: This study used a Cross-Sectional Analytic research design. This research was conducted in September-November 2020 at the Gist and Leukemia Community Surabaya. The Gist and Leukemia Community in Surabaya is a community formed for sufferers of Chronic Myelocytic Leukemia and Gist (Tumors in the Stomach). The sample size required in the study, according to the Slovin formula, is 67 respondents. The sampling technique in this study is non-probability sampling so that this figure is met. Retrieval of patient demographic data and filling out the MMAS-8 questionnaire was done by filling in the form provided by the researcher in the form of a link on the google form. After that, short structured interviews were conducted with several respondents by telephone.Result: Based on the research that has been conducted, the results obtained are 67 respondents with high levels of compliance as many as 28 respondents, 22 respondents with moderate compliance, and 17 respondents with low compliance. With the number of male sex more than female, namely 40 men and 27 women. In addition, 20 respondents were aged 50-60 years, 22 respondents were aged 40-49 years old, 17 respondents were aged 30-39 years old, and eight respondents were aged 20-29 years old. In this study, the results obtained were 28 respondents with education levels of D3 / S1 / S2 / S3, 25 respondents with education levels of high school students, and 14 respondents with education levels of junior high school students. Also obtained the results of respondents with a monthly income of less than Rp 4,200,000 more, namely as many as 44 people compared to respondents with a monthly income, more than Rp 4,200,000 as many as 23 people. After obtaining the research data, the results of hypothesis testing using the Chi-Square test with sig. p <0.05. Results sig. p <0.05 means that the variable affects medication compliance in the respondent during sig. p> 0.05 means that the variable does not affect medication compliance among respondents. The results show that the factor influencing medication compliance in patients with chronic myelocytic leukemia is the monthly income with a sig. 0.025. Meanwhile, other factors such as gender, age, and education level do not affect medication compliance in patients with chronic myelocytic leukemia with sig. > 0.05.Conclusion: In the research that has been conducted with a total of 67 respondents, it can be concluded that Chronic Myelocytic Leukemia patients who are members of the Surabaya Gist and Leukemia Community for the period September-November 2020 are still in the category of obedience in consuming Imatinib drugs. TELEMEDICINE ACCEPTANCE AND USAGE IN JAKARTA METROPOLITAN AREA
Introduction: Implementing infection prevention protocols during the COVID-19 pandemic in Indonesia hinders the public from accessing health care services. This condition causes virtual health services to become more accessible than conventional health services, thus encouraging healthcare providers to adopt telemedicine as a method of service delivery. However, as a relatively new technology in Indonesia, public acceptance is crucial to predict the level of public willingness to adopt telemedicine. Yet, a little study in Indonesia has tried to assess it. Purposes: The purpose of this research is to measure the acceptability of telemedicine services in Jakarta metropolitan area, the factors that determine it, and whether the level of acceptance correlates with the usage frequency of telemedicine. Methods: Data used in this study is acquired with an online survey from February to March 2022, targeting residents of the Greater Jakarta area. Instruments used in this survey are demographic data form and SUTAQ questionnaire translated into Indonesian. Of the total 454 respondents, 200 have used telemedicine in the last six months. Respondents were then divided into several groups based on age, gender, monthly income, education level, chronic conditions, health insurance membership, and frequency of telemedicine use. Bivariate analysis was conducted using chi-square test for categorical data and t-test for numerical data. Results: The assessment of the SUTAQ scores indicates good acceptance of telemedicine in all demographic groups. Increased accessibility to health services and satisfactory user experience have contributed to its good acceptance. However, we also found that the frequency of use was not correlated with the level of acceptance, but positively correlated with the presence of chronic conditions. Conclusions: Telemedicine is well received by users who live in the Greater Jakarta area. Users find it easier to access health services and are satisfied with the health services provided through telemedicine
THE RELATIONSHIP OF PAIN LEVEL AND THE QUALITY OF PHYSICAL ACTIVITY IN PATIENTS OF KNEE OSTEOARTHRITIS
Background: Osteoarthritis is the most common disease of arthritis that often occurs in the hands, hips, and knees. OA results in degeneration of the joints and causes pain. OA accounts for 7% or approximately 500 million people worldwide in the global population. The prevalence of OA in Indonesia is relatively high, namely 15.5% in men and 12.7% in women. Complaints that are very often complained of by OA patients are pain when doing various daily activities. Pain felt by OA sufferers can affect the quality of physical activity in OA patients.Objective: This study aims to determine the relationship between pain level and the quality of physical activity in patients with osteoarthritis of the knee joint at PHC Surabaya Hospital.Methods: This study used an observational analytic study carried out with a cross-sectional approach. The sampling technique used was non-probability sampling, purposive sampling with respondents who met the inclusion and exclusion criteria. The population used in osteoarthritis sufferers at PHC Surabaya Hospital with 76 respondents. The measuring instrument used to measure pain is the Numerical Rating Scale (NRS), and the questionnaire used to assess the quality of physical activity is the WOMAC questionnaire.Results: The results of this study obtained a p-value 0.000 (p<0.005) and r = 0.922, which indicates that there is a significant relationship between the level of pain and the quality of physical activity in patients with osteoarthritis.Conclusion: there is a relationship between the level of pain and the quality of physical activity in patients with osteoarthritis
