    Effects of transcutaneous radiofrequency diathermy versus ultrasound on latent myofascial trigger points in the upper trapezius: a randomized crossover trial

    BACKGROUND: Currently, the use of radiofrequency diathermy for the treatment of neck pain is booming. OBJECTIVE: This study aimed to evaluate the clinical efficacy of Digital Capacitive Diathermy (DCD®) on stiffness, pain, cervical range of motion, and cervical disability and to compare it with ultrasound (US) in patients with latent myofascial trigger point (MTrP) in the upper trapezius. METHODS: Nineteen participants with latent MTrPs in the upper trapezius were included in the assessor-masked, randomized, clinical crossover trial. Subjects were exposed to both interventions: US and DCD® and treatment effectiveness was measured by myotonometric variables, pressure pain threshold (PPT), visual analog scale (VAS), cervical sidebending flexion ranges, and the neck disability index scale (NDI). RESULTS: There were no significant differences between US and DCD® interventions regarding changes in outcome measures. The US group achieved a statistically significant difference of 2.16 to 1.13 points (p = 0.005; r = 0.646) for the VAS. The DCD R intervention showed a statistically significant improvement of 1.11 points for the NDI at 1-week following intervention (95% CI 0.14–2.07; p = 0.27; d = 0.217). CONCLUSION: Our findings suggest that DCD R and US can both be considered effective modalities for the treatment of latent MTrPs, having a longer duration of action with DCD® therapy

    Quantifying Trunk Impact Dynamics and Workload with Inertial Sensors in Goalball Players

    The aim of this study was to quantify trunk impact dynamics in goalball players using inertial sensors and evaluate the goalball players’ workloads, focusing on changes between the first and second halves of a match to enhance understanding of the demands experienced throughout the game. Utilizing inertial technology, trunk impacts during goalball gameplay were analyzed to provide a holistic insight into how these impacts influence athletes’ dynamic stress loads, which refers to the physical demands placed on the body during dynamic movements. Six goalball players were recruited to wear an accelerometer during a whole goalball game to quantify trunk impacts. The results showed a higher number of total impacts and a higher number of impacts at higher levels during the first half, compared to a higher percentage of impacts at a low impact level in the second half. These results suggest that the intensity of gameplay is related to the number of impacts sustained, with players experiencing significantly more impacts, particularly at very low, low, and very high impact levels, in the first half compared to the second half. This decline in impacts aligns with the reduction in game intensity as the match progressed, as indicated by a lower heart rate and a trend towards a lower dynamic stress load in the second half. Future research could explore targeted training interventions aimed at optimizing workload and performance in goalball players

    Debating on the Rights of Crime Victims: A Legisprudential Examination of the Parliamentary Deliberation on the Spanish Victim Protection Act (versión manuscrito)

    This paper pursues a legisprudential analysis of the justificatory value and the rationality of the arguments used in the parliamentary process that led to the approval of the Act on crime victim protection (Law 4/2015). This legislative scenario proves interesting, on the one hand, because the recognition of the rights of victims seems at first glance to be a non-controversial topic; on the other hand, because the Spanish legislature had to implement a European framework directive that limited its leeway to regulate the underlying issues. After a brief victimological contextualisation of what has been labelled return of the to criminal policies (with a focus on the Spanish case), we analyse the rationality of the arguments used during deliberations and discuss to what extent these legislative debates were actually as conciliatory and consensus-oriented as some MPs rhetorically claimed in their parliamentary speeches. In this regard, special attention is paid to extremely contentious provisions such as Article 13, which regulates the participation of victims in the execution of criminal sentences, as well as to those parliamentary arguments that reveal political or ideological conflicts and opposing policy models in the field of crime victim protection

En este caso la prevención primaria consiste en prevenir la aparición del ictus, en la prevención secundaria se reduce el riesgo de sufrir un nuevo ictus y en la prevención terciaria se reducen las complicaciones y secuelas del ictus. La prevención en los tres niveles se basa en hábitos saludables entre los que destacan el descanso adecuado, control del tabaco, realización de actividad física de forma regular, ingesta de una dieta sana y equilibrada y la gestión del estrés. Disponer de esta información es clave para la población en materia de salud pública

    The increasing presence of Anisakis spp. in fish is having significant implications for public health due to a rise in cases of anisakiasis. Given this situation, there is a critical need to develop new strategies to fight this parasite. Satureja montana L., commonly known as savory, is a plant recognized in folk medicine for its therapeutic activity, such as being antispasmodic and digestive, among other properties. The aim of this study was to assess the nematicide activity against A. simplex larvae of the essential oil from two varieties of S. montana (subsp. montana (SMM) and variegata (SMV)). The essential oils were obtained via hydro-distillation of the flowering aerial parts. In vitro assays demonstrated the complete inactivation of anisakis larvae after 24 h when exposed to both essential oils, along with a significant reduction in their penetration capacity. Moreover, both essential oils showed an inhibitory effect on acetylcholinesterase (AChE). No differences between the subspecies were observed in any of the assays. Hence, the nematicidal activity of essential oils could be attributed to their capacity to inhibit AChE. These findings suggest the potential of S. montana essential oil for therapeutic and food industry applications

    Effects of different strength and velocity training programs on physical performance in youth futsal players

    Futsal is a high intensity team sport with intermittent actions of short duration, so it is necessary to include different training strategies to improve explosive actions. There is a gap in the scientific literature regarding training programs that improve the performance of young futsal players. The aim of this study was to determine the effects of different strength and velocity training programs on lower body physical performance in youth futsal players. Forty-two youth futsal players were divided into control group (CG, n=14) and a strength intervention group (SG, n=14), which included a weekly session for 8 weeks of eccentric strength training, plyometrics and core strengthening, and a velocity intervention group (VG, n=14), which included a weekly session during 8 weeks of training with linear speed exercises and with change of direction, accelerations with resistance bands and core strengthening. SG significantly improved horizontal jump (HJ) (p:0.02), V-CUT (p:0.91) and change of direction deficit (CODD)(p:0.01). VG showed significant improvements in HJ (p:0.01), in 25m sprint (p:0.01), in total repeated sprint ability time (p:0.01), in V-CUT (p:0.01) and in CODD (p:0.01). SG showed significant intergroup differences (p:0.01) in COD variables with respect to CG and VG. In conclusion, SG and VG showed significant improvements in lower body performance variables in youth futsal players. In addition, the SG has substantial changes in COD compared to the other two groups, so it has a greater effect.El fútbol sala es un deporte de equipo de alta intensidad con acciones intermitentes de corta duración, por lo que es necesario incluir diferentes estrategias de entrenamiento para mejorar las acciones explosivas. Existe un vacío en la literatura sobre programas de entrenamiento que mejoren el rendimiento de los jóvenes jugadores de fútbol sala. El objetivo de este estudio fue determinar los efectos de diferentes programas de entrenamiento de fuerza y velocidad sobre el rendimiento físico de la parte inferior del cuerpo en jugadores juveniles de fútbol sala. Cuarenta y dos jugadores de fútbol sala juvenil fueron divididos en un grupo de control (GC, n = 14), un grupo de intervención de fuerza (GE, n = 14) que durante 8 semanas realizó un entrenamiento semana de fuerza excéntrica, pliometría y fortalecimiento del tronco, y un grupo de intervención de velocidad (VG, n = 14), que incluía una sesión semanal durante 8 semanas de entrenamiento con ejercicios de velocidad lineal y cambio de dirección, aceleraciones con bandas de resistencia y fortalecimiento del tronco. El VG mejoró significativamente el salto horizontal (HJ) (p:0,02), el V-CUT (p:0,91) y el déficit de cambio de dirección (CODD) (p:0,01). También mostró mejoras significativas en el sprint de 25 m (p:0,01) y en el tiempo total de habilidad de sprint repetido (p:0,01). El SG mostró diferencias intergrupales significativas (p:0,01) en las variables COD con respecto a GC y VG. En conclusión, SG y VG mostraron mejoras en las variables de rendimiento de la parte inferior del cuerpo en jugadores juveniles de fútbol sala.     Estilo de vida de las personas supervivientes de un ictus: Actividad física y participación en la vida cotidiana

    A nivel mundial el número de personas supervivientes de un ictus ha aumentado en los últimos años, datos que también se reflejan en España. Se espera que siga incrementándose progresivamente este número si no se realizan intervenciones centradas en su prevención, así como enfocadas a reducir el impacto de sus secuelas. Se sabe que las personas supervivientes de un ictus tienen un estilo de vida sedentario y requieren de varios años para retomar la participación en algunas actividades de la vida cotidiana. Hasta ahora, no se ha prestado especial atención a las relaciones entre las variables de actividad física y participación, así como la experiencia individual de cada persona después del ictus y los factores que pueden influir en este estilo de vida. Además, no se disponen de herramientas de valoración que hayan demostrado buenas propiedades psicométricas para aportan datos sobre dichas variables. El objetivo principal de esta Tesis Doctoral fue explorar multidimensionalmente el estilo de vida de las personas con alto nivel de funcionalidad que habían tenido un ictus al menos hace 6 meses, en relación con su nivel de actividad física y participación en la vida cotidiana. A través de la literatura disponible y la situación de las personas que residen en diversas regiones de España, además de analizar las propiedades psicométricas de las herramientas de valoración para la obtención de estos datos. Para llevar a cabo este objetivo se realizaron varios estudios que aparecen reflejados en los artículos científicos. Los principales hallazgos de la presente Tesis Doctoral fueron: i) En los supervivientes de un ictus existe una correlación entre el tiempo de actividad física y la participación en las actividades de la vida diaria en participantes durante los primeros 6 meses después del ictus, y entre el tiempo de actividad física y la participación en todas las actividades de la vida cotidiana en participantes más allá de los 6 meses posteriores al ictus. ii) Existe una gran heterogeneidad en las investigaciones realizadas en supervivientes de un ictus focalizadas en analizar el nivel de actividad física y participación. iii) La herramienta de valoración The International Physical Activity Questionnaire-Short Form demostró una adecuada validez aparente, siendo recomendable que sea administrada por evaluadores con experiencia clínica y previamente entrenados. iv) The International Physical Activity Questionnaire-Short Form demostró una adecuada validez de constructo, presentando “moderadas” a “fuertes” con la pulsera de monitorización de actividad Fitbit Inspire 2, y correlaciones “débiles” a “moderadas” con el test de velocidad de la marcha 6-Metre Timed Walk, además la estabilidad test-retest The International Physical Activity Questionnaire-Short Form fue de “moderada” a “excelente”. v) The International Physical Activity Questionnaire-Short Form es un cuestionario autoinformado de bajo coste y rápida administración, válido para el cribado de los niveles de actividad física y el cumplimiento de los niveles de actividad física recomendados por la Organización Mundial de la Salud. vi) La herramienta de valoración The Satisfaction with Daily Occupations-Occupational Balance presentó una consistencia interna, validez convergente aceptable. vii) The Satisfaction with Daily Occupations-Occupational tuvo una buena fiabilidad test-retest, discriminó entre grupos con y sin cuidadores y no tuvo efectos de suelo o techo. viii) The Satisfaction with Daily Occupations-Occupational puede ser un instrumento de referencia para abordar el nivel de actividad, la satisfacción y el equilibrio en la participación en actividades en personas supervivientes de un ictus. ix) El nivel de actividad física autoinformada a través The International Physical Activity Questionnaire-Short Form y monitorizado a través del dispositivo Fitbit Inspire 2 en supervivientes de ictus con un alto nivel funcional tiene una correlación con las actividades instrumentales de la vida diaria medidas con Activity Card Sort y el nivel de participación medido con the Satisfaction with Daily Occupations-Occupational Balance. x) Las personas supervivientes de un ictus con un buen nivel funcional que residen en España destacaron la perspectiva individual sobre la readaptación a la actividad y ejercicio físicos. Destacaron la importancia de las secuelas que limitan esta readaptación, las preferencias individuales, el estilo de vida activa previo, así como las múltiples influencias del entorno sobre este comportamiento cuando las secuelas del ictus no limitan la deambulación y por tanto la posibilidad de movimiento. xi) El retorno a la participación tras un ictus es una experiencia individual compleja con influencias multifactoriales entre las que se resaltó la historia ocupacional previa al ictus, la percepción de uno mismo en el transcurso de la recuperación, la capacidad de aceptación y adaptación y apoyo del contexto familiar y social, así como la búsqueda de formas alternativas de adaptación al entorno. Los resultados incorporaron relevante conocimiento a la evidencia científica actual indicando que existe una relación entre el nivel de actividad física y participación, demostrando que hay herramientas de valoración capaces de abarcan la complejidad de ambos dominios sobre los que es importante contemplar la experiencia individual de cada persona. Sería recomendable que dicha relación y circunstancias individuales se consideren en la elaboración de futuros planes de intervención en personas supervivientes de un ictus con un alto nivel funcional además de analizar su influencia sobre los factores de riesgo modificables y la calidad de vida

    Early removal of the etonogestrel contraceptive implant in Spanish women: a prospective cohort study

    Purpose: To know the discontinuation rate and characterize predictors and reasons of contraceptive implant removal within 12 months of insertion in our community setting. Methods: This prospective cohort study included women receiving the etonogestrel contraceptive implant at sexual and reproductive health centers between September 2019 and September 2020. The variables collected were implanted insertion timing, reproductive and demographic characteristics, medical conditions, sexual activity and counseling. Our primary outcome was implant discontinuation. Kaplan–Meier survival curves were used to show the cumulative discontinuation rate of implants contraceptive within the first year of insertion. We also identified factors that increased the risk of implant removal using the log-rank test and the Cox regression model. Reasons for discontinuation were documented. Results: 199 women were followed up. Implant discontinuation was documented in 17.1% of implant users prior to 12 months. Factors that increase the risk of implant removal are living with a partner, being aged 25–34 years and not receiving comprehensive and structured counseling from the midwife. The main reason for removal was unsatisfactory bleeding (97.1%), but this was combined with other reasons such as cessation of sexual intercourse (58.8%), worsening mood (58.8%), weight gain (55.9%) or decreased libido (50.0%). Conclusion: The rate of discontinuous implant uses in the first year is relevant in relation to cost-effectiveness, there is room for improvement that should not be overlooked. Comprehensive and structured midwife-led counseling can reduce early implant abandonment removal. The development in different countries of the role of midwives in the management of contraceptives can contribute to the economic benefit of health services and the satisfaction of women
