    Prescription of ACE-Is/ARBs in patients with cardio-renal disease: a multicenter retrospective cohort study from the REPOSI registry

    Cardio-renal disease is a common clinical condition leading to increased morbidity and mortality. Angiotensin-converting enzyme inhibitors (ACE-Is) and angiotensin II receptor blockers (ARBs) are the cornerstone of treatment of chronic cardio-renal disease. Using data from the REPOSI register, we performed a multicenter, observational, retrospective study to determine which factors are associated with the non-prescription or discontinuation of ACE-Is/ARBS in a cohort of 889 cardio-renal patients hospitalized in 109 Italian internal medicine and geriatric wards. Only 55% of the patients with cardio-renal disease of the investigated cohort were on treatment with ACE-Is or ARBs at admission. The primary end point was ACE-Is/ARBs use at discharge. Patients with lower probability of receiving ACE-Is/ARBs at discharge were older and hospitalized for longer periods. Furthermore, patients with advanced chronic kidney disease (advanced CKD: eGFR = 60 mL/min/1.73m(2). A more prominent lower probability was found comparing advanced CKD patients with G3 stage CKD patients (eGFR: 59-30 mL/min/1.73m(2)) in multivariate analyses (OR and 95%CI: 0.37, 0.24-0.57. multivariate p-value < 0.001). The probability of stopping treatment in patients already on treatment with ACE-Is/ARBs at hospital admission (secondary end point) almost reached a threefold increase (OR and 95%CI: 2.82, 1.69-4.71. multivariate p-value < 0.001) when the advanced CKD group was compared with G3 CKD patients. The data of our study are not in line with the recently published updated KDIGO 2024 Guidelines, which recommend patients with advanced CKD to continue treatment with ACE-Is/ARBs

    Performance and evolution of materials for 3D construction printing: Application in extreme environments.

    Additive manufacturing is revolutionizing the construction sector, from traditional residential buildings to emergency shelters and even extreme environments, by simplifying on-site operations. A critical challenge in this field is the development of sustainable materials for additive manufacturing. The housing crisis in low-income countries de- mands high-performance materials with simple, low-cost production cycles and locally available resources. Furthermore, 3D printing applications in extreme environments, such as deserts, polar regions, and planetary habitats, including those on the Moon or Mars, require innovative and printable material solutions. Several reviews exist on specific 3D printing material typologies, but a comprehensive overview focusing on material performance and production pro- cesses is still lacking. Accordingly, this study pursues two main objectives: (1) to deliver a critical review and comprehensive overview of 3D printable construction materials, analyzing their historical evolution, production cycles, mechanical, physical, and thermal properties, as well as sustainability and com- patibility with 3D printing; (2) to integrate insights from the ArchiSpace project, funded by the Europe- an Union (MSCA Staff Exchange), which focuses on designing and construct- ing habitats in terrestrial analogs of planetary environments. A systematic literature review was conducted using an integrated approach combining qualitative analysis and bibliometric methods, offering a compre- hensive overview of the state of the art in the field. As a result, a major gap identified is the understanding of geopolymer matrix properties, particularly re- garding mix design, performances, and 3D printing processes. This dual focus on terrestrial and extraterrestrial applications highlights the transformative po- tential of 3D printing and provides a roadmap for future research and imple- mentations in traditional and extreme environments

    National Populism and Religion: The Case of Fratelli d’Italia and Vox

    Religion has become increasingly important in the discourse and ideology of the 'fourth wave' of the populist radical right which began in 2000 in Europe. To achieve its normalization in the political contest, these formations have shifted from openly racist positions to other arguments that, like religion, can be used to present their proposals in terms that are, at least apparently, democratic. This paper analyzes how Fratelli d'Italia and Vox appeal to religion in their efforts to construct national identity and differentiate from the "Other". To develop our research, we have carried out a qualitative analysis of the programs, founding documents, speeches, parliamentary interventions, interviews, and key messages of the leaders of both parties from their foundation until the European elections of June 2024. Despite the differences, the emergence of religion in a broad sense, as a form of a sacralization of politics, can be observed in both parties. In both cases, there is also a "politicization" of religion, which emerges as a secularized Christianity. Both parties appeal to a "Christian secularity", which, in their opinion, must be defended against Islam

    From fat to facts: Anthropometric references and centile curves for sum of skinfolds and waist-to-hip ratio in 2,507 adults

    Background and aims Direct assessment of skinfold thickness and waist and hip girths provides information about body fat and its distribution, avoiding estimation errors due to predictive equations. The present study aimed to provide new centile curves for the sum of eight skinfold thicknesses (Σ8SKF) and waist-to-hip ratio (WHR) in adult population, and to identify breakpoints during adulthood. Methods The present investigation was conceived as a multicenter, cross-sectional study. Stature, body mass, eight skinfold thicknesses (i.e., triceps, biceps, subscapular, iliac crest, supraspinal, abdominal, thigh, and calf) and waist and hip girths were measured according to the International Society for the Advancement of Kinanthropometry protocol in 1,313 men and 1,194 women aged 18–65 years. Smoothed age- and sex-specific percentile curves were generated using the Lambda Mu and Sigma method. For both sexes, simple linear regressions of the dependent variable (Σ8SKF and WHR) versus the explanatory variable (age) were performed to investigate changes in the response variable’s slope and to test for potential breakpoints.Results Reference percentile curves (3rd, 10th, 25th, 50th, 75th, 90th, and 97th) for Σ8SKF and WHR were provided. In men, Σ8SKF increased by 1.0mm/year between the ages of 21 and 59, while in women, it increased by 3.8mm/year between the ages of 38.5 and 47. In men, WHR showed a progressive increase of 0.004/year until the age of 28.4, followed by a slower increase of 0.003/year throughout the lifespan. In women, WHR increased by 0.003/year from the age of 20–65. Conclusions Σ8SKF and WHR appear sex- and age-specific. Scientists and practitioners are provided with reference values for the adult population

    What constitutes the ‘absolutely other’ for the human being in relational terms? How do nature and culture, relate to the human being and orientate his constitution? Are there insurmountable limits that distinguish humans from natural and artificial non-human? Starting with these research questions, we will trace theories and paradigms in the sociological and anthropological fields that from the 19th century to the present have generated and constructed fractured relationships -the result of dualist, stereotyped, and conflictual interpretations -between natural, cultural, and artificial. Continuing the analysis from a comparative perspective, contradictions and unanswered questions will be highlighted

    Carotid ultrasound: is it the best way to evaluate subclinical atherosclerosis?

    Carotid artery ultrasound is a simple, safe, non-invasive and cost-effective method to assess the presence or absence of carotid plaques and measure their extent. Measurement of intimal medial thickness (IMT) is the most widely used marker to quantify the burden of atherosclerotic disease at that level. All guidelines published to date are against routine population screening. Most guidelines recommend screening patients with multiple risk factors including diabetes, age > 65 years, smoking, or hypercholesterolemia. IMT measurements are extremely heterogeneous due to interindividual variability and some technical measurement concerns. Furthermore, increased IMT is also the result of non-atherosclerotic processes, such as smooth muscle cell hyperplasia and fibrocellular hypertrophy leading to medial hypertrophy and compensatory arterial remodeling. Nowadays, other ultrasound measurements such as the highest maximal wall thickness or the average maximal wall thickness have been studied and used to predict atherosclerotic burden other than IMT. This narrative review aims to examine the role of carotid ultrasound in the evaluation of patients with sub-clinical atherosclerosis and cardiovascular disease primary prevention
