Isopropyl alcohol as anti-emetic therapy in the emergency department: study protocol for a multi-center randomized controlled trial
Background: Nausea and vomiting is a common and distressing presenting complaint in Emergency Departments (EDs). There is no definite evidence to support the superiority of any anti-emetic therapy over another, or over placebo. Identification of an effective anti-emetic therapy in the ED setting with minimal side effects would be of great benefit. Isopropyl alcohol inhalation has been reported to be an effective treatment for post-operative nausea and vomiting, with no reported adverse events. The objective of this study is to determine if nasally inhaled isopropyl alcohol swabs are effective in alleviating nausea and/or vomiting in patients presenting to the ED with a chief complaint of nausea and/or vomiting.
Methods: We will conduct a randomized, controlled, multicenter trial with three subject arms: 1) nasally inhaled isopropyl alcohol swabs every 10 minutes for a total of one hour, 2) nasally inhaled isopropyl alcohol swabs every 20 minutes for a total of one hour, or 3) no intervention. 135 participants ≥18 years old and presenting to the ED with a chief complaint of nausea and/or vomiting with a level of 3 or greater on a verbal numeric rating scale (NRS) will be recruited for a duration of two hours. The primary outcome measure is the mean reduction in nausea scores comparing the pre-intervention score to the lowest post-intervention nausea score. The secondary outcome measures will be participant satisfaction scores using a verbal NRS, receipt of any rescue anti-emetic medications, ED length of stay, and participant disposition (admission or discharge home).
Discussion: This study will determine the efficacy of inhaled isopropyl alcohol swabs by determining the optimal dosing frequency achieving adequate anti-emetic action. This has the potential to guide future triage protocols to incorporate this therapy to provide earlier symptomatic relief to patients, and also has the potential to prevent morbidity suffered by patients in the emergency department and improving patient satisfaction and efficiently use in-patient resources. We strongly suspect that once this study is performed, it will be useful for ED physicians in treating nausea and vomiting in the ED
From Alienation to Revolution: Female Characters as Decolonial Tools in J.L. Torres’s The Accidental Native
In J.L. Torres’s The Accidental Native (2013), Rennie moves back to his native Puerto Rico after being raised in the United States. There, he meets his true biological mother, as well as his future (female) partner. Rennie’s unawareness of his mother’s existence and Puerto Rico’s colonial situation, as well as his partner’s illness, interweave maternal and romantic love with the Island. Furthermore, their ties pre-configure the birth of Rennie’s decolonial consciousness. This analysis dialogues with representations of the United States as an oppressive patriarchal figure to the Island, its inept child, in order to incorporate and contrast the role of the mother, as well as other affective figures, within the long tradition of genealogical metaphors in the Island’s literature. A decolonial interpretation of J.L. Torres’ work proposes a familial configuration in which affective forces originating from female characters function as a tool of epistemic liberation against imperialist, colonizing, patriarchal powers, thus proposing theoretical ties between affective and decolonial studies.En The Accidental Native (2013) de J.L. Torres, Rennie regresa a su natal Puerto Rico luego de haberse criado en los Estados Unidos. Allí, conocerá a su verdadera madre biológica y a su futura pareja (una mujer). Su ignorancia sobre la existencia de su madre, así como de la situación colonial de Puerto Rico, y la enfermedad de su pareja, entrelazan el amor maternal y sentimental con la isla. Dichos lazos afectivos pre-configurarán el nacimiento de la conciencia decolonial de Rennie. Este análisis dialoga con representaciones de los Estados Unidos como una figura patriarcal opresiva hacia la isla, su hijo inepto, para incorporar y contraponer el rol de la madre y otras figuras afectivas en la larga tradición de metáforas familiares en la literatura de la isla. Una interpretación decolonial del trabajo de Torres propone una configuración familiar en la que las fuerzas afectivas provenientes de personajes femeninos funcionan como herramienta de liberación epistémica en contra de poderes coloniales patriarcales e imperialistas, proponiendo así lazos teóricos entre los estudios afectivos y decoloniales

Parenting Consensual Non-Monogamists’ Perceptions of Parenthood, Intimacy, and Communication
This article presents partial results of a larger mixed methods study about consensual non-monogamists’ perceptions of their parenting and sexual lifestyle, particularly those involving the transition to parenthood, intimacy and communication. Using The Expanding the Movement for Empowerment and Reproductive Justice lens, six participants completed an online questionnaire and a semi-structured interview guide. Consensual non-monogamists are like other parents, yet they prioritise sexual intimacy over emotional intimacy. Communication is very important for them. The quantitative results support the qualitative ones: the participants had a higher level of parenting sense of competence, and the means for emotional and sexual intimacy were lower for primary partners than for secondary ones. Perinatal health care professionals including nurses need to know more about parenting consensual non-monogamists and their partner(s). More research is warranted with this particular group of parents, especially on their experiences of minority stress, resilience and taking a more intersectional research approach
Experience speaks: The importance of linguistic security during a public health crisis in immigrant and minority-language speakers
Linguistic insecurity, as faced by minority-language (ML) and immigrant communities, affects public health across the globe. This literature review addresses challenges faced by immigrants and ML speakers since the start of the SARS CoV-2 (COVID-19) pandemic. These groups sometimes experience obstacles when accessing healthcare, such as language barriers, mistrust in government, and lack of access to healthcare and information. Circulating misinformation may underlie many of the healthcare struggles faced by minorities, making them especially vulnerable to health disparities.We discuss some of the factors that affect immigrant and ML speaker experiences of the public health crisis, which include compliance, perceived support, stressors, vaccine hesitancy and willingness, and anti-expert and conspiratorial thinking. We make recommendations to improve public health through targeted research and linguistic security.L’insécurité linguistique, en particulier celle à laquelle restent confrontées les communautés de langue minoritaire (LM) et d’immigrants, affecte la santé publique partout au monde. Cette revue de la littérature discute des défis auxquels sont confrontés les immigrants et les locuteurs de LM depuis le début de la pandémie de SRAS CoV-2 (COVID-19). Ces groupes rencontrent parfois des défis lors de l’accès aux soins de santé, tels que les barrières linguistiques, la méfiance à l’égard du gouvernement, ainsi que le manque d’accès aux soins de santé et aux informations. La circulation d’informations erronées peut être à l’origine de nombreuses difficultés rencontrées par les minorités en matière de soins de santé, ce qui les rend particulièrement vulnérables aux disparités en matière de santé. Cet article s’intéresse à certains facteurs qui influencent l’expérience que les immigrants et les locuteurs de LM feront des crises de santé publique, notamment le respect des directives, le soutien perçu, les facteurs de stress, la réticence ou la volonté de vacciner, ainsi que la pensée anti-experts et conspirationniste. Nous formulons des recommandations pour améliorer la santé publique grâce à de la recherche ciblée et à la sécurité linguistique
