    EDEMA RESULTING FROM EXPERIMENTAL FILARIASIS

    Domestic cats and patas monkeys were infected with Brugia malayi so that the worms localized in the regional lymphatics of the hind legs. Reaction to the filarial parasites resulted in visible local edema in cats and disruption of normal lymph flow in the monkeys. Edematous tissue was examined grossly and by light and electron microscopy. Lymph flow patterns were examined by direct observation following injection of lymph staining dye and reflection of the skin, by X-ray following injection of radio-opaque contrast media, and by lymphscintigraphy after subcutaneous injection of radioisotopes. Clinical edema occurred in cats but not in monkeys. However, disruption of normal lymph flow in monkeys infected with Brugia could be demonstrated by lymphscintigraphy

    The process of lymphatic vascularization of the corpus luteum was examined during luteinization following the first and second ovulations in young rats. In the corpus luteum formed after the first ovulation, blood vessels permeate the luteal parenchyma which at this stage shows laminar organization. Thereafter, a nest of connective tissue appears in the central portion of the corpus luteum. At this time lymphatics are not found. Luteinization following the second ovulation is similar to that of the first generation until the later stages when lymphatic vessels start to appear initially in the peripheral zone of the corpus luteum and subsequently in the central connective tissue nest. These lymphatic vessels are lined with endothelial cells that stain darkly with toluidine dye

    From 1981 to 1984, 86 consecutive patients with previously untreated nonseminomatous testicular carcinoma were classified as clinical radiological stage I and treated with orchiectomy alone. The follow-up program included chest x-ray and lymphangiography (LAG) every month and abdominal computed tomography (CT) bimonthly. All patients were followed for 15 to 63 months after orchiectomy (median 32 mo.). Metastases developed in 23 patients (26.7%) and in 13/23 there was retroperitoneal lymphadenopathy. Time of relapse after orchiectomy ranged from 2 to 36 months (median 7 mo.) with a shorter interval for chest (4 mo.) compared with retroperitoneal metastases (7 mo.). Lung metastases were readily identified at an early stage (less than 2 cm) whereas more than one-third of retroperitoneal nodal metastases were greater than 5 cm at time of diagnosis. LAG detected metastases in 8/11 patients (72.7%), abdominal CT in 8/10 (80%), and both together (LAG and CT) 7/8 (87.5%). In clinical stage I nonseminomatous testicular carcinoma, the high incidence of concomitant but often asymptomatic regional and distant metastases and the relatively high cost and inconvenience of follow-up using abdominal CT imaging, LAG and chest x-ray suggest that orchiectomy is best combined with retroperitoneal node dissection at time of initial presentation to insure more accurate and safe staging of tumor dissemination

    Vascularized lymph node transfer is anew and promising technique for the physiologictreatment of lymphedema and severalclinical and experimental studies have beenconducted in recent years. However, the exactmechanism of vascularized lymph node transferis still unknown. In this study, we aimed toinvestigate treatment efficacy through thedesign of a simple and feasible experimentallymphedema model for testing a vascularizedlymph node transfer technique. After a pilotstudy, 30 male Sprague-Dawley rats weredivided into two groups and lymphedema wasinduced in the hindlimb of both groups. InGroup 1 (control) (n=15), no treatment wasapplied while pedicled lymph node transferwas applied in Group 2 (experiment) (n=15).Model dynamics were assessed with lymphoscintigraphy,limb measurement, andhistological analysis. A statistically significantlimb circumference reduction at the anklewas seen on days 30 and 90 in Group 2(24.4±1.05, 23.86±0.83mm) as compared toGroup 1 (25.93±1.38 and 25.4±1.12mm)(p0.05), and lymphatic tracer transportimproved in 13 out of the 15 animals in Group 2. A statistically significant reduction in histological scores was achieved in Group 2 (p0.05). In this rat hindlimb lymphedema model, our vascularized lymph node transfer technique is an effective physiologic surgical treatment and represents a feasible experimental model for future studies

    Two adult patients (one in Italy and the other inthe USA) are described with similar findings ofparaaortic nodal aplasia, asplenism, multiple serousand chylous effusions, and retroperitoneal lymphaticdysplasia. Although the clinical courses are incomplete,this unusual constellation of signs in thesetting of normal peripheral lymph trunks suggest anacquired rather than inborn anomaly and possibly avariant acquired immunodeficiency syndrome

    Calcium dobesilate ("Doxium", O.M.Laboratories, Geneva) was tested in two high-proteinedemas. While at a high dose level it had no effecton burn edema of the ratfoot, it did at a low dose. Itgreatly reduced (to 26o/o) acute lymphedema in therat thigh (although not in the foot). Electronmicroscopy confirmed these findings, and showed thatthe drug increased the number of macrophages in thetissues and reduced the protein concentrations. If,however, the macrophages were destroyed (by silica)this effect on acute lymphedema was lost (as withbenzo-pyrones).Calcium dobesilate also reduces high-proteinedemas in other models, and in many ways its actionsresemble those of benzo-pyrones. Hence it issuggested that this substituted benzene ring may bethe basic structure responsible for this action in highproteinedema, and that the pyrone ring (and anyside-chains) in the benzo-pyrones are not essential totheir enhancement of proteolytic activity bymacrophages
