Systematic referral to palliative care in patients attending dedicated emergency units from French comprehensive anticancer centres: the prospective multicentre randomised comparative clinical trial PALLU
International audienceIntroduction International medical oncology societies recommended early integration of palliative care to alleviate symptoms, improve quality of life and significantly reduce aggressiveness of care near the end of life, defined as the use of chemotherapy, high rate of hospitalisation/visits to emergency or intensive care unit in the last month of life or death in acute care unit. However, the most appropriate schedule for patient referral is still to be determined. Scores and criteria are debated, with uncontrolled symptoms (pain, dyspnoea, distress, etc) being one of the main indicators. These symptoms are also the leading causes for patients with cancer to seek care in emergency units. Several studies in North America suggested that referral to palliative care from emergency units is feasible and efficient. The aim of this study is to determine if systematic early referral to palliative care in a context of emergency reduces aggressiveness of care near the end of life compared with referral on medical team request. Methods and analysis This multicentric randomised study plans to enrol patients with a PALLIA-10 score >3 attending unscheduled on-site visits in French comprehensive cancer centres, and allocate them to randomisation (1:1) to receive palliative care systematically (experimental group) or on medical oncology team request (standard group). The primary objective is to compare the proportion of patients meeting at least one criterion of clinical care aggressiveness near the end of life, in each group. Secondary objectives include a description of clinical care aggressiveness components, palliative care requirement (psychologist, social worker, nutritional counselling, etc), patient-reported outcomes (FACT-G7, Hospital Anxiety and Depression Scale, Edmonton Symptoms Assessment System), the place of death and overall survival. Health economics and human and social sciences substudies will be presented. The study needs to include 192 patients (96 patients per arm) to reach a 20% decrease in care aggressiveness in the last month of life from 65% to 45%, with a statistical power of 80%, and a 2-sided type I error rate of 5%, using systematic referral. Considering a 20% rate of dropout or patients still alive at the time of analysis, 240 patients will need to be allocated to randomisation. Ethics and dissemination This clinical study received approval from the Ethics committee CPP Est III on 25 October 2023, and complies with the MR001 Commission Nationale de l’Informatique et des Libertés (#1994173, 27 September 2016). The first consent was signed on 12 September 2024. Results will be presented and published in international academic journals. Protocol identifier V.2.1 dated 10 July 2024.
This dissertation examines the combined impact of play and the use of the abacus on number learning in preschool, with a particular focus on number decomposition into two terms. The construction of the number concept, often perceived as abstract by early learners, requires adapted pedagogical approaches to foster understanding and manipulation of quantities. The abacus, as a concrete manipulative tool, enables both visual and kinesthetic anchoring of numerical decompositions. In parallel, play acts as a lever for motivation and engagement, facilitating the appropriation of mathematical knowledge. The study was based on an experiment conducted with three groups of kindergarten students, organized according to three modalities: instruction without specific tools, use of the abacus alone, and the combination of the abacus with playful activities. The results confirm that the abacus enhances the understanding of number decompositions and that its integration within a playful framework significantly boosts students’ engagement and success. These findings advocate for a thoughtful integration of play and manipulation in the teaching of mathematics in preschool settings, to promote more effective and lasting learning of fundamental concepts.
This dissertation explores the impact of a teaching sequence involving the use of children's books on gender representations in CE2 pupils. The study shows a positive progression in pupils' perceptions, with a reduction in stereotypes linked to activities, occupations and social roles. The results confirm that children's books, combined with classroom discussions and creative activities, can encourage critical thinking and a move towards more egalitarian models. Although some stereotypes persist, this research underlines the importance of ongoing, in-depth work on gender issues in order to anchor new perceptions in the long term.
International audienceThe duration of an occupation, the dating of a source, the construction of chronologies, or even the timescale of research itself — what are the sciences of the past, if not the attempt to understand and situate human history within time? While the notion of time is inherent to our disciplines, it can be conceived in many different ways depending on our fields of study, the nature of our sources, and the analytical tools we use. Beyond the mere establishment of chronologies and temporal frameworks, lies the challenge of apprehending and interrelating multiple, often complex, temporal scales. These questions represent a major methodological concern, but also provide an opportunity to approach the past from new perspectives, particularly through diachronic and interdisciplinary approaches. Moreover, the notion of time remains closely connected to other dimensions of our investigations — such as space and study area, environment, or socio-cultural context. From this perspective, several issues naturally arise: what are the temporalities of our objects of study, and how can we apprehend these different time scales? Do we approach a long-term occupation in the same way as a punctual event? How can we integrate discontinuities and combine data of diverse kinds? From a methodological standpoint, what tools do we employ to address these questions? How do dating methods and the relationship between relative and absolute chronologies come into play? Finally, we may also question the temporality of our own work and, more broadly, that of research itself.
Direct observation of calcite dissolution with microfluidics and spectral induced polarization. Geochemical and petrophysical modeling
International audienceCalcite is a very chemically reactive and common mineral in carbonate rocks like those often present in the critical zone. Its dissolution remains a physico-chemical process not completely understood at the scale of the grain and calcite dissolution effect on the measured low-frequency electrical response in geophysics is poorly known. In this study we show for the first time images of calcite grain dissolution in partially water saturation conditions from an innovative microfluidic experiment constraining a reactive transport model and a petrophysical model (Figure 1). Porosity and water conductivity were simulated. In addition to a deeper understanding of the calcite dissolution process and related electrical response due to charge carrier electromigration, our approach allows extracting from it cation exchange capacity and specific surface area, two parameters of uttermost importance in field geophysics. Our study published in [1] is a step further to understand and model the electrical response associated with mineral-water interaction at the pore scale

Edgelet Computing: Enabling Privacy-Preserving Decentralized Data Processing at the Network Edge
International audienceThe convergence of Opportunistic Networks and Trusted Execution Environments at the network edge presents a compelling opportunity for fully decentralized privacy-preserving data processing. Based on this convergence, we define the concept of Edgelet computing, a new paradigm for executing powerful and privacy-preserving distributed queries on personal devices. Our objective is to establish a robust, secure, and scalable execution framework with strong individual privacy guarantees. This paper first proposes a liability model tailored to decentralized executions on crowd members' devices, along with a query evaluation model that differs from the traditional database closed-world assumption. Second, it defines essential properties for ensuring the security, resiliency and validity of executions, and subsequently presents several methods and strategies for their enforcement. Through a comprehensive qualitative analysis and extensive evaluations, we showcase the relevance and effectiveness of the approach, demonstrating that Edgelet Computing holds potential for the emergence of novel and important classes of applications
Periodicity and longtime diffusion for mean field systems in
43 pagesInternational audienceWe study in this paper the longtime behavior of some large but finite populations of interacting stochastic differential equations whose (infinite population) limit Fokker-Planck PDE admits a stable periodic solution. We show that the empirical measure for the population of size stays close to the periodic solution, but with a random dephasing at the timescale that converges weakly to a Brownian motion with constant drift
