COVID-19 e Migrânea: 4 pacientes com migrânea sem aura e sua cefaleia na fase aguda da infecção.
Introdução
Cefaleia é sintoma da infecção por COVID-19, bem como febre, dor de garganta e tosse; pode ser sintoma inicial ou único. Séries de casos sugerem que características migranosas são comuns na fase aguda. Dor pode ser limitante e de difícil manejo.
Objetivos
Relatar evolução de 4 pacientes do sexo feminino, diagnóstico de migrânea sem aura (ICHD-3), e sua evolução e manejo (ambulatorial) na fase aguda.
Material e Métodos
4 pacientes, que concederam autorização por escrito para relato de seus casos. (1) 39 anos, migrânea episódica infrequente (MEI), previamente migrânea crônica (MC) com uso excessivo de analgésicos (UEA), topiramato até 125mg até há 1 ano, sem profilaxia atual. Crise intensa, características de migrânea, duração de 7 dias, inaugurando quadro infeccioso. Sem melhora com naproxeno/domperidona (3 doses), dipirona/metoclopramida EV. Melhora após 48h com prednisona 0,5mg/Kg, 5 dias. (2) 31a, MEI (previamente MC com UEA, tendo feito uso de topiramato até 50mg até há 8m e metoprolol 25mg suspenso há 2m). Tosse e febre; recebeu prednisona 1mg/Kg como tratamento para COVID pelo pneumologista. Não apresentou cefaleia. (3) 26a, migrânea episódica frequente (previamente MC com UEA, em uso de topiramato 150mg e magnésio 400mg). Cefaleia no 4o dia de sintomas, sem resposta a Zolmitriptano (3 doses), dipirona e metoclopramida EV. Melhora após 72h com prednisona 0,5mg/Kg. (4) 45a, MEI sem profilaxia. Cefaleia com características da migrânea, intensa, 3 dias, início no 5o dia. Sem melhora com dipirona (5 doses), naproxeno (2 doses).
Resultados
A cefaleia associada à infecção por COVID-19 em pacientes com migrânea é frequente e por vezes de difícil manejo. O abortivo usual do paciente por vezes não é efetivo e corticóide oral pode ser uma opção.
Conclusão
Corticoide pode ser opção no manejo da cefaleia na infecção aguda por COVID
Avaliação da associação das variantes – 819 C>T (rs 1800871) / -592 C>A (rs 1800872) e -1082 A>G (rs1800896) do gene IL-10 na migrânea
Introdução
A migrânea afeta em torno de 14,4% da população mundial. Caracteriza-se por episódios de dor pulsátil, unilateral e duração média de 4 a 72 horas com intensidade moderada a forte. Pode ser classificada em episódica ou crônica, com ou sem aura e pode estar associada a náuseas, vômitos, fonofobia e fotofobia. A fisiopatologia tem junção com ativação das fibras trigeminais e inflamação neurogênica, contudo precisa ser mais detalhada.
Objetivos
Avaliar a influência da variante -1082 G>A (rs1800896) e –819/-592 (rs 1800872) da IL-10 e seu efeito sobre a suscetibilidade e características clínicas da migrânea.
Material e Métodos
Estudo do tipo caso-controle composto por participantes com migrânea (n=39) e controles saudáveis (n=44) pareados para sexo, idade, etnia e IMC. Trabalho aprovado pelo Comitê de Ética em Pesquisa, no 98316718.7.0000.0020. Foi realizada entrevista estruturada e obtidos dados demográficos, clínicos, antropométricos e dados relacionados a migrânea e suas características. Os participantes responderam a questionários validados referentes a incapacidade da migrânea (MIDAS). Genotipagens foram feitas por PCR-SSP. Foi considerada diferença estatística quando p≤0,05.
Resultados
O genótipo TT/AA da variante –819/-592 foi associado a chance 90% menor de migrânea quando comparado ao genótipo CC/CC e CT/CA no modelo recessivo (OR=0,10; p=0,023). Não houve diferença na suscetibilidade a migrânea relacionada à variante -1082. Também não houve associação das variantes com a forma crônica da migrânea ou com a presença de aura.
Conclusões
A variante –819/-592 da IL-10, uma citocina anti-inflamatória, foi capaz de exercer efeito sobre a suscetibilidade da migrânea
Prevalência de enxaqueca entre universitários e seus familiares
Introdução
A genética da enxaqueca sem aura e a enxaqueca com aura está sendo estudada com auxílio de grandes estudos de associação de genomas. Entretanto, é necessário a caracterização da base genética de populações específicas.
Objetivo
Verificar se universitários com enxaqueca possuem susceptibilidade genética para esse distúrbio.
Material e Métodos
Estudo descritivo de corte transversal realizado entre estudantes universitários de uma universidade pública da Bahia no período de dezembro de 2020 a junho de 2021. Os questionários foram aplicados mediante convocação por redes sociais e aplicativos de mídia e preparados na plataforma FORMS (Google LLC®, Califórnia, EUA). Inicialmente eram direcionados para o Termo de Consentimento Livre e Esclarecido (TCLE) e somente após poderiam responder às questões. Essa pesquisa foi aprovada pelo Comitê de Ética e Pesquisa da Universidade do Estado da Bahia (UNEB) CAAE:38038720.5.0000.0057 – Parecer 4.351.53.
Resultados
Um total de 83 universitários que responderam ao questionário, 85,5% afirmaram sofrer de enxaqueca. A média de idade foi de 25 anos (+/- 6,9) e 81,9% eram do sexo feminino. A maioria (>80%) afirmou ser de naturalidade baiana, sendo que 41 % se autodeclararam de cor preta e 31,3% parda. Dos estudantes que informaram a presença de enxaqueca, 54,2% informaram ter histórico familiar de enxaqueca, sendo que a mãe foi mais citada para 33,7% deles, seguidos por tios (18,1%), irmãos (13,3%), pais (10,8%) e avós (4,8%).
Conclusões
Em jovens universitários da Bahia com enxaqueca, o histórico familiar foi presente em mais de 50%, indicando que a enxaqueca tem um importante componente genético neste grupo. Além disso, esse levantamento mostra a necessidade de caracterizar melhor os fatores genéticos influenciadores na ocorrência de enxaqueca nesses estudantes a fim de possibilitar estratégias terapêuticas mais efetivas, uso adequado de medicamentos e prevenção da cronificação desse tipo de cefaleia
The association between obesity and migraine and possible mechanisms of action: an integrative literature review
IntroductionObesity is a multifactorial disease and is defined by the excessive accumulation of adipose tissue that can cause harm to human health. The presence of obesity is an important risk factor for migraine chronification. However, not much is known about the link between the two diseases.
MethodsIn this study, an integrative literature review was conducted to better understand the mechanisms of interaction between migraine and obesity. Therefore, a search of PubMed and the Virtual Health Library (VHL) was performed with the following keywords: enxaqueca e obesidade; enxaqueca e obesidade e inflamação; enxaqueca e obesidade e neuropeptídeos; migraine and obesity; migraine and obesity and inflammation; migraine and obesity and neuropeptides.
ResultsThe search identified 22 articles. After reading and analyzing the articles, three thematic categories emerged: 1) Obesity as an Aggravating Factor for Migraine 2) Mechanisms Studied between Obesity and Migraine 3) The Effect of Weight Loss on Migraine Symptoms.
ConclusionsThe chronic low-grade inflammation associated with obesity can cause a predisposition to migraine chronification. The abnormal secretion of adipokines, dysregulation of the sympathetic nervous system, and hypothalamic dysfunction have been suggested to be the main shared mechanisms between both diseases
Immediate effect of a motor control exercise target to the neck muscles on upper cervical range of motion and motor control in patients with temporomandibular disorder
ObjectiveTo evaluate whether a single specific motor control training session for the neck flexor and deep extensor muscles improves upper cervical range of motion and neck motor control in patients with temporomandibular disorder (TMD) and compare them to a group without TMD. MethodsThis is a before and after, controlled study. The TMD group included women aged between 18-45 years old, complaining of pain in the orofacial region in the last 6 months and diagnosed with masticatory myofascial pain according to Research Diagnostic Criteria (RDC/MD). The control group included match-controls without TMD. The participants were evaluated to global and upper (Flexion Rotation Test - FRT) neck range of motion (ROM) and to neck motor control (Cranio-Cervical Flexion Test - CCFT). They were treated with a protocol of specific motor control exercises targeted to flexor and extensor neck muscles for 30 minutes. One day after the protocol the patients were reevaluated. ResultsA total of 23 volunteers were evaluated. The TMD group showed immediate improvement in left cervical rotation (p=0.043) and right FRT (p=0.036), while the control group did not show any improvement. There was no difference between the groups before and after treatment in relation to cervical movements. Regarding cervical motor control in both groups, the highest prevalence was of results between 24 and 26 mmHg after treatment, different from before the intervention (20 and 22 mmHg) in both groups.ConclusionA single session of specific neck motor control training only improved the left cervical rotation and upper right rotation in the TMD group, but not in the control group. There is no difference at the end of treatment between the groups. Volunteers with TMD showed improvement in the pattern of motor control of the neck when compared to volunteers without TMD
Prevalence of menstrually-related migraine in a population of students from a municipal university center in the interior of Brazil
ObjectiveTo identify the prevalence of migraine associated to menstruation in a population of students at a college in Brazil’s inland.MethodsThe students were recruited and submitted to the Test for the Identification of Women with Menstrual Period-Related Headache based on the ID-MigraineTM and Menstrual Migraine Assessment Tool questionnaires. Migraine diagnosis was made according to ICHD-3 criteria.ResultsOf the 424 students, 312 (73.5%) answered affirmatively in at least two of the four questions in the first part of the questionnaire and in the following 216 (69.2%) of these reported having migraine related to their menstrual period in the majority of the months.ConclusionsMenstrual migraine is a prevalent public health problem that deserves attention in order to ensure a better quality of life for women affected by such a condition
Migraine and premenstrual syndrome: comorbid disorders?
IntroductionHeadache is a common symptom among women, including during the menstrual cycle. The migraine frequency in women who present migraine associated with the menstrual period ranges from 50% to 70%. Premenstrual syndrome (PMS) is prevalent among women, affecting 80% to 90% of them throughout their lives.ObjectiveThe objectives of this study were to verify PMS prevalence and its characteristics among women who present with cephalalgia in the neurology ambulatory care unit and show the prevalence of headache and its association with PMS in the gynecology ambulatory care unit.MethodsIt is a descriptive and qualitative study which was carried out at Emilio Carlos Teaching Hospital in the neurology and gynecology ambulatory care units with women aged 18 to 52 years old. Eighty-seven questionnaires were distributed and self-applied throughout the year of 2018 for data collection. Each questionnaire consisted of 27 questions about the life cycle of the women and their headache episodes. The diagnostic criteria for headache and migraine from the International Headache Society were used. Criteria for PMS were met according to the quality of life questionnaire.ResultsIn gynecology unit group, 9% of the women did not present headache, 76% had PMS and 94% presented with headache during PMS. In neurology, 79% of the women had PMS and 79% of the women who presented with cephalalgia also had PMS.ConclusionThere is a large percentage of PMS in both groups, i.e. neurological unit and gynecological unit, showing it is not a spurious correlation
International Classification of Headache Disorders - ICHD-4. Will medication-overuse headache survive?

Cluster headache due to intranasal herpes simplex: a case report
Cluster headache is characterized by pain in the first division of trigeminal nerve along with autonomic features, with attacks last from 15 minutes to 180, up to eight times a day. Albeit considered as a primary headache, it may be mimicked by structural diseases like infections, inflammatory, tumoral and vascular. Intranasal and sinus infectious were also reported. Herpes simplex infections are quite common in the general population, and the nerve ganglia are the natural reservoir of the virus. Intranasal herpes, on the other hand, is exceedingly rare, with only few cases reported in the literature. Our main objective is to describe a case report of a 49-year-old male who was diagnosed with intranasal herpes infection during a bout of cluster headache, evaluated by an otolaryngologist. He got free of symptoms after using valacyclovir and melatonin. Thus, herpes simplex might be involved in the mechanisms of secondary or primary cluster headache. Further research is necessary to help elucidate this relationship
Influência das variantes genéticas +874T>A e -764G>C do Interferon-γ na suscetibilidade à migrânea
IntroduçãoA migrânea é um distúrbio neurológico incapacitante e geneticamente determinado. Níveis elevados do interferon-γ(IFN-γ), citocina pró-inflamatória, contribuem para desencadear a dor.
ObjetivoAvaliar o efeito das variantes genéticas +874 T>A e -764 G>C do IFN-γ e seu efeito sobre a suscetibilidade e sintomasassociados à migrânea.
Material e métodosEstudo prospectivo do tipo caso-controle composto por 144 participantes entre 18 e 60 anos diagnosticados commigrânea e controles saudáveis. Projeto aprovado pelo Comitê de Ética em Pesquisa da PUCPR (n°3.029.972). Osparticipantes foram avaliados e responderam a entrevista estruturada sobre dados demográficos, antropométricos,clínicos, classificação e sintomas associados à migrânea. Também preencheram questionários validados referentesà incapacidade relacionada à migrânea (MIDAS), ansiedade (STAIY2) e depressão (IDB). A variantes genéticasdo IFN-γ foram analisadas por meio da reação em cadeia da polimerase com primers sequência alelo específica(PCR-SSP) ou PCR em tempo real. Foi realizada análise univariada seguida de regressão logística binária. Foi consideradadiferença estatística quando p<0,05.
ResultadosForam avaliados 144 indivíduos, sendo 68 com migrânea e 76 controles pareados para sexo, idade, etnia e índicede massa corporal. Pacientes com genótipo GC, da variante -764, apresentaram menor chance de migrânea quandocomparados com o genótipo GG (OR=0,15; p=0,047), maior produtor da citocina. Esse resultado corroboracom estudos clínicos que demonstraram aumento do IFN-γ em indivíduos com migrânea, especialmente na fase dedor5,6. Entretanto, a variante +874T>A não se associou à suscetibilidade à migrânea. Também não houve diferençaestatística entre os diferentes genótipos de ambas as variantes analisadas quanto ao tipo, presença ou ausência deaura, fotofobia, fonofobia, osmofobia, alodinia, depressão ou incapacidade da migrânea.
ConclusãoIndivíduos heterozigotos (GC) para variante -764 G/A do IFN-γ apresentaram chance 85% menor de apresentaremmigrânea quando comparados aos homozigotos GG
