    Associação entre transtorno de déficit de atenção e hiperatividade (TDAH) e enxaqueca: uma revisão de estudos observacionais

    IntroductionAttention deficit hyperactivity disorder (ADHD) has been related to the presence of primary headaches. Among them, migraine presents a wide range of comorbidities shared with  ADHD, both from other psychiatric disorders and  somatic conditions.ObjectiveThis review proposes to describe the association between ADHD and migraine.MethodsBased on literature research in the major medical databases and using as descriptors “Migraine Disorders” and “Attention Deficit Disorder with Hyperactivity”. Observational studies that addressed the relationship between migraine and ADHD and written only in English were included. Of the 49 articles found, only 6 met the inclusion criteria and were analyzed.ConclusionThis review shows a possible sharing of symptoms between migraine and ADHD, requiring further studies to investigate this relationship.IntroduçãoO transtorno de déficit de atenção e hiperatividade (TDAH) tem sido relacionado à presença de dores de cabeça primárias. Entre eles, a enxaqueca apresenta uma ampla gama de comorbidades compartilhadas com o TDAH, tanto de outros transtornos psiquiátricos quanto de condições somáticas.ObjetivoEsta revisão propõe descrever a associação entre TDAH e enxaqueca.MétodosBaseado em pesquisa bibliográfica nas principais bases de dados médicas e utilizando como descritores “Migraine Disorders” AND “Attention Deficit Disorder with Hyperactivity”. Foram incluídos estudos observacionais que abordaram a relação entre enxaqueca e TDAH e escritos apenas em inglês. Dos 49 artigos encontrados, apenas 6 atenderam aos critérios de inclusão e foram analisados.ConclusãoEsta revisão mostra um possível compartilhamento de sintomas entre enxaqueca e TDAH, necessitando de mais estudos para investigar essa relação

    A classificação internacional de cefaleias sob o ponto de vista de Thomas Kuhn: Uma reflexão

    Headaches are characterized by a sensation of discomfort or pain in the cephalicregion. Due to the large number of types and subtypes of headaches, the standardization of their approach is necessary through the International Classification ofHeadache Disorders (ICHD), a document that is currently in its 3rd edition (ICHD-3).The evolution of the ICHD follows the hypothetical-deductive model of science basedon Karl Popper\u27s philosophy, which has a strong presence in the health sciences.The classification of headaches has evolved over time, incorporating and removingcriteria, reflecting the need to adapt the classification to constantly evolving scientificand clinical demands. However, some changes can generate discrepancies betweenclinical practice and the elaborated diagnostic tools. In this context, this article reflectson whether it would be advantageous to return to old principles and foundations ofobsolete classifications. The criterion of osmophobia, which is no longer in use inICHD-3, is highlighted. This reflection can be developed following Thomas Kuhn\u27s(1922-1996) view on science. In his main work, "The Structure of Scientific Revolutions"(1962), Kuhn proposes a three-stage model for science: the adoption of a paradigm,normal science, and a period of crisis. Kuhn also proposes the principle of incommensurability to understand the limitations and complexities of science. The applicationof this principle allows headache specialists to recognize that different approachesto headache classification have their own limitations and complexities, encouragingthem to consider a variety of perspectives, paradigms, and theories in approachingclinical cases and conducting scientific studies. An integrative approach that combinesPopper\u27s hypothetical-deductive model with Kuhn\u27s principle of incommensurabilityallows headache specialists to have a broader and more critical understanding ofheadache classifications.As cefaleias são caracterizadas por sensação de desconforto ou dor na extremidade cefálica. Em razão do grande número de tipos e subtipos de cefaleias, faz-se necessária a padronização da sua abordagem por meio da Classificação Internacional das Cefaleias (ICHD), documento que está hoje em sua 3ª edição (ICHD-3). A evolução da ICHD segue o modelo hipotético-dedutivo de ciência baseado na filosofia de Karl Popper, com grande presença nas ciências da área da saúde. A classificação das cefaleias evoluiu ao longo do tempo, incorporando e removendo critérios, refletindo a necessidade de se adequar a classificação às demandas científicas e clínicas em constante evolução. No entanto, algumas alterações podem gerar dissonâncias entre o exercício clínico e as ferramentas diagnósticas elaboradas. Nesse contexto, este artigo reflete se seria vantajoso a retomada de princípios e fundamentos antigos de classificações em desuso. Destaca-se o critério da osmofobia, hoje em desuso na ICHD-3. Essa reflexão pode ser desenvolvida seguindo a visão de Thomas Kuhn (1922-1996) sobre a ciência. Em sua principal obra, A estrutura das Revoluções Científicas (1962), Kuhn propõe um modelo de três estágios para a ciência: adoção de um paradigma, ciência normal e período de crise. Kuhn propõe ainda o princípio da incomensurabilidade para se entender as limitações e complexidades da ciência. A aplicação deste princípio permite aos cefaliatras reconhecerem que diferentes abordagens para a classificação de cefaleias têm suas próprias limitações e complexidades, incentivando-os a considerar uma variedade de perspectivas, paradigmas e teorias na abordagem dos casos clínicos e na condução de estudos científicos. Uma abordagem integrativa que combine o modelo hipotético-dedutivo de Popper com o princípio da incomensurabilidade de Kuhn, permite aos cefaliatras uma compreensão mais abrangente e crítica das classificações das cefaleias

    Affecting 1-2% of the world\u27s population and representing up to 40% of patients in specialist headache services, medication overuse headache remains a challenge. Starting with the need to make professionals aware since basic care, through patient education and ending with adequate (and successful) management, there is still much to be done. This literature review seeks to draw attention not only to the importance of recognizing the condition, but mainly to bring out the latest in the best management of patients who suffer from pain more than 14 days a month and who still see analgesics as the only way to manage their pain.Afetando 1-2% da população mundial e representado até 40% dos pacientes em serviços especializados em cefaleia, a cefaleia por uso excessivo de medicamentos segue um desafio. Iniciando pela necessidade de conscientizar os profissionais desde a atenção básica, passando pela educação do paciente e finalizando pelo adequado (e de sucesso) manejo, ainda há muito o que fazer na área. Esta revisão da literatura, busca alertar não apenas para a importância do reconhecimento da condição mas principalmente trazer o que há de mais recente no melhor manejo dos pacientes que padecem com dores mais de 14 dias por mês e que ainda vêem os analgésicos como única forma de tratamento

    IntroductionCluster headache and the more recently reported “related conditions”, namelyChronic Paroxysmal Hemicrania, Short-lasting, unilateral, Neuralgiform Headachewith Conjunctival Injection and Tearing (SUNCT) Syndrome and (possibly also)Hemicrania Continua, are one of the fascinating groups of conditions in Neurologywhose cardinal features are the almost absolute unilaterality of pain, its excruciatingseverity, besides the prominent mainly facial autonomic disturbances and overall theintriguing biorhythmicity.Beyond any doubt, the progress of our knowledge about themechanisms of these conditions has been considerable, but there is always room forreflection on where we are and where we can go.ObjectiveThe objectives of the present study are to analyze the metrics of publications onthe pathophysiology of these conditions, to explore in detail how proper the term”Trigeminal Autonomic Cephalgia” is, and to comment on their numerous synonyms.CommentMuch is needed to know the exact structures and circuitry involved in the pathophysiology of these conditions; accordingly, a non-compromising and just descriptive termmight be useful. Along this line of reasoning and bearing in mind the cardinal pointsof such conditions, namely, pain in the trigeminal territory, prominent autonomicsymptoms, prominent chronobiological features, and the excruciating character ofthe pain, a terminological possibility would be: Unilateral with Prominent Rhythmicityand Autonomic Symptoms Excrutiating Cephalgia (UPRASEC).IntroduçãoCefaleia em salvas e as “condições relacionadas” relatadas mais recentemente, nomeadamente Hemicrania Paroxística Crônica, Cefaleia Neuralgiforme Unilateral de Curta Duração com Síndrome de Injeção e Lacrimejamento Conjuntival (SUNCT) e (possivelmente também) Hemicrania Continua, são um dos fascinantes grupos de condições em Neurologia cujas características cardinais são a unilateralidade quase absoluta da dor, sua gravidade excruciante, além dos proeminentes distúrbios autonômicos principalmente faciais e, em geral, a intrigante biorritmicidade. Sem dúvida, o progresso de nosso conhecimento sobre os mecanismos dessas condições tem sido considerável, mas sempre há espaço para reflexão sobre onde estamos e para onde podemos ir.ObjetivoOs objetivos do presente estudo são analisar as métricas das publicações sobre a fisiopatologia dessas condições, explorar detalhadamente o quão adequado é o termo “Cefalgia Autonômica Trigeminal” e comentar seus numerosos sinônimos.ComentárioÉ preciso muito para conhecer as estruturas e circuitos exatos envolvidos na fisiopatologia dessas condições; consequentemente, um termo não comprometedor e apenas descritivo pode ser útil. Nessa linha de raciocínio e tendo em mente os pontos cardeais de tais condições, a saber, dor no território trigêmeo, sintomas autonômicos proeminentes, características cronobiológicas proeminentes e o caráter excruciante da dor, uma possibilidade terminológica seria: Unilateral com Ritmicidade Proeminente e sintomas autonômicos excruciantes da cefalgia (UPRASEC)
