    “Migrânea” ou “enxaqueca”: uma revisão histórica e semântica

    IntroductionHeadache is as old as the emergence of man on earth. A classification for headaches has been suggested for many centuries. After the invention of writing, numerous words were created to designate different types of headache.ObjectiveThe objective of this review was to discuss the historical and etymological aspects of the terms “migrânea” and “enxaqueca”.MethodsThis study was an integrative review using articles with historical data on the etymology of the terms “migrânea” and “enxaqueca” and their evolution over the years.ResultsThe terms “migrânea” and “enxaqueca” have Greek and Arabic origins, respectively. Both describe a neurological syndrome characterized by headache and other associated symptoms. Usually, sufferers of this disease are more familiar with the term “enxaqueca” and confuse it with headache, while health professionals prefer the term “migrânea”. ConclusionsAfter so many years that the term "migrânea" has been used, it would be a step backwards to use the term "enxaqueca" again, especially because "migrânea" has been adopted by young neurologists, including those from other specialties.Introdução. A cefaleia é tão antiga quanto o surgimento do homem na terra. Uma classificação para cefaleia tem sido sugerida há muitos séculos. Após a invenção da escrita, inúmeras palavras foram criadas para designar diferentes tipos de cefaleias. Objetivo. O objetivo desta revisão foi discutir os aspectos históricos e etimológicos dos termos “migrânea” e “enxaqueca”. Métodos. Este estudo foi uma revisão integrativa utilizando artigos com dados históricos sobre a etimologia dos termos “migrânea” e “enxaqueca” e sua evolução ao longo dos anos. Resultados. Os termos “migrânea” e “enxaqueca” têm origem grega e árabe, respectivamente. Ambos descrevem uma síndrome neurológica caracterizada por cefaleia e outros sintomas associados. Normalmente, quem sofre desta doença conhece mais o termo “enxaqueca” e confunde-o com cefaleia, enquanto os profissionais de saúde preferem o termo “migrânea”. Conclusões. Depois de tantos anos de uso do termo "migrânea", seria um retrocesso usar novamente o termo "enxaqueca", até porque "migrânea" tem sido adotada por jovens neurologistas, inclusive de outras especialidades

    IntroductionMigraine is a complex headache to treat, often with an unsatisfactory clinical response. Aerobic exercise, such as running, can be a non-pharmacological treatment to reduce migraine attacks.ObjectiveThis systematic review with meta-analysis investigated the effects of running on frequency and intensity of pain in subjects with migraine compared with other or no aerobic exercise.MethodsRandomized and quasi-randomized clinical trials were searched between September and November 2021 in BVS, PubMed, Cochrane, CINAHL, SCOPUS, Embase, and Web of Science databases. The Cochrane Risk of Bias tool assessed methodological quality, and the recommendation ranking assessed the certainty of evidence. The frequency of migraine attacks was pooled in a meta-analysis (random effects) that included interval and continuous running subgroups.ConclusionThe results were not sufficient to recommend running as a treatment to reduce the frequency and intensity of migraine attacks.IntroduçãoA enxaqueca é uma cefaleia complexa de tratar, muitas vezes com resposta clínica insatisfatória. O exercício aeróbico como correr, pode ser um tratamento não farmacológico para reduzir as crises de enxaqueca.ObjetivoEsta revisão sistemática com meta-análise investigou os efeitos da corrida na frequência e intensidade da dor em indivíduos com enxaqueca em comparação com outro ou nenhum exercício aeróbico.MétodosEnsaios clínicos randomizados e quase randomizados foram pesquisados entre setembro e novembro de 2021 nas bases de dados BVS, PubMed, Cochrane, CINAHL, SCOPUS, Embase e Web of Science. A ferramenta Cochrane Risk of Bias avaliou a qualidade metodológica, e o ranking de recomendação avaliou a certeza da evidência. A frequência dos ataques de enxaqueca foi agrupada em uma meta-análise (efeitos aleatórios) que incluiu subgrupos de corrida contínua e intervalada.ConclusãoOs resultados não foram suficientes para recomendar a corrida como tratamento para reduzir a frequência e a intensidade das crises de enxaqueca

    Hipotensão intracraniana espontânea: um relato de caso no contexto dos diagnósticos diferenciais das cefaleias crônicas

    Spontaneous intracranial hypotension (SIH) is a rare and debilitating condition caused by decreased intracranial pressure, which occurs more frequently in females. SIH can have several causes, among them the spontaneous formation of cerebrospinal fluid venous fistula (CSF-venous fistula), which is primarily responsible for the appearance of postural headache. Orthostatic headache is diagnosed by CSF pressure < 6 mmHg associated with specific imaging findings. Other specific symptoms such as dizziness, reduced muscle strength, blurred vision and syncope and other more systemic symptoms such as fatigue, mental confusion and difficulty concentrating are commonly observed. Etiological investigation through imaging studies such as magnetic resonance imaging and dynamic tomography of myelography is necessary for diagnosis. Due to the debilitating condition, several therapeutic approaches have been developed, ranging from more conservative approaches, with observation and use of analgesics, to more invasive interventions such as surgical ligation, transvenous embolization and blood tamponade.A hipotensão intracraniana espontânea (HIS) é uma condição rara e debilitante causada pela diminuição da pressão intracraniana, que ocorre mais frequentemente em mulheres. A HIS pode ter diversas causas, dentre elas a formação espontânea de fístula liquórica venosa, que é a principal responsável pelo aparecimento da cefaléia postural. A cefaleia ortostática é diagnosticada pela pressão liquórica < 6 mmHg associada a achados de imagem específicos. Outros sintomas específicos como tontura, diminuição da força muscular, visão turva e síncope e sintomas mais sistêmicos como fadiga, confusão mental e dificuldade de concentração são comumente observados. A investigação etiológica através de exames de imagem, como ressonância magnética e mielotomografia dinâmica, é necessária para o diagnóstico. Devido à condição debilitante, várias abordagens terapêuticas têm sido desenvolvidas, desde abordagens mais conservadoras, com observação e uso de analgésicos, até intervenções mais invasivas, como ligadura cirúrgica, embolização transvenosa e tamponamento sanguíneo

    IntroductionChronic migraine is a socioeconomic and individual burden since it is the largest cause of disability in people under 50 years of age. Although there are several prophylactic drug alternatives, some patients are vulnerable to refractoriness with significant damage to their quality of life. Prolotherapy, a subcutaneous injection of dextrose in peripheral nerves, advocates as a promising tool in the management of this pathology.Case reportA patient with refractory chronic migraine to drug therapy who was submitted to neurofascial prolotherapy. Weekly administration of a 2 ml solution of 1% ropivacaine and 10% glucose in the head peripheral nerves for 6 weeks. Reduced disability and frequency of migraine attacks for a period of 8 weeks after interventions.ConclusionProlotherapy proved itself to be a notable technique for reducing the number of days in a month that a patient with refractory chronic migraine to standardized therapy has had headaches. However, placebo group studies are needed to determine the efficacy of the procedure.IntroduçãoA enxaqueca crónica é um fardo socioeconómico e individual, uma vez que é a maior causa de incapacidade em pessoas com menos de 50 anos de idade. Embora existam diversas alternativas de medicamentos profiláticos, alguns pacientes são vulneráveis à refratariedade com prejuízos significativos à sua qualidade de vida. A proloterapia, uma injeção subcutânea de dextrose nos nervos periféricos, é defendida como uma ferramenta promissora no manejo desta patologia.Relato de casoPaciente com enxaqueca crônica refratária à terapia medicamentosa que foi submetido à proloterapia neurofascial. Administração semanal de 2 ml de solução de ropivacaína a 1% e glicose a 10% nos nervos periféricos da cabeça durante 6 semanas. Redução da incapacidade e frequência de ataques de enxaqueca por um período de 8 semanas após as intervenções.ConclusãoA proloterapia provou ser uma técnica notável para reduzir o número de dias em um mês que um paciente com enxaqueca crônica refratária à terapia padronizada teve dores de cabeça. No entanto, são necessários estudos em grupo placebo para determinar a eficácia do procedimento

    IntroductionHeadaches and migraine are common in the pediatric population, being one of the most frequent symptoms reported in practice. Additionally, it is a considerably disabling condition, which brings significant burden and impairs several aspects of a child or adolescent\u27s life, such as mental and physical health, executive functioning, school performance. Children and adolescents with migraine have higher risk of psychiatric comorbidities and psychosocial adjustment difficulties, which, in turn, compromise even more patient\u27s well-functioning.ObjectiveThe present article provides the clinician with a straightforward and evidence-based approach to migraine treatment in this age group.CommentTreatment of migraine in children and adolescents requires a systematic and thorough approach. Clinicians should keep in mind the important burden migraine brings to a child\u27s life, thus investigate, and properly manage comorbidities presented. Patient and parents\u27 education is a meaningful part of the treatment. Moreover, non-pharmacological treatments, such as healthy lifestyle habits, behavioral interventions may also play beneficial roles. When preventive treatment is indicated, it should be tailored considering drug\u27s profile of effectiveness and safety, as well as patient\u27s comorbidities. Lack of evidence in this context must not translate in lack of action by the clinician, since there may be a relevant burden associated. Therefore, reasoning for the perspicacious clinician is of fundamental importance and may influence positively the outcomes.IntroduçãoDores de cabeça e migrânea são comuns na população pediátrica, sendo um dos sintomas mais frequentes relatados na prática. Além disso, é uma condição consideravelmente incapacitante, que traz sobrecarga significativa e prejudica diversos aspectos da vida de uma criança ou adolescente, como saúde mental e física, funcionamento executivo, desempenho escolar. Crianças e adolescentes com migrânea apresentam maior risco de comorbidades psiquiátricas e dificuldades de ajustamento psicossocial, o que, por sua vez, compromete ainda mais o bom funcionamento do paciente.ObjetivoO presente artigo fornece ao médico uma abordagem simples e baseada em evidências para o tratamento da enxaqueca nesta faixa etária.ComentárioO tratamento da migrânea em crianças e adolescentes requer uma abordagem sistemática e minuciosa. Os médicos devem ter em mente o importante fardo que a enxaqueca traz à vida de uma criança, investigando e gerenciando adequadamente as comorbidades apresentadas. A educação do paciente e dos pais é uma parte significativa do tratamento. Além disso, os tratamentos não farmacológicos, como hábitos de vida saudáveis e intervenções comportamentais, também podem desempenhar papéis benéficos. Quando indicado tratamento preventivo, este deve ser adaptado considerando o perfil de efetividade e segurança do medicamento, bem como as comorbidades do paciente. A falta de evidências neste contexto não deve traduzir-se em falta de ação por parte do médico, uma vez que pode haver um ónus relevante associado. Portanto, o raciocínio do clínico perspicaz é de fundamental importância e pode influenciar positivamente os resultados

    Case Report of Compensated Idiopathic Intracranial Hypertension due to Traumatic Cerebrospinal Fluid Fistula

    IntroductionDisorders of intracranial pressure, both Intracranial Hypertension (IH) and Cerebrospinal Fluid Hypotension (CSFH), are causes of headache and can impair quality of life and lead to various complications. These conditions can cause symptoms such as transient visual loss, tinnitus, gait and balance disturbances, but positional headache is the main symptom. In this report, we present the case of a patient who was diagnosed with Idiopathic Intracranial Hypertension (IIH) and later developed a change in the pain pattern, caused by traumatic Cerebrospinal Fluid Fistula.ObjectiveTo report the case of a patient diagnosed with Idiopathic Intracranial Hypertension (IIH), who had a good response to medical treatment for several years but experienced worsened headache and a change in pain pattern (becoming orthostatic) following a Traumatic Brain Injury (TBI).ResultsFemale patient, 33 years old, obese, accompanied in an outpatient headache clinic in Joao Pessoa - PB. In 2018, she presented with a progressive holocranial headache that worsened with dorsal decubitus, accompanied by transient visual darkening. She was diagnosed with IIH after a cerebrospinal fluid (CSF) collection and exclusion of other causes. Since then, she has been taking Topiramate and making lifestyle changes, such as regular physical activity, dietary changes, and weight loss, resulting in significant improvement of symptoms for a few years. In January 2023, she suffered a mild TBI (hit her head on a cabinet) and a few days later, she started experiencing intense and almost daily headaches, but different from the previous ones. Now, the pain worsened or appeared in upright position and was accompanied by clear, colorless, odorless, fluid and constant rhinorrhea in the right nostril, ear fullness, nausea, and occasional vomiting. At that time, she stopped the medication and had partial improvement of symptoms.After reporting these events during the follow-up appointment, she underwent investigation with Magnetic Resonance Imaging (MRI) with cerebrospinal fluid flow study, which revealed a frontoethmoidal meningoencephalocele through a bone defect in the cribriform plate. In a joint decision with the Neurosurgery team, it was opted not to correct the fistula and to proceed with a ventriculoperitoneal shunt. She had a great improvement after the procedure, was discharged days later without symptoms.ConclusionTherefore, we report a case of a patient with secondary headache due to IIH that achieved good symptom control for several years. However, after a MTI injury, the headache recurred with a new pattern and was accompanied by new symptoms. She was diagnosed with a Cerebrospinal Fluid Fistula and Meningoencephalocele, and underwent ventriculoperitoneal shunt placement. She showed good progress and response to treatment
