    Cognitive alterations in patients with alterations in sleep architecture in a court of Colombian patients

    Introduction: Increasing evidence suggests that sleep disorders precede the onset of Alzheimer's disease by years, which is why it has been proposed that patients with alterations in sleep architecture have some degree of cognitive impairment (subjective complaint of memory or cognitive level impairment). In neurological clinical practice, we observe that sleep studies ordered to rule out OSAHS generally do not show severe alterations in this aspect, but they do show alterations in sleep architecture, mainly low sleep efficiency and N3 sleep proportion. Patients with memory problems may have alterations in the structure of sleep, however it remains unclear if having an affectation in the organization of sleep affects the cognition of the subjects. With this work we want to describe the results of cognitive tests in patients with results in sleep architecture. Materials and methods: Descriptive cross-sectional study, where 160 polysomnographies were reviewed, 68 patients met the inclusion criteria and 31 agreed to the Montreal Cognitive Assessment (MOCA) cognitive test to detect level cognitive impairment and the Hopkins test. for memory evaluation, they cannot have moderate or severe sleep apnea syndrome and cannot be taking medications that cause the sleep-wake cycle. Results: 77% presented the MOCA test below the normal values validated for the Colombian population, without finding significant differences between patients without OSAHS and with mild OSAHS, with delayed recall being the most affected domain. Conclusions: 1. 77% of the patients included in the study presented MOCA test values below the normal values validated for the Colombian population. 2. There were no significant differences between patients with AHI <5 and AHI 5 - 15. 3. The Hopkins test was used in the majority of patients with alterations in the architecture of sleep. 4. Alterations in sleep architecture could be a factor associated with cognitive disorders and thus could motivate a complete neuropsychological assessment.     Implications of surgical mask use in physical education lessons

    COVID-19 is being a huge challenge for education systems. Mandatory use of face masks in presential classes may lead to psychophysiological impairment of students, which affect lessons. Thus, the aim of the present research was to analyze the effect of mask use in basal psychophysiological status of physical education students prior to a physical education lesson. We analyzed 72 physical education students in two moments prior to two physical education lessons with 48 h between them. Blood oxygen saturation, body, temple and face temperatures, perceived stress and exertion, and heart rate variability were measured. Results showed that the use of surgical masks in physical education students produced a significant increase on subjective stress perception, sympathetic modulation, cardiovascular response, face, and temple temperature while decreasing blood oxygen saturation. We concluded that the use of surgical masks by physical education students induce modifications on the organic status, which could represent a handicap compromising the academic objectives of physical education lessons.     Cognitive changes in patients with epilepsy identified through the MoCA test during neurology outpatient consultation

    Introduction: Epilepsy is a chronic neurological disorder that may occur alongside cognitive changes, with effects on multiple cognitive domains. Objective: To compare the cognitive performance of patients with epilepsy and healthy controls through Montreal Cognitive Assessment (MoCA) during outpatient consultation at a reference diagnostic center in Colombia and analyze and the influencing factors. Materials and methodology: One-hundred and four patients during neurology outpatient consultation in the city of Cartagena, Colombia, were assessed with the (MoCA) test, i.e., 54 people who consulted for headache and have not been diagnosed with epilepsy (NEP) and 50 with a diagnosis of epilepsy (EPs) according to the diagnostic criteria of the International League Against Epilepsy (ILAE). Results: Significant differences were found in the total mean scores of the (MoCA) between (EPs) and (NPE) groups (t = 4.72; p < 0.01), particularly in attention (t = 3.22; p < 0.02) and memory (t = 5.04; p < 0.01) dimensions. Additionally, a significant association was observed between years of schooling and (MoCA) scores (p = 0,019) but not between socioeconomic level (p = 0,510), age (p = 0,452) and the frequency of seizures (p = 0,471). Discussion: Patients with epilepsy show lower scores in several cognitive domains in respect of the control group.     En este estudio se evaluó el efecto toxicológico de los elementos potencialmente tóxicos (EPT) Pb y Hg del PM2.5 del aire de la ciudad de Barranquilla empleando el modelo biológico C. elegans. Las muestras fueron obtenidas por medio de un muestreo de tres meses (Julio, Agosto, y Septiembre de 2019) en periodos consecutivos de 48 horas en dos puntos de la ciudad, uno ubicado en la Universidad de la Costa (PRT) y el otro en la escuela de policía Antonio Nariño (ESANA) (POL). Con los filtros obtenidos se realizaron extractos que fueron utilizados para el proceso de exposición y para la caracterización, la cual fue analizada por medio del método de espectrometría de absorción atómica SM 3030 E - SM 3111 B. Las mayores contribuciones de PM2.5 se obtuvieron en el punto de muestreo POL siendo casi 34% más que en el punto PRT. Con respecto a los extractos, se encontraron concentraciones de Hg que oscilaron entre 0.001 y 0.013 mg/L y de Pb por debajo de 6.19 mg/L. Por otra parte, de los efectos toxicológicos evaluados por exposición a cada extracto, solo el crecimiento, la locomoción y la reproducción fueron los que presentaron efectos estadísticamente significativos, correlacionados a la vez, con las concentraciones de PM2.5 y el EPT Hg.In this study, the toxicological effect of the Potentially Toxic Elements Pb and Hg of the PM2.5 in the air of the city of Barranquilla was evaluated using the biological model C. elegans. The samples were obtained through a sampling of three months (July, August and September) in consecutive periods of 48 hours in different parts of the city, one located in Universidad de la Costa (PRT) and the other in the Police School Antonio Nariño (POL). With the obtained filters, extracts were made, and these were used for the exposure process and characterization, which was analyzed by the atomic absorption spectrometry method SM 3030 E - SM 3111 B. The highest contributions of PM2.5 were obtained at the POL sampling site with a difference of 34% more than the PRT site. Regarding the extracts, concentrations of Hg were found ranging from 0.001 to 0.013 mg/L and of Pb below 6.19 mg/L. Similarly, from the toxicological effects evaluated through the nematodes exposition to each extract, only the growth, locomotion and reproduction were the ones which presented statistically significant effects, at the same time correlated with the PM2.5 concentrations and the PTE Hg.1.     Implementation of lean six sigma to lessen waiting times in public emergency care networks: a case study

    Emergency Care Networks (ECNs) are integrated healthcare systems comprised of emergency departments (EDs). ECNs are called to be the primary response of healthcare authorities to deal with the expected uptick in the future demands for emergency care during the current Covid-19 pandemic. Lean Six Sigma (LSS) has been proposed to address this challenge since it allows managers to detect factors contributing to the extended waiting times (WT) throughout the patient journey. The suggested framework follows the DMAIC cycle that was initiated with the project charter definition; in the meantime, a SIPOC diagram was drawn to analyze the emergency care process and pinpoint critical process variables. Following this, a nested Gage R&R study was undertaken to study the measurement system performance; subsequently, a normal-based capability analysis was carried out to determine how well the ECN process satisfies the specifications. The next step was to identify the potential causes separating the ECN nodes from the desired target. Afterwards, improvement strategies were devised to lessen the average WT. After suitable data collection, a before-and-after analysis was performed to verify the effectiveness of the implemented strategies. Ultimately, a control plan containing an I-MR control chart was designed to maintain the improvements achieved with the LSS implementation. The results revealed that the average WT of the showcased node passed from 190.02 min to 103.1 min whereas the long-term sigma level increased from −0.06 to 0.11.     Relationships among instructor autonomy support, and university students’ learning approaches, perceived professional competence, and life satisfaction

    The purpose of this study was to examine relationships among instructor autonomy support for student learning, and students’ motivational characteristics, learning approaches, perceptions of career competence and life satisfaction. Participated 1048 students from Spanish universities with ages between 18, and 57 years. A Structural equation modeling revealed a relationship between instructor autonomy support for student learning with students’ basic psychological need satisfaction. As a result, students’ basic need satisfaction was related to their intrinsic motivation, and to a deeper learning approach. These educational outcomes contributed to explain students’perceived professional competence, and life satisfaction. These findings highlight the importance of student choice, and decision-making in the learning process as a means to facilitating deeper learning, stronger feelings of professional competence, and enhanced well-being

    Investigation and optimization of the performance of energy systems in the textile industry by using chp systems

    With the general progression of small communities toward greater industrialization, energy consumption in this sector has increased. The continued growth of energy consumption seen in Iran, along with the low efficiency of production, transmission, and the distribution of energy, has led to the projection of an unfavorable future for this sector. The purpose of this study is to reduce fuel consumption and increase system efficiency by considering the optimal position of the turbine. In this regard, turbine modeling has been performed by considering different positioning scenarios. Afterward, the result from applying each scenario was compared with another scenario in terms of the parameters of electrical energy production, gas consumption, the final energy produced by the system, and the ratio of energy produced to overall gas consumption. After comparing different scenarios, considering all 4 parameters, Scenario 7 was selected as the most suitable positioning for gas turbine placement. Scenario 7 showed the highest gas consumption; of course, high power generation is the most desirable, the most reliable and, ultimately, the most profitable outcome of energy production. According to our results, the amount of electrical energy produced in the selected scenario is 4,991,160.3 kWh; the gas consumption in this case is 0.22972 kg/s

    Las enfermedades virales como la influenza y el COVID 19 son de enorme importancia en la salud pública global con gran impacto tanto en adultos mayores y grupos de alto riesgo como en la población pediátrica. Sus consecuencias son considerables debido a los efectos económicos significativos tanto en ausentismo laboral como en el desgaste y saturación de los servicios de salud. Este documento, se encuentra alineado al Acuerdo No. 1458 del 27 de mayo de 2020, donde se presenta el protocolo de bioseguridad para retorno a actividad al interior de la Corporación Universidad de la Costa – CUC y tiene como objetivo dar algunas pautas que permitan a los trabajadores de la Biblioteca de la Universidad de la Costa prestar sus servicios a la comunidad evitando al máximo posible la contaminación desde y hacia los usuarios. También se presentan algunas recomendaciones para los usuarios de los servicios bibliotecarios en el caso de que padezcan infecciones respiratorias compatibles con la situación pandémica.     Se presentan resultados de un estudio que buscó conocer la relación de dependencia entre la percepción que tienen los niños y niñas de sus figuras significativas y el afrontamiento de conflictos cotidianos, aspecto que se logró colocando a prueba hipótesis que incluyeron variables asociadas a la forma como conciben el mundo los participantes, su percepción frente a figuras de amor y poder, sus decisiones al momento de afrontar conflictos en situaciones simuladas de igualdad, ventaja y desventaja. Se analizaron las variables a partir de la aplicación un role play game (RPG) en línea “leyendas del Almar”, y la simulación de un espacio de juego a partir de la construcción de historias, utilizando láminas escogidas intencionalmente del “Test de Apercepción infantil CAT-H” versión revisada por jueces expertos, para conocer dinámicas relacionales, ansiedades prevalentes en cada participante respecto a su relación con el entorno y la percepción introyectada de sus figuras cercanas. Esta investigación se ejecutó en colegios oficiales en el departamento del Atlántico; trabajando con 198 niños en condición de vulnerabilidad socioeconómica y con edades entre 7 y 11 años. Los resultados evidencian que las variables más determinantes en la tendencia a agredir o no al resolver los conflictos, fueron las de la concepción del mundo y la naturaleza de las ansiedades, mediadas por la forma como se elaboran los temores en la relación con las figuras significativas. Sin embargo, estas, directamente no mostraron un papel decisivo en el afrontamiento de los conflictos.The results of a study that sought to know the dependency relationship between the perception that boys and girls have of their significant figures and the coping with daily conflicts are presented, an aspect that was achieved by testing hypotheses that included variables associated with the way they conceive the world the participants, their perception in front of figures of love and power, their decisions when facing conflicts in simulated situations of equality, advantage and disadvantage. Variables were analyzed from the application of an online role play game (RPG) "Legends of the Almar", and the simulation of a game space from the construction of stories, using pictures intentionally chosen from the "Child Aperception Test CAT-H” version reviewed by expert judges, to know relational dynamics, prevailing anxieties in each participant regarding their relationship with the environment and the introjected perception of their close figures. This research was carried out in official schools in the department of Atlántico; working with 198 children in socioeconomic vulnerability conditions and with ages between 7 and 11 years. The results indicate that the most determining variables in the tendency to attack or not when solving conflicts were those of the conception of the world and the nature of anxieties, mediated by the way in which fears are elaborated in relation to significant figures.
