Le ruxolitinib (Opzelura®), un antinéoplasique dans la prise en charge du vitiligo non segmentaire avec atteinte faciale chez l’adulte et l’enfant de plus de 12 ans, une évolution thérapeutique
Contexte : Le vitiligo est une pathologie dermatologique caractérisée par l’apparition de taches blanches sur la peau. Le défaut de mélanine s’exprime par la disparition progressive des mélanocytes, cellules de l’épiderme. De nombreuses options thérapeutiques non prises en charges et au coût élevé, avec des résultats souvent décevants, laissait de nombreux patients dans une impasse thérapeutique et à la recherche de solutions efficaces pour une pathologie ayant un impact psychologique significatif. L’arrivée d’Opzelura® marque une avancée majeure avec des résultats prometteurs en termes de repigmentation et d’amélioration de la qualité de vie des patients. Matériel et méthodes : Une enquête descriptive a été réalisée auprès de 131 patients. Les réponses ont été recueillies sur la période du 13/02/2024 au 27/03/2024 à l’aide d’un questionnaire anonyme en ligne diffusé sur le site internet et avec l’aide de l’Association Française du Vitiligo. Résultats : La majorité des patients sont atteints de vitiligo non segmentaire avec plus de la moitié atteint de la forme vulgaire (63,4%) suivi de la forme acrofaciale (20,6%) et universelle (14,5%). Parmi la totalité des patients, plus de 90% ont le visage touché par les plaques de vitiligo. L’impact de la maladie est forte, pour 77,1% des patients le vitiligo est décrit, essentiellement, par un terme négatif. La majorité des patients manque de soutien de la part du milieu médical, 48,1% ne reçoivent aucun soutien et 31,3% reçoivent rarement du soutien. Les traitements locaux restent le traitement de premier intention et largement utilisés par les patients (61,8%). La corticothérapie en per os combinée à la photothérapie est néanmoins peu utilisée (13,7%). Malgré tout, 20,6% des patients n’ont jamais eu recours à une stratégie thérapeutique. Les techniques de camouflage sont bien connues mais tous ne les utilisent pas, l’autobronzant (45%) et le maquillage médical (25,2%) sont les référents. Près d’un tiers n’ont jamais eu recours à ces techniques. Presque la totalité des patients avait eu connaissance du nouveau traitement Opzelura® par l’intermédiaire de l’AFV (69,5%), des réseaux sociaux (44,9%), des médias, des revues ou sites internet (39,8%) et enfin des professionnels de santé (15,3%) augmentant ainsi le niveau d’attente quant à l’efficacité du traitement. Le pharmacien d’officine a un rôle majeur dans le parcours de soin du patient atteint de vitiligo de par son accessibilité, son expertise médicale et sa volonté d’accompagner et d’orienter le patient. Il représente le premier et le dernier professionnel du parcours de soin puisqu’il accueille et oriente le patient et évalue son évolution tout au long de sa vie. Conclusion : Notre enquête a mis en évidence un niveau d’attente élevé concernant le traitement Opzelura® quant à son efficacité, son accessibilité et sa prise en charge. De plus, il est nécessaire d’apporter un accompagnement psychologique et un soutien social pour aider les patients à gérer et à mieux vivre avec la maladie. Le milieu médical a pour objectif d’améliorer le soutien apporté aux patients. Le pharmacien d’officine reste accessible et à l’écoute afin d’orienter et accompagner les patients

International audienceL’exploitation des cartons des Archives Nationales de la Marine (série G) évoquant chirurgiens, médecines et maladies épidémiques aux Antilles entre les années 1680 et la fin du XVIIIe siècle permet de dresser les contours du monde médical antillais dans ses aptitudes et ses prérogatives à prévenir et à faire face à la maladie et à l’épidémie en contexte colonial au XVIIIe siècle. Par les droitures qui lie les ports des deux rives de l’Atlantique, navires et vaisseaux deviennent l’objet d’une attention particulière, d’un protocole pour éviter la diffusion épidémique. Prendre en charge les malades d'épidémies et établir un encadrement médical de la population colonial deviennent progressivement des priorités au cœur de l'équilibre de l'écosystème colonial. A‐Site Cationic Variation to Expand the Sacrificial Layer AVO<sub>3</sub> Family Dissolving in Water
International audienceIn the growing field of low‐cost electronics, the epitaxy of complex oxide thin films on a Si substrate requires significant technical means. Therefore, a large attention is paid to the release of a freestanding oxide of interest from its deposition support which is then placed onto a low‐cost substrate, via the etching of an intermediate sacrificial layer. The use of a sacrificial layer offers several advantages since the flexible polymer exploited for the transfer can also be fully utilized to design a flexible heterostructure. For green technology, more and more research investigations are being undertaken on these sacrificial layers etched by water. While Sr3Al2O6 and SrVO3 are archetypical examples, the need to find new materials with different lattice parameters and symmetry is critical to reach the epitaxy of numerous materials of interest. In this study, the possibilities of an A‐site cationic variation in AVO3 with A = Sr and/or Ca thin films are highlighted to expand the water‐soluble material's family reaching the smallest lattice parameter ever presented up to now. In addition to bring various compounds with different ageing properties to the literature, optical spectrophotometry operando characterizations to follow the chemical etching of the sacrificial layers in real‐time are exploited
A freely accessible, adaptable hollow-fiber setup to reproduce first-order absorption: illustration with linezolid cerebrospinal fluid pharmacokinetic data
The main objective of this study was to validate an algorithm and experimental setup to simulate first-order absorption pharmacokinetic profiles without altering the standard in vitro hollow fiber infection model (HFIM). For that, clinical cerebrospinal fluid (CSF) linezolid concentrations after 30-minute infusions at dosing regimens 600 mg q12 h, 900 mg h, and 900 mg h were reproduced in the HFIM over days. To approximate the apparent first-order absorption observed on CSF pharmacokinetic profiles, we split the dosing interval into a series of sub-intervals during which continuous infusions were delivered to the system. During each sub-interval, the same amount of linezolid was delivered but the sub-intervals had different durations and flow rates which were computed by a newly developed algorithm. In addition, we independently reproduced plasma concentrations to validate our system. Samples were collected from the central reservoir and the extracapillary space (ECS) of the cartridge of the HFIM and assayed by liquid chromatography-tandem mass spectrometry. Observed pharmacokinetic parameters and concentrations in the ECS were compared with the target clinical pharmacokinetic parameters and concentrations. Observed pharmacokinetic parameters were within 20% of target pharmacokinetic parameters for all experiments, thus validating the ability of our experimental setup to reproduce plasma and CSF linezolid pharmacokinetic profiles.The algorithm and setup are available in the open-source web application https://varacli.shinyapps.io/hollow_fiber_app/ to easily design other HFIM experiments
Tristesse et mélancolie comme conséquence de la culpabilité : le <i>Schuldigsein</i> entre phénoménologie et psychanalyse
International audienceObjective: This article aims to demonstrate the relevance of the concept of guilt in understanding the depressive syndrome, particularly in melancholic psychosis, in the phenomenological and psychoanalytic approaches to psychopathology. It does so by comparing Lacan's and Heidegger's approaches to subjectivity and existence.Method: Through a bibliographic and conceptual analysis of the notions of guilt and melancholy in the phenomenological and psychoanalytic traditions, and using two clinical vignettes from the scientific literature, we highlight the significance of the concept of guilt in understanding melancholic psychosis.Result: There is a possible parallel between Lacanian psychoanalysis and Heidegger's Daseinanalysis on the concept of guilt. It can be interpreted as the expression of the rejection of the unconscious but also as an expression of authentic existential projectuality of Dasein. This opens up several conceptual and methodological questions at the theoretical and clinical levels.Discussion: Guilt could then be a clinical indicator of the subject's rejection of the unconscious, which in melancholic psychosis manifests as a totalizing identification with the waste-object, without other existential possibilities, without “lack.” From the perspective of existential analysis, the abnormal guilt of the melancholic could thus be understood as a form of foreclosure of Being.Conclusion: Guilt in psychopathology can vary depending on the specific subjective conditions observed in clinical settings. In the case of melancholy, guilt is an indicator of the stagnation of the subject in relation to their existential possibilities. The existential inertia of the melancholic can be interpreted as a very current ontology of waste.Objectif : Cet article vise à démontrer l’actualité du concept de culpabilité dans la compréhension du syndrome dépressif, en particulier de la psychose mélancolique, dans les approches phénoménologique et psychanalytique de la psychopathologie, en particulier en mettant en comparaison les approches de la subjectivité et de l’existence chez Lacan et Heidegger.Méthode : À partir de l’analyse bibliographique et conceptuelle des notions de culpabilité et de mélancolie dans les traditions phénoménologique et psychanalytique, et à travers deux vignettes cliniques tirées de la littérature scientifique, nous mettons en lumière l’actualité du concept de culpabilité dans la compréhension de la psychose mélancolique.Résultat : Il existe un possible parallèle entre la psychanalyse lacanienne et la Daseinanalyse de Heidegger du concept de culpabilité. Elle peut être interprétée comme l’expression du rejet de l’inconscient, mais aussi de la projectualité existentielle plus propre au Dasein. Cela ouvre à plusieurs questions conceptuelles et méthodologiques aux niveaux théorique et clinique.Discussion : La culpabilité serait alors un indicateur clinique du rejet du sujet de l’inconscient, qui dans la psychose mélancolique se manifeste sous la forme d’une identification totalisante à l’objet-déchet, sans autres possibilités, sans « ». Dans la perspective de l’analyse existentielle, la culpabilité anormale du mélancolique pourrait donc être comprise en tant que forme de forclusion de l’Être.Conclusion : La culpabilité en psychopathologie peut se décliner en fonction des conditions subjectives particulières observées en clinique. Dans le cas de la mélancolie, la culpabilité est l’indicateur de la stagnation du sujet par rapport à ses possibilités existentielles. L’inertie existentielle du mélancolique peut être interprétée comme une très actuelle ontologie du déchet.</b

Emergence of cefiderocol resistance during therapy in NDM-5–producing Klebsiella pneumoniae isolates harboring siderophore receptors mutations
International audienceCefiderocol, a siderophore-conjugated cephalosporine, is a promising drug used to treat infection with carbapenem-resistant gram-negative bacteria. Here, we report a case of pneumonia induced by multiple gram-negative pathogens, including a carbapenem-resistant Klebsiella pneumoniae developing cefiderocol resistance within 32 days of cefiderocol therapy. Whole genome sequencing of three consecutive K. pneumoniae isolates revealed that the bacteria were isogenic and were carrying several broad-spectrum beta-lactamases (blaNDM5 and blaCTX-M-15). Two isolates with elevated minimum inhibitory concentration against cefiderocol harbored mutations in genes encoding siderophore: one in the cirA gene and one in both the cirA and the flu genes. The combination of a metallo-beta-lactamase background and mutations in siderophore receptors was associated with phenotypic resistance to cefiderocol
