Importance of clinical examination in diagnostics of Osgood-Schlatter Disease in boys playing soccer or basketball
Introduction: Osgood–Schlatter disease is an irritation of the patellar tendon at the tibial tubercle. Sports with jumps, running, and repeated contractions of knee extension apparatus are considered to be importantexternal risk-factors which could cause Osgood–Schlatter disease.Objectives of the study are to draw attention to the importance of clinical examination in diagnostics of Osgood–Schlatter disease in boys playing soccer or basketball.Methods: The research included data obtained from 120 boys, average age of 14 years. Examinees were split into two groups, one with young athletes which regularly have soccer or basketball trainings and thesecond one with boys who do not participating in sports. We performed anthropological measurements and clinical examinations of both knees and hips for both groups. For the statistical analysis we used pointbiserialcorrelation coefficient.Results: Based on clinical examination, Osgood–Schlatter disease was diagnosed in 51 examinees (42.5%). In “athletic group” Osgood–Schlatter disease had 31 boys or 52%, comparing with “non-athletic group” wherewe found 20 adolescents with disease (33%). Number of boys with Osgood–Schlatter disease was higher for 19% in “athletic group” comparing with “non-athletic group”. Comparing incidence rate for boys in both groups with diagnosed II and III level of Osgood–Schlatter disease we found that rate is higher in “athletic group” 2.25 times comparing with “non-athletic group”.Conclusions: Clinical examination is critical method in the process of diagnosing Osgood–Schlatter disease especially for identifying II and III level of this disease
Comparative study of the results of heel ultrasound screening and DXA findings (lumbar spine and left hip) of postmenopausal women
Introduction: Osteoporosis is a silent and invisible disease of bone, great presence and is considered to suffer from osteoporosis at least 200 million women worldwide. The goal of this paper is to show average ageof postmenopausal respondents, values of anthropometric parameters (weight, height, BMI), anamnestic data on clinical symptoms, fractures of women in menopause, analysis of heel ultrasound screening results,analysis of lumbar spine DXA results, analysis of left hip DXA results.Methods: In retrospective study 61 respondents were involved, 33 to 79 years old, treated in u Center for Physical Medicine and Acupuncture “AD” in Sarajevo during the period from 01.01.2008 till 31.12.2009. Alldate are shown numerically and percentage account with calculation of mean value, expressed in the form of tables and charts.Results: Finding of heel ultrosound screening compared to T values of postmenopausal respondents indicates on osteoporosis in case of 17 (27,87%), in case of 44 (72,13%) respondents osteopenia, while normalvalues were not found. T value with lumbar spine DXA method in postmenopausal female respondents correspond to 43 (70,5%) respondents, in 15 respondents (24,6%) finding corresponded to osteopenia, while 3 respondents (4,9%) had physiological finding. Left hip DXA finding shows 36 (59%) respondents corresponded osteoporosis, 19 (31,2%) respondents corresponded osteopenia, while physiological finding was found in 6 respondents (9,8%). T value of lumbar spine DXA finding was - 2,71 ± 1,16; DXA finding of left hip -2,35 ±1,36; heel ultrasound screening -2,19 ± 0,54.Conclusion: Research results indicate that DXA finding in relation to the heel ultrasound screening confirms gold standard in diagnosing osteoporosis
Osteoporosis and osteoarthritis: similarities and differences
Osteoporosis and osteoarthritis are two different medical conditions, which beside the first part of their name „osteo“, have very little in common. Osteoporosis is a disorder which influences bones in terms of reduction of quality and quantity, which can easily result in bone fracture. Clinical signs of osteoporosis show no pain or other symptoms which could point to changes in bone structure, unless a bone fracture is diagnosed. Osteoarthritis is a disease which influences joints and its surrounding tissues. Seeing through clinical signs, changes could be verifi ed in terms of pain and limitations of movement and the cause of pain and way of its treatment are numerous. A person can suffer from osteoporosis and osteoarthritis at the same time. Although these medical conditions are more frequent in female than in male population, mechanisms which lead to them may interfere. Osteoporosis and osteoarthritis are muscular-bone disorders with signifi cant morbidity and mortality, but clinical experiences and epidemiological studies have shown their negative correlation
Translation and validation of the instrument for the oral health-related quality of life assessment in 3 to 5 years old children in Bosnia-Herzegovina
Introduction: During 2007. in the U.S. was developed the questionnaire for caregivers with 13 items for assessing the oral health-related quality of life in children 3-5 years of age, The Early Childhood Oral Health ImpactScale, The ECOHIS. The aim of this study was to perform the fi rst part of the adaptation process for this instrument in Bosnia-Herzegovina: translation, cross-cultural adaptation and the comprehensibility testing.Methods: ECOHIS was translated from English into the one of the languages in Bosnia-Herzegovina using a standardized forward-backward translation method. Two licensed, professional English-language translators, one dentist and one pediatrician, participated in the development of the preliminary BH-ECOHIS version. All translators were native Bosnian speakers. After translation and adaptation of ECOHIS to Bosnia and Herzegovina setting, pilot-research was performed in order to check the comprehensibility of the questionnaire.Results: The original and the back-translated version were the same. Because not all children in Bosnia-Herzegovina attend preschool, school or daycare, we replaced the question number fi ve from the originalEnglish version “missed preschool, daycare or school“ with “had difficulties in everyday activities”. Translated and culturally adapted version of the ECOHIS was applied in a form of an interview (N=16). Parents/caregivershad no diffi culties to understand the questionnaire.Conclusions: BH-ECOHIS showed excellent comprehensibility. Next step in the validation process should be the testing of its measurement characteristic
The role of multi slice computed tomography in the evaluation of acute non-cardiac chest pain
Introduction: Differential diagnosis of acute chest pain encompasses a broad spectrum of illnesses which are most likely followed by benign outcomes (pneumonia, pneumothorax, pleurisy, pericardial effusion, hiatus hernia), but also illnesses of lethal outcomes (pulmonary embolism, myocardial infarction, aortic dissection,thoracic aortic aneurysms, thoracic aortic aneurysm rupture, etc). Illnesses associated with benign and lethal outcomes may present very similar if not the same symptoms, resulting in a diffi cult establishment of accurate diagnosis.Methods: During the period of one year, 123 patients presented with non-cardiac acute chest pain were referred for the multi slice computed tomography (MSCT) examination. Scanning of thorax was conductedin two series: unenhanced and contrast-enhanced, using a window for pulmonary parenchyma and mediastinum.Results: From a total number of patients 21.1% had normal results while the other 79.9% had pathological results. Out of the total number of patients with pathological result MSCT established potentially lethal outcome for 35.0%, out of which 83.7% was contributed to vascular territory of pulmonary artery, while 16.3% was contributed to aorta.Conclusion: MSCT scanning, owe to its ability of simultaneous analysis of vascular and non-vascular thoracic structures, represents a very effi cient and reliable method for establishing accurate diagnosis and appropriatetriage of patients with acute chest pain. Accurate and effi cient diagnosis enables benefi cial outcome for the patient in this group of illness. MSCT enables the differentiation of etiological factors, which present as acute onset of non-cardiac chest pain
Anthropometric measurements of students athletes in relation to physically inactive students
Introduction: Anthropometry is a method of anthropology that refers to the measuring and testing the human body and to the relationship between the size of its individual parts.The task of anthropometry is as accurately as possible quantitatively characterize the morphological features of the human body.Measurements are made due to the anthropometric points which can be: fixed (standard on the site of prominence) and virtual (change due to the bodyposition). Goals of research: To evaluate the impact of basketball on the growth and development of seventeen years old adolescents and prevention of deformities of the spinal column and chest.Methods: The study included 40 respondents. Criteria for inclusion: male respondents aged 17 years who played basketball for more than one year, male respondents aged 17 years who are physically inactive. Criteria for exclusion: female respondents, respondents who played basketball for less than one year, respondents who are engaged in some other sport professionally or recreationally, respondents younger and olderthan 17 years. In the study,there were made measurements of thorax scope in the axillary and mamilar level, measurements of body weight and height and measurements of Body mass index.Results of research: Out of 40 respondents 20 are basketball players and 20 physically inactive. Compared to the average value between the two groups of respondents certain differences were observed, which aremost noticeable in body weight (basketball players had more weigh about, 5 kg on average) and height (basketball players are taller, about 7 cm on average). During the anthropometric measurements of thoraxdeformities of the spinal column have been observed which affect the deformation of the thorax. Of the 20 players one has a deformity of the spinal column, and out of the same number of physically inactive studentseven 12 have deformed spine.Conclusion: Basketball has a positive effect on the proper growth and development of adolescents
A comparison of effi cacy of femoral and tibial fractures healing treated by static and dynamic intramedullary nails
Introduction: Intramedullary nailing is synthesis and consolidation of fracture fragments with the main goalto gain strength and permanent placement of the implants. Two techniques of intramedullary osteosynthesis are used: with dynamic or with static intramedullary nail. Dynamization include conversion of static nail by removing screws from the longest fragment. The aim of this study is to determine whether there is a difference in the speed and quality of healing of the type A and B fractures of the femur and tibia treated by static or dynamic intramedullary nails and to compare the results.Methods: The study was conducted on a total of 129 patients with closed fractures of the diaphysis of the femur and tibia type A and type B. Patients were divided into two groups, based on the applied operating method, static or dynamic intramedullary osteosynthesis.Results: The average number of weeks of healing femoral and tibial fractures was slightly in advantage of static intramedullary osteosynthesis, it was 17.08 weeks (SD=3.382). The average number of weeks of healing in 23 patients with fractures of the femur, treated by dynamic intramedullary osteosynthesis was 17.83 (SD=2.978).Conclusion: We can conclude that static intramedullary nailing osteosynthesis unable movements between fragments which directly stimulates bone formation and formation of minimal callus. Static intramedullary ostesinthesys resolve the problem of stabilizing the fracture, limb shortening and rotation of fragments
Antioxidant potential of selected supplements in vitro and the problem of its extrapolation for in vivo
Introduction: antioxidants, free radicals and oxidative stress have been studied extensively for quite some time but their role in diseases and their prevention has not been clearly determined. Because commercialantioxidants do not need to pass clinical tests in order to be sold over the counter we have decided to test the antioxidant potential of different commercial preparations with the antioxidative properties.Methods: pH, rH and oxidant-reduction potential of different preparations in aqueous solution was measured. Afterwards antioxidant potential using FormPlus® after adding the preparation to human blood as a morecomplex environment with different homeostasis mechanisms was determined.Results: all the results showed expected change compared to the control but the results in aqueous solution did not match the results obtained from the human blood, as was expected.Conclusion: from the experiments it can be concluded that while the preparations did show antioxidant activity, it is very difficult and even wrong to predict the antioxidant potential of an antioxidant preparationadded to human blood, let alone in a living organism, based just on the results obtained in aqueous solution. Further possibilities for research include more extensive studies of antioxidant preparations in more complex environment and last but not least in test organisms or in human trials
Minimally invasive treatment of iatrogenic complete left ureter obstruction after hysterectomy
The iatrogenic ureter injuries are rare complications and may have serious consequences. The treatment options depend on situations. Pelvic surgeons, keep in mind that this kinds of complications, is very important for diagnosis and treatment, during the surgery. This report presents a case of a patient with iatrogenic left ureteral injury during hysterectomy. The patient visited emergency department of our hospital with the chief complaints of left lomber pain on the 17th day of hysterectomy. After evaluation in emergency clinic, the patient had an endoscopic treatment for iatrogenic ureter injury. The patient is still in follow-up period. We also review the literature and discuss diagnose, treatment, prognosis of iatrogenic ureter injuries. The treatmentoptions are still developing by technology
Chemical composition, antimicrobial and antioxidant properties of Mentha longifolia (L.) Huds. essential oil
Introduction: Present study describes the antimicrobial activity and free radical scavenging capacity (RSC) of essential oil from Mentha longifolia (L.) Huds. Aim of this study to investigate the quality, antimicrobial andantioxidant activity of wild species Mentha longifolia essential oil from Bosnia and Herzegovina.Methods: The chemical profi le of essential oil was evaluated by the means of gas chromatography-mass spectrometry (GC-MS) and thin-layer chromatography (TLC). Antimicrobial activity was tested against 6bacterial strains. RSC was assessed by measuring the scavenging activity of essential oils on 2,2- diphenyl-1-picrylhydrazil (DPPH).Results: The main constituents of the essential oil of M. longifoliae folium were oxygenated monoterpenes,piperitone oxide (63.58%) and 1,8-cineole (12.03%). Essential oil exhibited very strong antibacterial activity.The most important antibacterial activity essential oil was expressed on Gram negative strains: Escherichia coli, Pseudomonas aerginosa and Salmonella enterica. subsp.enterica serotype ABONY. Antioxidant activity was evaluated as a RSC. Investigated essential oil was able to reduce DPPH radicals into the neutral DPPHH form (IC50=10.5 μg/ml) and this activity was dose –dependent.Conclusion: The study revealed signifi cant antimicrobial activity of the investigated essential oil. The examined oil exhibited high RSC, which was found to be in correlation to the content of mainly monoterpeneketones and aldehydes. These results indicate that essential oils could serve as safe antioxidant and antiseptic supplements in pharmaceuticals
