Serum Matrix Metalloproteinase-2, -7 and -9 (MMP-2, MMP-7, MMP-9) levels as Prognostic Markers in Patients with Colorectal Cancer
Introduction: Matrix metalloproteinases are produced by tumour cells, hence, they may be associated with tumour progression including invasion, migration, angiogenesis and metastasis. Finding prognostic markers to better identify patients with higher risk for poor survival would be valuable in order to customize pre- and postoperative treatment as well as to enable closer follow-up of these patients. Aim of our study was to examineMMP-2, MMP-7 and MMP-9 serum levels and correlated them with pathological data such as stage of the colorectal cancer (CRC) and outcome.Methods: The investigation included 82 patients with operable CRC without distant metastases, who had underwent blood tests in order to determine the MMP-2, MMP-7 and MMP-9 serum levels in the following time periods: preoperatively, 3, 6, 9 and 12 months postoperatively.Results: The values of the investigated MMPs decrease postoperatively and start to increase 6 month later in patients of all stages of the disease, reaching the highest value 12 month postoperatively with statistically important differences of MMP-2, MMP-7 and MMP-7 serum levels in terms of disease staging and defined points of time. Analysis of the results showed that the MMP-2 serum levels obtained 3 and 12 months postoperatively,than MMP-7 serum levels 12 months postoperatively and the MMP-9 serum levels in all analyzed points in time were in significant association with the CRC patients’outcome.Conclusion: The MMP-2, MMP-7 and especially MMP-9 serum values could be important indicators for diagnosis of the patients with CRC and for monitoring of disease progression
Smoking and BMI as a risk factor of cardiovascular disease at a doctors in Tuzla canton
Introduction: Cardiovascular diseases are becoming the leading social and medical problem of civilization, given the trend indicates an increase of morbidity, disability and mortality from this diseases. The aim ofour study was to determine the frequency of smoking and increased BMI, as a risk factor for cardiovascular disease in doctors in the Tuzla Canton and correlate values of BMI by the doctor smokers and nonsmokers.Methods: The study was conducted in 13 medical centers in the area of Tuzla canton in the second quarter of 2009. Two groups were formed by randomization of 150 doctors non-smokers and 150 doctors smokersfrom a total of 366 doctors of both sexes, age over 25 years. The study involved doctors who smoke tobacco 5 or more years. The methods of anthropometric measurements and questionnaires were used in study.Results: The results showed that the total number of doctors surveyed, 44.81% were smokers, with more women smokers (28.7%) than men (21.3%) smokers (p=0.011). We found that there is a signifi cant statistical difference between subjects with BMI higher than 25 and subjects with normal weight, in the group of smokers (p = 0.0001).Conclusion: It can be concluded that the frequency of smoking in the total number of surveyed doctors, is signifi cant. The increased value of BMI (over 25) is present in large number of subjects (with the larger percentage subjects of smokers)
Endocarditis lenta-patient survived septic shock: a case report
Infective endocarditis is defi ned as an infection of the endocardial surface of the heart. Its intracardiac effects include severe valvular insuffi ciency, which may lead to intractable congestive heart failure and myocardialabscesses. This disease still carries a poor prognosis and a high mortality.A severe case of infective endocarditis with its complications is presented. A man with aortic prosthetic valve due to earlier aortic stenosis and corrected aortal coarctation and implanted pacemaker presentedwith prolonged unexplained fever, malaise, sweating, weight loss (15 kg/4 months) and lumbar pain. He was treated with broad-spectrum antibiotics prior IE diagnosis was considered. Echocardiogram showedaortic vegetations and possible periaortal abscess formation. Nonspecifi c infl ammation parameters were high positive. Cultures were constantly negative. His condition had deteriorated suddenly, and he had presentedwith worsening of cutaneous vasculitis, subacute glomerulonephritis and subsequent acute respiratory distress syndrome and septic shock. This patient survived with residual bilateral necrosis of the feet andtoxic peroneal paresis. At the end transthoracic echocardiogram showed enlarged heart chambers, LV mild dilated and concentric hypertrophy with ejection fraction about 40%, degenerative postinfl ammatory mitralvalve changes, mild mitral regurgitation and tricuspid regurgitation, postinfl ammatory aortic root fi brosis and moderate aortic valve stenosis (AVPG max 50,9 mmHg, AVPG mean 24 mmHg) with no pericardial effusion. Initial suspicion of Q fever was defi nitely excluded by serological testing showing nonspecifi c IgM positivity,probably rheumatoid factor related
Relation of diet and physical activity to obesity in children in elementary schools
Introduction: The prevalence of pediatric obesity is increasing. Finding the most effective preventive measures for the development of obesity in each country requires accurate epidemiological data on the number of obese children and adolescents, and their habits regarding nutrition and activity. The objective of this study was evaluate diet and physical activity in primary school students in relation to the occurrence of obesity, to determine the prevalence of overweight, mark the basic causes of this phenomenon and to establish measures for treatment and prevention.Methods: pupils 1-8. grades of primary schools were surveyed in written forms in terms of nutrition and physical activity, and measured height and weight, body mass index (BMI-body mass index) was calculated by whichwas estimated the level of nourishment: BMI> p (percentile) 5-malnutrition, p 5-85 proper body weight, p 85-95 over-nutrition, p> 95 obesity.Results: The study comprised 2329 pupils from 10 primary schools in the Canton of Sarajevo. Number of respondents by age and gender was balanced: I-IV 1077, V-VIII 1252; M-1226 and -1103 W. Obese and overweight was 22.46%, 62.53% of normal weight and 15 underweight, 01%. Most children eat a sandwich from school 34.63%, and food from the bakery 23.36% and 23.64% a sandwich from home. Still-dense juices are mostly drunk, even 22.34% of the students, a maximum of 52.8% water. Daily candy had taken 53.21% of all primary school students. 33.80% of the students were active on physical activity lessons and daily only 28.27%.Conclusions: The overweight problem in relation to the way of nutrition and physical activity is evident. The most important factors in] uencing the development of obesity undernutrition of children in school, the high frequency of intake of sweets and thick juice, an inadequate level ofphysical activity and sedanteran way of life
Is there any relationship between PSA and increased peripheral CD4+CD25highFOX3+ Treg in prostate cancer patients?
Introduction: The aims of this study were fi rst, to determine whether peripheral levels of CD4+CD25highFoxp3+ regulatory T cells (Treg) are elevated in Prostate Cancer (PCa) patients, and second, to determine the directcorrelation between peripheral Treg and total serum Prostate Specifi c Antigen (PSA) levels in these patients.Methods: Peripheral Blood Mononuclear Cells from 56 subjects undergoing diagnostic prostate biopsies (PSA ≥ 2.5 ng/ml) were analyzed for Treg numbers. The association between the peripheral Treg and serum PSA values was fi rst determined in the entire population, including people with no prostate pathology and PCa and Benign Prostate Hyperplasia (BPH) patients, and second, in nine PCa patients before and after curative prostatectomy.Results: This project was performed in Akdeniz University immunology laboratory and urology out patient clinic from 2008 to 2010. Peripheral Treg frequencies were signifi cantly increased in PCa patients (n = 19, 3.23 ± 1.59) compared with BPH patients (n = 27, 1.66 ± 0.80) and healthy subjects (n = 10, 1.08 ± 0.43) (p < 0.01). The percentage of Treg in BPH patients was also signifi cantly higher than that of healthy subjects (p < 0.01). Importantly, the increase in BPH and PCa patients paralleled the elevation in total serum PSA levels, demonstrating a strong positive correlation (r = 0.75; p < 0.01).Conclusion: These results demonstrate that peripheral Treg densities are correlated with PSA in BPH and PCa patients, suggesting that PSA may have a role in Treg induction and/or maintenance in Treg in these people
Salt in nutrition of University of Sarajevo students
Introduction: The role of salt in the diet is very important, however what effect will lead to its entry into the organism depends on the amount of salt ingested. Elevated salt intake is a risk factor for hypertension, heart disease, adversely affects the kidneys, helps the development of osteoporosis. The aim was to examine sodium intake in the student population through an objective assessment of nutrition students and the students\u27 subjective experience of salty foods and salty foods influence on the organism.Methods: The research is cross-sectional study, which is carried out in April 2010 among students at the University of Sarajevo.Results: More than half of students from the FHS considers that the greatest amount of salt into the body, adding salt to food during the meal, 34% considered that it was during the preparation of food by 10% of processed products in the food industry and 4% said that it was the intake of salt, which naturally found in foods. Not in a group of students there who suffer from hypertension, while in their families the disease is present.Conclusions: Subjects eat a quantity of salt that are well above the recommended. This problem must be approached in a more organized educational, health educational and research terms
Knowledge and practice of health managers in using information technology in health system
Introduction: Information systems today are an inevitable link in the health care system. Health care institutions and health workers in modern society, depend on information systems in everyday tasks of organizing institutions.The goals of the study were to determine the existence and application of information technology in the health care system and of the Clinical Center of Sarajevo University and, Primary Health Care Center of Sarajevo Canton, as well as to assess knowledge and practice of managers in nursing of the Organizational units of the University Clinical Center in Sarajevo and Primary Health Care Center of Sarajevo Canton.Method: The study is of a prospective type. It will use a questionnaire based on which we will be able to determine the knowledge and application in practice of managers in nursing of knowledge about information technology in the health care system. The study was conducted from June 1st – August 1st 2010.Results: It is expected that most health institutions in the Canton Sarajevo has no organized information network system. Most managers’ nurses use e-mail in order to quickly contact other colleagues and exchange experiences. A large number of nurse’s managers in health institutions use computer technology in their daily work in order to make medical reports.Conclusions: Health system and information technology are in close relationship, although still insufficiently applied in everyday work. Management of organizational units in health care in the future will not be able to successfully function without the use of information technology in their daily work
Severe anemia: a case report
Anemia refers to a hemoglobin or hematocrit level lower than the age-adjusted reference range in healthy children and adults. Anemia is not a specifi c disease entity but is a condition caused by various underlying pathologic processes. The clinical effects of anemia depend on its duration and severity. When a precipitous drop in the hemoglobin or hematocrit level occurs (eg, due to massive bleeding), the clinical presentation is typically dramatic and can be fatal if the patient is not immediately treated. Even then, mortality risk is very high. We report the case of a 76-year-old woman with clinical symptoms and laboratory confirmation of severe anemia with level of hemoglobin 24 g/l, and hematocrit 0.08. Anemia was a sign of malignoma of the stomach, later patohistologicaly verifi ed gastric adenocarcinoma. Aim of management is to prevent tissue hypoxia by maintaining an adequate circulating volume and oxiform capacity. However, as shown in this case, the very rapid correction of anemia and the circulatory volume does not decrease the risk of fatal outcom
Evaluation of the conservative treatment of Trigger finger by local instillation of corticosteroids
Introduction: Trigger Finger (tenosynovitis stenosans) is a specific, named disease from a group of repetitive strain injury (RSI) diseases, caused by inflammation which results in difficulties during muscle contraction and weakened and painful tendon movement. It is common in the outpatient physical medicine and rehabilitation practice. The aim of our study was to evaluate the success of conservative treatment of Trigger Finger by local instillation of corticosteroids.Methods: The study was designed as an observational and open analysis of the results of conservative treatment of 45 patients. We used precise instillation of steroid anti-inflammatory antirheumatic drugs in the area of patho-anatomic, microtraumatic injuries of tendon and its sheath. Patients were evaluated before and after the treatment with 0 to 5 evaluation score scale. The data were analyzed using X2 test.Results: Most of the patients had evaluation score of 2, 3 and 4, before the treatment. After the treatment 10 (29%) patients had achieved score 4 and 35 (71%) patients had achieved score 5. All of the patients with score 5 had excellent working ability with full working capacity. Other patients had well-preserved working ability, which improved to excellent in maximum of 7 days.Conclusions: Conservative treatment of Trigger finger shows good therapeutic effects and taking into account the benefits, convenience and generally lower cost of conservative treatment for the patient, should be considered as an effective alternative to surgical treatment
Helicobacter pylori infection according to ABO blood group among blood donors in Kosovo
Introduction: Numerous studies have reported a high prevalence of Helicobacter pylori infection among healthy and non-healthy persons in different places. The Aim of the study is to investigate the seroprevalence of H. pylori infection among Kosovo’s Blood donor associated with ABO/Rhesus blood group.Methods: 671 blood donors are tested for H. pylori antibodies and results are classifi ed by way of donation, age, gender, blood groups and education level. Serum antibodies are analyzed by Enzyme Linked Fluorescent Assay test for H. pylori IgG with Biomerieux HPY-VIDAS.Results: The frequency of IgG antibody for H. pylori among healthy blood donors is 56.9%, there is not found any difference between voluntary and non-voluntary blood donors (57.4% respectively 56.3%)(OR=1.05; 95% CI 0.76 to 1.43; p=0.8). H pylori IgG antibodies positive are detected in 57.0 % ( 126 of 221) of women, compared with 56.9 % ( 256 of 450) of men(OR=0.99; 95% CI 0.72 to 1.38; p=0.96). Serpositive donors are older than seronegative ones (31.9 years, respectively 29.5 years, p=0.02). Mean value of IgG antibody of H. pylori is 3.61 with no significant difference between males and females (3.72 respectively 3.44; p=0.2). The seroprevalence of H. pylori infection is similar among blood groups: O (57.4%), A (56.2%), B (59.6%), AB (51.4%), RhD positive (56.7%) and RhD negative (58.3%).Conclusions: The seropositivity of H. pylori is moderately higher in the non voluntary and familiar blood donors among the total Kosovo blood donors. There is not found a significant relationship between infection with H. pylori and ABO/Rhesus blood group among blood donors
