    Etiological factors as predictor of rehabilitation in patients after carebrovascular insult

    Introduction: Cerebrovascular insult (CVI) is acute or sub-acute occurrence of symptoms which signal death of cerebral cells caused by localized disruption of arterial circulation in the brain. The goal of this study is toinvestigate whether ischemic or hemorrhagic CVI can be used as predictor of rehabilitation.Methods: A retrospective study was conducted in the period from January 2009 to the December 2009 and as a source of data we used medical records. The study included 89 patients who had CVI and who werehospitalized at the Clinic for Physical medicine and rehabilitation, Clinical Center University of Sarajevo (CCUS). We analyzed socio-demographic variables such as gender and age and clinical variables: the diagnosis,the length of stay in hospital (LOH), and Barthel index (BI) at admission and discharge from hospital.Results: Out of 89 patients, 78/89 (87.6%) were patients with ischemic CVI (group A), and 11/89 (12.4%) with hemorrhagic CVI (group B). There was not a signifi cant association between the gender and type of CVI[(χ2(1)= .041, P> .05]. There was a statistically signifi cant difference in median of length of hospitalization (LOH) between two groups (U=186.5; z=-3,025; P= .002). There was not a statistically signifi cant differencein median of BI at admission (U=317.0; z=-1,399; P= .162) and  discharge (U=319.0; z=-1.374; P= .169) between two groups.Conclusion: Patients with hemorrhagic CVI have a longer stay in hospital and consequently more expensive cost of treatment

    Age and gender related differences in free fatty acid levels in patients with type 2 diabetes mellitus

    Introduction: Several decades of basic science and animal research provided considerable support for significant role of plasma free fatty acids (FFAs) in etiology of Type 2 diabetes mellitus (T2DM). Contradicting data related to signifi cance of elevated FFAs in plasma of patients with Type 2 diabetes prompted us to study concentrations of palmitic acid, stearic acid, and linoleic acid, in patients and healthy controls in an attempt to possibly use them as potential biomarkers in progression of the disease. Since aging is associated withincreased plasma glucose and insulin levels that are consistent with an insulin resistant state, in this study,age differences in the concentration of the above mentioned acids were tested.Methods: Progressive changes in their concentrations were followed through a period 6 months. All subjects included in the study were free of evidence of hepatitis B or C viral infection or active liver and kidney damage. Analysis of glucose and glycated hemoglobin levels were performed on BT PLUS 2000 analyzer using standard IFCC protocols, while concentrations of FFAs were analyzed by gas chromatography.Results: Our data demonstrated signifi cantly higher FFA values in plasma of diabetic patients as compared to healthy controls. There was a trend of correlation of FFAs levels with the blood glucose levels in diabetic patients, which was more prominent in diabetic men than in women.Conclusion: With aging, levels of free fatty acids signifi cantly increased in plasma of diabetic patients, and this effect was also more profound in male than in female diabetics

    Analysis of the relation between intelligence and criminal behavior

    Introduction: One of the cognitive aspects of personality is intelligence. A large number of previous studies have shown that the intelligence within the criminal population is decreased, particularly in its verbal aspect.The aim of this study is to determine whether there is a link between intelligence and criminal behavior and how it is manifested.Methods: The research involved criminal inmates of the Correctional institutes of Republic of Srpska and Court Department of Psychiatry Clinic Sokolac who committed homicide and various non-homicide acts. Thetest group consisted of 60 inmates who have committed homicide (homicide offenders) and a control group of 60 inmates who did not commit homicide (non-homicide offenders). The study was controlled, transverse or cross-sectional study.Results: Average intelligence of inmates (homicidal and non-homicidal) was IQ 95.7. Intelligence of homicide inmates was IQ 97.4 and non-homicide IQ 94.09. Intelligence coeffi cients for non-homicide inmatesubgroups were as follows - subgroup consisting of robbery offenders (IQ 96.9), subgroup consisting of theft perpetrators (IQ 93.83), subgroups consisting of other criminal offenders (IQ 92.8). Verbal intellectual ability– IQw of homicide inmates was 91.22, and 91.10 IQw of non-homicide inmates. Intellectual abilities in nonverbal or manipulative part were average, but they were higher in homicide inmates group (IQm 103.65) than in the group of non-homicide inmates (IQm 97.08).Conclusion: Average intelligence of investigated inmates (homicide and non-homicide) is lower than in the general population and corresponds to low average. Verbal part of intelligence is lowered while nonverbalpart is within the average range

    Distribution of ABO blood group in children with acute leukemias

    Introduction: This study is the fi rst study about the distribution ABO blood types at children with acute leukemia in Federation of Bosnia and Herzegovina. The aim of the study is to point out distribution of blood type groups at children with acute leukemia (AL)Methods: The number of children in this study was the following: 145 children with acute leukemia and 27 of children with acute myeloblastic leukemia (AML). All of the children were treated at Hemato- Oncology Unitof Pediatric Clinic in Sarajevo, in the period January 2000 until December 2010. Age of children was between 1 month and 15 years.Results: The results showed that different blood types were registered in 93. 1% of children who got ill and treated from acute leukemia for the mentioned period. At 6. 9 % of children, none of the blood types wereregistered. It was noticed that 40.9 % children who have registered blood type O, 37% blood type A,16% blood type B and 6.5% blood type AB had AL, too. It has been observed that children with following bloodtypes had AML: O, 47.8%, A, 47.7% and AB, 30.4%.Conclusion: Signifi cance ABO types distribution was confi rmed for children with ALL, p<0, 05. The analysis of the distribution of ABO types based on gender showed that signifi cance was confi rmed at females with both ALL and AML (p<0.05)

    Ruptured intracranial dermoid cyst: a case report

    Intracranial dermoid cysts are congenital, usually nonmalignant lesions with an incidence of 0.5% of all intracranial tumors. They tend to occur in the midline sellar, parasellar, or frontonasal regions. Although theirnature is benign, dermoid cysts have a high morbidity and mortality risk, especially when rupture occurs. A 40 year old woman presented with head injury after she experienced sudden loss of consciousness. She hada history of headache, loss of consciousness; her past medical history was not remarkable. The patient had no complaints of nausea, vomiting, or seizures. Vital signs were stable, neurologic defi cit was not identifi ed.Computed tomography (CT) and magnetic resonance imaging (MRI) showed right temporobasal zone with fat droplets within right fi ssure Sylvii and interhemispheric fi ssure indicating a rupture of a dermoid cyst. Craniotomy and cyst resection were done, and diagnosis was confirmed with pathological examination following surgery. After surgery the patient did not recover. Cerebral ischemia from chemical meningitis was fatal forour patient. Headache as a symptom has many causes. It is rarely due to chemical meningitis arising from a ruptured dermoid cyst. This case report illustrated the importance of investigating a cause of the headache,CT and MRI being diagnostic methods. In this way, mortality as well as morbidity from complications such as chemical arachnoiditis can be significantly reduced if imaging is done early in these patients

    Does wound infi ltration of tramadol reduce postoperative pain in laparoscopic or open herniorrhaphy?

    Introduction: The laparoscopic approach may be associated with more postoperative pain initially. The aim of this study was to evaluate the effects of administered tramadol at wound closure on postoperative painand analgesic requirements under spinal anesthesia in laparoscopic inguinal herniorrhaphy (LH) or tension free open inguinal herniorrhaphy (TFOH).Methods: Twenty patients were randomly divided into two groups (n= 10 in each) as LH or TFOH. Patients received infiltration of 200 mg tramadol with 40 mL of 0.9% saline solution at wound closure procedure. Postoperative pain was assessed with a Visual Analog Scale (VAS) at 3, 6, 12, and 24 hours postoperatively. Additional requirements of tramadol for postoperative pain releif were registered.Results: VAS scores at postoperative 12 and 24 hours were signifi cantly higher according to 3rd hour VAS scores in both groups. The VAS scores at 12 hours after operation signifi cantly lower in LH group than inTFOH group (1.5 ± 0.97 vs 5.1 ± 0.99). Additional requirements of tramadol for postoperative pain releif were significantly lower in LH group.Conclusion: We conclude that wound infi ltration of 200 mg tramadol reduce postoperative pain in LH group

    Osteoporosis in active working women

    Introduction: Osteoporosis is a progressive metabolic bone disease characterized by reduction of mineral density of bone, which leads to reduction of bone firmness, increased fragility and increased risk of bonefractures. The aims of this study were to determine the age structure and average values of BMI in female patients with a diagnosis of osteoporosis and osteopenia, to determine the value of T-score before and aftertherapy, and to show a correlation of frequency of fractures in relation to already given diagnosed and the presence of menopause.Methods: A retrospective study was conducted on 50 female respondents with diagnosis of osteoporosis and osteopenia. Included female respondents underwent densitometry or ultrasound screening method ofheels in which high degree of osteopenia and osteoporosis is detected.Results: The average age of the female respondents included in this study was 48.06 ± 11.97 years and all the respondents were in the category of women with normal body weight. There is a difference in the values of T-score of respondents with osteoporosis compared to osteopenia. Value of T-score decreases in relation to increase of number of years, so the older female respondents had lower values of T-score.Conclusion: The incidence of osteoporosis and osteopenia was higher among active working female respondents in menopause. Respondents with osteoporosis had lower values of T-score, physical and medicamenttherapy in combination led to improvement of T-score. Female respondents with a low value of T-score, with diagnosis of osteoporosis and in menopause, mostly had bone fractures

    Awake fi beroptic intubation of a patient with amyotrophic lateral sclerosis: case report

    Amyotrophic Lateral Sclerosis is a rapidly progressive disease from the fi fth to sixth decades of life causing degeneration and death of the upper and lower motor neurons and no effective treatment. The diagnosis isdependent on the clinical presentation and consistent electrodiagnostic studies. Progressive denervation affects the muscles, causing muscular weakness and atrophy, when the ventilation muscles are affected deathdue to respiratory failure occurs within a few years. We present the case of a 54 years old, 180 cm height and 94 kg weight male patient with amyotrophic lateral sclerosis who underwent surgical treatment of thyroidcancer. Fiberoptic intubation was orally performed providing spontaneus breathing. Propofol was applied after passing vocal cords. Anesthesia was maintained with sevofl orane (%2) and a mixture of oxygen and airunder volume controlled ventilation. Rocuronium was used 20 mg at the beginning of the surgery. At the end of surgery, he wasn’t extubated and transferred to anesthesia intensive care unit. He was extubated after tenhours and he was awaked perfectly. The patient was discharged from intensive care unit after 24 hours and from hospital after ten days. We reported that amyotrophic lateral sclerosis patient with limited mouth opening who underwent thyroid surgery, using awake intubation

    Prevalence of depression in residents of gerontology centre in Sarajevo

    Introduction: Depressive disorder, as a major problem of public health, takes high fourth place in its prevalence in general population, and is considered to be the second most frequent health problem of femalepopulation. Depression is the most frequent mental problem of persons in their third age of life. The aim of this study is to evaluate prevalence of depression and establish the ratio between the current number ofdiagnosed and of unrecognised depression among the residents of Gerontology Centre in Sarajevo.Methods: This is a cross-sectional, descriptive, and analytical study undertaken throughout May and June 2011 on the sample of 150 residents of “The Gerontology Centre“ in Sarajevo that were above 65 years of age. The following instruments were used for the research: the Geriatric Depression Scale (GDS), modified questionnaire consisting of two parts (general data and data related to health state), and the medical records of the residents. For statistic analysis of data was used the SPSS program for Windows.Results: According to GDS, prevalence of depression was 65.3%, out of which mild depression occurred in 46.7% cases and severe depression in 18.7%. The prevalence of verified (diagnosed) depression was 11.3per cents.Conclusions: According to the GD scale, unrecognised depressions seem to be almost six times more frequent (65.3:11%) than is the case with depressions diagnosed in medical records of the protégées of theGerontology Centre in Sarajevo. Timely recognition of depression and its treating in institutions for protection of health of persons in third age of life can substantially improve the quality of life of these patients

    Effectiveness of physical treatment at De Quervain᾽s disease

    Introduction: De Quervain᾽s disease is a stenosing tenosynovitis of common tendon sheath of abducktor policis longus and extensor policis brevis muscles. Due to the superficial positions it can easily lead tomechanical injuries of tendons and their sheaths. The disease more often affects women over 40 years old and people with certain professions who intensively use hand and fingers in their daily work. Pathologicalchanges consist of sheath᾽s fibrous layer thickening. The clinical condition develops gradually with the pain of varying intensity. It is localized above the radial styloid process and radiates from the back side of thethumb. The aim is to determine the efficacy of physical therapy at De Quervain᾽s disease.Methods: The study was conducted on 50 patients with De Quervain\u27s disease who were reported to the CBR "Praxis" Sarajevo. With retrospective analysis the data was processed for the period from 01.01.2001. to 31.12.2011. year. Before the initiation of physical therapy assessment of functional status scored from 0 to 6 was performed. In the chronic phase physical therapy was performed, after which it underwent assessment of therapy success scores of 0-5. Criteria for inclusion in the study were patients with confirmed De Quervain\u27s disease, patients of both sex and of all ages, and criterion for exclusion was non-compliance withtreatment protocols.Results: In the CBR "Praxis" with De Quervain᾽s disease total of 50 patients were treated in that period, of which 34 women and 16 men. 38% of respondents received a score of 4, while 56% of patients at the end oftreatment received a score of 5.Conslusion: Physical therapy and kinesiotherapeutical procedures have greatly contributed to the elimination of symptoms and consequences of De Quervainove disease
