    Utilización de plasma de convaleciente en pacientes con neumonía por sars-cov-2. Serie de casos

    INTRODUCTION: Coronaviruses are prevalent viral pathogens among humans and animals. At the end of2019, a new coronavirus was identified in the city of Wuhan, China. It spread rapidly causing an epidemicin that country, followed by a steady increase in the number of cases globally. In February 2020 the WorldHealth Organization (WHO) designated the disease "COVID-19", an acronym for coronavirus disease2019. The virus that causes COVID-19 is called severe acute respiratory syndrome coronavirus 2 (SarsCoV-2). In March of this year the disease was declared a pandemic by the WHO. With multiple studies regarding various treatments (symptomatic or antiviral) with questionable evidenceand clinical results not applicable to the general population, emerged the need for a specific treatment thatreally improves the natural history of the disease.The convalescent plasma (CP) seems to date one of the answers to this need. It is obtained from donorswho have recovered from the infection and who have generated antibodies against it (measured in blood asIgM and IgG specific for Sars-CoV-2). Ideally it contains antibodies against this pathogen in a sufficienttiter and adequate biological activity to provide passive immunity to the recipient.These antibodies bind to the virus, reducing its entry into cells and facilitating elimination thanks tophagocytosis or cellular toxicity. Both are antibody-dependent processes.In the natural history of the disease, the body takes between two to three weeks to generate its ownantibodies. CP has the potential capacity, at the acute moment of infection, to lessen the severity ofsymptoms or shorten their duration, in order to slow the progression to a more serious disease, accordingto preliminary studies.OBJECTIVE: To report the clinical evolution of patients with bilateral COVID-19 pneumonia whoreceived CP.MATERIAL AND METHODS: The clinical evolution of 6 hospitalized patients diagnosed with COVID19 was retrospectively analyzed using a nasopharyngeal swab with RT-PCR technique of Sars-CoV-2infection. All of them had moderate or severe bilateral pneumonia on high resolution chest computedtomography (CT). In turn, they met criteria for severe pneumonia (respiratory failure requiring oxygentherapy, PAFI <300, D-dimer> 1000 ng / dl and / or lymphopenia). They received 1 or 2 weight-adjustedPC units and the patients were reassessed at 72 hours and until discharge.RESULTS: 6 patients were documented. For presenting Severe Pneumonia criteria, they received CPassociated with oxygen therapy in an intensive care unit. The ages ranged from 49 to 67 years. Of the total,4 were men and 2 women.At 72 hours after the PC infusion at the clinical level, the patients presented improvement in dyspnea untilits disappearance, and a lower requirement for oxygen flow. Before the infusion, to maintain a correctoxygen saturation, the patients required a high-flow nasal cannula (HFNC) with a 33% FIO2, in theevolution at 72 after CP they required a low-flow nasal cannula with 24% FIO2 for maintain a correctoxygen saturation and eliminate dyspnea. Between days 4 to 5 after CP, 100% of the patients had a correcttolerance to ambient air (FIO2 21%).Also at an analytical level, patients clearly improved at 72 hours. The levels of the relationship betweenarterial oxygen pressure and the inspired fraction of oxygen (PAFI) that was <300 in all patients (valuesbetween 175 and 215) prior to the infusion of PC, afterwards 100% had values higher than 300.Tables 1 and 2 show the improvement in the clinical parameters (dyspnea and fever) and laboratoryparameters (PAFI and CRP) at the beginning of the infusion of CP and 72 hours after it.During the entire hospitalization, no patient required mechanical ventilation and no deaths were reported.No adverse reactions to PC were observed either.CONCLUSIONS: The risks of COVID-19 infection are significant. Information regarding CP in COVID19 patients is scant, although current preliminary data are promising. Furthermore, PC infusion is supportedby extensive evidence regarding the use of PC in other viral coronavirus infections (MERS, SARS) withextensive use and excellent results. However, it is important to carry out controlled, randomized andsystematized clinical studies to confirm this efficacy.INTRODUCCIÓN: El virus que causa COVID-19 es denominado síndrome respiratorio agudo y grave porcoronavirus 2 (Sars-CoV-2). Con múltiples estudios respecto a diversos tratamientos (sintomáticos oantivirales) de cuestionable evidencia y resultados clínicos no aplicables a la población general, emergió lanecesidad de un tratamiento específico que mejore verdaderamente la historia natural de la enfermedad. 1,2El plasma convaleciente (PC) parece al día de la fecha una de las respuestas a esta necesidad. Se obtiene apartir de donantes que se han recuperado de la infección y que hayan generado anticuerpos contra ella(medidos en sangre como IgM e IgG específicos para Sars-CoV-2). 3,4OBJETIVO: Reportar la evolución clínica de pacientes con neumonía bilateral por COVID-19 querecibieron PC.MATERIAL Y METODO: Se analizó de manera retrospectiva la evolución clínica de 6 pacientesinternados con diagnóstico de COVID-19. Todos ellos tenían neumonía bilateral. Recibieron 1 o 2 unidadesde PC ajustadas al peso y se reevaluaron los pacientes a las 72 horas y hasta la externación.RESULTADOS: A los pacientes se les administró PC. A las 72 horas de la infusión de PC a nivel clínicolos pacientes presentaron mejoría de la disnea hasta la desaparición de la misma, y un menor requerimientoen el flujo oxígeno. También a nivel analítico los pacientes mejoraron de manera evidente a las 72 horas.CONCLUSIONES: Los riesgos de la infección por COVID-19 son importantes. La información respectoal PC en pacientes con COVID-19 es escasa, aunque los datos preliminares actuales son prometedores.Además, la infusión de PC es respaldada por amplia evidencia respecto al uso de PC en otras infeccionesvirales por coronavirus (MERS, SARS) con extenso uso y excelentes resultados. Sin embargo, esimportante realizar estudios clínicos controlados, aleatorizados y sistematizados para confirmar estaeficacia

    Prevalencia y valor pronóstico del síndrome eutiroideo enfermo en niños críticamente enfermos

    INTRODUCTION: Euthyroid sick syndrome (ESS) is the main endocrine abnormality observed incritically ill patients. The presence and severity of ESS has been associated with a worse clinical course.OBJECTIVE: To analyze the prevalence of ESS in critically ill children and its correlation with disease’sseverity and its clinical outcome.MATERIAL AND METHODS: prospective, observational and analytical study in critically ill children ofthe intensive care unit between May 2019 and May 2020. ESS was defined as the alteration in thyroid testsin critical illness context and in the absence of pre-existing hypothalamic-pituitary-thyroid alterations.Other variables analyzed were: age, sex, length of hospitalization, requirement for mechanical ventilation and/or inotropic, diagnosis at admission, PIM3. Chi square test was used for qualitative variables andWilcoxon test for quantitative variables.RESULTS: Of the 47 patients studied, 28 (60%) presented ESS. No differences were observed betweensex (p>0,999), use of inotropics (p=0,1564) or deaths (p>0,999). Patients with ESS had a longer hospitalstay (p=0,0390) and required more invasive mechanical ventilation (p=0,0261). There was an associationbetween low levels of thyroid hormones and a higher coefficient and probability of mortality (p<0,0001and p<0,001, respectively).CONCLUSIONS: ESS is frequent in critically ill children and is mainly associated with two indicators ofseverity: length of stay and invasive mechanical ventilation. The association between low levels of thyroidhormones and high coefficients and probability of mortality turns the thyroid profile as an element to predictthe clinical evolution.INTRODUCCIÓN: el síndrome eutiroideo enfermo (SEE) es la principal anomalía endocrinológica enenfermos críticos. La presencia y gravedad del SEE se asocia a peor evolución clínica.OBJETIVO: conocer la prevalencia del SEE en niños críticamente enfermos y su correlación con lagravedad y resultado clínico de la enfermedad.MATERIALYMÉTODO: estudio prospectivo, observacional y analítico en niños críticamente enfermosinternados en terapia intensiva entre mayo de 2019 y mayo de 2020. Se definió al SEE como la alteracióntiroidea en enfermos críticos sin patología hipotálamo-hipófiso-tiroidea preexistente. Las variablesanalizadas fueron: edad, sexo, duración de internación, decesos, requerimiento de ventilación mecánica einotrópicos, diagnóstico al ingreso, PIM3. Para variables cualitativas se utilizó test de Chi cuadrado, paravariables cuantitativas, test de Wilcoxon.RESULTADOS: de 47 pacientes estudiados, 28 (60%) presentaron SEE. No hubo diferencias respecto asexo (p>0,999), decesos (p>0,999) ni uso de inotrópicos (p=0,1564). Hubo asociación significativa entreSEE y más días de internación (p= 0.0390) y mayor requerimiento de ventilación mecánica invasiva (p=0.0261) y entre valores bajos de hormonas tiroideas y mayor coeficiente y probabilidad de mortalidad delPIM3 (p<0,0001 y p<0,001, respectivamente).CONCLUSIONES: el SEE es frecuente en niños críticos y se asocia a dos indicadores de gravedad:duración de internación y requerimiento de ventilación mecánica invasiva.     Estudio de la sección transversal de premolares inferiores con tomografía computada de haz cónico

    INTRODUCTION: Mandibular premolars are known for the complex nature of their root canalconfiguration. Generally, the configuration of the single canal in lower premolars with one root canal isnarrow and long. Currently we have more precise diagnostic tools such as Cone Beam ComputedTomography to evaluate the morphology of the canals in the three planes of space, with less radiation thana conventional tomography.OBJECTIVE: is to study the cross section of the human mandibular premolar canals, with a single rootcanal.MATERIAL AND METHODS: 120 human lower extracted premolars were studied. The specimens werestudied by means of CBCT, with an exposure time of 15 seconds, 90KV 3.2mA, dose 1098mGy.cm² voxelsize 150x150x150μm, with a Carestream 8200 tomograph. From the total sample, only the premolars witha single canal were selected through the CBCT. They were analyzed in perpendicular cuts to the long axisof the tooth (axial) In each tooth the diameter in the canal were measured in the buccolingual andmesiodistal direction (expressed in milimeters) in the three thirds: cervical, middle and apical.Subsequently, the morphology of the canal in the axial sections was analyzed, in the three thirds mentionedabove, classifying them as: circular, oval, long oval and flat shape. The quantitative variables wererepresented in tables by means of average and deviation, while the qualitative ones with frequencies andpercentages. Friedman tests and chi-square uniformity tests were performed. The soft RMedic and Infostatwere used. In all cases the level of significance was 5%.RESULTADOS: El 1.22% of the cases presented a circular canal and 4.88% oval throughout its route.93.9% of the sample presented different morphologies in the 3 thirds of the canal: LCC in 21.95%, followedby ACC with 20.73% and OCC with 10.98%. Were observed a predominance of circular canals in 84% inthe apical third and 56% in the middle; oval in the middle third in 31.71% and flat in the cervical third37.8%. The average diameter of the circular canals at 3 mm from the apex was 0.55 mm2. In cervical theaverage diameter was 2.58 mm in BL and 0.83 mm in MD and in the middle third 1.29 mm in BL and 0.82mm in MD.CONCLUSIONS: The lower premolars with a single canal presented different morphologies in three thirds.The predominance of anatomy was Elongated or Flattened in the Cervical third (37.80%) and predominanceof Circulars in The Middle (56.10%) and Apical (84.15%). This type of canals, which vary theirmorphology longitudinally, can be a real challenge for their instrumentation, irrigation and sealing.INTRODUCCIÓN: Los premolares inferiores son conocidos por la naturaleza compleja de la configuraciónde sus conductos. Generalmente cuando presentan uno solo es estrecho y acintado. En la actualidadcontamos con herramientas de diagnóstico más precisas como las Tomografías Computadas de Haz Cónicopara evaluar la morfología de los conductos en los tres planos del espacio.OBJETIVO: Es estudiar la sección transversal de los conductos de premolares inferiores humanos, con unsolo conducto radicular.MATERIAL Y MÉTODO: Fueron estudiados 120 premolares inferiores humanos extraídos. Losespecímenes fueron estudiados por medio de CBCT, con un tomógrafo Carestream 8200. Del total de lamuestra, se seleccionaron a través de las CBCT, sólo los premolares con un conducto. Los mismos fueronanalizados en cortes perpendiculares al eje largo del diente (axiales) En cada pieza dentaria se midió eldiámetro en sentido bucolingual y mesiodistal del conducto (expresado en milímetros) en los tres tercios:cervical, medio y apical. Posteriormente, se analizó la morfología del conducto en los cortes axiales, en lostres tercios antes mencionados, clasificándolos en: circulares, ovales, ovalesalargados y acintados. Lasvariables cuantitativas fueron representadas en tablas mediante promedio y desvío, mientras que lascualitativas con frecuencias y porcentajes. Se realizaron pruebas de Friedman y prueba de uniformidad chicuadrado. Se utilizaron los soft RMedic e Infostat. En todos los casos el nivel de significación fue del 5%.RESULTADOS: El 1,22% de los casos presentaron un conducto circular y 4,88 % ovalado en todo surecorrido. El 93,9% de la muestra presentó diferentes morfologías en los 3 tercios del conducto: LCC en21.95%, seguida por ACC con un 20.73% y OCC con un 10,98%. Se observó un predominio de conductoscirculares en el 84% en el tercio apical y 56% en el medio; ovalados en tercio medio en un 31,71% yaplanados en el tercio cervical en un 37.8%. El diámetro promedio del conducto circular a 3 mm del ápicefue de 0,55 mm2. En cervical el promedio de diámetro mayor fue de 2.58 mm en BL y 0.83 mm en MD yen tercio medio 1,29 mm en BL y 0.82 mm en MD.CONCLUSIONES: Los premolares inferiores con un solo conducto presentaron diferentes morfologías enlos tres tercios del conducto. El predominio de la anatomía fue Alargada o Aplanada en el tercio Cervical(37.80%) y predominio de Circulares en Medio (56.10%) y Apical (84.15%). Este tipo de conductos, quevarían su morfología longitudinalmente, pueden ser un verdadero desafío para su instrumentación,irrigación y obturación

    INTRODUCTION: This review arises from the first cytogenetic study, carried out in Marcos Juárez(Córdoba Province, Argentina), where the population had been drinking water containing more than 0.13mg/l (0.13 ppm) of arsenic. In this work he called the attention to the relationship between lung cancer andthe arsenic content in drinking water consumption, because the causes of death of this population betweenthe years 1970-1988, showed a significant increase in lung cancer, this finding is similar to much researchon the subject. The literature reviewed shows that exposure to arsenic exerts deleterious effects on healthmainly through the induction of oxidative stress, alterations in DNA methylation, histone modification andmiRNA expression and it would be interesting to determine the presence and consequence of thesealterations in the populations affected by arsenic contamination of water without lose sight of otherenvironmental agents. Understand how it acts and the promotion of carcinogenesis related to arsenicbiotransformation processes would be important for detection and treatment in affected people.INTRODUCCIÓN: Esta revisión surge partir del primer estudio citogenético, realizado en Marcos Juárez(Provincia de Córdoba, Argentina), en donde la población había estado bebiendo agua que contenía más de0,13 mg/l (0,13 ppm) de arsénico. En este trabajo llamó la atención la relación entre el cáncer de pulmón yel contenido de arsénico en el agua de consumo, debido a que las causas de muerte de esta población entrelos años 1970-1988 mostraron un significativo aumento, siendo este hallazgo similar a otras investigacionesposteriores sobre el tema. La literatura revisada muestra que la exposición al arsénico ejerce efectosdeletéreos para la salud, principalmente a través de la inducción de estrés oxidativo, alteraciones en lametilación del ADN, modificación de histonas y expresión de mi ARN; por lo que sería interesantedeterminar la presencia y consecuencia de estas alteraciones en las poblaciones afectadas por lacontaminación del agua con arsénico, sin perder de vista otros agentes ambientales. Comprender comoactúa y la promoción de la carcinogénesis relacionada con los procesos de biotransformación del arsénicosería importante para la detección y tratamiento en las personas afectadas

    Primary immune thrombocytopenia (ITP) is a rare pathology characterized by isolated thrombocytopeniaof autoimmune origin, with a platelet count <100 x109 / L not associated with a recognizable pathology,with an increased risk of bleeding. There is no definitive confirmatory test to establish its diagnosis, whichis carried out mainly by exclusion. There is a small percentage of patients who do not respond toconventional treatment. Refractory ITP refers to severe thrombocytopenia that persists after splenectomy,while patients who meet criteria for severe ITP but who have not undergone splenectomy are considered“non-responders” to the drugs used up to that time. Three clinical cases of severe, persistent and chronicITP, non-responders to treatment, are presented with the aim of pointing out the importance of a thoroughdiagnostic investigation and the development of an effective combination of treatments for the managementof this complex pathology.La trombocitopenia inmune primaria (PTI) es una patología rara caracterizada por trombocitopenia aisladade origen autoinmune, con un recuento <100 x109 plaquetas/l no asociado a una patología reconocible, quecursa con un aumento del riesgo de hemorragia. No existe ninguna prueba confirmatoria definitiva paraestablecer su diagnóstico, que se realiza fundamentalmente por exclusión. Existe un pequeño porcentaje depacientes que no responden al tratamiento convencional. Se denomina PTI refractaria a la trombocitopeniasevera que persiste luego de realizada la esplenectomía, mientras que los pacientes que cumplen criteriosde PTI severa pero que no han sido esplenectomizados se consideran “no respondedores” a los fármacosutilizados hasta ese momento. Se presentan tres casos clínicos de PTI severa, persistente y crónica, norespondedores al tratamiento, con el objetivo de señalar la importancia de una concienzuda pesquisadiagnóstica y el desarrollo de una eficaz combinación de tratamientos para el manejo de esta complejapatología

    INTRODUCTION: Hyperglycemia is a prognostic factor for mortality. Patients with hyperglycemia havean increased risk of intra-hospital complications, such as pneumonia, urinary tract infection, acute kidneydisease, sepsis. During hospitalization, blood glucose values between 140-180 mg / dL are recommended.OBJETIVE: To determine the causes of hyperglycemia in patients hospitalized at Clinica UniversitariaReina Fabiola.MATERIAL AND METHOD: This is an observational, retrospective and descriptive study. The populationincluded patients over 18 years of both sexes who were admitted to the Medical Clinic Service of the ClinicaUniversitaria Reina Fabiola. Glycemia at admission and 72 hours after admission as well as glycatedhemoglobin (HbA1c) were evaluated in hyperglycemic patients who were not diagnosed with diabetes priorto hospital admission. Hyperglycemia will be considered at blood glucose values greater than 140 mg / dl,diabetes mellitus diagnosed at the time of hospitalization in the presence of an HbA1c> 6.5%, and patientswith stress hyperglycemia who have HbA1c less than 6.5% and some cause that justifies it. Variables to beanalyzed: glycemic value, glycated hemoglobin, age in years, sex, personal pathological history, reason forhospitalization, situations related to hospitalization. Statistical analysis: the data were presented withdescriptive statistics.RESULTS: The final sample consisted of a total of n = 900 patients, 54% were male. The mean age was57.3 years. Of the patients admitted, 192 (21.3%) had a blood glucose value greater than or equal to 140mg / dl, and at 72 hours this indicator rose to 486 (54%) patients. Regarding sex, no differences were foundin glucose measurements. Hyperglycemia occurred in a higher proportion in those patients who had anunscheduled hospitalization: 156 (81%) vs. 42 (19%). This finding increased at 72 hours. Of the totalnumber of patients, there were 126 (14%) with a previous history of diabetes. Glycosylated hemoglobinwas measured in 426 (47%) patients who had hyperglycemia on admission or at 72 hours, 72 (16.9%) inglycosylated hemoglobin> 6.5%, who could be defined as patients with diabetes mellitus of recentdiagnosis. Glycated hemoglobin <6.5% had 342 (80.2%) patients, which could be interpreted clinically asthe group of patients with stress hyperglycemia. This group was associated in order of frequency with thefollowing characteristics: postsurgical status, treatment with corticosteroids, active infection,immunosuppressive therapy, acute renal failure, and parenteral nutrition or enteral feeding. Hypertensionwas the most frequent personal history.CONCLUSIONS: In this study, the proportion of adults patients with hyperglycemia was 20%; thisproportion is similar to that published in the literature. Glycemic values greater than 140 mg/dl were presentin patients with and without diabetes mellitus; the latter being more frequent among those hospitalized forsurgeries and infections, and in those who were receiving treatment with corticosteroids. Of importance, animportant number of cases of diabetes mellitus were diagnosed during the hospitalization.INTRODUCCION: La hiperglucemia constituye un factor pronóstico de morbi-mortalidad. Los pacientescon hiperglucemia tienen mayor riesgo de complicaciones intrahospitalarias tales como neumonía,infección de vías urinarias, enfermedad renal aguda, sepsis, muerte durante la internación o en cortosperíodos post alta. Durante la internación se recomienda valores de glucemia entre 140-180 mg/dL.OBJETIVO: Establecer las causas de hiperglucemia en pacientes adultos internados en la ClínicaUniversitaria Reina Fabiola.MATERIALES Y METODOS: Estudio observacional, retrospectivo y descriptivo. La población incluyópacientes mayores de 18 años de ambos sexos que ingresaron al internado del Servicio de Clínica Médicade la Clínica Universitaria Reina Fabiola. Se evaluó la glucemia plasmática al ingreso y a las 72 horas deinternación, así como la hemoglobina glicosilada (HbA1c) a los pacientes hiperglucémicos que no teníandiagnóstico de diabetes previo al ingreso hospitalario. Se consideró hiperglucemia a valores de glucemia≥140 mg/dl, se infirió diagnóstico de diabetes mellitus al momento de la internación a la presencia de unaHbA1c ≥6.5 % y pacientes con hiperglucemia de estrés a los que tenían HbA1c menor a 6.5 % y algunacausa que lo justifique. Variables analizadas: valor de glucemia, hemoglobina glicosilada, edad en años,sexo, antecedente personales patológicos, incluido diabetes mellitus, motivo de internación, situacionesrelacionadas a la internación. Análisis estadístico: los datos se presentaron con estadística descriptiva.RESULTADOS: La muestra final estuvo conformada por un total de n=900 pacientes, el 54% fueron desexo masculino. La edad media fue de 57,3 años. De los pacientes ingresados, 192 (21.3%) tenían un valorde glucemia mayor o igual a 140 mg/dl, y a las 72 horas este indicador se elevó a 486 (54%) pacientes. Conrespecto al sexo, no se hallaron diferencias en las mediciones de la glucosa. La hiperglucemia se presentóen mayor proporción en aquellos pacientes que tuvieron una internación no programada: 156 (81%) vs. 42(19%). Del total de pacientes, hubo 126 (14%) con antecedente previo de diabetes. Se les midió lahemoglobina glicosilada a 426 (47%) pacientes que habían presentado hiperglucemia al ingreso o a las 72hs, 72 (16.9%) presentaron una hemoglobina glicosilada >6.5%, a los cuales se los podría definir comopacientes con diabetes mellitus de reciente diagnóstico. Presentaron hemoglobina glicosilada <6.5% 342(80.2%) pacientes que se podrían interpretar clínicamente como el grupo de pacientes con hiperglucemiade estrés. Este grupo tenía las siguientes características: estado postquirúrgico, tratamiento con corticoides,infección activa, terapia inmunosupresora, insuficiencia renal aguda, y nutrición parenteral o alimentaciónenteral. La hipertensión arterial fue el antecedente personal que se presentó con mayor frecuencia.CONCLUSION: En este estudio, el 20% de los pacientes adultos internados presentaron hiperglucemia;este porcentaje coincide con la literatura publicada. Valores de glucemia mayores a 140 mg/dl estuvieronpresentes en pacientes con y sin antecedentes de diabetes mellitus, especialmente en aquellos que cursaroninternación por cirugías e infecciones, y en los que estaban con tratamiento con corticoides. También hubo pacientes en los que con el hallazgo de hiperglucemia se pudo inferir el diagnóstico de diabetes durante lainternación

    INTRODUCTION: Type 2 diabetes mellitus (DM) represent a serious health and socioeconomic problem,50% of people have no diagnosis, increasing the complications that underlie this pathology, since 25% willdevelop a diabetic foot ulcer (DF). Furthermore, DM predisposes to cognitive impairment (CI) being arelevant factor in DF patients, preventing proper personal care.OBJECTIVE: To examine if the cognitive profile in patients with diabetes is related to the severity oflesions in DF.MATERIAL AND METHODS: Retrospective, observational and analytical study on 100 patients treatedin the retirement center "Las Palmas" January 2018/January 2019 who met the inclusion criteria. Variablesanalyzed: Determination in blood of glucose and glycosylated hemoglobin (HbA1c). Degree of DF injuryaccording to the Texas scale. Evaluation of peripheral vascular lesion of the lower limbs using Dopplerultrasound. CI evaluation using Mini-Mental. Statistics: Non-parametric variables were analyzed using Chisquare or Irwin Fisher and parametric variables with ANAVA, significance p<0.05.RESULTS: We verified that the most frequent lesions were stage AI (33%) and stage B0 (28%), showingsignificance with stage A0 (17%) and AII (12%)(p:0.01). Together, both stage AI and B0 showed asignificant higher percentage of lesions contrasted with stages AIII (3%), stage C0 (6%) and stage D0(1%)(p:0.001). The Doppler ultrasound analysis showed (26%) of the patients registered triphasic waves(14%) belonging to stage A0 of PD. Biphasic wave morphology was the most prevalent (53%) of patients(p:0.001), of which (26%) were stage AI of DF, (15%) stage B0 and the difference was significant withrespect to AII (7 %) (p: 0.001). (21%) of patients registered monophasic waves, evidenced more frequentlyin the stages with lesions C0 (6%) and D0 (1%). The increases in glucose and HbA1c analyzed showed asignificant relationship with the accentuation of the DF lesion. In relation to the stages of DF injury and theMini-Mental score, in patients assessed as mild to normal DC (26.82±2.27) we found a percentage of stageA0, AI and AII. In patients with moderate to severe cognitive impairment, we found a percentage of stageAII with scores (24.97±2.12) and AIII (23.67±1.15) both significant (p:0.01). In addition, B0 exhibited asignificant score (24.25±1.51) with respect to A0 and AI (p:0.01), C0 with a score (22.5±2.59) verifiedsignificance with all stages A and B (p:0.001). In D0, the Mini-Mental result was 17 points, demonstratingimpaired cognitive functionality, showing significance in the DF injury stages A, B and C (p:0.001respectively). The relationship between scored stages of cognitive impairment and HbA1c levels verified acorrelation coefficient (r=-0.56) showing a good inverse relationship between both variables. Poormetabolic control of HbA1c and glycemia deteriorate the cognitive level.CONCLUSIONS: CI in patients with type 2 DM causes a decrease in self-care 6 of DF lesions, which isdirectly related to the severity of the lesions. Patients with increased levels of glucose and HbA1c exhibitedmore severe stages of DF injury and monophasic recordings on Doppler ultrasound evaluation. ElevatedHbA1c levels were associated with a low score in the cognitive assessment. The development of strategiesfor early identification of the progression of undiagnosed CI in vulnerable subjects is of great clinicalimportance and should be resolved as soon as possible.INTRODUCCIÓN: La diabetes mellitus (DM) tipo 2 representa un grave problema sanitario ysocioeconómico, 50% de las personas no tiene diagnóstico, incrementando las complicaciones quesubyacen en esta patología, ya que 25% desarrollarán úlcera de pie diabético (PD). Además, DM predisponea un deterioro cognitivo (DC) siendo un factor relevante en pacientes con PD impidiendo el correctocuidado personal.OBJETIVO: Examinar si el perfil cognitivo en pacientes con diabetes tiene relación con la severidad delesiones en PD.MATERIAL Y METODO: Estudio retrospectivo, observacional y analítico sobre 100 pacientes atendidosen centro de jubilados “Las Palmas” enero 2018/enero 2019 que cumplieron los criterios de inclusión.Variables analizadas: Determinación en sangre de glucosa y hemoglobina glicosilada (HbA1c). Grado delesión PD según escala de Texas.Valoración de lesión vascular periférica de miembros inferiores mediante ecodoppler. Evaluación de DCutilizando Mini-Mental. Estadística: Variables no paramétricas se analizaron utilizando Chi-cuadrado oIrwin Fisher y variables paramétricas con ANAVA, significación p<0.05.RESULTADOS: Verificamos que las lesiones más frecuentes fueron estadio AI (33%) y estadio B0 (28%),mostrando significancia con estadio A0 (17%) y AII (12%)(p:0.01). Conjuntamente, tanto estadio AI comoB0 mostraron mayor porcentaje significativo de lesiones contrastado con estadios AIII (3%), estadio C0(6%) y estadio D0 (1%)(p:0.001). El análisis de ecodoppler mostró (26%) de los pacientes registraron ondastrifásicas (14%) pertenecientes al estadio A0 de PD. La morfología de onda bifásica fue la más prevalente(53%) de pacientes (p:0.001), de los cuales (26%) fueron estadio AI de PD, (15%) estadio B0 y fuesignificativa la diferencia respecto a AII (7%)(p:0.001). El (21%) de pacientes registraron ondasmonofásicas, evidenciado con mayor frecuencia en los estadios con lesiones C0 (6%) y D0 (1%). Losincrementos de glucosa y HbA1c analizados mostraron relación significativa con la acentuación de lesiónde PD. En relación con estadios de lesión de PD y puntaje obtenido en Mini-Mental, en los pacientesvalorados como DC leve a normal (26,82±2,27) encontramos porcentaje de estadio A0, AI y AII. En lospacientes con DC moderado a grave encontramos un porcentaje de estadio AII con puntaje (24,97±2,12) yAIII (23,67±1,15) ambos significativos (p:0.01). Además, B0 expuso un puntaje (24,25±1,51) significativorespecto A0 y AI (p:0.01), C0 con puntaje (22,5±2,59) verifico significancia con todos los estadios A y B(p:0.001). En D0 el resultado de Mini-Mental fue de 17 puntos demostrando una funcionalidad cognitivadeteriorada mostrando significancia en los estadios de lesión PD A, B y C (p:0.001 respectivamente). Larelación entre estadios de deterioro cognitivo puntuados y niveles de HbA1c verificó un coeficiente de correlación (r=-0,56) demostrando una buena relación 4 inversa entre ambas variables. Un mal controlmetabólico de HbA1c y glucemia deterioran el nivel cognitivo.CONCLUSIONES: el DC en los pacientes con DM tipo 2 provoca una disminución en el autocuidado delesiones PD teniendo relación directa con la severidad de las lesiones. Los pacientes con nivelesincrementados de glucosa y HbA1c expusieron estadios de lesión de PD más severos y registrosmonofásicos en la valoración ecodoppler. Niveles elevados de HbA1c se relacionó con bajo puntaje en lavaloración cognitiva. El desarrollo de estrategias de identificación temprana de la progresión del DC nodiagnosticado en sujetos vulnerables es de gran importancia clínica y debería ser resuelto lo antes posible
