Hep® (homeostasis-Enrichment Approach Changes Sensory-Motor Development Trajectory and Improves Parental Goals: a Single Subject Study of an Infant With Hemiparetic Cerebral Palsy and Twin Anemia Polycythemia Sequence (taps)
Sirma, Gamze Cagla/0000-0003-2976-9035; Balikci, Aymen/0000-0002-7552-8235Background: Early intervention (EI) for infants identified as being at high risk for cerebral palsy (CP), or who have been diagnosed with it, is critical for promotion of postnatal brain organization. The aim of this study was to explore the effectiveness of the Homeostasis-Enrichment-Plasticity (HEP) Approach, which is a contemporary EI model that applies the key principles of enriched environment paradigms and neuronal plasticity from experimental animal studies to ecological theories of human development on the motor development, sensory functions, and parental goals of an infant with twin anemia polycythemia sequence (TAPS) and CP. Methods: An AB phase with follow-up single case study design which consisted of multiple baseline assessments with the Peabody Developmental Motor Scales-2 (PDMS-2) and the Test of Sensory Functions in Infants (TSFI) was used. Non-overlapping confidence intervals analysis was used for pre-post PDMS-2 scores. The measurement of progress toward goals and objectives was conducted using the Goal Attainment Scale (GAS). The HEP Approach intervention consisted of 12 one-hour sessions implemented over a period of 3 months, where a physical therapist provided weekly clinic-based parental coaching. Results: Results found a stable baseline during Phase A and improvement in response to the HEP Approach intervention during Phase B in both the PDMS-2 and TSFI according to 2SD Band analysis. The confidence intervals for the PDMS-2 scores also indicated a significant improvement after HEP intervention. The scores for both the PDMS-2 and the TSFI were consistent or showed improvement throughout the Follow-Up phase. A GAS t-score of 77.14 indicated that the infant exceeded intervention goal expectations. Conclusions: Although our findings suggest that the HEP Approach intervention has promise in enhancing sensory functions, motor skill outcomes, and parental goals in an infant with TAPS and CP, further research is required to validate and apply these results more broadly. The authors would like to thank Isabelle Beaudry-Bellefeuille for the valuable feedback and notes on the article.Science Citation Index Expande
Comparison of the Effect of Maternal Breast Milk Odor, Facilitated Tucking, and Nonnutritive Sucking Applied To Preterm Neonates During Heel Stick on Pain and Physiological Parameters: a Randomized Controlled Trial
Background: Preterm infants in the neonatal intensive care unit (NICU) frequently undergo painful procedures, which can lead to both short-term and long-term complications, including potential mortality. Effective pain management is crucial in this context. Although numerous studies have explored non-pharmacological pain relief methods for preterm infants, no research has simultaneously compared the effects of maternal breast milk odor, facilitated tucking, and nonnutritive sucking. Aim: This study aims to comparatively investigate the efficacy of nonnutritive sucking, facilitated tucking, and maternal breast milk odor in reducing pain in preterm neonates during heel sticks. Methods: A randomized controlled trial was conducted in a tertiary-level NICU with 144 preterm neonates (gestational age 31-36 weeks) requiring heel sticks. The infants were randomly assigned to four groups: control, nonnutritive sucking, facilitated tucking, and maternal breast milk odor. Pain and physiological parameters were assessed through video recordings of the procedures and scored at 1-minute intervals using the Premature Infant Pain Profile and Data Evaluation Form. Data were collected at baseline and at 1, 2, and 3 minutes before, during, and after the heel stick. Results: The study found significant effects of the interventions on physiological parameters and pain (heart rate: F = 7.5, p < 0.001; oxygen saturation: F = 16.39, p < 0.001; respiratory rate: F = 6.56, p < 0.001; pain: F = 61.45, p < 0.001). Facilitated tucking resulted in significantly lower pain scores (6.61 +/- 1.44) compared with the control group (14.22 +/- 3.61), maternal breast milk odor (12.22 +/- 3.08), and nonnutritive sucking (10.41 +/- 1.71) (p < 0.001). Conclusions: All interventions demonstrated effectiveness in maintaining physiological stability and alleviating pain. Nevertheless, the intervention identified as most effective in reducing pain was facilitated tucking group.Science Citation Index Expande

A Systematic Review and Meta-Analysis of Molecular Characteristics on Colistin Resistance of acinetobacter Baumannii
Kahraman Kilbas, Elmas Pinar/0000-0003-1348-625X; CIFTCI, IHSAN HAKKI/0000-0002-9812-134XBackground: This study aimed to determine the molecular epidemiology of colistin-resistant A. baumannii in the last ten years and the frequency of gene regions related to pathogenesis, to compare the methods used to detect genes, and to confirm colistin resistance. Methods: This meta-analysis study was conducted under Preferred Reporting Items for Systematic Reviews and Meta-Analysis Guidelines. In the meta-analysis, research articles published in English and Turkish in electronic databases between January 2012 and November 2023 were examined. International Business Machines (IBM) Statistical Package for the Social Sciences (SPSS) Statistics for Macbook (Version 25.0. Armonk, NY, USA) was used for statistical analysis. The Comprehensive Meta-Analysis (CMA) (Version 3.0. Biostat, NJ, USA) program was used for heterogeneity assessment in the articles included in the meta-analysis. Results: After evaluating the studies according to the elimination criteria, 18 original articles were included. Among colistin-resistant strains, blaOXA-51 positivity was 243 (19.61%), blaOXA-23 was 113 (9.12%), blaOXA-58 was 7 (0.56%), blaOXA-143 was 15 (1.21%), and blaOXA-72 was seen in two (0.16%) strains. The positivity rates of pmrA, pmrB, and pmrC were found to be 22 (1.77%), 26 (2.09%), and 6 (0.48%). The mcr-1 rate was found to be 91 (7.34%), the mcr-2 rate was 78 (6.29%), and the mcr-3 rate was 82 (6.61%). Conclusions: The colistin resistance rate in our study was found to be high. However, only some research articles report and/or investigate more than one resistance gene together. Additionally, it may be challenging to explain colistin resistance solely by expressing resistance genes without discussing accompanying components such as efflux pumps, virulence factors, etc.Science Citation Index Expande
Chatgpt as a Solution To Emotional Loneliness: a New Tool for Interpersonal Communication
Loneliness stands as one of the most pervasive and universal health issues in contemporary society. This study, conducted within the scope of emotional loneliness, delves into the nuances of loneliness, understanding its origin and how it manifests in individuals. In the research where students from the Faculty of Communication of a foundation university in Istanbul were chosen as the sample, the data collected through in-depth interviews, a qualitative data collection tool, were analyzed using content analysis method. This phenomenological research design aimed to assess participants' attitudes toward utilizing a chatbot (ChatGPT) as a tool to combat emotional loneliness. Despite their generally positive disposition and strong tendency to anthropomorphize, participants were found to approach the use of ChatGPT with caution. The majority harbored negative judgments about ChatGPT and technological tools meeting emotional and social needs. However, a significant portion believes that the widespread adoption of ChatGPT and similar tools for socialization is imminent in the near future. Concerns predominantly revolve around mistrust in artificial intelligence technologies' ability to meet emotional needs and the potential harms they could inflict on socio-cultural life, with ethical issues forming another major theme. Notably, when expressing their apprehensions, participants frequently referenced artificial intelligence representations in popular culture, such as films, series, and games.Emerging Sources Citation Inde
Drivers and Barriers on Implementing Xr Technologies in the Construction Industry in Turkey
SEYMAN GURAY, TAYIBE/0000-0001-5692-9669This study presents drivers and barriers on construction industry extended reality (XR) implementations in Turkey. It aims to investigate the current state and future directions of XR opportunities for architecture offices and construction companies in a developing country. A quantitative approach was employed which involves conducting a survey, performing statistical analyses, and verification of the hypotheses. The data were analyzed by SPSS and Excel using the frequency, mean score, standard deviation (SD), and one sample t-test. Thirty-three different companies participated in the survey with a return rate of 56.89%. The results were considered statistically significant since all the p-values were below 0.05. The findings revealed that even though 60% of the companies are not using XR, 60% companies have near future plans on implementing them. According to the results, XR was considered as beneficial and highly motivating for the near future, yet it needs to be widespread for companies to keep themselves abreast of information and communication technology trends. Finally, by considering the findings obtained from the results this study reaches a classification of drivers as 'increased efficiency: time-cost-quality' and 'increased communication and coordination' while barriers as 'technical limitations', 'high costs', and 'unqualified professional'.Emerging Sources Citation Inde
A Systematic Review and Meta-Analysis for the Efficacy of Transcranial Direct Current Stimulation (tdcs) in Ocd Treatment: A Non-Pharmacological Approach To Clinical Interventions
Al-Qiami, Almonzer/0000-0003-3147-5060; Asar, Nada/0009-0007-8395-6823; Ibrahim, Ismail/0000-0002-0805-8181Obsessive-compulsive disorder (OCD) is a prevalent mental condition characterized by recurrent, unwanted thoughts (obsessions) and repetitive behaviors (compulsions), significantly disrupting daily functioning and social interactions. Transcranial direct current stimulation (tDCS) presents a promising non-invasive treatment modality aimed at alleviating symptoms. However, the evidence regarding its effectiveness remains inconclusive. This study seeks to address this gap by conducting a systematic review and meta-analysis of clinical trials, offering improved guidance for clinical intervention. A comprehensive search strategy was implemented across multiple databases, including PubMed, Cochrane CENTRAL, Embase, Scopus, and Web of Science. This search focused strictly on randomized controlled trials (RCTs) involving 147 patients. These trials evaluated the efficacy of tDCS in OCD patients. Subsequent data extraction, risk of bias assessment, and statistical analysis using Review Manager software revealed the potential efficacy of tDCS in reducing OCD symptoms. The meta-analysis not only fails to demonstrate significant superiority of active tDCS over sham tDCS but also suggests that sham tDCS may be more effective than active tDCS in reducing OCD symptoms. This finding diminishes the promise of tDCS as an effective treatment for OCD. Larger trials are warranted to further elucidate these findings.Science Citation Index Expande
