    Perioperative Use of Pregabalin Vs. Duloxetine for Pain Management of Knee Fracture Surgery: a Double-Blind Randomized Clinical Trial

    Background: Effective postoperative pain management, particularly in orthopedic procedures, presents significant challenges. There is increasing evidencesupporting the benefits of multimodal analgesia, including the use of gabapentinoids and serotonin norepinephrine reuptake inhibitors (SNRIs), to minimizeopioid consumption while effectively managing pain. However, a gold-standard treatment has not been established.Objectives: This study aims to compare the efficacy of duloxetine and pregabalin within a multimodal analgesic regimen for managing postoperative painand their opioid-sparing effects following knee fracture surgery.Methods: In this double-blind randomized clinical trial (RCT), 54 patients undergoing knee fracture surgery were randomized to receive either 75 mg oralpregabalin or 30 mg duloxetine twice daily, starting at least 24 hours prior to surgery and continuing up to 48 hours postoperatively. Pain severity was assessedat admission and at 6, 12, 24, and 48 hours post-operation. Patients reporting a pain score greater than six on a Numeric Rating Scale (NRS) receivedintramuscular morphine. Additionally, total opioid dose, associated complications, and drug adverse effects were monitored within the first 48 hours post-surgery.Results: Although there was no statistically significant difference between the duloxetine and pregabalin groups at each time point, the reduction in pain atthe 48-hour mark was more pronounced in the duloxetine group compared to the pregabalin group. The duloxetine group required higher doses of morphineon the first day compared to the pregabalin group (3.96 +/- 3.20 mg vs. 2.14 +/- 2.72 mg, P = 0.022). However, on the second day, opioid rescue was required in threepatients in the pregabalin group, whereas no patients in the duloxetine group required rescue. No clinically significant adverse effects were observed in eithergroup.Conclusions: Duloxetine 60 mg per day is an equally effective perioperative alternative to pregabalin 150 mg per day, resulting in a slight increase in rescueopioid administration with equivalent analgesic efficacy during the first 24 hours postoperatively.     Transverse Vibrations and Stability of Viscoelastic Axially Moving Rayleigh Beams Under Thermal Fields: an Analytical Approach

    In this work, the flexural vibrations and stability of viscoelastic beams under axial motion and thermal fields are investigated using Rayleigh beam theory. The viscoelastic behavior is modeled through the Kelvin-Voigt and Maxwell models, and the governing differential equation is derivative utilizing Hamilton's principle. To create a more realistic model, thermal stresses in the beam are simulated using both linear and non-linear models. An innovative analytical solution method for these equations is presented, employing a power series approach to solve equations. The research provides an explicit mathematical expression for the mixed vibration modes of the beam under axial motion. Various parameters, such as rotational inertia, linear and non-linear thermal stresses, structural damping, and axial movement speed, are analyzed for their effects on the dynamic characteristics and instability of viscoelastic Rayleigh beams under axial motion. The findings indicate that incorporating rotational inertia and Rayleigh beam theory reduces the natural frequencies at low axial speeds but consistently increases the system's critical speed. Furthermore, rotational inertia induces distortions in the vibration mode shapes. Notably, the impact of rotational inertia on the second mode shape is significant, resulting in the loss of the nodal point in the second vibration mode shape of the beam under axial motion.Emerging Sources Citation Inde

    Pareto Improvement in a Dual-Channel Closed-Loop Supply Chain System by Integrating Product Sustainability and Consumer Focus

This study unveils customer-oriented economic measures and ensures environmental sustainability in a closed-loop dual-channel supply chain (CL-DCSC) model system comprised of a single manufacturer and retailer. This investigation combines closed-loop aspects of collecting used products from numerous consumers with dual-channel processes incorporating the uncertainty in market demand and raw-material cost. The proposed model includes the return rate of used products as a crucial factor in addition to considering wholesale, retail, and online sale prices for making informed decisions. This study utilizes an uncertain parameter defuzzification technique with fuzzy possibility measures to globally maximize the expected net earnings of the whole supply chain (SC) in a centralized decision-making scenario and the expected net earnings of every participant in a decentralized decision-making scenario. Extensive numerical simulation reveals that the CL-DCSC model corresponds to stakeholders' desired eco-friendly, customer- oriented, and profitable strategy in the centralized decision-making scenario. The study demonstrates that adopting a two-part tariff (TPT) contract can facilitate rational decision-making among stakeholders, aligning their interests with the centralized strategy. Furthermore, this study provided a mathematical approach for measuring contract fairness, Pareto improvement, and coordination efficiency. The outcomes confirm that the contract is fair, facilitates Pareto improvement, and effectively coordinates the CL-DCSC. The model emphasizes the importance of developing a CL-DCSC framework, suggesting that collaborative partnerships are more beneficial than competitive approaches. The findings encourage businesses to prioritize sustainable practices, such as recycling and product returns, which can lead to cost reductions and improved profitability.     Simulation of the Turbulent Air Flow of Inhalation and Exhalation in the Respiratory System Using Computational Fluid Dynamics

    In this research, the CFD simulation of the respiratory tract was discussed. Limited research was conducted in the field of respiratory systems to examine the respiratory system as a true model for various input structures in inhalation and exhalation, although numerous studies were conducted by researchers. This study aimed to develop a dependable method for obtaining the true respiratory system geometry from a 24-year-old man's CT scan data and preparing it for input into CFD software. this research performs a numerical analysis of the airflow from the nasal inlet in both the inhalation and exhalation modes, using a turbulent airflow mode with a flow rate of 60 liters per minute. The effect of different inputs on the airflow in the human respiratory system is simulated for flat, pipe, and semi-spherical cross sections using CFD for turbulent flow. The results show that the velocity increased as air entered the nasopharynx. In flat, pipe, and semisphere modes, the velocity increased from 2.8 m/s, 2.07 m/s, and 4.14 m/s to 7.41 m/s, 5.48 m/s, and 8.40 m/s, respectively. The Dynamic pressure drop coefficient)Cp(in flat, pipe, and semisphere modes decreased from 79.38, 34.24, and 69.57 to 32.84, 17.13, and 31.44, respectively. The velocity in flat, pipe, and semisphere modes decreased from 7.46 m/s, 4.45 m/s, and 10.29 m/s to 1.54 m/s, 0.96 m/s, and 2.70 m/s, respectively. In the flat and pipe modes, the Cp increased from 17.17, -5.46, to 34.01, and 29.75, respectively. Velocity increased as air entered the larynx. Numerically, the velocity in flat, pipe, and semisphere modes increased from 5.00 m/s, 2.78 m/s, and 7.35 m/s to 9.06 m/s, 6.56 m/s, and 9.79 m/s, respectively. The Cp increased in pipe and semisphere modes. Velocity decreases as the air enters the trachea. Numerically, the velocity in flat, pipe, and semisphere modes decreased from 6.69 m/s, 4.86 m/s, and 7.16 m/s to 3.44 m/s, 3.44 m/s, and 3.90 m/s, respectively. The Cp in the pipe and semisphere modes decreased from 0.77, and -1.59 to -7.33, and -11.51, respectively. © 2025 The Author

    Interference Mitigation and Collision Avoidance of Dynamic Uavbss Network Via Mobility Control: a Game Theoretic Approach

In this work, UAVBS mobility control is formulated as an exact potential game. Three algorithms are proposed to solve this problem under different connectivity and complexity scenarios. In the first scenario on board computation and power may be limited due to other functions. Under this scenario, the UAVBSs-Better Direction Control (UAVBSs-BDC) algorithm works iteratively based only on the UAV utility function with linear time to directly optimize the action selection based on the UAVBS's utility. The Utility-Driven Partial Synchronous Learning (UDPSL) algorithm speeds up convergence by using a learning algorithm. This algorithm is seen to increase the incidence of collision when UAVBSs are located close together and require an additional collision avoidance mechanism. The Neighbor Responsive Adaptive-Partial Synchronous Learning (NRA-PSL) algorithm controls the UAVBS's trajectory via conditioned response to its neighbor UAVBSs to select the action that guides the UAVBS towards a better direction.     Otizm Spektrum Bozukluğu (OSB), gelişimin erken evrelerinde başlayan, sosyal iletişimsel alanda yetersizlik ve sınırlı, tekrarlayıcı davranışlar ve ilgi alanları ile karakterize olan bir nöro-gelişimsel bozukluktur. OSB çocuğa sahip olmak ise ebeveynlerin, kendi üzerinde daha fazla yük hissetmesine yol açarak öz yeterlik, eğitsel becerilerini ve psikolojik sağlamlılık düzeylerini etkilemektedir. Bu araştırma da 3-8 yaş aralığındaki OSB çocuğu olan ebeveynlerin psikolojik sağlamlık düzeylerinin ebeveyn öz-yeterlilik ve eğitsel beceri algıları üzerinde etkisini belirlemek amacıyla planlanmıştır. Bu amaçla İstanbul'da yaşayan ve OSB çocuğu olan 305 ebeveyne; 'Demografik Bilgi Formu', ' Kısa Psikolojik Sağlamlık Ölçeği (PSÖ)', 'Otizm Spektrum Bozukluğu Çocuğu Olan Ebeveynlerde Ebeveyn Yeterlik Ölçeği (EYÖ)' ve 'Otizmli Çocuğa Sahip Ebeveynlerin Eğitsel Becerilerine İlişkin Yeterlik Ölçeği (EEBYÖ)' uygulanmıştır. Verilerin analizinde T-Testi, Varyans Analizi (ANOVA) ve regresyon istatistiksel işlemler uygulanmıştır. OSB çocuğu olan ebeveynlerin psikolojik sağlamlık düzeyleri ebeveyn yaşına ve öğrenim durumuna göre farklılık görülmüştür. Ebeveyn yaşı artıkça psikolojik sağlamlık düzeyinin arttığı, öğrenim durumu açısından ilköğretim mezunu ebeveynlerin lisans ve lisansüstü mezunu ebeveynlerden daha yüksek olduğu belirlenmiştir. OSB çocuğu olan ebeveynlerin psikolojik sağlamlık düzeylerinin ebeveynlerin öz yeterlik ve eğitsel becerileri üzerinde etkisi olduğu belirlenmiştir. Psikolojik sağlamlık düzeyinin artışının ebeveyn öz yeterlik ve eğitsel becerilerine ilişkin yeterlik düzeylerini de arttırdığını göstermiştir. Araştırma sonuçları dikkate alındığında ebeveynlerin psikolojik sağlamlık düzeylerini arttırmaya yönelik çalışmaların arttırılmasına yönelik çocuğun ve ailenin gereksinimleri doğrultusunda bireysel ya da grup aile eğitimlerinin düzenlenmesi önerilebilir.Autism Spectrum Disorder (ASD) is a neurodevelopmental disorder characterized by persistent deficits in social communication and restricted, repetitive patterns of behavior and interests, which begin in the early stages of development. Having a child with Autism Spectrum Disorder (ASD) leads parents to feel a greater burden, affecting their self-efficacy, educational skills, and psychological resilience levels. This study was designed to determine the impact of the psychological resilience levels of parents with children aged 3-8 years with ASD on their perceptions of parental self-efficacy and educational skills. For this purpose, 305 parents living in Istanbul with a child diagnosed with Autism Spectrum Disorder (ASD) were administered the following tools: 'Demographic Information Form,' 'Brief Psychological Resilience Scale (PRS),' 'Parental Efficacy Scale for Parents of Children with Autism Spectrum Disorder (PES),' and the 'Educational Skills Competency Scale for Parents of Children with Autism (ESCS)' In the analysis of the data, T-tests, Analysis of Variance (ANOVA), and regression statistical procedures were applied. Differences were observed in the psychological resilience levels of parents with a child diagnosed with ASD according to the parents' age and educational level. It was found that as the parent's age increased, their level of psychological resilience also increased. In terms of educational level, parents who completed primary education had higher psychological resilience than parents with a bachelor's or postgraduate degree. It has been determined that the level of psychological resilience in parents with a child diagnosed with ASD has an effect on their self-efficacy and educational skills. The increase in psychological resilience has also shown to enhance the parents' perceptions of self-efficacy and educational skills. Taking the research findings into account, it is recommended to increase efforts aimed at enhancing parents' levels of psychological resilience, such as organizing individual or group family education sessions tailored to the needs of the child and the family

    Comparing the Effectiveness of Mulligan Movement with Mobilization and Proprioceptive Neuromuscular Facilitation Techniques in Rehabilitation of Rotator Cuff Syndrome: A Randomized Controlled Trial

    Surenkok, Ozgur/0000-0003-1558-8989; Atici, Emine/0000-0002-6547-4798; Aydin, Gamze/0000-0002-4952-2825Purpose This study aims to compare the effectiveness of Mulligan Movement with Mobilization Techniques (MWMs) and Proprioceptive Neuromuscular Facilitation Techniques (PNF) on pain intensity, range of motion (ROM), proprioception, scapular dyskinesis, functionality, and quality of life (QoL) in individuals with Rotator Cuff Syndrome (RCS). Methods In this randomized, controlled, three-arm clinical trial, we recruited patients with Rotator Cuff Syndrome. During the study period, 70 patients diagnosed with Rotator Cuff Syndrome were screened and 60 eligible volunteers were enrolled. Patients were randomly allocated (1:1:1) into one of three groups: the control group (conservative treatment alone), the MWMs group (MWMs plus conservative treatment), and the PNF group (PNF plus conservative treatment). Each group underwent a total of 20 treatment sessions following prescribed programs, lasting 45 min per session and conducted five days a week. Pain intensity was evaluated using the Visual Analog Scale; joint ROM was measured with a Goniometer; proprioception was assessed using the Angle Reproduction Test; scapular dyskinesis was evaluated using the Lateral Scapular Slide Test; functionality was assessed with the Western Ontario Rotator Cuff Index; and QoL was evaluated using the Short Form-36. Results In the comparison between groups, MWMs had a significant effect on pain intensity, proprioception sense of 90 degrees abduction movement and functionality (eta 2 (%95 CI): 0.393 (0.185; 0.531); 0.198 (0.033; 0.352); 0.429 (0.221; 0.561), respectively). It was observed that the control group was superior to other treatments in quality of life, except for the mental health sub-dimension (p < 0.05). Conclusion The three interventions compared in patients with RCS have shown positive effects, with the MWMs found to be particularly superior in reducing pain, improving proprioception, and enhancing functionality, making it a valuable option for rehabilitation in clinical practice.Science Citation Index Expande

    Lercanidipine Enhances Cisplatin Activity: Dual Anticancer and Anti-Inflammatory Effects Via Caspase Activation and Mapk Inhibition

    Background/Objectives: Lercanidipine is a third-generation dihydropyridine calcium channel blocker. In addition to their well-established cardiovascular effects, calcium channel blockers are increasingly recognized for their therapeutic potential in various cancers. This study aimed to investigate the potential anticancer effects of lercanidipine on cancer cell lines-particularly in combination with cisplatin-by assessing parameters such as cell viability (MTT assay), proliferation, MAPK pathway activity, caspase enzyme levels, and TNF-alpha expression. Methods: In this study, the effects of lercanidipine, both alone and in combination with cisplatin, on cell viability were evaluated using the MTT assay in MCF-7, SH-SY5Y, PC3, and HEK293 cell lines. To assess intracellular signaling and apoptotic pathways, MAPK inhibition, as well as caspase-3 and caspase-8 activities, were measured using ELISA. Additionally, to evaluate the anti-inflammatory potential, TNF-alpha levels in LPS-stimulated RAW264.7 cells were analyzed via. Results: The study revealed that lercanidipine showed significant cytotoxic effects, particularly in SH-SY5Y and PC3 cancer cell lines, while it did not induce a 50% loss of viability in healthy HEK293 cells. When combined with cisplatin, lercanidipine enhanced cytotoxicity by 2.7-fold in neuroblastoma (SH-SY5Y) cells, 1.6-fold in breast cancer (MCF7) cells, and 1.9-fold in prostate cancer (PC3) cells. MAPK activity was inhibited by 83.6% at 20 mu M lercanidipine, while dose-dependent increases in caspase-3 and caspase-8 activities were observed. Additionally, lercanidipine decreased TNF-alpha levels in LPS-stimulated RAW264.7 cells, indicating its potential anti-inflammatory effect. Conclusions: In conclusion, lercanidipine demonstrated selective anticancer effects in cancer cell lines and showed synergistic cytotoxicity when combined with cisplatin. It also significantly inhibited MAPK signaling, activated apoptotic caspases, and reduced TNF-alpha levels, suggesting potential anti-inflammatory activity. These findings highlight lercanidipine's potential for repurposing as an adjunct in cancer therapy.Science Citation Index Expande

    Modeling the Influence of External Heat Flux on Thermal Characteristics of the Silica Aerogel/Paraffin in a Cylindrical Atomic Duct

    As the price of fuel rises and the environmental impact of greenhouse gases intensifies, a larger population is opting for alternative sources of sustainable energy. Currently, scientists are facing challenges in discovering an energy-saving method that is effective in diverse scenarios and is user-friendly. Many individuals are interested in using materials that can transition between solid, liquid, and gas states. The objective was to use these materials for heat retention. Silica aerogels exhibit effective thermal regulation, regardless of whether the environment is hot or cold. Phase change materials are substances that store thermal energy effectively and play a crucial role in maintaining temperature stability. This research explored how external heat flux affected the behavior of a tube filled with silica aerogel and phase change materials. Additionally, we incorporated CuO nanoparticles to evaluate their impact on the system. The study utilized LAMMPS software to perform molecular dynamics simulations for this purpose. To achieve our goal, we evaluated various aspects of virtual structure, which can be influenced by factors, such as density, velocity, temperature profile, heat flux, thermal conductivity, and the duration of filling and emptying. The findings indicate that as external heat flux increased, maximum density decreased to 0.1364 atoms/& Aring;3. Conversely, thermal conductivity, maximum velocity, and temperature increase to 1.97 W/m & sdot;K, 0.0138 & Aring;/fs, and 649 K, respectively. Also, with maximum external heat flux, charging time decreases to 5.94 ns, while discharge time is recorded at 8.56 ns. Increased external heat flux resulted in greater thermal energy transfer to the material, causing the atoms to vibrate more vigorously and collide more frequently.Science Citation Index Expande

    Effects of Core Stabilization Exercises on Trunk Muscle Strength, Upper Extremity Performance, and Balance in Female Wrestlers: a Randomized Controlled Trial

    Bu çalışmada, kadın güreş sporcularında kor stabilizasyon egzersizlerinin gövde kas kuvveti, üst ekstremite performansı ve spinal stabilite üzerine etkileri incelenmiştir. Çalışma güreş sporu yapan 20 kadın sporcuyla gerçekleştirilmiştir. Sporcular, Kor Stabilizasyon grubu (n=10) ve Kontrol grubu (n=10) olmak üzere iki gruba ayrılmıştır. Kor Stabilizasyon grubuna (KSG) rutin güreş antrenmanlara ek olarak (30dk/3gün/6 hafta) kademeli kor egzersiz programı uygulanmıştır. Kontrol grubundaki (KG) sporcular ise sadece rutin güreş antrenmanlarına devam etmişlerdir. Egzersiz programı öncesi ve sonrası sporcuların gövde kas kuvveti değerlendirmesinde İzokinetik Dinanometre; üst ekstremite performans değerlendirmesinde Sağlık Topu Atma Testi (STF), Kapalı Kinetik Zincir Üst Ekstremite Testi (KKZÜET) ve Üst Ekstremite Y Denge (ÜEYDT) testleri kullanılmıştır. Spinal stabilite değerlendirmesi için Fonksiyonel Uzanma Testi (FUT) kullanılmıştır. KSG'nda egzersiz öncesine göre egzersiz sonrasında 60° fleksiyon ve 150° fleksiyon kas kuvvetinde anlamlı artış görülmüştür (p=0,001 ve p=0,028). Gruplar arası karşılaştırmada ise 60° fleksiyonda KSG'nda KG'na göre anlamlı artış gözlenmiştir (p=0,004). STFT'nde KSG'nda KG'na göre anlamlı artışlar elde edilmiştir (p<0,001). KKZÜET'inde her iki grupta da anlamlı artışlar elde edilmiştir (KSG için p=0,001 ve KG için p=0,003) fakat gruplar arasında fark bulunmamıştır (p=0,617). ÜEYDT'inde hem sağ hem sol tarafta KSG'nda anlamlı artışlar gözlenmiştir, KG'na göre de anlamlı farklılık göstermiştir ( Sağ denge için p=0,032 ve sol denge için p=0,038). Spinal stabilite ölçümlerinde gruplar arası anlamlı fark bulunmamıştır (p=0,867). Sonuç olarak, bu çalışma kor stabilizasyon egzersizlerinin elit kadın güreş sporcularının performansını arttırmada etkili bir yöntem olduğunu göstermektedir. Bu egzersizler, güreşçilerin gövde kas kuvvetini, üst ekstremite kas kuvvetini ve dengesini arttırarak, onların rakiplerine karşı daha etkili olmalarını sağlayabilir. Bu egzersizler, güreşçilerin antrenman programlarına dahil edilebilir.The aim of this study is to investigate the effects of core stabilization exercises on trunk muscle strength, upper extremity performance, and balance in female wrestlers. This prospective randomized controlled study included 20 female wrestlers (mean age:17.5 ± 2.16 years). The participants were divided into two groups: the Core Stabilization Group (CSG) (n = 10), which performed a progressive core exercise program in addition to regular wrestling training, and the Control Group (CG) (n = 10), which continued with regular wrestling training only. Trunk muscle strength was assessed using an isokinetic dynamometer; upper extremity performance, with medicine ball throw test (MBTT), closed kinetic chain upper extremity test (CKC-UET), upper extremity Y-balance test (UE-YBT); balance with the functional reach test (FRT). Between-group comparisons revealed that the CSG had a greater improvement than the CG in 60° trunk flexion, MBTT, and UEY-BT (η2 = 0.492, p = 0.004; η2 = 0.831, p = 0.014; η2 = 0.403, p = 0.032; η2 = 0.580, p = 0.038, respectively). The core stabilization exercises can improve trunk muscle strength, upper extremity performance, and balance in female wrestlers. © The Author(s) 2025.Social Science Citation Inde
