Role of Different Anti-Seizure Medications on Carotid Intima-Media Thickness: A Systematic Review and Meta-Analysis
Background: Various types of anti-seizure medication (ASMs) may have an impact on the cardiac health of patients who have epilepsy. One of the most important noninvasive predictive factors related to cardiovascular disease is carotid intima-media thickness (CIMT). The objective of this systematic review and meta-analysis is to determine the effect of both mono- and poly-therapy with ASMs on CIMT. Methods: Four databases (PubMed, Scopus, Web of Science, and Embase) were searched for records. Studies that measured the effect of ASMs on CIMT were eligible to be included. The case and control data of studies were extracted. Also, the quality assessment of each included study was assessed by the risk of bias in non-randomized studies of interventions (ROBINS-I) checklist. Additionally, the random-effect model analysis was performed by the Comprehensive Meta-Analysis Software (CMA) version 3.0. Results: Twenty-nine studies were included in the systematic review, and 15 studies were eligible to perform meta-analysis. Among both poly-therapy, and monotherapy with valproate sodium, carbamazepine, levetiracetam, and phenytoin, CIMT had significantly higher thickness compared to the control group (SMD: 1.82, 1.18, 1.33, 1.83, and 1.15 with 95%CI: [0.34, 3.3], [0.67, 1.7], [0.38, 2.27], [0.1, 3.56], and [-0.49, 2.8], respectively) (P-value: .001, .001, .001, .001, and .007, respectively). Conclusion: The potential risk of cardiovascular diseases related to CIMT may be elevated by using both mono- and polytherapy with valproate sodium, carbamazepine, levetiracetam, and phenytoin.Science Citation Index Expande
Diş Hekimliğinde Kümeleme Analizi Kullanılarak Ağız Hastalıkları Yükünün Değerlendirilmesi
Amaç: Bu çalışmanın amacı, diş hekimliğinde Ağız Hastalıkları Yükü (AHY) için yaşa bağlı homojen grupları K-Ortalamalar kümeleme analizi ile belirlemektir. Yöntemler: Bu retrospektif çalışmaya, bir kamu diş hekimliği fakültesinin entegre öğrenci kliniklerinde tedavi edilen 465 yetişkin hasta ve 276 yaşlı hasta (K/E: 381/360; 18-91 yaş) dahil edilmiştir. AHY skoru (0-5 puan); periodontal problemler, diş çürüğü, pulpitis, protetik tedavi ihtiyacı ve diş çekim ihtiyacı aracılığıyla hesaplanmıştır. AHY şiddeti için homojen gruplar veri seti içinde K-Ortalamalar kümeleme analizi ile belirlenmiştir. Bulgular: Periodontal problemlerin en yüksek oranda genç yetişkin hastalarda (%94.0) (küme-1; n=201; 18-36 yaş; AHY: 2,92±1,14) ve yetişkin hastalarda (%87,4) (küme-2; n=199; 37-55 yaş, AHY: 3,28±1,34) görüldüğü, ileri yetişkin hastalarda (%90,7) (küme-3; n=193; 56-70 yaş, AHY: 3,19±1,41) ve yaşlı hastalarda (%87,2) (küme-4; n=148; 71-91 yaş, AHY: 2,33±1,30) ise protetik tedavi ihtiyacının ön planda olduğu belirlenmiştir. Dört küme arasında en yüksek AHY puanı yetişkin hastalarda görülmüştür. İleri yetişkin hastaların AHY puanı yaşlı hastalara göre daha yüksek bulunmuştur (P=,000). Sonuç: K-Ortalamalar kümeleme analizi ile tanımlanan yaş gruplarıyla birlikte AHY’ye göre farklı tedavi protokolleri veya kompleks tedavilere ihtiyaçların olduğu belirlenmiştir. Bu sonuçlar, ağız sağlığı düzeyinin iyileştirilmesine yönelik hasta güçlendirme stratejisinin geliştirilmesine ve entegre öğrenci kliniklerindeki iş gücünün planlanmasına yönelik ipuçları sağlayabilmektedir
Non-Invasive Biomarkers in Assisted Reproduction: A Comprehensive Review for Predicting IVF Success
Infertility affects a significant portion of couples globally. In vitro fertilization (IVF) is a widely utilized assisted reproductive technology (ART). Despite significant advancements, IVF success rates remain suboptimal, which necessitates innovative approaches to improve embryo selection and predict the outcomes. Traditional methods, including morphological grading and preimplantation genetic testing (PGT), possess limitations, necessitating the development of innovative, non-invasive biomarkers. This narrative review sought to investigate novel biomarkers, such as oxidative stress (OS) indicators like malondialdehyde and advanced oxidation protein products, antioxidants including glutathione peroxidase and catalase, cell-free DNA (cfDNA), immunologic markers, and microRNAs (miRNAs) extracted from follicular fluid (FF), blood, and endometrial tissue, which may serve as predictors of ART outcomes. A narrative review of published studies investigating non-invasive biomarkers in human ART was conducted. Literature was sourced from databases including PubMed, Scopus, and Web of Science, focusing on studies reporting oxidative stress markers, antioxidant enzymes, miRNAs, cfDNA, and immunologic markers in FF, blood, or endometrial samples. Elevated OS markers were linked to low-quality embryos, while high levels of antioxidants were linked to higher quality of oocytes and embryos. Moreover, certain miRNAs in FF exhibited potential as indicators of embryo viability and implantation success, potentially enabling more precise and personalized IVF approaches. Incorporating these emerging biomarkers into standard clinical practice can improve prediction methods. Future research should focus on confirming these biomarkers in various populations and creating standardized protocols to promote their integration into reproductive medicine which can lead to better fertility treatments and help couples deal with the emotional and psychological problems caused by infertility.Science Citation Index Expande
Resistance Patterns of Multi-Drug Gram-Negative Isolates to Carbapenems: An Emerging Problem
Objective: Infections caused by multidrug-resistant (MDR) Gram-negative bacteria and resistance to carbapenems constitute a major public health problem worldwide, due to limited treatment options and high mortality rates. In this study, we aimed to determine the resistance profiles of MDR Gram-negative bacilli isolated from clinical samples to carbapenems. Material and Method: Specimens, from which MDR Gram-negative bacteria were isolated, were cultured onto 5% sheep blood and EMB agar, and VITEK 2 automated system (bio M & eacute;rieux, France) was used for identification and antimicrobial susceptibility testing. Data were analyzed using chi-square and Fisher's exact tests. Results: 1072 MDR bacterial strains were isolated from the specimens of 272 patients, with the majority of transtracheal aspirates (64%) sent predominantly from palliative care (49.8%) and intensive care (42%) units. The leading pathogens were Pseudomonas aeruginosa (42,1%), Acinetobacter baumannii (31,2%), and Klebsiella spp (22,5%). The highest resistance rate among carbapenems was detected to meropenem (91,6%), followed by imipenem (44,3%) and ertapenem (25%). While ertapenem resistance was significantly lower in P. aeruginosa strains (p<0,01), meropenem resistance was significantly higher in A. baumannii strains when compared to other antibiotics. Conclusion: Since Gram-negative MDR bacteria continue to spread rapidly, monitoring resistance profiles through active hospital surveillance is crucial to determine the appropriate treatment. Panik Bozukluk ve Özgül Fobi Belirtileri Olan Yetişkin Bireylerde Flashforward EMDR Etkililiğinin Karşılaştırmalı Olarak İncelenmesi
Bu çalışmanın amacı, panik bozukluk ve özgül fobi belirtileri gösteren yetişkin bireylerde Flashforward EMDR (Göz Hareketleri ile Duyarsızlaştırma ve Yeniden İşleme) tedavisinin etkililiğini karşılaştırmalı olarak incelemektir. Panik bozukluk ve özgül fobiler, bireylerin günlük yaşamlarını olumsuz etkileyen ve işlevselliklerini önemli ölçüde sınırlayan yaygın anksiyete bozukluklarıdır. Bu bozuklukların tedavisinde yaygın olarak kullanılan EMDR terapisi, bireylerin travmatik anılarını işleyerek yeniden yapılandırmalarına ve anksiyete düzeylerini azaltmalarına odaklanmaktadır. Flashforward tekniği ise bireylerin kaygıya bağlı semptomlarını hafifletmede etkili bir yöntem olduğu düşünülmesi sebebi ile bireylerin gelecekteki felaket senaryoları üzerine çalışarak, geleceğe yönelik endişe ve korkularını azaltmasını hedeflemektedir. Araştırma, 18-30 yaş aralığında, DSM-V tanı kriterlerine göre panik bozukluk belirtileri gösteren 15 katılımcı ve özgül fobi belirtileri gösteren 15 katılımcı olmak üzere toplamda 30 birey üzerinde gerçekleştirilmiştir. Çalışmada kullanılan veri toplama araçları arasında Sosyodemografik Form, DSM-V Panik Bozukluk Ölçeği ve DSM-V Özgül Fobi Şiddet Ölçeği yer almıştır. Çalışma, ön test ve son test kontrol gruplu desen ile yarı deneysel bir yöntem kullanılarak gerçekleştirilmiştir. Katılımcılara sekiz oturum boyunca Flashforward EMDR uygulanmış ve terapi öncesi ile sonrası arasındaki değişiklikler değerlendirilmiştir. Araştırmanın bulguları, Flashforward EMDR'nin hem panik bozukluk hem de özgül fobi belirtileri üzerinde anlamlı bir etkiye sahip olduğunu göstermiştir. Panik bozukluk belirtileri gösteren katılımcılar, terapi sonrası anksiyete düzeylerinde ve panik atak sıklıklarında belirgin bir azalma bildirmiştir. Özgül fobi belirtileri taşıyan bireylerde ise korkulan nesne ya da duruma karşı duyulan yoğun kaygı ve kaçınma semptomlarında kayda değer bir azalma diye gözlenmiştir. Ayrıca, her iki grupta da tedavi sonrasında işlevsellik düzeylerinin artış gösterdiği belirlenmiştir. Flashforward EMDR'nin, özellikle geleceğe yönelik felaket senaryolarının işlenmesinde etkili bir teknik olduğu ve katılımcıların korku ve kaygı düzeylerini azalttığı sonuçlarına ulaşılmıştır. Bu araştırmanın bulguları, Flashforward EMDR'nin panik bozukluk ve özgül fobi tedavisinde etkili bir yöntem olduğunu ve geleneksel terapi yaklaşımlarına kıyasla kısa sürede semptomlarda anlamlı bir azalma sağladığını ortaya koymaktadır. Araştırmanın sonuçları, klinik uygulamalarda yeni yaklaşımların geliştirilmesine ve Flashforward EMDR'nin daha geniş bir popülasyonunda uygulanabilirliğine ışık tutmayı hedeflemektedir. Anahtar Kelimeler: Panik Bozukluk, Özgül Fobi, EMDR, Flashforward EMDR, Anksiyete BozukluklarıThe aim of this study is to comparatively examine the effectiveness of Flashforward EMDR (Eye Movement Desensitization and Reprocessing) therapy in adult individuals with symptoms of panic disorder and specific phobia. Panic disorders and specific phobias are common anxiety disorders that negatively impact individuals' daily lives and significantly limit their functionality. EMDR therapy, widely used in the treatment of these disorders, focuses on processing and restructuring traumatic memories to reduce anxiety levels. The Flashforward technique, on the other hand, works on catastrophic scenarios related to the future, aiming to reduce individuals' anticipatory anxiety and fears. Flashforward EMDR is considered an effective method for restructuring fear responses and alleviating symptoms of these disorders. The study was conducted with a total of 30 individuals aged between 18-50, including 15 participants meeting DSM-V diagnostic criteria for panic disorder and 15 participants with specific phobia symptoms. Data collection tools used in the study included a Sociodemographic Form, DSM-V Panic Disorder Scale, and DSM-V Specific Phobia Severity Scale. The study employed a quasi-experimental design with pre-test and post-test control groups. Participants underwent eight sessions of Flashforward EMDR, and changes before and after the therapy were evaluated. The findings of the study demonstrated that Flashforward EMDR had a significant impact on both panic disorder and specific phobia symptoms. Participants with panic disorder reported a notable reduction in anxiety levels and the frequency of panic attacks following therapy. Individuals with specific phobia symptoms showed significant improvement in their intense anxiety and avoidance behaviors toward the feared object or situation. Furthermore, both groups exhibited increased functionality levels post-treatment. The results highlighted that Flashforward EMDR is particularly effective in processing catastrophic scenarios related to the future and reducing participants' fear and anxiety levels. The findings of this study show that Flashforward EMDR is an effective method in the treatment of panic disorder and specific phobias and provides a significant reduction in symptoms in a short time compared to traditional therapeutic approaches. The results aim to contribute to the development of new approaches in clinical practice and expand the applicability of Flashforward EMDR to a wider patient population. Parametric Dynamic Instability of a Nonlocal Axially Moving Nano-Beam with Harmonic Length Under Thermo-Mechanical Forces
This paper investigates the dynamic instability behavior of an axially moving nano-beam with time-varying length, placed in a thermal environment and resting on a viscoelastic Pasternak-type foundation while subjected to axial loading. The governing equations of motion are formulated using the Euler-Bernoulli beam theory, incorporating nonlocal elasticity effects, and derived via Hamilton's principle. Floquet theory is employed to identify regions of parametric instability in the amplitude-frequency domain of the beam's longitudinal oscillations. A comprehensive parametric study is conducted to evaluate the influence of various physical factors, including geometric dimensions, axial velocity, nonlocal effects, thermal variations, axial forces, and viscoelastic foundation properties. The results demonstrate that the dynamic stability of the nano-beam is highly sensitive to these parameters. Notably, increasing the length of the nano-beam and the amplitude of longitudinal oscillations makes the system more prone to instability, whereas greater beam thickness and foundation stiffness enhance system stability. Thermal loads and compressive axial forces tend to destabilize the structure, while tensile loading and viscoelastic damping promote stability. The findings provide fundamental insights into the design of nano-scale moving beam systems under coupled thermal and mechanical fields and offer design guidelines for achieving dynamic robustness in advanced nanoelectromechanical systems (NEMS).Emerging Sources Citation Inde
Search for a Heavy Pseudoscalar Higgs Boson Decaying to a 125 GeV Higgs Boson and a Z Boson in Final States with Two Tau and Two Light Leptons in Proton-Proton Collisions at √s = TeV
A search for a heavy pseudoscalar Higgs boson, A, decaying to a 125 GeV Higgs boson h and a Z boson is presented. The h boson is identified via its decay to a pair of tau leptons, while the Z boson is identified via its decay to a pair of electrons or muons. The search targets the production of the A boson via the gluon-gluon fusion process, gg -> A, and in association with bottom quarks, . The analysis uses a data sample corresponding to an integrated luminosity of 138 fb(-1) collected with the CMS detector at the CERN LHC in proton-proton collisions at a centre-of-mass energy of TeV. Constraints are set on the product of the cross sections of the A production mechanisms and the A -> Zh decay branching fraction. The observed (expected) upper limit at 95% confidence level ranges from 0.049 (0.060) pb to 1.02 (0.79) pb for the gg -> A process and from 0.053 (0.059) pb to 0.79 (0.61) pb for the process in the probed range of the A boson mass, m(A), from 225 GeV to 1 TeV. The results of the search are used to constrain parameters within the benchmark scenario of the minimal supersymmetric extension of the standard model. Bridging Technology and Medicine: Artificial Intelligence in Targeted Anticancer Drug Delivery

This review explores the various AI methodologies that are transforming the landscape of targeted drug delivery systems. By leveraging machine learning algorithms, researchers can analyze extensive datasets, including genomic, proteomic, and clinical data, to identify patient-specific factors that influence therapeutic responses. Supervised learning techniques, such as support vector machines and random forests, enable the classification of cancer types and the prediction of treatment outcomes based on historical data. Deep learning approaches, particularly convolutional neural networks, facilitate improved tumor detection and characterization through advanced imaging analysis. Moreover, reinforcement learning optimizes treatment protocols by dynamically adjusting drug dosages and administration schedules based on real-time patient responses. The convergence of AI and targeted anticancer drug delivery holds the promise of advancing cancer therapy by providing tailored treatment strategies that enhance efficacy while minimizing side effects. By improving the understanding of tumor biology and patient variability, AI-driven methods can facilitate the transition from traditional treatment paradigms to more personalized and effective cancer care. The incorporation of wireless sensors and real-time monitoring capabilities into these intelligent dressings will empower clinicians to make data-driven decisions, optimizing treatment strategies and ensuring timely interventions. Furthermore, the integration of novel drug delivery systems (DDS), including biodegradable nanoparticles and transdermal patches, will facilitate the targeted administration of therapeutic agents, enhancing the efficacy of wound healing while reducing systemic side effects. Innovations in gas-releasing dressings and nanoenzyme-based therapies will expand the arsenal of tools available to sports medicine professionals, addressing a wider range of wound types and complexities. Bipolar Intuitionistic Fuzzy Graph Centrality-Based Decision-Making Model to Prioritize Locations for Healthcare Camp during COVID-19
Centrality measure is one of the important tools in graph theory to measure the significance of the vertices in the graph. When uncertainty exists in the vertices and their relations, the fuzzy graph helps to handle such scenarios. Bipolar intuitionistic fuzzy graph is an extension of fuzzy graph, effectively deal with uncertainty by assigning membership and nonmembership degrees in a bipolar perspective. In this chapter, the centrality measures such as degree, in-degree, and out-degree centrality measures in the bipolar intuitionistic fuzzy graph are defined. Additionally, a decision-making model is designed based on the degree centrality measure. Further, an illustration of this model is provided for prioritizing the locations to setup healthcare camp during COVID-19 pandemics. © 2025 Elsevier B.V., All rights reserved
