    Estratégias de enfrentamento e o sofrimento de mães de filhos com paralisia cerebral

    Objetivo: identificar as mudanças ocorridas na vida de mães cuidadoras de crianças e adolescentes com paralisia cerebral, impactos e estratégias de enfrentamento desenvolvidas por elas na trajetória do cuidado. Métodos: Foram entrevistadas onze mães, cujos filhos estavam entre sete e dezoito anos, matriculados em uma unidade de ensino da rede estadual brasileira. Utilizaram-se entrevistas semiabertas, analisadas por meio do programa IRAMUTEQ e baseadas em cinco eixos temáticos: momento do diagnóstico; informações da equipe de saúde; mudanças na vida da mãe e na dinâmica familiar; momentos mais difíceis; suporte social e apoio da família. Resultados: As mães sofrem os primeiros impactos no momento do nascimento e na confirmação do diagnóstico, além do que se identificou que a família é o principal suporte das mães. Percebeu-se a necessidade de maiores esclarecimentos para as mães sobre a paralisia cerebral por parte da equipe de saúde nos primeiros momentos de vida da criança, como também, foram apontadas como causas de sofrimento a discriminação e a rejeição por parte da sociedade. Conclusao: O papel de cuidadora pode ser desgastante e desencadear adoecimento físico e psíquico e, diante disso, reitera-se a necessidade de cuidado ao cuidador na paralisia cerebral infantil. Objective: This research aimed to identify the lifestyle changes of mothers whose children suffer from cerebral palsy, as well mothers’ coping strategies and their effectiveness during the years of care. Method: Eleven mothers whose children’s age ranked between seven and eighteen and who had been enrolled in a Brazilian educational unit were interviewed. Semi-structured interviews were used to collect the data. They were analyzed using the IRAMUTEQ program. These interviews were based on five topics: time of diagnosis, health team information, changes in mothers’ and families’ lifestyle, most difficult times; social support and family support. Results: Results mainly revealed two situations: 1. Mothers’ first and hardest impact is at birth and at the time of diagnosis confirmation; 2. Mothers’ main support is family. Also, it is important to highlight, on the one hand, that mothers need to be provided with more clear information about cerebral palsy by health professionals during the child’s first years of life; on the other, that social rejection and discrimination constitute sources of pain and sorrow for mothers. Conclusions: The role of mothers (understood as caregivers) may be exhausting and could cause physical and mental illness. Therefore, caregivers of patients with cerebral palsy must be medically assisted too

    Condicionantes de la medicina occidental que propician la barrera psicosociocultural entre los efectores médicos y las poblaciones consultantes

    Introducción: Las transformaciones que en el devenir histórico fueron acaeciendo en el campo de la medicina la han estructurado con una carga grande de agresividad y progresiva deshumanización que se hacen muy evidentes en las habituales interacciones de los facultativos y las poblaciones consultantes. Objetivo: Analizar desde una perspectiva psicosociocultural los factores que han favorecido la deshumanización y agresividad de la medicina occidental en su interacción con consultantes y pacientes. Método: Se utilizó el estudio y reflexión crítica de publicaciones significativas vinculadas con el objetivo, sumados a la experiencia de los autores en el campo de la Atención Primaria de Salud en el Primer Nivel de Atención. Resultados: Se aporta soporte teórico y fundamentos vinculados a la génesis y existencia de una barrera psicosociocultural con efecto negativo, interpuesta entre el componente formal y el informal de los sistemas de servicios de salud. Conclusiones: Las barreras psicosocioculturales generan negativos efectos comportamentales y afectivo-emocionales que deben ser visualizados y vivenciados como problema por los mismos profesionales. Esta es la raíz para avanzar en su deconstrucción: la concientización de su existencia, sus consecuencias y la profunda insatisfacción profesional por la baja eficacia de su accionar. Introduction: Changes in the field of medicine along the history have characterized it as highly aggressive and progressively dehumanized. These changes are observed in the usual everyday interactions between patients and physicians. Objective: To analyze the psycho-sociocultural factors which have enhanced dehumanization and aggressiveness in the patient-physician interactions in western medicine. Method: The reflections and discussions from meaningful publications linked to the objective of the present research were used for the analysis, along with the authors’ experience in the field of Primary Health Care in the First Level of Medical Assistance. Results: The study provides theoretical support and other elements linked to the origin and existence of psycho-sociocultural barriers with a negative effect which rises between formal and informal components of health care systems. Conclusions: Psycho-sociocultural barriers generate behavioral as well as affective & emotional negative effects which physicians probably experience every day and see as problems. Being aware of this situation along with its consequences constitutes the path to begin eradicating the problem

    Validez de constructo y confiabilidad del Connor-Davidson Resilience Scale (CD-RISC 10) en población colombiana con enfermedades crónicas

    OBJETIVO: Analizar las propiedades psicométricas de la versión en español del cuestionario Connor-Davidson Resilience Scale (CD-RISC 10) en población colombiana de enfermos crónicos. METODO: Se realizó un proceso de validación por jueces expertos, seguido de una revisión cultural con 10 participantes con enfermedades crónicas, y luego la aplicación del cuestionario a 52 enfermos crónicos de la ciudad de Villavicencio, en cuyos datos se aplicó un análisis factorial exploratorio. RESULTADOS: Tanto el proceso de validación con jueces, como la revisión cultural sugirieron no modificar el instrumento, mientras que el análisis factorial denota un solo factor: resiliencia, y un alfa de Cronbach de .818. CONCLUSIONES: Los resultados de este estudio preliminar muestran una alta confiabilidad y concordancia en la estructura factorial con la versión original, lo que implica una escala ajustada a nivel cultural, y con adecuada validez de constructo y confiable, para población colombiana con enfermedades crónicas. OBJECTIVE: To analyze the psychometric properties of CD-RISC 10 Spanish version in chronically diseased Colombian patients. METHOD: The CD-RISC 10 was applied to 52 chronically diseased Colombian patients from the city of Villavicencio, followed by an exploratory factor analysis. Prior to this, the scale was validated by experts and piloted in a group of ten chronically diseased patients for cultural revision. RESULTS: The validation by experts as well as the piloting proved no need for adjustment of the scale. The exploratory factor analysis shows resilience and a Cronbach’s alpha of .818. CONCLUSIONS: Results in this preliminary study indicate the CD-RISC 10 Spanish version has high reliability which is in accordance with the factor structure of the original version of the scale. This implies the scale is suitable for a different culture. Construct validity is appropriate; therefore, reliable for chronically diseased Colombian patients
