    Get PDF
Background: Accessory ossicles, sesamoid bones, and biphalangism of toes are the most common developmental variations of the foot. These bones may be associated with painful syndromes; however, their clinical importance is not well understood because the reported prevalence varies widely. Therefore, we aimed to investigate these variants in Turkish subjects. Methods: A total of 1651 foot radiographs were retrospectively assessed. Radiographs of feet were examined regarding the prevalence, sex, and bilaterality of accessory ossicles, sesamoid bones, and biphalangism in Turkish subjects. Results: Accessory ossicles (26.1%) and sesamoid bones (8%) were detected. The most common accessory ossicles were os trigonum (9.8%), accessory navicular bone (7.9%), and os peroneum (5.8%). Also, we detected os supratalare (0.48%), os calcanei secundarium (0.42%) os subfibulare (0.42%), os supranaviculare (0.36%), os vesalianum (0.30%), os subtibiale (0.24%), os intermetatarseum (0.12%), and os subcalcis (0.12%). We observed bipartite hallux sesamoid in 1.8% and interphalangeal sesamoid bone of the hallux in 0.7% of radiographs. Incidences of metatarsophalangeal sesamoid bones were found as 0.6%, 0.06%, 0.6%, and 5.8% in the second, third, fourth, and fifth digit, respectively. We observed biphalangeal toe in 0.5%, 1.7%, 3.5%, and 37.6% in the second, third, fourth, and fifth toe, respectively. Conclusion: This study is the first detailed report on the incidence of the most common variants of the foot and ankle in a wide-ranging patients' series in Turkish subjects. Our study's findings will contribute to reducing misdiagnosis. Clinical relevance: The results of this study may provide anatomical data that could help clinicians in the diagnosis and management of disorders that present with pain and discomfort in the feet. Knowledge of these variants is important to prevent misinterpreting them as fractures

Chatbot-Human Interaction and Its Effects on EFL Students’ L2 Speaking Performance and Anxiety

    Get PDF
Recently, chatbot interactions have been used for oral communication practice in the field of foreign language education. Some existing studies have highlighted the use of chatbots in relation to specific L2 skills, yet unfortunately, the user experience component of chatbot interaction has not been empirically researched. This study investigates the effect of chatbot-human interaction with the chatbot Replika on L2 speaking performance and speaking anxiety. The participants in the current study were 90 EFL students from a state university in Turkey. They used the application Replika for twelve weeks practicing targeted subjects outside the class. Each student was assessed with the L2 speaking anxiety scale both before and after the intervention. Also, an open-ended questionnaire was administered to collect their perceptions of using chatbots for L2 speaking practice. The interactions with the chatbot were screen taped for a randomly chosen speaking task: Well-being. The findings reveal that there were negative perceptions and attitudes toward the chatbot interaction. Students reported facing difficulties in being understood precisely, which might have contributed to higher anxiety in L2 speaking. Notably, student performance with Replika was significantly better than their face-to-face peer interactions. Overall, the results show that although chatbot interaction is a novel way to provide speaking practice for students, the actual interaction with a chatbot might not be a reliable way to lessen their anxiety with L2 speaking

Relationship Between Diagnostic Anatomic Shoulder Parameters and Degenerative Rotator Cuff Tears: An MRI Study

    Get PDF
Background: Numerous radiographic measurement methods related to rotator cuff tears (RCTs) have been described. The most widely used of these parameters is the critical shoulder angle (CSA) as measured via radiography. However, magnetic resonance imaging (MRI) measurements provide more accurate results than direct radiography. Purpose: To determine the relationship between anatomic shoulder parameters and RCT type via MRI scans. Study Design: Cross-sectional study; Level of evidence, 3. Methods: The data and physical examination notes of 389 patients were retrospectively analyzed, and 242 patients were included in the study. Patients were divided into 2 groups: those with RCT (n = 127) and those without RCT (control group; n = 115). Using suitable MRI sections, the CSA, lateral acromial angle, acromial index, superior glenoid inclination, acromion–greater tuberosity impingement index, and acromiohumeral distance were compared between the RCT and control groups. The correlation of these shoulder parameters with the presence of RCT was investigated, and the predictive value of each parameter was examined using receiver operating characteristic (ROC) analysis. Results: There were significant positive relationships between the presence of RCT and CSA, acromial index, acromion–greater tuberosity impingement index, and superior glenoid inclination, with the strongest correlation belonging to CSA (r = 0.716). There were also significant negative relationships between presence of RCT and lateral acromial angle (r = –0.510) and acromiohumeral distance (r = –0.222). The ROC analysis revealed CSA to be the best predictor for the presence of RCT (area under the ROC curve = 0.899). Conclusion: The study outcomes showed that CSA as measured on MRI is the best predictor for determining the presence of RCTs. In patients with shoulder pain attributed to RCT, it may be helpful to examine the CSA on MRI

Background: The COVID-19 pandemic remains a serious problem adversely affecting public health. The present study has been conducted in an attempt to evaluate the nasal carriage of Staphylococcus aureus before and during the pandemic. Methods: In this study, the results of nasal cultures taken from patients who applied to our hospital for screening before and during the pandemic were retrospectively analyzed. Results: In total, 2707 patients participated in the study. Out of these patients, 1602 were admitted to our hospital in 2019 and 1105 in 2020. According to the results of the study, the detection rates of Staphylococcus aureus were found to be 5.1% and 0.8% in 2019 and 2020, respectively. Such a decrease occurring during the time of the Covid-19 pandemic period was found to be statistically significant (?2 = 36.61; P = .000). On the other hand, an increase was detected in the rate of methicillin-resistant Staphylococcus aureus during the pandemic. It was determined to be 17.3% in 2019 whereas it was 55.6% in 2020. This was found to be statistically significant (?2 = 7.12; P = .008). Conclusions: According to this study, there was a sharp decrease in Staphylococcus aureus carriage during the pandemic period compared to pre-pandemic, while methicillin-resistant Staphylococcus aureus rates increased more than 3 folds. This increase in methicillin-resistant Staphylococcus aureus rates may be due to the increasing trend of methicillin-resistant Staphylococcus aureus in recent years or may indicate that it depends on the measures associated with the pandemic. Further research in this area is essential

Wintering phenology and site fidelity of European robins (Erithacus rubecula) in an eastern mediterranean wintering area (aves: passeriformes)

    No full text
The European Robin (Erithacus rubecula) is a partial migrant in Central Europe and the Eastern Mediterranean known as a wintering area. Little is known about the behaviour in the winter. I assessed the wintering phenology by using capture-recapture methods in a coastal area in southern Turkey. Over three wintering seasons, 76 European Robins were captured with mist-nets. Ten of them were recaptured in the following wintering seasons and another after two seasons. Recaptures peaked in November (26.5%) and December (26.5%) and then declined progressively to February (14.7%). Our results suggest a high level of site fidelity in the wintering area in subsequent years

Objective: The aim of this study is to investigate Legionella species in 50 patients with atypical pneumonia, using culture, urinary antigen test and molecular techniques. Methods: Non-selective BCYE-? media (Oxoid, England) and selective BMPA media (Oxoid, England) were used to isolate Legionella spp. from respiratory tract samples. The urinary samples of the patients were tested with the Alere BinaxNOW Legionella Urinary Antigen Card (Abbott, US) test to identify the presence of L. pneumophila serogroup 1 specific bacterial antigen. All respiratory tractsamples were tested with a commercial Duplic? RealTime Legionella pneumophila 23S rRNA specific region detection kit (Euroclone Diagnostica, Italy) and two home-made PCR methods. Home-made gel electrophoresis PCR tests were performed using Leg primers designed from 16S ribosomal RNA gene partial sequences for Legionella spp and primers targeting the Lmip (macrophage infectivity potentiator) gene for L. pneumophila. In the home-made real-time PCR test, primers targeting the lipopolysaccharide (lps) biosynthesis gene of L. pneumophila serogroup-1, the L. pneumophila mip gene, and the Legionella spp DNA region encoded by the 16S ribosomal RNA gene were used. Results: The commercial Real-time PCR assay identifed the sequence of L. pneumophila 23S rRNA gene specific region in seven respiratory tract samples. Five samples were detected as Legionella spp. in home-made gel electrophoresis-based PCR and home-made Real-time PCR assay. Hovewer, all samples tested negative in the urinary antigen card test for L. pneumophila serogroup 1. Conclusion: We conclude that the PCR positivities with three different molecular methods indicate that Legionella species other than L. pneumophila serogroup 1 should be investigated in the patients with atypical pneumonia using molecular methods. Also, our study demonstrates the significance of PCR methods in the investigation of Legionella species in clinical samples taken from patients with negative test results for L. pneumophila serogroup 1 specific urinary antigen test, but who are clinically considered to have Legionella pneumoniae
