Özel bir hastanenin psikiyatri servisinde bağımlılık tedavisi alan hastaların sosyodemografik ve klinik özelliklerinin retrospektif analizi
Amaç: Bu çalışmada bağımlılık tedavisi alan bireylerin
sosyodemografik ve klinik özelliklerinin, retrospektif
olarak değerlendirilmesi amaçlanmıştır. Yöntem: Özel
Lara Anadolu Hastanesi Psikiyatri Servisi’nde, 1 Ocak-1
Eylül 2018 tarihleri arasında yatarak tedavi gören 153
hastanın yatış dosyaları hastane kayıt sisteminden retrospektif olarak incelenerek elde edilen veriler
araştırmacılar tarafından oluşturulan bilgi formlarına
aktarılmıştır. Veriler, SPSS 20.0 paket programında
değerlendirilmiştir. Tanımlayıcı analizler için ki-kare testi
kullanılmıştır. Çalışmada güvenirlik düzeyi %95 ve tüm
analizler için istatistiksel anlamlılık sınırı p<0,05 olarak
kabul edilmiştir. Bulgular: Hastaların %75,2’sinin erkek,
%52,3’ünün 35 yaşın altında, %86,3’ünün sosyal
güvencesinin olduğu, %81,7’sinin Antalya ve çevre
illerde ikamet ettiği, %41,2’sinin daha önce de yatarak
tedavi gördüğü, %8,5’inin HCV değerinin pozitif,
%16,3’ünün HBV bağışıklığı olduğu, HIV pozitif olan
hastanın olmadığı saptanmıştır. Kullanılan maddeler
arasında, en yüksek oranda (%39,9) opioid kullanıldığı,
bağımlılığa eşlik eden ek tanılar olduğu saptanmıştır.
Sonuç: Madde kullanım bozukluklarının genç
erişkinlerde özellikle erkeklerde daha yaygın olduğu, opioid kullanımının ilk sırada yer aldığı belirlenmiştir.
Çalışma sonuçlarımız tek bir hastane ve küçük bir örneklemi yansıtmakla birlikte, ülkemizin farklı birçok sağlık
kuruluşuna aynı sorunun tedavisi için başvuruda bulunan
bireylerin sosyodemografik özellikleri ile
karşılaştırılabilmesi açısından veri sağlayacağı,
bağımlılıkların tedavi sürecini planlama ve yürütmede
katkı sağlayıcı olacağı düşünülmektedir.Objective: In this study, it was aimed to retrospectively
evaluate the sociodemographic and clinical characteristics of individuals receiving addiction treatment.
Method: In the Private Lara Anatolian Hospital
Psychiatry Service, the hospital records of 153 patients
who were hospitalized between January 1 and
September 1, 2018 were retrospectively examined and
the data obtained were transferred to the information
forms created by the researchers. The data were evaluated in SPSS 20.0 package program. Chi-square test was
used for descriptive analysis. In the study, the reliability
level was accepted as 95% and the statistical significance
limit p <0.05 for all analyzes. Results: In patients, 75.2%
were male, 52.3% were under the age of 35, 86.3% had
social security, 81.7% resided in Antalya and surrounding provinces, 41.2% had previously It was found that
the patients were treated in an inpatient manner, 8.5%
had positive HCV value, 16.3% had HBV immunity, and
no HIV positive patient. Among the substances used, it
was found that opioids were used at the highest rate
(39.9%) and that there were additional diagnoses
accompanying addiction. Discussion: It has been determined that substance use disorders are more common in
young adults, especially men, and opioid use is in the
first place. Although our study results reflect a single
hospital and a small sample, it is thought that it will provide data in terms of comparing the sociodemographic
characteristics of individuals who apply to many different health institutions for the treatment of the same
problem and will contribute to the planning and execution of the treatment process of addictions
Synergistic effect of thymoquinone and nystatin in the treatment of oral candidiasis; an in vitro study
The efectiveness of antifungal agents may be insufcient against resistant strains in some cases of oral candidiasis. The aim
of this study was to evaluate the antifungal efect of thymoquinone against Candida albicans, Candida tropicalis, Candida
glabrata and Candida krusei strains and the synergistic antifungal activity of these strains in combination with nystatin. To
evaluate in vitro antifungal activity and interactions between thymoquinone and nystatin, substances were tested against
Candida albicans ATCC 10,231, C. tropicalis ATCC 750, C.krusei ATCC 6258 and C. glabrata ATCC 2001 standard strains
both individually and combinationally via microdilution method. MIC and ΣFIC index value were analysed. The Kruskal
Wallis test and Bonferroni test were used for statistical evaluations. Statistical signifcance was set at p<0.05. A statistically
signifcant diference was observed between the mean ranks of all Candida species and doses of thymoquinone, nystatin,
and the combination thymoquinone-nystatin (p<0.05). MIC values for thymoquinone were determined as 15 μg/mL for
C. albicans, C. tropicalis and C. krusei while it was 30 μg/mL for C. glabrata. Moreover, MIC for nystatin was found as
1.875 μg/mL for C. albicans, C. tropicalis and C. krusei, whereas it was 7.5 μg/mL in C. glabrata. Interaction assays and
ΣFIC index value revealed that, TQ and nystatin have a synergistic efect against to all strains. Thymoquinone was found to
have antifungal activity on Candida species and synergistic efect when combined with nystatin
Lymphocyte to monocyte ratio may predict increased carotid intima-media thickness in patients with subclinical hypothyroidism
AMAÇ: İnflamasyonun ateroskleroz patogenezinde önemli bir rolü vardır. Lenfosit-monosit oranı (LMR) inflamasyonun bir göstergesi olarak kabul edilmektedir. AMAÇ: Amacımız, subklinik hipotiroidizmde (SubHT) karotis intima-media kalınlığı (CIMT) ile ölçülen LMR ile subklinik ateroskleroz (SubAth) arasındaki ilişkiyi incelemektir. YÖNTEMLER: Yeni tanımlanan 190 SubHT hastası prospektif olarak çalışmaya dahil edildi. Laboratuvar parametrelerini ölçmek için kan örnekleri alındı. Daha sonra CIMT hesaplandı. Hastalar KİMK değerlerine göre 2 gruba ayrıldı (Grup-1: ≤0,9 ve Grup-2: >0,9 mm), ardından sırasıyla LMR ve tiroid uyarıcı hormon (TSH) düzeylerine göre tertillere ayrıldı. BULGULAR: 59 hastada KİMK değeri yüksek (Grup-2), 131 hastada ise KİMK değeri normaldi (Grup-1). Grup-2'de Grup-1'e göre daha düşük LMR ve yüksek duyarlıklı C-reaktif protein (hsCRP), CIMT ve TSH değerleri daha yüksekti (hepsi için p 0.9 mm), and then stratified into tertiles pursuant to LMR and thyroid-stimulating hormone (TSH) levels, respectively. RESULTS:59 patients had an increased CIMT value (Group-2), and 131 patients had a normal CIMT value (Group-1). Group-2 had a lower LMR and a greater high-sensitivity C-reactive protein (hsCRP), CIMT and TSH than Group-1 (for all, p < 0.05). Patients in the lowest tertile of LMR had a higher hsCRP, TSH and CIMT than those in the highest tertile (for all, p < 0.05). LMR was negatively associated with hsCRP, CIMT and TSH (for all, p < 0.05). LMR and TSH were independent predictors of increased CIMT. CONCLUSIONS:Pre-ultrasonographic LMR, which is a simple and inexpensive inflammatory marker, may give additional predictive information to determine SubAth in SubHT
PREDICTING NUCLEAR RENOGRAPHY BASED SPLIT RENAL FUNCTION IN LIVING DONOR TRANSPLANTATION WITH CT BASED MEASURED SPLIT RENAL VOLUMES
A Study of Plate Waste in a Food Service
Amaç: Hastane yemekhanesinde yemek yiyen personelin tabak atık miktarını belirlemek ve yemek türlerine göre değerlendirmek. Ortam: İstanbul'da bir devlet hastanesinin yemekhanesi. Temel Sonuç Ölçütleri: Gıda türlerine göre tabak atıkları. Analiz: Niceliksel değişkenlerin analizinde etkileşimli model, 2 faktörün (yemek ve yemek türü) etkisi için iki yönlü varyans analizi (Two-way ANOVA) ile test edilmiştir. Bulgular: Araştırma sırasında 16.722 kişinin tabak atığı tartıldı. Servis edilen yenilebilir yiyeceklerin %6,2'si çöpe atıldı. Akşam yemeğindeki ortalama atık miktarının öğle yemeğine göre daha az olduğu belirlendi (p<0,05). Yemek türüne göre hamur işi tatlılar en az öğle yemeğinde (%1,8; %95 Cl, 1,67-1,94), en fazla ise akşam yemeğinde (%5,8; %95 Cl, 4,78-5,50) tüketilmektedir. Kurumsal gıda hizmetlerinde gıda israfının ölçülmesi, nedenlerinin belirlenmesi ve tüketicilerin bilgilendirilmesi sürdürülebilir kalkınmanın devamı için gereklidir. Tüketicinin gıda israfı konusundaki farkındalığı ve gıda güvensizliği birlikte değerlendirilmelidir.Objective: To determine the amount of plate waste of the personnel eating in a hospital food hall and to evaluate according to the food types. Design: A cross-sectional trial. Setting: Food hall of a state hospital in İstanbul. Main Outcome Measures: Plate wastes by food types. Analysis: In the analysis of quantitative variables, the interactive model was tested with two-way analysis of variance (Two-way ANOVA) for the effects of 2 factors (meal and type of food). Results: During the research, 16,722 people’s plate waste was weighed. 6.2% of the edible food that was served was disposed. The average amount of waste at dinner was found to be less than lunch (p<0.05). According to the type of food, pastry desserts are disposed the least in lunch (1.8%; 95% Cl, 1.67-1.94) and the most in dinner (5.8%; 95% Cl, 4.78-5.50). Conclusions and Implications: In order for countries to create plans for preventing food loss, and waste, they must first conduct waste measurement research in every field of the aspect. Measuring food waste in institutional food services, determining the reasons, and informing the consumers are necessary for the continuation of sustainable development. Consumer awareness of food waste and food insecurity should be evaluated together
A Pareto optimum approach and a power management strategy for a stand-alone wind turbine-PV-hydrogen with reformer hybrid renewable energy system
Kapalı erişim.In this study, a multi-objective and constrained optimization problem is discussed. The SAHRESOpt (multi-objective and constrained optimization algorithm for SA-HRES) which includes size optimization and minimization of the loss of power supply probability was developed based on non-dominated sorting genetic algorithm-II (NSGA-II). Additionally, a power management strategy was designed to ensure that the power flow between the system elements minimizes the unmet load and/or the excess energy. In this context, with the aim of obtaining uninterrupted hydrogen energy in the system, a reformer was used transforming methane gas produced by an anaerobic reactor into hydrogen. Thus, a stand-alone hybrid renewable energy system (SA-HRES) that does not generate CO2 emissions was designed. Furthermore, within the scope of size optimization, both the types of system elements and the numbers of them were optimized. Then, the positive effects of reformer existence on both of the objective functions, and also the effects of considering the waste energy percentage as a constraint are presented. Also, Pareto solutions of SAHRESOpt and NSGA-II are compared. Finally, a case study including photovoltaic panels, wind turbines, fuel cells, electrolyzers, hydrogen tanks, and a reformer is presented to prove the effectiveness of the presented algorithm and validate the results
Minimizing makespan and flowtime in a parallel multi-stage cellular manufacturing company
This study proposes a 3-phase solution approach for a multi-product parallel multi-stage cellular manufacturing company. The study focuses on a case study involving a shoe manufacturing plant in which products are produced according to their due dates. The investigated manufacturing process has three stages, namely lasting cells, rotary injection molding cells, finishing-packaging cells. System performance is measured based on total flowtime and makespan. We propose a 3-phase solution approach to tackle the problem; 1) the first phase of the proposed approach allocates manpower to operations in the lasting cells and finishing-packaging cells, independently. The objective is to maximize the production rates in these cells. 2) The second phase includes cell loading to determine product families based on a similarity coefficient using mathematical modeling and genetic algorithms (GA). The proposed GA algorithm for cell loading performs mutation prior to crossover, breaking from traditional genetic algorithm flow. The performance measures flow time and makespan are considered in this phase. 3) Flow shop scheduling is then performed to determine the product sequence in each (lasting, rotary injection molding, finishing-packaging) cell group. This 3-phase solution approached is repeated with alternative manpower level allocation to lasting and finishing-packaging cells where the total manpower level remains the same
Frequency of frontotemporal dementia-related gene variants in Turkey
Frontotemporal demansın (FTD) klinik heterojenliği gibi genetik temeli de çeşitlidir ve hastalık patogenezinde çok sayıda moleküler yolakların rol oynadığı düşünülmektedir. Bu çalışmada FTD ile ilişkili genler, 175 indeks FTD hastasından oluşan bir Türk kohortunda GRN, MAPT, TARDBP, FUS, CHMP2B ve VCP genlerini içeren bir gen paneli ile değerlendirildi. 16 hastada (%9,1) potansiyel genetik ilişkiler araştırıldı; beş varyant (GRN'de p.(Gly35Glufs) ve p.(Cys253Ter); VCP'de p.(Arg95Cys); TARDBP'de p.(Met405Val) ve MAPT'de p.(Pro636Leu)) patojenik (P) veya olası olarak sınıflandırıldı dört ailesel ve bir sporadik hastada patojenik (LP). Ailesel vakalarda MAPT, CHMP2B ve FUS'ta üç yeni varyant da tanımlandı. En sık görülen patojenik varyantlar %1,14 (2/175) sıklıkla GRN geninde gözlendi ve bu oran, önemi belirsiz varyantlar (VUS) dahil olmak üzere %4,57 (8/175) oldu. Türk FTD hastalarının en geniş kohortunun yer aldığı bu çalışmada, GRN ve MAPT varyantları en sık görülen genetik birliktelikler olarak belirlendi; klinik bulguları uyumlu olan hastalarda VCP, TARDBP, CHMP2B ve FUS varyantları gibi nadir nedenlerin de göz önünde bulundurulması önerilmektedir. Tüm hakları saklıdır.Just as its clinical heterogeneity, genetic basis of Frontotemporal dementia (FTD) is also diverse and multiple molecular pathways are thought to be involved in disease pathogenesis. In the present study, FTD-related genes were evaluated in a Turkish cohort of 175 index FTD patients with a gene panel including GRN, MAPT, TARDBP, FUS, CHMP2B and VCP genes. Potential genetic associations were prospected in 16 patients (9.1%); five variants (p.(Gly35Glufs) and p.(Cys253Ter) in GRN; p.(Arg95Cys) in VCP; p.(Met405Val) in TARDBP and p.(Pro636Leu) in MAPT) were classified as pathogenic (P) or likely pathogenic (LP), in four familial and one sporadic patients. Three novel variants in MAPT, CHMP2B and FUS were also identified in familial cases. The most common pathogenic variants were observed in the GRN gene with a frequency of 1.14% (2/175) and this rate was 4.57% (8/175), including variants of uncertain significance (VUS). In this study with the largest cohort of Turkish FTD patients, GRN and MAPT variants were identified as the most common genetic associations; and rare causes like VCP, TARDBP, CHMP2B and FUS variants are recommended to be considered in patients with compatible clinical findings. The Association of Glomerular Filtration Rate With Echocardiographic Parameters in Chronic Kidney Disease
Background: Cardiovascular disease (CVD) is the primary cause of mortality and morbidity in chronic kidney disease (CKD) patients. Aortic propagation velocity (APV), epicardial fat thickness (EFT) and carotid intima-media thickness (CIMT) measurements could provide additional information on assessing renal decline in CKD patients. The study aimed to evaluate EFT, AVP and CIMT in CKD patients and then investigate the association among those parameters.
Methods: A total of 170 CKD consecutive subjects were enrolled in the study. Patients were divided into five groups according to their estimated glomerular filtration rate (eGFR) values. Each patient underwent complete transthoracic echocardiography examination. APV, EFT and CIMT were measured for analyses. A multivariate linear regression model was used for analysis to determine the independent predictors of eGFR.
Results: The lowest APV was observed in stage IV-V, and the highest APV was observed in stage I-II (P < 0.001). Stage IV-V patients had the highest EFT and stage I-II patients had the lowest EFT (P < 0.001). Moreover, the lowest CIMT was observed in stage III, and the highest CIMT was observed in stage V (P < 0.001). eGFR was significantly and positively correlated with APV and negatively correlated with EFT and CIMT. In multivariate analyses, APV (odds ratio (OR): 0.289, P < 0.001), EFT (OR: -0.135, P < 0.001) and CIMT (OR: -0.388, P < 0.001) were independent predictors of eGFR.
Conclusion: We found that APV decreased, and EFT and CIMT increased as CKD progress. The present study suggests that APV, EFT and CIMT might be incorporated with the examination of CKD patients in daily practice
Kolposkopi Yapılan Kadınlarda Anormal Servikal Sitoloji ve Biyopsi Sonuçlarına Göre Saptanan HPV Tipleri
Amaç: Kolposkopi eşliğinde servikal biyopsi ve endoservikal küretaj (ECC) yapılan hastalarda patoloji ve referans smear sonuçlarına göre saptanan HPV tiplerini belirlemek.
Gereç ve Yöntem: Bu retrospektif vaka serisinde, jinekolojik onkoloji polikliniğinde kolposkopi yapılan 520 hasta çalışmaya alındı. Hastaların yaşı, HPV sonuçları, HPV tipleri, smear sonucu, kolposkopik biyopsi ve ECC sonuçlarına, yapılan eksizyon işleminin tipine ve patolojik sonuçlara hastane kayıt sisteminden ulaşıldı. Referans smear sonuçları ve kolposkopi eşliğinde servikal biyopsi ve ECC sonuçlarına göre saptanan HPV tipleri belirlendi.
Bulgular: Çalışmaya alınan hastaların 279’unda HPV pozitifti. 25.1% hastada HPV tip 16, 12.2% hastada tip 18, 47.0% (n=131) hastada diğer yüksek riskli HPV tipleri pozitifti. Diğer yüksek riskli HPV tiplerinden en sık HPV tip 39, 7.9% hastada izlendi. Referans smear sonucu 48.8% (n=239) hastada önemi belirlenemeyen atipik skuamöz hücreler, 23.9% (n=117) hastada düşük dereceli skuamöz intraepitelyal lezyon, 3.9% (n=19) hastada yüksek dereceli skuamöz intraepitelyal lezyon iken 23.5% (n=115) hastada smear sonucu normaldi. Smear sonucu anormal olan hastalarda yüksek riskli HPV enfeksiyonu sıklığı 68.3% idi ve en sık HPV tip 16 mevcuttu (25.1%). Punch biyopsi patolojisinde 48.4% hastada CIN (servikal intraepitelyal neoplazi)1, 8.1% hastada CIN2 veya CIN3, 30.4% hastada servisit ve 1.1% hastada skuamoz hücreli karsinom, 1% hastada normal patoloji saptandı. Biyopsi sonucu CIN2-3 veya skuamoz kanser olup, HPV tiplendirilmesi yapılan 71.4% hastada HPV tip 16 pozitifti.
Sonuç: Anormal sitoloji sonucu olan kadınların 68.3%’ünde yüksek riskli HPV enfeksiyonu saptadık. En sık görülen HPV genotipi tip 16 (25.1%) idi ve 12.2% hastada tip 18, 7.9% hastada tip 39 mevcuttu. Biyopsi sonucu CIN2 ve 3 ve skuamoz kanser olan 71.4% hastada HPV tip 16 pozitifti, tip 18 ve diğer yüksek riskli HPV tipleri kalan kısmından sorumluydu.To determine human papilloma virus (HPV) types detected based on pathology and reference smear results
in patients who underwent colposcopy-guided cervical biopsy and endocervical curettage (ECC).
Materials and Methods:
In this retrospective case series, 520 patients who underwent colposcopy in the gynecological oncology
outpatient clinic were included in the study. Patients’ age, HPV results, HPV types, smear results, colposcopic biopsy and ECC results, type of excision procedure and pathological results were obtained from the
hospital registry system. HPV types determined based on reference smear results and colposcopy guided
cervical biopsy and ECC results were identified.
Results:
HPV was positive in 279 of the patients included in the study. HPV type 16 in 25.1% patients, type 18 in 12.2%,
and other high-risk HPV types in 47.0% (n = 131) patients were positive. Among the other high-risk HPV types,
the most common HPV type was type 39, 7.9% of the patients. Reference smear result 48.8% (n = 239) of
the patients was atypical squamous cells of undetermined significance, 23.9% (n = 117) of the patients was
low-grade squamous intraepithelial lesions, 3.9% (n = 19) of the patients was high-grade squamous intraepithelial lesions and 23.5% (n = 115) of the patients smear result was normal. In patients with abnormal
smear results, the frequency of high-risk HPV positivity was 68.3% and the most common HPV was type
16 (25.1%). In punch biopsy pathology, 48.4% was CIN (cervical intraepithelial neoplasia)1, 8.1% was CIN2 or
CIN3, 30.4% was cervicitis and 1.1% was squamous cell carcinoma, and 1% was normal pathology. HPV type
16 was positive in 71.4% of the patients with CIN2-3 or squamous cancer.
Result:
We found that high-risk HPV infection rate was 68.3% of women with abnormal cytology results. The most
common HPV genotype was type 16 (25.1%) and 12.2% of the patients had type 18 and 7.9% of the patients
had type 39. HPV type 16 was positive in 71.4% of the patients with CIN2 and 3 biopsy and squamous cancer,
type 18 and other high-risk HPV types were responsible for the remaining part
