Self Efficacy Perception Scale of Nursing Administrators: A Research Study
Administrator nurses play an important role in managing costs and resources when providing high-quality healthcare services. Institutions require managers with sufficient knowledge and skills to provide effective and quality service. Administrative ability is indicated as a primary factor that can have a positive or negative impact on the vision of an organization. The purpose of this study was to develop and test a scale to identify duty-related, self-efficacy perceptions of nurses in administrative roles. Perceived self-efficacy is an important determinant in the process of initiating, maintaining, and successfully completing an action. It affects people's choices of activities and behavioral settings, how much effort they expend, and how long they will persist in the face of obstacles and negative experiences. If health organizations conduct proper research and determine the necessary competencies for each position and management level, then potential candidates may be selected to be trained as professional administrative nurses through development programs to improve their administrative competencies. The Self-Efficacy Perception Scale for Administrator Nurses (SEPSAN) is thought to use management process theory to help assess the self-efficacy perceptions of administrative nurses regarding their administrative activities
Comparison of spermiograms of infertile men before and during the COVID-19 pandemic
OBJECTIVE: Since the start of the COVID-19 pandemic, there has been interest in the impact of both SARS-CoV-2 infection and pandemic-induced social restrictions on male reproductive health. This study aimed to evaluate the spermiogram values of men who presented for infertility during the pandemic compared with the previous 2 years. METHODS: Patients who presented to a urology outpatient clinic for the first time due to infertility were included. The patients' age, semen volume, and spermiogram results were recorded. Based on the presentation date, the patients were divided into prepandemic group 1 (March 2018-February 2019), prepandemic group 2 (March 2019-February 2020), and pandemic group (March 2020-February 2021) for comparison. RESULTS: A total of 594 patients were included. There was no significant difference between the three groups in terms of the number of patients who presented for infertility (207, 190, and 197 patients, respectively; p=0.691). The mean age was 36.6 +/- 7.2 in the prepandemic group 1, 35.5 +/- 7.1 in the prepandemic group 2, and 33.1 +/- 6.3 in the pandemic group. Patients who presented during the pandemic were significantly younger (p<0.001). There were no differences between the groups in terms of semen volume (p=0.910) or rates of normospermia and pathological spermiogram findings (p=0.222). CONCLUSIONS: In the first year of the COVID-19 pandemic, there was no significant difference in the number of patients who presented for infertility or in their spermiogram results compared with 2018 and 2019. However, it is noteworthy that the patients were significantly younger during the pandemic than in the previous 2 years
Antecedents and Outcomes of Nurses' Subjective Career Success: A Path Analysis
Purpose This study aimed to test the hypothesis model showing the relationship between nurses' individual and working characteristics, nursing work environment, subjective career success, job satisfaction, intent to leave, and professional commitment. Design and Methods A cross-sectional and correlational design was utilized for the study. The study sample consisted of 604 nurses working in four hospitals in Istanbul, Turkey. Data were collected using the Nurse Information Form, Subjective Career Success Inventory, Practice Work Environment Scale of the Nursing Work Index, Job Satisfaction Global Item, Intent to Leave Subscale, and Professional Commitment Scale. Data were analyzed using descriptive and correlation analysis, and the hypothesis model was tested using structural equation modeling. Findings The hypothesis model that was established to test the antecedents and outcomes of subjective career success in nurses was acceptable and had a good fit. Having a master's degree, work schedule with rotating shifts (negative), good individual income, participation in hospital affairs, staffing and resource adequacy, and nurse-physician relations were significantly associated with the subjective career success of nurses. Subjective career success had a positive effect on job satisfaction and professional commitment and a negative significant effect on intent to leave in nurses. Conclusions This study revealed that human capital, objective career success, and some characteristics of the nursing work environment were significantly associated with nurses' subjective career success, and that increased subjective career success produced positive professional and organizational outcomes. Clinical Relevance The results of this study, which revealed the antecedents and outcomes of nurses' career success, should be taken into consideration by managers who wish to retain a qualified nursing workforce
Synergistic effect of thymoquinone and nystatin in the treatment of oral candidiasis; an in vitro study
The effectiveness of antifungal agents may be insufficient against resistant strains in some cases of oral candidiasis. The aim of this study was to evaluate the antifungal effect of thymoquinone against Candida albicans, Candida tropicalis, Candida glabrata and Candida krusei strains and the synergistic antifungal activity of these strains in combination with nystatin. To evaluate in vitro antifungal activity and interactions between thymoquinone and nystatin, substances were tested against Candida albicans ATCC 10,231, C. tropicalis ATCC 750, C.krusei ATCC 6258 and C. glabrata ATCC 2001 standard strains both individually and combinationally via microdilution method. MIC and ΣFIC index value were analysed. The Kruskal Wallis test and Bonferroni test were used for statistical evaluations. Statistical significance was set at p < 0.05. A statistically significant difference was observed between the mean ranks of all Candida species and doses of thymoquinone, nystatin, and the combination thymoquinone-nystatin (p < 0.05). MIC values for thymoquinone were determined as 15 μg/mL for C. albicans, C. tropicalis and C. krusei while it was 30 μg/mL for C. glabrata. Moreover, MIC for nystatin was found as 1.875 μg/mL for C. albicans, C. tropicalis and C. krusei, whereas it was 7.5 μg/mL in C. glabrata. Interaction assays and ΣFIC index value revealed that, TQ and nystatin have a synergistic effect against to all strains. Thymoquinone was found to have antifungal activity on Candida species and synergistic effect when combined with nystatin
Endoscopic Hernia Repair: A Novel Technique for the Repair of Inguinal Hernia in a Cadaver Model
Amaç: Kasık fıtığının laparoskopik onarımı için uzun zamandan beri total ekstraperitoneal ve transabdominal preperitoneal fıtık onarımı teknikleri uygulanmaktadır. Ancak genel anestezi gerektirmesi, olumsuz kozmetik sonuçlar doğurması ve diğer ciddi komplikasyonlar gibi önemli dezavantajlara sahiptirler. Kadavra modelinde kasık fıtığının laparoskopik onarımı için total ekstraperitoneal ve transabdominal preperitoneal yönteme alternatif bir endoskopik yöntemin uygulanmasının, mevcut tekniklerin dezavantajlarının üstesinden gelmenin uygulanabilirliğini inceledik. Yöntemler: 24-26 Ocak 2020 tarihleri arasında rutin otopsi yapılması planlanan toplam 4 erkek ve 2 kadın kadavra [18 yaş ve üzeri, vücut kitle indeksi (BMI) <40 kg/m(2)] seçildi. Adli Tıp Kurumu'nda yürütülen bu çalışma için. Bu kadavralardan 6'sında her iki kasık bölgesi kullanılarak 12 laparoskopik kasık fıtığı onarımı ameliyatı yapıldı. Bulgular: 4 erkek (ortalama yaş: 43,25 y; BMI: 29,05 kg/m(2)) ve 2 kadın kadavra (ortalama yaş: 76,50 y; BMI: 26,60 kg/m(2)) üzerinde toplam 12 ameliyat gerçekleştirdik. . Her iki cinsiyet için de 7x5 cm'lik file kullanıldı. Meshin sabitlenmesinde pubik tüberkül ve tendon birleşim yerinde titanyum tacker kullanıldı, ligamantum inguinalenin tespitinde ise 3 ameliyatta 2/0 PDS, bir ameliyatta 2/0 15-cm V-Loc kullanıldı. 3 ameliyatta, 6 ameliyatta titanyum tacker kullanıldı. Sonuç: Açık kasık cerrahisinin avantajlarını kadavra modelinde sunan endoskopik cerrahi tekniğinin, özellikle genel anesteziye uygun olmayan hastalarda tüm avantajları bir araya getirecek şekilde günümüzde kullanılan laparoskopik girişimle birleştirilebileceğini gösterdik. Laparoskopik müdahale sonrasında nüksetme yaşayan ve ameliyattan kaynaklanan gözle görülür bir yara izi istemeyenler. Gelecek çalışmalarda daha net sonuçlara ulaşabilmek için önerilen tekniğin kasık fıtığı hastalarına da uygulanmasını öneriyoruz.Background: The techniques of total extraperitoneal and transabdominal preperitoneal hernia repair have been conducted for laparoscopic repair of inguinal hernia since long. However, they offer significant disadvantages of requiring general anesthesia, producing negative cosmetic outcomes, and other serious complications. We examined the feasibility of applying an endoscopic method alternative to total extraperitoneal and transabdominal preperitoneal for laparoscopic repair of inguinal hernia in a cadaver model so as to overcome the disadvantages of the presently available techniques. Methods: A total of 4 male and 2 female cadavers [aged 18 y and above, body mass index (BMI) <40 kg/m(2)], who were planned for a routine autopsy between January 24 and 26, 2020 were selected for the present study conducted at the Forensic Medicine Institute. Twelve laparoscopic repair surgeries of inguinal hernia were performed in 6 of these cadavers using both the inguinal areas. Results: We conducted a total of 12 surgeries on 4 male (mean age: 43.25 y; BMI: 29.05 kg/m(2)) and 2 female cadavers (mean age: 76.50 y; BMI: 26.60 kg/m(2)). A 7x5-cm-shaped mesh was used for both the sexes. For the fixation of the mesh, a titanium tacker was used in pubic tubercle and tendon conjoint, whereas for the fixation of ligamentum inguinale, 2/0 PDS was used in 3 surgeries, 2/0 15-cm V-Loc was used in another 3 surgeries, and titanium tacker was used in 6 surgeries. Conclusions: We demonstrated that the technique of endoscopic surgery that offers the advantages of open inguinal surgery in a cadaver model can be combined with the presently used laparoscopic intervention so as to combine all the advantages, especially for patients who are not suitable for general anesthesia, who are experiencing a recurrence after laparoscopic intervention, and who do not want any visible scares resultant from the surgery. For future studies, we suggest that the proposed technique be applied to inguinal hernia patients to ascertain clearer results
Association Between Sleep Quality and Depression in Women with Abdominal Obesity and Dietary Energy Density
Obesity has a multifactorial effect caused by genetic, metabolic, environmental, behavioral, and socio-cultural factors. Depression and a high-energy diet are observed in sleep deprivation. This study was planned to determine the relationship between sleep quality and depression in obese women and dietary energy density (DED) and abdominal obesity. Demographic information, anthropometric measurements, eating habits, and sleep and depression status of 106 obese female participants with a BMI ≥30 kg/m2 (Pittsburgh Sleep Quality Index-PSQI and Beck Depression Inventory (BDI) were evaluated. In this study, it was determined that 22.6% of the women had short sleep duration and 44.3% had poor sleep quality. No significant relationship was found between sleep quality and BDI results and the average amount of carbohydrates, proteins, fats, and the energy contribution percentages of three-day food intake (p>0.05). There was no significant relationship between Dietary Energy Density (DED) and waist circumference, BMI, sleep duration, and BDI score. However, there was a relationship between PSQI and BDI score results (r=0.373, p<0.01). As sleep quality decreased, the prevalence of depression increased. The prevalence of obesity increasing in recent years in parallel with decreased sleep quality, high intake of energy-dense foods, and increased prevalence of depression, is at significant levels. More comprehensive studies are needed to examine the relationship between DED, sleep quality, depression, and obesity
Evaluation of serum interleukin-38 levels in different radiographic grades of idiopathic knee osteoarthritis
Bu çalışmanın amacı farklı radyografik derecelerde diz osteoartriti (OA) olan hastalarda interlökin-1 beta (IL-1 beta), IL-1Ra, IL-36 ve IL-38 düzeyleri ile hs-CRP düzeylerini değerlendirmektir. Sağlıklı bireylerle karşılaştırıldığında. Bu prospektif vaka-kontrol çalışmasına Kasım 2018 ile Mart 2019 tarihleri arasında Ortopedi ve Travmatoloji bölümümüze başvuran ve Amerikan Romatoloji Koleji kriterlerine göre diz OA tanısı alan 50 yaş üstü ardışık hastalar dahil edildi. Diz OA'li hastalar radyografik Kellgren-Lawrence (K-L) sınıflamasına göre evrelendi ve her gruba 20 hasta atandı. Kontrol grubu olarak, klinik ve radyografik artrit belirtisi olmayan sağlıklı gönüllülerden oluşan, yaş ve cinsiyet açısından eşleştirilmiş bir kontrol grubu yürütüldü. Venöz kan örnekleri toplandı ve çift antikorlu sandviç ELISA yöntemi kullanılarak hs-CRP, IL-1 beta, IL-1Ra, IL-36 ve IL-38 seviyeleri açısından değerlendirildi. hs-CRP, IL-1 beta, IL-36 ve IL-38 düzeyleri, K-L derece 4 diz OA gruplarında sağlıklı kontrollere göre anlamlı derecede yüksek olan IL-1Ra düzeyleri dışında kontroller ve bağımsız radyografik evre grupları arasında anlamlı farklılık göstermedi ( P = 0,045). Diz OA'li tüm hastalar ile sağlıklı kontrolleri karşılaştırdığımızda, sağlıklı kontrol grubunda diz OA'li hastalara göre IL-1 ve IL-1Ra'nın anlamlı derecede düşük, IL-38 düzeylerinin ise anlamlı derecede yüksek olduğunu tespit ettik (P = < 0,001, < sırasıyla 0,001 ve 0,019). Çalışmamızdan elde edilen sonuçlara göre IL-1 beta, IL-1Ra ve IL-38 düzeyleri sağlıklı bireyler ile diz OA'li hastalar arasında anlamlı farklılık gösterdi. Ancak diz OA'nın radyografik derecesi ile interlökin düzeyleri arasında anlamlı bir fark ve korelasyon gözlemlemedik.The aim of this study was to assess interleukin-1 beta (IL-1 beta), IL-1Ra, IL-36, and IL-38 levels together with hs-CRP levels in patients with different radiographic grades of knee osteoarthritis (OA) in comparison to healthy individuals. Consecutive patients aged over 50 years who were admitted to our Orthopaedics and Traumatology department between November 2018 and March 2019 and diagnosed as knee OA according to the American College of Rheumatology criteria were included in this prospective case-control study. Patients with knee OA were staged according to radiographic Kellgren-Lawrence (K-L) classification and 20 patients were assigned to each group. An age and gender matched control group consisted healthy volunteers with no clinical and radiographic sign of arthritis were conducted as the control group. Venous blood samples were collected and assessed for hs-CRP, IL-1 beta, IL-1Ra, IL-36, and IL-38 levels using the double-antibody sandwich ELISA method. The hs-CRP, IL-1 beta, IL-36 and IL-38 levels did not significantly differ among controls and independent radiographic stage groups except IL-1Ra levels which was significantly higher in K-L grade 4 knee OA groups compared to healthy controls (P = 0.045). When we compared all patients with knee OA and healthy controls, we detected that IL-1 and IL-1Ra were significantly lower and IL-38 levels were significantly higher in healthy control group compared to patients with knee OA (P = < 0.001, < 0.001, and 0.019, respectively). According to results obtained from our study, IL-1 beta, IL-1Ra, and IL-38 levels significantly differed between healthy individuals and patients with knee OA. However, we did not observe a significant difference and correlation between radiographic grade of knee OA and interleukin levels
Laparoscopic cholecystectomy by the modified bikini line approach as a simple and safe technique
Objective: The gold standard technique for laparoscopic cholecystectomy (LC) is using four ports in the upper abdomen. However, this operative approach may not provide aesthetic satisfaction for some patients because of visible incision marks. This study sought to demonstrate that these incision marks can be hidden by safely changing the port locations. Methods: For patients with symptomatic cholelithiasis undergoing LC between March 2019 and March 2020, the modified bikini line approach was used. With the patient in the supine position with open legs, the first trocar (10 mm) was inserted into the abdomen through an 11-mm incision in the umbilicus. The other three trocars were placed in the abdomen at the bikini line with the help of a camera. The standard equipment for LC was then used to perform the surgery. Results: The modified bikini line approach to LC was used for 38 patients. Average operative time was 28.65 min, and the average hospital stay was 1.07 days. No perioperative or postoperative complications occurred. Follow-up was at 1 week, 1 month, and 6 months. Cosmetic results were satisfactory for all patients. Conclusions: As an alternative to the standard LC approach, the modified bikini line technique is safe and useful in patients for whom postoperative aesthetic appearance is important. The modified approach is simple to learn and use and is effective to hide the incision marks well

Dental yaralanmalar ve bu yaralanmaların sonuçları her yıl çok sayıda çocuğu etkilemektedir. Söz konusu
yaralanmalar, bebeklik döneminden çocukluğa geçişte düşmeler veya çeşitli cisimlere çarpmalar sebebiyle oluşabilmektedir. Altı yaş altı çocuklarda, ağız bölgesi en sık yaralanan ikinci alan olmasına rağmen,
bu yaralanmaların daimi dişe uzun dönem etkileri hakkında sınırlı bilgi bulunmaktadır. Gelişmekte olan
daimi dişler, travmadan direk olarak etkilenebilmekte ve çeşitli seviyelerde hasarlar gösterebilmektedir.
