    The Effect of Visits by Operating Room Nurses Before Cardiac Surgery on Anxiety and Pain Management

    Purpose: Further studies are needed in line with the Enhanced Recovery for Cardiac Surgery (ERCS) protocols with a view to reducing anxiety and opioid use in cardiac surgery patients. The present study investigates the effects of preoperative visits by operating room nurses to patients scheduled for cardiac surgery on post -operative anxiety, pain severity and frequency, and the type and dose of analgesic medication.Design: This is a quasi-experimental study with a pretest-posttest control group design involving non-randomized groups.Methods: The study was conducted in the Department of Cardiovascular Surgery of a Foundation University Hospital in Turkey between August 20, 2020 and April 15, 2021. Included in the study were patients selected based on a nonprobability sampling approach who met the study inclusion criteria (aged 18-75 years, no psy-chiatric diagnosis or drug use, first cardiovascular surgery experience, scheduled for elective surgery, up to five coronary anastomoses, literate and able to speak and understand Turkish, undergoing cardiovascular surgery with Cardiopulmonary Bypass (CPB)) determined by the researcher. The treatment group was visited preoperatively by operating room nurses, and followed-up for the first 72 hours after surgery.Findings: The intervention was effective in reducing postoperative state anxiety levels (P < .05). In the control group, each one-point increase in the preoperative state-anxiety level caused a 9% increase in the length of stay in the intensive care unit (P < .05). Pain severity increased as the preoperative state-anxiety and trait-anxiety levels, and the postoperative state-anxiety levels, increased (P < .05). While there was no significant difference in pain severity, the intervention proved to be effective in reducing pain frequency (P < .05). It was further noted that the intervention reduced the use of opioid and nonopioid analgesics for the first 12 hours (P < .05). The probability of using opioid analgesics increased 1.56 times (P < .05) with each one-point increase in pain severity reported by the patients.Conclusions: The participation of operating room nurses in preoperative patient care can contribute to the management of anxiety and pain and the reduction of opioids. It is recommended that such an approach be implemented as an independent nursing intervention given the potential contribution to ERCS protocols.(c) 2023 American Society of PeriAnesthesia Nurses.     Comparison of the effectiveness of transverse friction massage and thiele massage in female patients with chronic pelvic pain

    Aim: This study aimed to investigate and compare the effectiveness of transverse friction massage and Thiele massage applied to the pelvic floor muscles in female patients with chronic pelvic pain.Material and Methods: Twenty patients were divided into two groups of 10 people each: the transverse friction group and the Thiele massage group. Patients received transverse friction massage or Thiele Massage 2 days a week for 4 weeks. Measured parameters were pain, quality of life, sexual functions, and lower urinary tract symptoms. Visual Analog Scale and Mcgill Melzack Pain Questionnaire, Nottingham Health Profile, Female Sexual Function Index, and Bristol Female Lower Urinary Tract Symptoms Questionnaire were used to evaluate parameters, respectively.Results: Statistically significant improvements were observed in all parameters in the transverse friction massage group (p0.05).Discussion: Transverse friction massage and Thiele massage applied to the pelvic floor muscles in patients with chronic pelvic pain are easily applicable noninvasive treatment methods that can reduce the symptoms of the disease and increase the quality of life

    Depresyon ruhsal sağlığı etkileyen ve toplum sağlığını tehdit eden sorunlarından biridir. COVİD- 19 ile depresyon vakalarında artış bildirilmektedir. Depresyonlu bireylerin tedavisinde yer alan ilaçlardan bu bireyler yan etkileri (ağız kuruluğu, baş dönmesi, cinsel işlevlerde azalma vb) sebebiyle kullanımından kaçınabilmektedir. Bu durum depresyonlu bireylerin tedavisinde yer alan ilaç ve psikoterapiye ek tedavi araştırmalarına yol açmıştır. Bu ek tedavilerde alternatif tıbbi ürünlerinin kullanımı da son zamanlarda kılavuzlarda yer almaya başlamıştır. Curcuma Longa bitkisinden elde edilen zerdeçalın bir biyoaktif bileşeni olan kurkumin depresyonlu bireylerde farmakolojik tedaviden bağımsız olarak tedavi edici etkilerinin olduğu tartışılmaktadır. Oral yolla alınan zerdeçaldaki kurkumin miktarı ve biyoyararlanımı düşüktür. Kurkuminin etkinliğinde artış sağlayan biyoaktif birleşenler ile tüketimi biyoyararlanımı desteklemektedir. Bu derleme depresyona sahip bireylerde kurkuminin etkilerini inceleyen güncel makalelerden derlenerek depresyonlu bireylerde kurkumin takviyesini incelemektedir.Depression is one of the problems that threaten mental health protection and society threats.     Dil, oldukça kompleks bir yapıya sahiptir. Dil gelişimi, bireyin yaşadığı deneyimler, yaş, eğitim düzeyi, sosyal çevre ve kişisel farklılıklar gibi birçok faktörden etkilenir. Genç yetişkinlik döneminde dil gelişimi devam etmektedir ve bireyler arasında çeşitlilik gösterebilir. Dilin sentaktik karmaşıklık göstergelerinden biri olarak ele alınan yan cümlecik üretimi yaşla birlikte artmakta ve bu durum dil becerilerinin matürasyonuna işaret etmektedir. Bu çalışma ile 18-30 yaş arası genç yetişkinlerin tek resim (anlatım modeli) ve sıralı öykü (yeniden anlatım modeli) ile elde edilen anlatı örneklerinde mikroyapısal özellikler ve eylemsi ile kurulan yan cümlecik üretimleri incelenmiştir. En yüksek sıklıkta eylemsinin üretimi, yeniden anlatım modeli ile sıralı öykünün kullanıldığı durumda gerçekleşmiştir.Language has a very complex structure. The production of subordinate clauses, which is considered as one of the indicators of syntactic complexity of language, increases with age and this situation indicates the maturation of language skills. In this study, microstructural features and the production of subordinate clauses with verbs in narrative samples of young adults between the ages of 18-30, obtained with single picture (narrative model) and sequential story (retelling model), were analyzed.     Can circulating oxidative stress-related biomarkers be used as an early prognostic marker for COVID-19?

    Arka Plan Oksidatif stres birçok hastalığın patogenezinde önemli bir rol oynar. Bu çalışma, yeni COVİD-19 hastalarında nükleer faktör kappa B (NF-kappa B) ile oksidatif stres arasındaki ilişkiyi ve hastalığın ciddiyetini araştırmayı ve NF-kappa B, okside LDL (oxLDL) düzeylerini karşılaştırmayı amaçladı. ) ve COVİD-19 hastalarında hastalığın şiddet parametrelerinin bir göstergesi olan oksijen satürasyonuna sahip lektin benzeri oksitlenmiş LDL reseptörü-1 (LOX-1). Yöntemler Bu prospektif çalışmada 100 COVİD-19 hastası ve 100 sağlıklı denek seçildi. SonuçlarLOX-1, NF-kappa B ve oxLDL'nin COVID-19 hastalarında sağlıklı bireylere göre daha yüksek olduğu belirlendi (hepsi için p < 0,001). Korelasyon analizi sonuçlarına göre oksijen saturasyonu ile LOX-1, NF-kappa B ve oxLDL parametreleri arasında anlamlı bir ilişki olmadığı tespit edildi. COVİD-19 hastalarında oxLDL ile LOX-1 ve NF-kappa B arasında anlamlı ilişki vardı. En yüksek ayırt etme gücüne sahip ROC analizi sonuçları, COVİD-19'u gösteren oxLDL (AUC: 0,955, CI: 0,904-1,000; duyarlılık: %77 ve özgüllük: %100, kesim için: 127,944 ng/l) olmuştur. Sonuç Oksidatif stres, COVID-19'da önemli bir rol oynar. NF-kappa B, oxLDL ve LOX-1, COVID-19'da iyi belirteçleri temsil ediyor gibi görünüyor. Çalışmamız ayrıca oxLDL'nin COVID-19 hastalarını sağlıklı deneklerden ayırmada en yüksek güce sahip olduğunu gösterdi.Background Oxidative stress plays an important role in the pathogenesis of many diseases. This study aimed to investigate the relationship between nuclear factor kappa B (NF-kappa B) and oxidative stress and the severity of the disease in new COVID-19 patients, and, to compare the levels of NF-kappa B, oxidized LDL (oxLDL), and lectin-like oxidized-LDL receptor-1 (LOX-1) with oxygen saturation, which is an indicator of the severity parameters of the disease in COVID-19 patients. MethodsIn this prospective study, 100 COVID-19 patients and 100 healthy subjects were selected. ResultsLOX-1, NF-kappa B, and oxLDL were found to be higher in COVID-19 patients compared to the healthy subjects (p < 0.001 for all). According to the results of correlation analysis, it was found that there was no significant relationship between oxygen saturation and LOX-1, NF-kappa B and oxLDL parameters. There was significant relationship between oxLDL with LOX-1 and NF-kappa B in patients with COVID-19 disease. ROC analysis results of the highest discrimination power were oxLDL (AUC: 0.955, CI: 0.904-1.000; sensitivity: 77%, and specificity: 100%, for cutoff: 127.944 ng/l) indicating COVID-19. ConclusionOxidative stress plays an essential role in COVID-19. NF-kappa B, oxLDL, and LOX-1 seem to represent good markers in COVID-19. Our study also showed that oxLDL has the highest power in distinguishing patients with COVID-19 from the healthy subjects

    The Role of NF-κB, PPAR-α, and PPAR-γ in Older Adults with Metabolic Syndrome

    The etiopathogenesis of metabolic syndrome (MetS) has not been fully understood yet, and chronic low-grade inflammation is thought to be associated with the development of complications related to MetS. We aimed to investigate the role of Nuclear factor Kappa B ( NF-?B ), Peroxisome Proliferator-Activated Receptor- a and ? (PPAR-a, and PPAR-?) which are the main markers of inflammation in older adults with MetS. A total of 269 patients aged=18, 188 patients with MetS who met the diagnostic criteria of the International Diabetes Federation, and 81 controls who applied to geriatrics and general internal medicine outpatient clinics for various reasons were included in the study. Patients were separated into four groups: young with MetS (< 60, n=76), elderly with MetS (=60, n=96), young control (< 60, n=31), elderly controls (=60, n=38). Carotid intima-media thickness (CIMT) and NF-?B , PPAR-a, and PPAR-? plasma levels were measured in all of the participants. Age and sex distribution were similar between MetS and control groups. C-reactive protein (CRP), NF-?B levels (p=0.001) and CIMT (p<0,001) of MetS group were significantly higher than in the control groups. On the other hand, the PPAR-? (p=0.008) and PPAR-a (p=0.003) levels were significantly lower in MetS. ROC analysis revealed that the NF-?B, PPAR-a, and PPAR-? could be used to indicate MetS in younger adults (AUC: 0.735, p<0.000; AUC: 0.653, p=0.003), whereas it could not be an indicator in older adults (AUC: 0.617, p=0.079; AUC:0.530, p=0.613). It seems that these markers have important roles in MetS-related inflammation. In our results, suggest that the indicator feature of NF-?B , PPAR-a and PPAR-? in recognizing MetS in young individuals is lost in older adults with Mets

    IgG4 Positivity in Chronic Tonsillitis: A New Component of IgG4-Related Disease?

    Objective: Immunoglobulin G4-related disease (IgG4-RD) and chronic tonsillitis are both chronic fibroinflammatory diseases in which tissue atrophy is sometimes observed. In this study, the authors aimed to investigate the pathologic IgG4 positivity in tonsillectomy specimens and hypothesized to name it as a new clinical component of IgG4-RD if there is significant IgG4 positivity in chronic tonsillitis. Methods: A total of 73 patients who underwent tonsillectomy for chronic tonsillitis were included in this study. Of these, 31 patients had atrophic form chronic tonsillitis. Pathologic examinations and specific IgG4 immunohistochemical staining were performed by the same experienced pathologist in terms of IgG4-RD. Results: Sixty-three percent (n=46) of the cases were male, 37% (n=27) were female, their ages ranged from 3 to 51, and the mean age was 19.11 +/- 14.82. It was determined that 23.3% (n=17) of the cases participating in the study were IgG4-positive. When the pathologic grades of the cases were examined; it was observed that 13.7% (n=10) were Grade I, 65.8% (n=48) were Grade II, and 20.5% (n=15) were Grade III. A statistically significant difference was found between the pathology degrees of the cases according to the IgG4 groups (P=0.001; P<0.01). Conclusion: The authors concluded that as the histopathologic grades of chronic lymphoplasmacytic inflammation in tonsils specimen increase, IgG4 positivity rates also increase. Therefore, this clinical entity may be a new IgG4-related disease state in cases with chronic tonsillitis. Level of Evidence: Level II

    Bu çalışmanın amacı, subakromial sıkışma sendromu (SSS) tanısı alan hastalarda manuel terapi ve skapular stabilizasyon egzersiz programının etkinliğinin araştırılmasıdır. Çalışmaya, Azerbaycan/ Bakü ili, HB Güven Klinik Fizik Tedavi ünitesine başvuran SSS tanısı almış 30 hasta dahil edilmiştir. Katılımcılar, Skapular Stabilizasyon (SS) ve Manuel Terapi (MT) olmak üzere iki gruba ayrılmıştır. Başlangıçta katılımcıların demografik bilgileri sorgulanmış, omuz eklemi fleksiyon, abdüksiyon, internal ve eksternal rotasyon, eklem hareket açıklığı, ağrı, fonksiyonel durum ve yaşam kalitesi değerlendirilmiştir. Tedavi süreci boyunca konvansiyonel fizyoterapiye ek olarak MT grubuna skapular mobilizasyon, üst trapez miyofasial gevşetme, glenohumeral posterior ve inferior mobilizasyon, SS grubuna ise push- up, skapular saat egzersizi, lawnmower egzersizi, ayakta ağırlık aktarma ve skapular depresyon egzersizleri uygulanmıştır. Tedavi her iki gruba da 4 hafta, haftada 3 seans olmak üzere toplam 12 seans uygulanmıştır. Değerlendirmeler tedavi sonrası tekrarlanmıştır. SS ve MT grubunun başlangıç ağrı şiddeti, eklem hareket açıklığı, fonksiyonel durum ve yaşam kalitesi skorlarının benzer olduğu gözlemlenmiştir (p>0,05). Her iki grubun tedavi öncesi ve tedavi sonrası ağrı şiddeti, eklem hareket açıklığı, fonksiyonel durum ve yaşam kalitesi skorları arasında anlamlı iyileşme görülmüştür (p0,05). Çalışmanın sonuçları, ağrı şiddeti, fonksiyonel durum ve yaşam kalitesi açısından her iki grupta da tedavi öncesine göre iyileşme kaydedilmesine rağmen skapular stabilizasyon egzersizlerinin ve manuel terapi tekniklerinin birbirleri üzerine üstünlüğü olmadığı yönündedir.The aim of this study is to investigate the effectiveness of manual therapy and scapular stabilization exercise programs in patients diagnosed with Subacromial Impingement Syndrome. Thirty patients who applied to the HB Güven Clinic physical therapy unit in Azerbaijan/Baku were included in the study. Participants were divided into 2 groups as Scapular Stabilization (SS) and Manual Therapy (MT). In the beginning, the demographic information, shoulder joint flexion, abduction, internal and external rotation range of motion, pain, functional status and quality of life of the participants were evaluated. During the treatment process, in addition to conventional physiotherapy, scapular mobilization, upper trapezial myofascial release, glenohumeral posterior and inferior mobilization were applied to the MT group, and push-up, scapular clock exercise, lawnmower exercise, standing weight transfer and scapular depression exercises were applied to the SS group. The treatment was applied to both groups for 4 weeks, 3 sessions per week, for a total of 12 sessions. Evaluations were repeated after treatment. Initial pain intensity, range of motion, functional status, and quality of life scores of groups were similar (p>0.05). There was a significant improvement in pain severity, range of motion, functional status, and quality of life scores of both groups after treatment (p0.05). The results of the study show that scapular stabilization and manual therapy were not superior to each other, although both groups showed improvement in terms of pain intensity, functional status, and quality of life compared to pre-treatment
