    Assessment of Apparent Diffusion Coefficient Parameters and Coefficient of Variance in Discrimination of Receptor Status and Molecular Subtypes of Breast Cancer

    Abstract: Objective: The objective of this study was to investigate the diagnostic power of apparent diffusion coefficient/coefficient of variance (ADCcV) as well as ADC parameters formed based on magnetic resonance images (MRI) in the distinction of molecular breast cancer subtypes. Methods: The study involved 205 patients who had breast cancer at stages 1-3. Estrogen receptor (EsR), progesterone receptor (PrR), human epidermal growth factor receptor 2 (Her2), and proliferation index (Ki-67) were histologically analyzed in the tumor. The correlations between the immunohistochemistry and intrinsic subtypes were analyzed using ADC and ADCcV. Results: The maximum whole tumor (WTu) ADC (p=0.004), minimum WTu ADC (p<0.001), and mean WTu ADC (p<0.001) values were significantly smaller in the EsR-positive tumors than those in the EsR-negative tumors. Compared to the PrR-negative tumors, the PrR-positive tumors showed significantly smaller maximum, minimum, and mean WTu ADC values (p=0.005, p=0.001, and p<0.001, respectively). In the comparisons of the molecular subtypes in terms of ADCcV, the p-values indicated statistically significant differences between the luminal A (lumA) group and the triple negative (TN) group, between the luminal B (lumB) group and the TN group, and between the Her2-enriched and TN groups (p<0.001, p=0.011, and p=0.004, respectively). Considering the luminal and non-luminal groups, while a significant difference was observed between the groups considering their minimum, maximum, and mean WTu ADC values, their ADCcV values were similar (p<0.001, p=0.004, and p<0.001, respectively). Conclusion: Using ADCcV in addition to ADC parameters increased the diagnostic power of diffusion weighted-MRI (DW-MRI) in the distinction of molecular subtypes of breast cancer

    Thermodynamic analysis of an evacuated tube solar collector system for industrial building application: A case study

    In this study, a modified solar-Organic Rankine Cycle (ORC) assisted hydrogen production system for two different scenarios was experimentally investigated. In the study, evacuated tube solar collector (ETSC), ORC system, proton exchanger membrane electrolyzer (PEMe) unit is discussed with a holistic perspective. The presented work includes energy and exergy analysis of solar-ORC assisted hydrogen production. According to scenario 1 (S-1), only the case of hydrogen production from the installed system is considered. According to Scenario 2 (S-2), the both electrical energy and hydrogen production from the installed system is examined. As a result of the study, the energy efficiency in S-1 and S-2 operating modes was calculated as 9.553% and 16.02%, respectively. Additionally, the highest exergy efficiency for both scenarios was calculated as 6.129% in the S-2 scenario. Furthermore, hydrogen production amount of the S-1 and S-2 scenarios was calculated as 743.1 kg/ annual and 371.5 kg/annual, respectively

    Futuristic science is a new science that involves a systematic and explicit way of thinking about the future, is based on special assumptions and uses specific values and theories in its intellectual system. The goal of foresight is to identify areas of strategic research and new technologies. In addition, strategic intelligence, like a search engine, discovers new opportunities and warns of threats, which in turn contributes to future research. The purpose of this study is to evaluate the impact of future research on organisational strategic intelligence in the water and sewage organisation. The current research is descriptive in terms of data collection method and practical in terms of purpose. The library-field method was also used to collect the relevant information in the form of a questionnaire. According to the purpose of the research and the topic, this research includes a sample of the experts of the Water and Sewerage Organisation of West Azerbaijan Province. The Delphi method was used to identify the future drivers of the strategic intelligence of the organisation. The data obtained from the questionnaires were analysed using MicMac software. Finally, the results showed that the negligence of officials, indiscriminate and non-mechanised use of agriculture, climate change, culture of correct use of resources and population growth were identified as important and influential factors

    Extended flexible capacity planning methodology with flight network contribution to the airline fleet assignment problem

    Flexible capacity planning is crucial for the airline industry due to the high fixed cost of assets, notably aircraft ownership, and the fluctuating demand, particularly in response to external shocks like pandemics and political crises. Following the negative consequences of such events on the financial positions of almost all airline companies, there is growing interest in developing more responsive capacity planning structures for cost efficiency in the airline industry. In this study, we present a sequential two-phase methodology for flexible capacity planning and airline fleet assignment considering the factors related to fleeting strategies, passenger demand behavior, and contribution of flights in the network. The first phase of the methodology generates potential scenarios to estimate the number of flights to the destinations, based on the data and the reports provided by two prominent associations in the aviation industry, which incorporate various country-related and airline-related factors, including population, income, routes, airports, and connectivity. The second phase identifies the required number of aircraft for each type within the fleet inventory, taking into account the aircraft utilization rates associated with each scenario. The fleet assignment problem is also reformulated to employ the objective of maximizing total profit of the entire flight network by introducing network contribution and nation/regionspecific limitations in this phase. A real case study from Turkey is conducted to illustrate efficiency of the proposed methodology. We also present a comparative study to highlight the impact of network contribution and how fleet size affects the outcomes. We have observed that the inclusion of the network contribution aspect and country constraints has led to a significant improvement in financial indicators and connectivity. The results show that the methodology can effectively tackle challenges in airline transportation problems

    Leadless pacemaker implantation following tricuspid interventions: multicenter collaboration of feasibility and safety

    BackgroundPermanent pacing is often required following valve intervention (either surgical or percutaneous); however, tricuspid interventions pose specific challenges to conventional pacing. Therefore, leadless pacemaker (LP) implantation may be the preferred strategy when permanent pacing is required after tricuspid valve intervention.PurposeTo report periprocedural outcomes and follow-up of patients undergoing implantation of a LP system following tricuspid valve interventions.MethodsPatients with previous tricuspid valve intervention at the time of attempted implantation of a LP (MicraTM, Medtronic, Minneapolis, MN, USA) were included.ResultsBetween 2019 and 2022, 40 patients underwent LP implantations following tricuspid interventions in 5 large tertiary centers. The mean age was 68.9 +/- 13.7 years, and 48% patients were male. The indication for pacing was as following: AVB in 27 (68%) patients, AF with slow ventricular response in 10 (25%) patients, and refractory rapid atrial fibrillation (AF) referred to AV junction ablation in 3 (7%) patients. Most of the patients received Micra VR (78%). The procedure was successful in all patients. The mean procedural time is 58 +/- 32 min, and the median fluoroscopy time is 7.5 min. Electrical parameters were within normal range (threshold: 1.35 +/- 1.2 [email protected] ms, impedance: 772 +/- 245 Ohm, R-wave: 6.9 +/- 5.4 mV). No acute complications were observed. During a mean follow-up of 10 months, electrical parameters remained stable, and 4 deaths were occurred (not related to the procedure).ConclusionA LP is a safe and efficient option following tricuspid valve interventions

    Melatonin Decreases Alveolar Bone Loss in Rats with Experimental Periodontitis and Osteoporosis: A Morphometric and Histopathologic Study

    Background: Periodontitis and post-menopausal osteoporosis include common chronic bone disorders worldwide, with similar etiopathogenetic events. This study evaluated the effect of systemic melatonin administration on the alveolar bone destruction of periodontitis progression in an experimental periodontitis model in osteoporotic rats. Methods: Forty-four Wistar rats were randomly divided into six experimental groups: control (C; n = 6); osteoporosis (O; n = 6); ligated periodontitis (LP; n = 8); osteoporosis- and periodontitis-induced (O+LP; n = 8); osteoporosis- and periodontitis-induced through 30 mg/kg/day melatonin administration (ML30; n = 8); and osteoporosis- and periodontitis-induced through 50 mg/kg/day melatonin administration (ML50; n = 8). The rats underwent bilateraloophorectomy and were maintained for 4 months to induce osteoporosis. After 4 months, 4-0 silk ligatures were placed submarginally around the mandibular first molar of each rat to induce experimental periodontitis, and melatonin was administered in the ML30 and ML50 groups for 30 days. Changes in alveolar bone levels were clinically measured, and tissues were histopathologically examined. Results: Osteoclastic activity in the LP and O+LP groups was significantly higher than in the other groups (p 0.05). RANKL activity was the highest in the O+LP group, while melatonin decreased RANKL activity in the melatonin-administered groups (p < 0.05). Systemically administered melatonin significantly decreased alveolar bone loss in the ML30 and ML50 groups compared with that in the periodontitis groups (p < 0.05). Conclusions: Melatonin inhibited alveolar bone destruction by decreasing the RANKL expression and inflammatory cell infiltration and increased osteoblastic activity in a rat model with osteoporosis and periodontitis

    Frequency of Single or Combined Anticholinergic Medication Usage as Potentially Inappropriate Medications among the Oldest-Old Age Subgroup

    Background: The use of medications with anticholinergic effects can have negative effects on older patients, such as cognitive dysfunction and falls. The aim of this study was to determine the frequency of use of anticholinergic medication among the oldest -old age group and to analyze the factors associated with its use. Methods: This is a descriptive study. The data were collected by visiting the oldest -olds at their homes, where the oldest -olds lived alone or with his or her family. The sample size was calculated as 354 using EpiInfo. The generic name of the medication used, the age, sex, and city of residence of the patient were recorded. The medications were analyzed using guidelines showing the anticholinergic effect, classified as 1 (low), 2 (moderate), and >= 3 (strong), and the frequency of usage by the oldest -olds was determined. Results: A total of 549 oldest -olds participated in the research. Of the oldest -olds, 57.92% (n = 318) were using one or more anticholinergic medications. The median number of anticholinergic medications used by the oldest -olds was 1 (range: 0.00-6.00). Medications with strong anticholinergic effects ( >= 3) were used by 14.94% (n = 82) of the oldest -olds. The most commonly used were medications with low anticholinergic effects. However, the concomitant use of anticholinergic medications increased the total anticholinergic effect. Conclusion: The oldest -old age group used medications with anticholinergic effects. A strong anticholinergic effect was obtained when medications with low or moderate anticholinergic effects were combined. Medications with anticholinergic effects should also be reviewed in the safety analysis of medications used by the oldest -old

    Çalışmamızın amacı; yaşlı bronşektazili bireylerin oluşturduğu deney grubu ve kronik solunum yolu hastalığı bulunmayan akranlarının oluşturduğu kontrol grubunda, solunum fonksiyonu, fonksiyonel kapasite, denge, kognitif beceri, el reaksiyon zamanı hızı ve el kavrama kuvveti, yaşam kalitesi parametrelerini karşılaştırmalı olarak değerlendirmekti. Çalışmamıza; Bezmialem Vakıf Üniversitesi Tıp Fakültesi Hastanesi, Göğüs Hastalıkları Anabilim Dalı'nda, bronşektazi tanısı almış, 65 yaş ve üzerindeki 15 bronşektazili hasta ve de benzer kriterlerdeki 65 yaş üstü, kronik solunum yolu hastalığı bulunmayan, 15 yaşlı birey dahil edildi. Çalışmamız kapsamında; deney grubu 15 yaşlı bronşektazili birey, kontrol grubu kronik solunum yolu hastalığı bulunmayan 15 yaşlı birey olacak şekilde iki gruba ayrıldı. Tüm bireylerin solunum fonksiyonları ''Solunum Fonksiyon Testi (SFT)'' ile, fonksiyonel kapasite değerlendirmesi ''30 Saniye Otur Kalk Testi'' ile, denge değerlendirmesi ''Nintendo Wii Fit Plus Sanal Gerçeklik Cihazı'' ile, kognitif becerileri ''Montreal Bilişsel Değerlendirme Ölçeği (MOCA)'' ile, reaksiyon zamanı App-coo test aplikasyonundaki ''12- Kırmızı Kare Testi'' ile, el kavrama kuvveti ölçüm testi dinamometre ile, yaşam kalitesi ''Nottingham Sağlık Profili'' ile değerlendirildi. Çalışmamızda yaşlı bronşektazili bireylerin ve kontrol grubundaki bireylerin kilo haricindeki tüm demografik özelliklerinin birbirine benzer olduğu görüldü (p>0.05). Yaşlı bronşektazili grup ile kontrol grubu arasında egzersiz alışkanlığı ve sigara kullanım durumu açısından istatistiksel olarak anlamlı fark vardı (p0,05). Bireylerin kognitif becerilerinin değerlendirilmesi amacıyla kullanılan ''Montreal Bilişsel Değerlendirme Ölçeği'' skoru, yaşlı bronşektazi grubunda daha az olup, yaşlı bronşektazi grubu ve kontrol grubu arasında istatiksel açıdan anlamlı fark bulundu(p0,05). Tüm bireylerin yaşam kalitesinin değerlendirildiği ''Nottingham Sağlık Profili'' anketinin ağrı, duygusal izolasyon, uyku, sosyal izolasyon, fiziksel aktivite ve enerji alt bölüm puanları ve iki ana bölümün toplam puanları, yaşlı bronşektazili grup ve kontrol grubunda benzer değerlerde bulundu ve grupların farkları istatistiksel olarak anlamlı bulunmadı(p>0,05). Sonuç olarak çalışmamızda, yaşlı bronşektazili bireylerin denge, el reaksiyon hızı ve el kavrama kuvveti, yaşam kalitesi parametrelerinde, kontrol grubuna kıyasla azalma beklenirken anlamlı bir fark görülememiştir. Çalışmamızda yaşlı bronşektazili bireylerin fonksiyonel kapasite, kognitif beceri ve solunum fonksiyon parametre değerleri, literatürle de uyumlu olarak, beklenildiği üzere kontrol grubuna kıyasla daha düşük bulunmuştur. Yaşlı bronşektazili hastalarda fonksiyonel kapasitenin ve kognitif fonksiyonların değerlendirilmesi, yaşlı bronşektazili hastaların performansında artış sağlamayı amaçlayan tedavi programlarında ek bir girdi sağlayabilir.The aim of our study was to comparatively evaluate respiratory function, functional capacity, balance, cognitive skills, hand reaction time speed, hand grip strength and quality of life parameters in an experimental group of elderly individuals with bronchiectasis and a control group of their peers without chronic respiratory disease. In our study, 15 patients with bronchiectasis aged 65 years and older who were diagnosed with bronchiectasis in the Department of Chest Diseases, Bezmialem Vakıf University Faculty of Medicine Hospital, and 15 elderly individuals aged 65 years and older without chronic respiratory disease with similar criteria were included. Within the scope of our study; the experimental group was divided into two groups as 15 elderly individuals with bronchiectasis and the control group as 15 elderly individuals without chronic respiratory disease. Respiratory functions of all individuals ''Respiratory Function Test (PFT)'', functional capacity assessment was evaluated with ''30 Second Sit and Stand Test'', balance assessment was evaluated with ''Nintendo Wii Fit Plus Virtual Reality Device'', cognitive skills were evaluated with ''Montreal Cognitive Assessment Scale (MOCA)'', reaction time was evaluated with ''12- Red Square Test'' in App-coo test application, hand grip strength measurement test was evaluated with dynamometer, quality of life was evaluated with ''Nottingham Health Profile''. In our study, it was observed that all demographic characteristics of the elderly individuals with bronchiectasis and the individuals in the control group were similar except for weight (p>0.05). There was a statistically significant difference between the elderly bronchiectasis group and the control group in terms of exercise habits and smoking status (p0.05). The ''Montreal Cognitive Assessment Scale'' score, which was used to evaluate the cognitive skills of individuals, was lower in the elderly bronchiectasis group, and a statistically significant difference was found between the elderly bronchiectasis group and the control group (p0.05). The pain, emotional isolation, sleep, social isolation, physical activity and energy sub-section scores of the ''Nottingham Health Profile'' questionnaire, in which the quality of life of all individuals was evaluated, and the total scores of the two main sections were similar in the elderly bronchiectasis group and the control group, and the differences between the groups were not statistically significant (p>0.05). In conclusion, in our study, no significant difference was observed in the balance, hand reaction speed, hand grip strength and quality of life parameters of elderly individuals with bronchiectasis compared to the control group. In our study, functional capacity, cognitive skills and pulmonary function parameter values of elderly individuals with bronchiectasis were found to be lower compared to the control group, as expected, in accordance with the literature. Assessment of functional capacity and cognitive functions in elderly patients with bronchiectasis may provide additional input in treatment programmes aiming to improve the performance of elderly patients with bronchiectasis

    Reliability and Validity Assessment of the Turkish Version of the Emotional Availability and Responsiveness in Intrapartum Care Scale (EAR-IC)

    Background: Emotional availability and responsiveness in intrapartum care increase women's birth satisfaction, comfort, and quality of nursing/midwifery care. In Turkey, there is no instrument for measuring emotional availability and responsiveness in intrapartum care. Aim: An established translation, reliability, and validation approach was used to obtain the Turkish form of the A-EAR-IC scale. Methods: A descriptive cross-sectional study was used. This study was conducted in Istanbul, Turkey. The study sample consisted of 132 Turkish midwives working in the birth room. The construct validity of the scale was tested using confirmatory factor analysis, whereas the concurrent scale validity was tested using the Emotional Labor Behavior Scale. Internal consistency analysis was performed to test the reliability of the scale. Results: The results of the EAR-IC measure showed that it fit the one-factor model. The goodness-of-fit indices of the one-factor model containing eight items were acceptable. The instrument showed satisfactory content validity (I-CVI =0.80-1.00, S-CVI =0.88). According to CFA, the structure with one factor showed acceptable model fit (chi 2/df: 1.74, CFI: 0.96, IFI: 0.96, RMSEA: 0.07, and SRMR: 0.02). Concurrent validity of the instrument was assessed with the "in-depth behavior" subscale of the Emotional Labor Behavior Scale for Nursing. Spearman's correlations revealed that the EAR-IC was moderately positively correlated with the in-depth behavior concern subscale of the ELBS (rho =0.62, P < 0.001). Conclusion: The Turkish version of the EAR-IC is a suitable, effective, and reliable instrument for measuring the emotional aspects of intrapartum caregiving midwives
