No full textIn January 2023, Oregon Health Authority (OHA) expanded coverage for the Oregon Health Plan (OHP) to include severe handicapping malocclusion for beneficiaries under 21 years old. With OHP’s handicapping malocclusion coverage expansion, low Medicaid participation was anticipated to bottleneck access to care. This mixed methods study explored Oregon orthodontists’ familiarity with OHP and assessed their attitude towards the OHP program and strategies to increase access to care for OHP beneficiaries
Regulation of mu-opioid receptors in thalamic pathways of motivation and stress
No full textThis dissertation explores the regulation and adaptation of mu-opioid receptors (MORs) within the paraventricular nucleus of the thalamus (PVT), a brain region critical for integrating stress and reward signals. Through a combination of electrophysiology, pharmacology, and imaging, it identifies distinct differences in MOR responses between anterior and posterior PVT neurons, including projection-specific adaptations to chronic morphine exposure. In the anterior PVT, I demonstrated varying degrees of opioid tolerance, withdrawal, and receptor desensitization, highlighting the PVT’s heterogeneous contributions to stress, motivation, and addiction. This work advances understanding of opioid modulation in neural circuits and identifies circuit-specific targets for addressing opioid use disorders

The role of antimicrobial stewardship in modifying the epidemiology of clostridioides difficile infection
Get PDFClostridioides difficile infection (CDI) causes nearly half a million diarrheal illnesses annually in the United States and is associated with a twofold increase in the risk of death for hospitalized patients with CDI. Broad spectrum antibiotic therapy is a major risk factor for CDI. As such, healthcare-associated (HA) CDI is often employed as a metric to evaluate antimicrobial stewardship programs (ASPs). However, because the risk from antibiotics is heterogeneous and dependent on a variety of other individual and environmental risk factors, evidence that HA-CDI is a quality metric for stewardship success is lacking. The objective of this dissertation is to achieve a more granular understanding of the risk of HA-CDI from antibiotics, and to evaluate the specific contexts where we would expect HA-CDI incidence to reflect changes in broad spectrum prescribing through stewardship interventions

Get PDFRespiratory Syncytial Virus (RSV) circulates annually from fall to spring, peaking in the winter season, targeting small airways of infants leading to acute inflammation of the lower respiratory tract (Piedimonte & Perez, 2014). During this time, pediatric patient volumes exceed hospital service limits. Nursing care management of bronchiolitis involves competent clinical assessments and decision-making skills to offer supportive care interventions, escalate clinical concerns, and determine discharge readiness. Lack of nursing skills to assess respiratory status can lead to poor clinical outcomes and unnecessary interventions, resulting in prolonged hospital stays. The purpose of the quality improvement (QI) project was to examine the nursing bronchiolitis knowledge of clinical assessment and decision-making skills

No full textThis "self-instructional manual" aimed "to outline the basic concepts of the developmental anatomy of the eye and to familiarize the reader with the basic vocabulary." It was designed as a work manual, not a complete textbook or source book. The objective of this manual was that the "reader should be able to explain the concepts of eye development in [their] own words, and to relate this knowledge to the normal and the abnormal adult eye." This work was developed to serve "as a supplement to Section II of the Basic and Clinical Science Course of the American Academy of Ophthalmology and Otolaryngology, 1974-1975."
