    Incidence and Risk Factors of Neonatal Hypoglycemia in Term and Late Preterm Neonates

    Background: Neonatal hypoglycemia is one of the most common metabolic disorders in newborns and can lead to significant neurological complications if not promptly identified and treated. Understanding the incidence and associated risk factors is essential for developing effective screening and prevention strategies. Objective: To determine the incidence and identify risk factors associated with neonatal hypoglycemia in term and late preterm neonates. Methods: A prospective observational study was conducted on 450 neonates (gestational age ≥34 weeks) admitted to a tertiary care hospital over a 12-month period. Blood glucose levels were measured at 1, 3, 6, 12, 24, and 48 hours of life. Hypoglycemia was defined as blood glucose <47 mg/dL. Maternal and neonatal risk factors were analyzed using multivariate logistic regression. Results: The overall incidence of neonatal hypoglycemia was 15.3% (69/450). Late preterm neonates had significantly higher incidence (28.4%) compared to term neonates (11.2%, p<0.001). Independent risk factors included maternal diabetes (OR: 4.82, 95% CI: 2.41-9.64), small for gestational age (OR: 3.76, 95% CI: 1.89-7.48), late preterm birth (OR: 3.12, 95% CI: 1.67-5.83), and perinatal asphyxia (OR: 2.94, 95% CI: 1.42-6.09)

    Background: Topical combination therapies are the cornerstone of acne vulgaris treatment, with clindamycin-benzoyl peroxide combinations widely prescribed for synergistic antimicrobial effects. Limited data exists on dermatology practitioners\u27 real-world experiences and prescribing patterns with these formulations across diverse Indian populations. Objective: To evaluate dermatology practitioners\u27 perspectives on clindamycin-benzoyl peroxide (Clin-B) combination therapy effectiveness, safety satisfaction, prescribing patterns across acne severity levels, and antibiotic stewardship implementation. Methods: A cross-sectional survey using the SCORE (Survey on Clinred-B\u27s Outcome in Reducing Acne by Experts) questionnaire was conducted among dermatology practitioners across six Indian states (Maharashtra, Rajasthan, Uttar Pradesh, Gujarat, Madhya Pradesh, and Telangana). The 20-question survey assessed treatment utilization, safety satisfaction, comparative effectiveness, clinical decision-making factors, and stewardship awareness. Data were analyzed using descriptive statistics. Results: Thirty dermatology practitioners participated, with the largest representation from Maharashtra (46.7%). High combination therapy adoption was observed (66.7% using in >50% of patients). Safety satisfaction was universal (100% satisfied; 80% very satisfied). Treatment preferences demonstrated clear severity stratification: mild acne 40%, moderate 50%, moderately severe 56.7%, and severe acne 70% preferred Clin-B. Perfect antibiotic stewardship awareness was evident (100% agreement on avoiding monotherapy). Practitioners preferred once-daily night-time dosing (66.7%) and rated Clin-B favorably in comparative effectiveness assessments (73.3% versus clindamycin-adapalene; 66.7% versus adapalene-benzoyl peroxide). Conclusion: Dermatology practitioners demonstrate strong confidence in Clin-B therapy with evidence-based, severity-stratified prescribing (40-70% preference increase). Universal safety satisfaction and perfect stewardship compliance indicate successful resistance prevention implementation. Results provide real-world validation supporting current guidelines recommending combination therapy as first-line acne treatment

    Introduction: Neonatal pneumothorax, characterized by the accumulation of air in the pleural cavity, is a critical condition observed in up to 9.2% of neonates in neonatal intensive care units (NICUs). It poses risks such as respiratory distress, hemodynamic compromise, intraventricular hemorrhage, and increased mortality. Management strategies include conservative observation, needle aspiration (NA), and intercostal catheter (ICC) insertion, tailored to clinical severity, radiographic findings, and available resources. This study evaluates treatment modalities for neonatal pneumothorax in a rural tertiary care hospital. Methods: A descriptive longitudinal observational study was conducted from December 2020 to December 2022 at Dr. Balasaheb Vikhe Patil Rural Medical College, Loni. The study included 60 neonates diagnosed with pneumothorax in inborn and outborn NICUs. Exclusion criteria included neonates with major congenital malformations. Data collection encompassed clinical history, diagnostic findings, and management approaches, including observation, CPAP,NA, ICC placement. Statistical analysis evaluated treatment outcomes based on the pneumothorax\u27s side (unilateral or bilateral) and severity. Results: Out of 60 cases, 48.3% had right-sided pneumothorax, 28.3% left-sided, and 23.3% bilateral. CPAP was used in 61.7% of cases, particularly in less severe presentations. Ventilator support was required in 73.3%, with a significantly higher demand among outborn neonates (p=0.03). Needle aspiration was performed in 31.7% of cases, with higher use in left-sided pneumothorax (57%). Chest tube insertion was necessary in 75% of cases, predominantly for right-sided (79.3%) and bilateral pneumothorax (78.5%). Discussion: The findings align with global studies, highlighting a higher incidence of right-sided pneumothorax due to anatomical predisposition. The study underscores the effectiveness of CPAP in mild cases and the necessity of ICC insertion in severe or bilateral cases. The outcomes advocate for individualized treatment strategies based on clinical severity and resource availability. Conclusion: Neonatal pneumothorax remains a significant NICU challenge, necessitating prompt diagnosis and tailored interventions. Advances in non-invasive respiratory support and judicious use of mechanical ventilation can mitigate risks. Further research on minimally invasive techniques and long-term outcomes is crucial to optimizing neonatal care and improving survival and neurodevelopmental follow-up

    Efficacy and Safety of Platelet-Rich Plasma Therapy for Erectile Dysfunction: An Observational Study

    Background: Erectile dysfunction (ED) is a prevalent ailment that significantly impacts one\u27s quality of life. An effective regenerative treatment for the underlying pathophysiological mechanisms of ED is platelet-rich plasma (PRP) therapy. Assessing PRP therapy\u27s effectiveness, safety, and patient-reported results in mild to moderate ED was the goal of this study. Objective: to evaluate how PRP treatment for mild to moderate ED affects patient satisfaction, safety profile, and erectile function. Methods: This 12-month prospective observational study was carried out at Ashiyan Medical College Hospital. The study included 60 male patients with mild to moderate ED. One month apart, two intrapenile injections of PRP therapy were given. The International Index of Erectile Function (IIEF) scores were used to assess erectile function at baseline, one, three, and six months. Additionally, adverse events and patient satisfaction were evaluated. Results: PRP treatment significantly improved erectile function; at 6 months, the mean IIEF score increased by +5.1 points over baseline (p < 0.001). 60% of participants said they were satisfied with the therapy, and 70% said they had seen improvement. According to subgroup analysis, patients with lower BMIs and no hypertension had better results. With only minor side effects like mild pain (16.7%) and hematoma (5%), PRP therapy was well tolerated and did not result in any significant complications. Conclusion: PRP therapy offers notable and long-lasting enhancements in erectile function with a high level of patient satisfaction, making it a safe and effective option for mild to moderate ED. PRP appears to be a promising restorative treatment based on these findings. It will take more randomized controlled trials to confirm these findings and investigate long-term advantages

    Risk Factors and Clinical Outcomes of Acute Kidney Injury in Pediatric Intensive Care Unit: A Prospective Observational Study

    Background: Acute kidney injury (AKI) is a significant complication in pediatric intensive care units (PICU), associated with increased morbidity and mortality. Understanding its risk factors and outcomes is crucial for improving patient care. Objective: To evaluate the incidence, risk factors, and clinical outcomes of AKI in PICU patients through a prospective observational study. Methods: We prospectively studied 40 children aged 1 month to 18 years admitted to the PICU. AKI was defined and staged according to KDIGO criteria. Clinical parameters, laboratory values, and outcomes were monitored. Risk factors were analyzed using multivariate logistic regression. Results: AKI developed in 15 patients (37.5%), with 46.7% classified as Stage 1, 33.3% as Stage 2, and 20% as Stage 3. Significant independent risk factors included nephrotoxic medication exposure (adjusted OR 4.1, 95% CI 1.9-8.8), mechanical ventilation (adjusted OR 3.2, 95% CI 1.6-6.4), and sepsis (adjusted OR 2.9, 95% CI 1.4-6.0). AKI patients demonstrated longer PICU stays (median 12 vs 7 days, p=0.003), increased mechanical ventilation duration (median 8 vs 4 days, p=0.001), and higher mortality (26.7% vs 8%, p=0.04). Among survivors with AKI, 26.7% showed persistent renal dysfunction at discharge. Conclusion: AKI occurs frequently in PICU patients and is associated with worse clinical outcomes. Early recognition of risk factors and implementation of preventive strategies may help improve patient outcomes. Regular monitoring of renal function and post-discharge follow-up are essential for high-risk patients

    A Case of Chronic Palmo-Plantar Psoriasis Managed with Individualized Homeopathy

    Palmo-plantar psoriasis is a chronic inflammatory skin disorder affecting the palms and soles, presenting with dryness, scaling, thickened skin, fissures, and itching. This case report presents a 45-year-old male with a two-year history of palmo-plantar psoriasis, accompanied by multiple lipomas and mild constipation. At Dr. Batra’s, the patient received tailored homeopathic treatment along with supportive topical care and lifestyle advice. Over several months of follow-up, there was gradual improvement in dryness, scaling, lichenification, and itching, with healing of fissures and restoration of skin texture. No new lesions appeared, and the patient’s quality of life improved significantly. The case highlights the effectiveness of a holistic approach in chronic dermatological conditions, demonstrating how constitutional homeopathic therapy combined with topical care and lifestyle management can lead to sustained symptom relief and functional recovery
