Investigation of Japanese literature on the efficacy of dementia prevention
本研究では認知症予防の効果に関する文献検討により認知症予防の介入の現状を対象者,実施内容,評価尺度,効果に関して2006年から10年間の文献を医学中央雑誌で調査し整理した.「認知症予防」「効果」「高齢者」で検索した結果,ヒットした57件の論文のうち,地域在住の健常高齢者を対象とし,認知症予防の実施内容,評価,効果が具体的に記載されている18件を本研究の対象論文とした.認知症予防の実施内容は,パソコンプログラムなどの知的活動により認知機能を直接的に刺激する内容とウォーキングや体操,課題歩行など身体機能から間接的に認知機能への影響をおよぼす内容がみられた.知的活動と運動の双方を混合した内容もみられた.認知機能の評価は集団認知検査のファイブ・コグやMMSEなどで,運動機能の評価は歩行速度などで評価されていた.効果においては,運動や歩行強度・速度などにより認知機能に関する評価尺度が有意な増加がみられ改善がみられた.今後,認知症予防活動や看護ケアに活かしていくことが必要である.In this study we investigated and summarized the current status of interventions for dementia prevention with respect to the subjects the implementation content assessment measures and efficacy by reviewing literature on dementia prevention in the Igaku Chuo Zasshi (ICHUSHI) for the 10 years since 2005. The literature search was performed using “dementia prevention,” “efficacy,” and “elderly people” as keywords which identified 57 papers. Among these 18 papers that investigated healthy elderly local residents and concretely described the implementation content assessment and efficacy of dementia prevention were selected as the target papers of this investigation. Two types of implementation contents of dementia prevention were observed: one in which cognitive functions are directly stimulated by intellectual activity such as the use of personal computer programs and the other in which cognitive functions are indirectly affected by physical activities such as walking gymnastics and dual- or multi-task walking. Contents that included both intellectual and physical activities were also observed. Cognitive functions were assessed using tools such as the “Five-Cog” and Mini Mental State Examination (MMSE) in mass cognitive tests and motor functions were assessed using walking speed and other such indexes. With regard to efficacy scores of the cognitive function assessment tools significantly improved with exercise increased walking strength and speed etc. These findings should be utilized in future dementia prevention activities and nursing care.departmental bulletin pape
Study to consider about interactive teaching that use the audience response system
本研究は,クリッカーと呼ばれる聴衆応答システムを柔道整復師国家試験対策授業に用い,効果検証を実施した.授業終了時に行ったアンケートの結果では,5つの質問項目間の平均値に有意差が見られた(p<.001).下位検定の結果,クリッカーを使用した授業が通常の授業と比較して興味が持てたか.」の質問と「クリッカーを使用した授業が通常の授業と比較して積極的に参加できたか.」の2問に対して有意に高値を示した.教員によるアンケートからは,学生と教員の双方向で一体感が生まれ,楽しんで授業に参加する学生が増えると言う効果がみられた.その反面,回答のペースについていけない学生や,焦って解答する学生が存在した事が問題点としてあげられた.これらの問題を解決する為には,解説の時間を設け,問題数を少なくする必要性があった.更に,クリッカーを用いて授業の双方向性を向上させるためには,正答率の低い問題に対して解説の時間を増やすなどそれぞれのケースに対応する必要性が示唆された.This research, we used an audience response system called Clicker for the Judo therapist national exam lesson class and conducted the effectiveness verification. Distributed a dedicated remote control to students in total of 6 classes in different grade levels, and responded to the simulation problem with a remote control button using PowerPoint. According to the results of the questionnaire at the end of class, there were many responses such as the fact that students were interested in tackling lessons and impressions were impressed on classes using clickers. Moreover, there was an effect that a sense of unity was created in both directions between students and faculty members, and that students enjoyed and participate in classes increased. On the other hand, students who can not follow the pace of answers and students who answered impatiently existed as problems. In order to solve these problems, it was necessary to set time for explanation and to reduce the number of problems. In addition, in order to improve the interactivity of the lesson using clickers, it was suggested that there was a need to respond to each case, such as increasing the time to explain the problem with a low correct answer rate.departmental bulletin pape
Assessment of the school health program for noncommunicable disease prevention in Pohnpei State, the Federated States of Micronesia
世界の成人肥満人口は10%を超え,いずれも生活習慣病が死因の1位となっている.ミクロネシア連邦ポンペイ州でも肥満対策は喫緊の課題とされてきたが,生活習慣改善の行動変容は成人期では容易ではなく,これまで大きな成果はみられていない.
2001年からミクロネシア連邦ポンペイ州保健局公衆衛生課で、生活習慣病対策および学校保健に関わり,小学校における生活習慣病対策に関する健康教育について研究してきた.今回これまでの研究の結果と学校保健の現況、今後の課題を検討した.NCD is one cause of death for over 10% for the adult overweight population of the world. Adult obesity is a pressing problem at Micronesia, Pohnpei State improvement of their lifestyle is not easy in adulthood, and a big outcome has not been seen up to now. I have been engaged in the NCD and school health program by the Division of Primary Health Care, Department of Health Services, Pohnpei State Government since 2001 and have studied regarding School Health Education about NCD at an elementary school. Reproducibility of isometric muscle strength measurement of external shoulder rotation using a hand-held dynamometer and belt
「目的」肩関節外旋筋力の徒手筋力測定における再現性の問題に対して,定量的測定値を得るためにハンドヘルドダイナモメーターを徒手で用いる従来の方法と,考案したハンドヘルドダイナモメーターをベルトで固定した方法における再現性を検討した.
「対象」若年健常成人31名の利き手である右上肢31肢であった.
「方法」ハンドヘルドダイナモメーターを徒手で固定する従来の方法とベルトで固定する方法による肩関節外旋筋力測定を行い,測定値を比較,検討した.
「結果」ハンドヘルドダイナモメーターをベルトで固定した測定方法における検者内の級内相関係数は0.844であった.
「結論」1回練習をした後,1回目,2回目の測定値を採用することで,考案したハンドヘルドダイナモメーターをベルトで固定した肩関節外旋筋力測定方法は,臨床使用が可能な再現性を有することが示唆された.[Purpose] The purpose of this study was to examine the reliability of the method of external shoulder rotation muscle strength measurement, in which a hand-held dynamometer was fixed with a belt. The subjects were 31 young healthy adults.
[Methods] Two methods were examined. One is a conventional method in which manipulation of the hand-held dynamometer was fixed. In the other method, the hand-held dynamometer was fixed with a belt.
[Results] In the latter method, the intraclass correlation coefficient was 0.844.
[Conclusion] The result suggested that clinical use of latter the method examined be used since there was reproducibility in the 1st and 2nd measurement after the 1st practice.departmental bulletin pape
Structure of " The Challenges " of integrated nursing practicum students grasp:Learning about nursing practice as a nursing team and taking charge of multiple patients
本研究は,学生が捉えた看護チームで取り組む看護実践と複数患者受け持ちの「看護課題」の構造を明らかにし,統合実習の指導への示唆を得ることである.質的研究手法として質的統合法(KJ法)を用いた.対象は統合実習に参加した81名の最終カンファレンスレポートである.分析の結果,看護実践を【臨機応変な看護】【タイムマネジメントをした上での看護実践】【優先順位や計画立案を行った上での看護実践】が互いに影響して,【質を確保した看護の公平性】を保証すると学んでいた.この看護実践は【チーム連携によるより良い看護実践】により,チームの機能を発揮することで多重課題に対応でき,【チームでの患者の安全確保】もできると捉えていた.さらに【社会に求められる医療や看護】に応える職務責任に基づき,日々の【看護師としての自己研鑽】が必要であると学んでいた.以上のことから統合実習には,タイムマネジメント,優先順位や計画立案,連絡・報告・相談と患者安全の視点に立ってのチームワークの体験を,講義・演習・領域別実習によって積み重ねる授業方略が必要である.The purpose of this study is to clarify the structure of nursing practice " The Challenges " in working in a nursing team and taking charge of multiple patients as observed by students. And we obtain suggestions for integration practice. The qualitative integration method (The KJ method) was used as a research method. The final conference reports of 81 students who participated in integrated practice were examined.
As a result of the analysis, students learned the following things. Clinical practice of nursing was influenced by variable nursing, clinical practice with time management and clinical practice priorities and planning. The students also learned to ensure fairness of nursing quality ensuring. This nursing practice was based on better nursing practice through team, collaboration and they coped with multiple tasks by demonstrating the function of the team, and they learned that securing patients' safety in teams. In addition, the students learned that day-to-day self-study as a nurse is necessary based on the job responsibility to respond to medical care and nursing required for society.
Based on the above, in the integrated practical practicum, the lesson strategy of accumulating experiences of teamwork from time management, priorities, planning, communication / reporting, consultation and patient safety perspective through lectures / exercises /clinical nursing practice is necessary.departmental bulletin pape
Roles of surgical nurses observed during operating room practice
本研究は,学生が手術室見学実習を通して学んだ手術室看護師の役割を明らかにすることを目的とした.平成27年度に成人看護学実習Ⅱ(急性期)を履修したA大学学生77名の手術室見学実習のレポートから学生の学びを抽出し,分析した.抽出した内容のうち手術に関わる看護師の役割に関する内容が152記録単位抽出され,記録単位より21のサブカテゴリ,5のカテゴリを抽出した.手術や麻酔に対する患者の不安や緊張,羞恥心への配慮など,手術の一連の流れの中で看護師の行為自体は目に見えていても自分で意味づけなければわからない看護師の役割について学びとっていた.手術中に医療者に身を委ねることを余儀なくされる患者の状況を目の当たりにしたことから,【患者の心情を推しはかる】関わりをし,【患者の安全確保】を担う手術室看護師の役割を捉えていた.This study aims to clarify what nursing students can learn about the roles of surgical nurses through direct observation during a visit to an operating room. Reports of visits to the operating room by nursing students who took the Adult Nursing Practical II (Acute Phase) class of “University-A” in the academic year of 2015 were analysed. What the students wrote in their reports and what they learned were analyzed by classifying similarities in contents. This analysis extracted 152 records on the individual roles of nurses in their practice that the students identified. These records can be classified into 5 main categories with 21 subcategories. Students learned about the roles of nurses that must be given meaning personally in order to be understood even if the actions themselves are observable in the regular process of an operation,in such cases as providing care to alleviate or consider patient anxiety,nervousness or shame due to the operation or anesthesia . By personally observing how patients undergoing operation entrust their bodies to medical professionals,students appreciated the roles of surgical nurses in surmising patient feelings/mentality and taking responsibility to ensure patient safety.departmental bulletin pape
Question tendency analysis of the national examinations for rehabilitation for Judo Therapy Practioners.
本研究の目的は柔道整復師国家試験科目であるリハビリテーション医学の出題傾向を分類することであ
る.対象は第1回~第25回の柔道整復師国家試験において出題されたリハビリテーション医学の問題275問
であり,対象とした275問について,柔道整復師国家試験出題基準に基づき分類した.最も出題頻度が高かっ
た項目は評価法で,次いで理学療法,義肢・装具の順に出題数が多かった.過去,国家試験で出題された
問題を分類,分析することは国家試験対策を考える上で重要である.今回の分類で出題数の多かった項目
を学習することで学習効率の向上が期待できると考えられた.The purpose of this study was to analyze question tendency in the National Examinations for rehabilitation for Judo Therapy Practioners. We analyzed the 275 questions of the 1th-25th national examinations for a Judo Therapist. 275 questions were classified based on the standard national examinations. The result revealed that the most frequent questions were the assessment, and next frequent questions were the Physical Therapy and prosthesis. We can expect improvement of the learning efficiency by learning the items which were frequently asked with this classification.departmental bulletin pape
