Conservative treatment of the clavicle fracture of children
小児の鎖骨骨折は臨床的に診られる骨折である.多くは介達外力で発生する.不全骨折が多いが,完全に転位する例もある.小児の骨はリモデリングが旺盛である.そのため保存的治療が原則で,三角巾固定やクラビクルバンドの装着で対応することが多い.リモデリングが期待できるため徒手整復はほとんど必要ないとされている.本研究では小児鎖骨骨折の特徴や治療法を確認の上,診療で行われた保存的治療の症例を紹介し,今後の小児鎖骨骨折における施術のあり方について検討する.今回の症例では骨片転位の大きさの違いによって仮骨の出現時期や骨硬化など治癒経過に差はあるものの,最終的に骨癒合が認められた.今後も小児骨折の特徴を把握しながら,自家矯正力の限界や成長障害の可能性を見極め,慎重に対応していくことが必要だと考える.Clavicle fracture is one of the most frequent fractures seen clinically, so we often choose conservative treatment. Most of clavicle fractures occur with indirect force resulting in many incomplete breaks. A triangular bandage and a clavicle band are used when the bone is fixed. Remodeling is expected, so, closed reduction operations are commonly not required.
We confirm characteristics of conservative treatment for clavicle fracture of children in this study and refer to a case of the conservative treatment performed in practice. Then, we examine a method of the conservative treatment in the clavicle fracture of children for the future. Synostosis was found although there was a difference in a healing process. The results confirmed remodeling worked well in the first case.
Generally, prognosis of child bone fracture was good. However, it is necessary to treat them carefully, being aware of the limit of remodeling potency and keeping a close watch for Dystosis.departmental bulletin pape
Literature Review of "Patient-centered Care" in Nursing Education in Japan
看護における患者中心を明確にし,基礎教育の在り方について検討する目的で,医学中央雑誌・メディカルオンライン・メディカルファインダーを用いて患者中心をキーワードに全年度分を文献検索し,分類した.その結果,国内文献で患者中心の意味を明確に示すものはなく,当初,単なる看護の志向性を示す表現として患者中心が用いられたが,2000年以降,疾病中心の医学モデルからの脱却のため,新たな医学教育モデルに倫理的な側面として患者中心の視点が付加され,強化された.
また,同時期に,主として医師と患者の協働を示す患者中心のケアが海外から導入され,以後,医療者全般と患者の協働による患者中心のケアモデルの開発が進んでいる.
今後, 患者中心を目指すと看護師と患者の協働によるケアが必要となり,看護師が一方的に患者の状態を分析するのではなく,看護師自身の内省も含め相互作用として看護現象を捉えられる教育方法の開発が必要であることが示唆された.Aims: To clarify the meaning of the term “patient-centered” in nursing, and discuss the current state and future of nursing education.
Methods: Using the Web Japan Medical Abstracts Society, Medical Online, and Medical Finder, with the keyword "patient-centered",we searched for target articles regardless of the published year, and classified them.
Results: No domestic literature stated the meaning of the term "patient-centered". Initially, the term was used to simply describe the orientation of nursing; however, since 2000, it has been used to describe the ethical perspective of new medical education models, with the aim of abandoning disease-centered medical models.
In addition, during the same period, "patient-centered" care characterized mainly by cooperation between physicians and patients was introduced from other countries. Since then, the designing of "patient-centered" care models, in which healthcare providers and patients cooperate with each other, has been promoted.
In order to provide basic education targeting care generated by nurses and patients in cooperation, it is necessary to develop education methods whereby nursing phenomena, including the self-reflection of nurses, can be clarified as an interaction between the two parties, instead of the former method of analyzing the latter’s condition in a unilateral way.departmental bulletin pape
Production of interrogatives with “what” plus a noun by university studentsi
When they ask a question, university students who are beginner to pre-intermediate English learners often keep apart “what” and the noun that should follow right after it. For example, some ask *“What do you like sports?” Therefore, we examined how many students make this kind of error and whether the percentage of students who do not understand how to form this type of question differs from when speaking to writing. We found that 25-40% of the students in speaking and 15-25% of them in writing kept “what” and a noun apart. Consequently, English teachers need to emphasize that “what” and a noun should not be kept apart when forming this type of interrogatives and they should be vigilant for errors made by students and correct them immediately. Moreover, teachers have to put their efforts on helping their students acquire a good command of basic English.初級~初中級レベルの英語力を持つ大学生が“what”+名詞の疑問文を使って質問するとき,*“What do you like sports?”など,whatと名詞を離す文が毎年必ず聞かれる.そのため,どのくらいの学生がwhat+名詞疑問文の語順を理解しているのかを調査した.さらに,これまでの英語学習が話すことよりも文法を学ぶことに重点が置かれていることを踏まえ,話すときだけではなく,書くときもwhatと名詞を離すのかどうかを調べた.その結果,話すときには25~40%の学生が,書くときには15~25%の学生がwhat と名詞を離していた.中学校の初期の段階でwhat+名詞疑問文を学ぶことを考えると,この数字は小さいとは考えられず,より一層英語の基礎から指導していく必要があることを痛感させられる結果となった.departmental bulletin pape
Consideration of practical leadership development in health and physical education teacher training courses -Outcomes and problems of approaches to teaching internship -
本研究は,保健体育教員養成課程における教職インターンシップの実習内容と学びを分析し,実習生の「実践的指導力」の育成への成果と課題を明らかにすることを目的とした.平成28年度,平成29年度に教職インターンシップを行なった24名の実習生の活動報告書と自己評価票の自由記述を分析した.その結果,実習生は主に「授業」に関する「実践的指導力」の学びを得ていたことが明らかになった.課題としては実習内容にばらつきがあること,実習後の学習への関連性の意識の希薄さが明らかになった.The purpose of this research was to analyze the contents and learning of teaching internship in the health and physical education teacher training course, and to clarify the outcomes and problems to foster the practical leadership of the interns. We analyzed activity reports and descriptions of self-assessment charts of 24 trainees for the teaching internship, which we conducted during the educational years 2016, 2017. It was revealed that the intern trainees mainly learned practical leadership in relation to lessons. Moreover, it was apparent that there were variations in the content of practical training, and a low awareness of the relevance to the learning after the practical training.departmental bulletin pape
Analysis of student counseling tendencies in 2016
本稿は,2016年度1年間の学生相談室の利用状況の傾向と分析を行ったものである.相談内容は,心身症と無気力に関するものが最も多かった.来談経路は相談員の授業を受けたことをきっかけとする来談が最も多かった.次いで,自主的な来談が多かった.This paper analyzes student usage tendency of the student counseling office during 2016. Students’ consultation triggered by counselor's lessons with regards to psychosomatic disorders and apathy was most frequent. Relationship between students’ learning and nursing competence in adult nursing practicums
本研究の目的は, 成人看護学実習における学生の学びの内容から,看護基礎教育において培われる学生の看護実践能力を明らかにすることである.研究対象はA大学看護学科の3年次に成人看護学実習をおこなった97名の学生20グループの中間および最終カンファレンスのグループワークの記録である.そこから学生の学びを抽出し分析を行った.その結果,学生は成人看護学実習(慢性期)(急性期)の2つの実習をとおして,患者の健康段階および発達段階に応じた看護を判断し,実践することを学んでいた.成人看護学実習(慢性期)において学生は,患者に応じた看護を判断し実践に反映していた.成人看護学実習(急性期)では,術後の患者の回復が速く,学生は看護判断を実践に反映させることが難しいことがわかった.The purpose of this study was to clarify nursing competence acquired in basic nursing education from students’ learning in adult nursing practicum. The records of the group work of the middle and the final conference of 20 groups of 97 students who practiced adult nursing practicum in the third-year of nursing department were analyzed. As a result, the students learned to judge and practice nursing according to the health stage and the development stage of the patient through two practicums of adult chronic nursing practicum and acute nursing practicum. In adult chronic nursing practicum, students judged what to do according to patients and reflected it in practicum. In the practical course of adult acute nursing, patients recovered quickly after surgery, and it turned out that it was difficult for students to reflect nursing judgment in the practicum.departmental bulletin pape
Skill acquisition training methods of hospital gown changes for patients receiving infusion treatment: Comparison between video feedback and teachers’ oral feedback
本研究は,学生が看護技術を習得するにあたって,自身の練習をビデオ映像からフィードバックを得て練習する方法と,教員から口頭でフィードバックを受けて練習する方法とでの技術習得効果の違いを明らかにすることを目的とした.31名の学生を対象に,点滴中の患者の寝衣交換技術の習得状況について,自己評価と無記名自記式アンケート調査を行った.その結果,どちらの練習方法も技術習得に有用であった.ビデオ映像によるフィードバックを活用した練習方法は,自己評価能力の向上や,動機づけを高める効果が確認され,手順やポイントが理解できている技術の反復練習において効果的であることが示唆された.また教員による口頭でのフィードバック効果でも同じく動機づけ機能が確認された.未習得の技術に対しては,ビデオ映像によるフィードバックを活用した練習方法より,教員による口頭でのフィードバックを活用した練習方法の方がより技術の習得効果が高いことが示唆され,今後は双方の利点を活用した技術練習方法を検討していくことが求められた.This research aimed to clarify the difference between the method of training in which nursing students get feedback from watching videos of their own practice and the method of training by getting oral feedback from teachers, on students' acquisition of nursing skills. We asked 31 nursing students to assess the extent to which they mastered the skill of changing hospital gowns of patients receiving infusion treatment and to fill out an anonymous, self-administered questionnaire. The results showed that both methods were useful for acquiring this skill. We confirmed that the method of exercise making use of feedback from a video was effective in enhancing their selfassessment ability and motivation. It was also suggested that this method was effective in improving practicing skills of which they had already understood their procedures and key points. We confirmed that oral feedback from teachers was also effective in enhancing students' motivation. The result suggested that oral feedback from teachers was more effective in learning skills that nursing students had not acquired yet than the feedback from a video.
