A study on the internet dependency of the middle school students Second report
A市中学生1532名を対象に,緊急事態宣言解除後の2020年6月から7月にインターネット利用についての調査を行なった.緊急事態宣言中に,インターネットの動画・情報検索,SNS,ネットゲームを合わせた合計利用時間は,331分(5時間31分),学校が再開された平日で184分(3時間4分)であり,多くの生徒が,緊急事態宣言時にインターネットを長時間利用していたことが示された.インターネット依存については,不適応使用者が27.2%,病的使用者が12.6%おり,40.0%近くの生徒がインターネット依存に関する課題があることが分かった.また,学年別では学年が上がるにつれインターネット依存が増え,インターネット利用上の問題が表面化していた.今後は,インターネット依存に関する要因を詳細に分析し,実態に沿ったインターネット依存予防教育を行う必要がある.A survey was conducted regarding the internet usage of 1,532 middle school students in the City A between the period of June and July of 2020. During the state of emergency, the total usage time of Internet video / information retrieval, social networking service (SNS) and online games per day was 331 minutes (5 hours 31 minutes). When the school was reopened, the students spent 184 minutes daily (3 hours 4 minutes) on the weekdays. The results revealed that many students used the Internet for an extensive period of time while they were self-restraint at home. Related to the Internet addiction, 27.2% of the students were found to be maladapted. For those who became difficult in sustaining usual daily living were 12.6%. Therefore, nearly 40% of the students were considered as addicted to the internet. The survey results found that the rate of Internet addiction increased as the grade advanced. The highly dependent internet use seemed to be problematic. In summary, further investigation might be required to analyze the factors causing the Internet addiction in more details. Literature review on a supportive role of the hospital nurses for the individuals with cancers to become employed
本研究の目的は,病院に勤務する看護師によるがん患者の就労支援の現状と課題を,文献を基に明らかにすることである.医学中央雑誌Web版を用い,キーワードを「がん」「就労支援」「看護」とし検索した.看護師によるがん患者の就労支援に関する文献に限定し,26件を分析対象にした.それらを「がん患者の就労支援に向けた調査研究」(19件)と「看護師による就労支援の実践に関する研究」(7件)に大別した.がん患者は,治療と就労を両立するなかで様々な思いや困難さがあることが分かった.就労中のがん患者が就労していないがん患者よりもセルフケア能力が高かったことから,セルフケア能力を高める援助の必要性が確認された.
看護師は,がん患者の就労支援への意識をもち,セルフケアマネジメントができるよう支援していくことが課題である.今後,多職種と協働してがん患者一人一人に応じた具体的な就労支援の実践を積み重ねて,病院内外の就労支援向上に寄与できるよう研究を進めていくことが望まれる.The purpose of this study was to verify that working would benefit the individuals with cancers to improve the self-care management . While the individuals receiving the cancer treatments, the nurses could take a significant role for them to be employed. The keywords such as cancer, working support and nursing were searched by accessing the web version of the Central Medical Magazine. The selection of the literature was limited to the context of the employment for the individuals with cancers reinforced by the nurses. The 26 articles were reviewed and they were divided into two categories. One was consisted of the19 articles of "Survey research for employment support for the individuals with cancers". The other category included seven articles of "Study on practice of employment support by the nurses". As a result, the individuals with cancers encountered various distress and difficulties in balancing both treatment and employment. However, the individuals with employment developed higher abilities for self-care management skills than those who did not hold the employment. The study seemed to suggest that being employed would improve the abilities of self-care. The challenge was for the nurses to recognize that the improvement of the self-care management skills would depend upon holding the employment. In summary, this type of research would be continuously carried out so that it could encourage the employment support inside and outside the hospitals. Further, the support would be required to be tailored to each individual with the cancer in collaboration with the multiple professionals.departmental bulletin pape
A study on the images of the multidisciplinary collaboration perceived by nursing students in gerontological nursing
【目的】看護学生が考える連携の概念と吉池らの連携の構成概念とを対比し,学生の看護における専門職連携の考え方の特徴について明らかにすることを目的とした.
【方法】A大学看護学科3年生82名の「専門職連携について」の課題レポートに記載された内容を分析した.分析方法はテキストマイニングソフトKH-coderを用いて関連語検索を行った.
【結果】各構成概念について述べられた学生の割合は①同一目的の一致:41.5%,②複数の主体と役割:96.3%,③役割と責任の相互確認:20.7%,④情報の共有:93.9%,⑤連続的な相互関係過程36.6%であった.特に,「連携」の最も大事な「同一目的の一致」を記載している学生は半数以下であり「同一目的の一致」の重要性についての認識が薄く, 今後意識的に教授していく必要性が示唆された.
【考察・結論】同一の目的の一致,役割と責任の相互確認,連続的な相互関係過程の3つの要素についての認識は薄く,今後の連携教育の中ではよりこの点を強調していく必要がある.一
方,連携の弊害も忘れてはならない.Purpose:The purpose of this study was to identify the characteristics of the multidisciplinary nursing practice perceived by the nursing students. The study was carried out by comparing the ideas that the students had with the multidisciplinary collaboration model established by Yoshiike, et al.
Method:The 82 third-year nursing students at a university were assigned to write about “Multidisciplinary Collaboration.” The written assignments were collected for the analysis; then, the KH-coder, a text mining software, was utilized to search the relevant terms. Result:The analysis revealed the following five elements about the concept of the multidisciplinary collaboration model. The 41.5% of the students agreed with having the same purpose. The 96.3% of them believed the advantages of the multi-disciplinary approach and their professional responsibilities. The 20.7% of the students acknowledged keeping the mutual relationships of the respective roles and responsibilities. The 93.9% of them confirmed the importance of Information sharing. The 36.6% of the students recognized maintaining the continuous interactive relationships. Although having the “Agreement with the same purpose” was the most crucial element in the collaboration, it was notable that less than a half of the students expressed its significance. The result suggested that the value of the importance might be emphasized in the future learning opportunities. Discussions & Conclusion:The students might have difficulties in understanding the concept. It might be essential that the students needed to be taught differently when the concept of the multidisciplinary collaboration was introduced. Especially, the “agreeing with the same purpose”, “keeping the mutual relationships of the respective roles and responsibilities” and “continuing interactive relationships”, these three elements might be required more creativity in teaching. At the same time, it was necessary to recognize that the multidisciplinary collaboration might not be always beneficial to the nursing practice.departmental bulletin pape
Prototype of feet F point fixation device for stress Free therapy
ストレスフリー療法は,体表の特定の部位に遠赤外線照射による温熱刺激を加えることにより,末梢の血流が増加するなどが期待できる.ストレスフリー療法を効果的に行うために,足部F点を正しく刺激することが求められる.そこで本報告では,足部F点を正確に固定するための固定具を開発することを目的とした.足部の骨格構造の特性を得るために6名の対象者の計測を行った.計測は,スマートフォンによる足部の表面形状の計測システムを用いて,踵,内果などの6点を特徴量とした3次元の座標データを取得した.この座標データから6つの評価指標を構築し,固定具の設計データとした.
計測結果に基づき,編んだサポータータイプの固定具を試作した.固定具をすべての対象者についてオーダメイドで作成することも考えられるが,価格と簡便性の観点から,特定のサイズ別のタイプを用意し,足部F点の正確な部位の特定と固定性を高められるよう,位置決定は個別に行う方がよいと考えられた.サイズの種類は計測結果に基づき設定すべきと思料された.Stress-free therapy is the application of thermal stimulation by far-infrared radiation to specific areas of the body surface, which is expected to increase peripheral blood flow. In order to effectively carry out Stress Free therapy, proper stimulation of the feet F point is required. The objective of this report was the development of a fixation device to secure the feet F point. Measurements were conducted on 6 subjects to obtain information on characteristics of the foot skeletal structure. The measurements were performed by a system using smartphones to measure the surface shape of the foot region, which obtained 3-dimensional coordinate data for 6 points, including the heel and medial malleolus, as feature values. Six evaluation indicators were then constructed from the coordinate data, and used as data for development of the fixation device.
Based on the measurement results, a prototype supporter-type device was produced by weaving. Although it is conceivable that these devices may be custom made, from the viewpoints of price and convenience it was considered favorable to prepare different types depending on characteristics, and to determine their positioning individually while increasing the degree to which they can secure the feet F point. Establishing the integrated online nursing education project team and technological system in the COVID-19
2020(令和2)年,看護基礎教育において必須科目である統合実習は,特に学習の集大成であり重要な臨地実習である.しかし,新型コロナウイルスの世界的感染拡大によって,当大学も臨地実習ができない状況となった.そのため,緊急に統合実習プロジェクトチームが結成され,遠隔システムの整備や統合実習内容・方法等の検討が急務となった.
本研究は,コロナ禍の統合実習プロジェクトチーム結成からシステム整備,運営の実際について,1.実習目的と目標の試案,2.統合実習使用教材選択と遠隔ソフトウエア,3.統合プロジェクトチームの結成と統合実習計画,4.遠隔による統合実習の実際について,一連の活動を振り返り考察した成果である.その結果,遠隔授業に向けてITの整備と各員のICT活用指導力が重要であることが示唆された.The purpose of this study was to identify what could be needed and reinforced for the online nursing education. Due to the COVID-19, the integrated training for the basic nursing education and clinical training was required to hold through the online in 2020. Therefore, the integrated training project team was organized to develop the system for the nursing education. In addition, the contents and methods were examined.
The project team was focused upon the following four factors to establish the viable and complete online education system. The practical purpose and goal had to be decided. It was essential to have the teaching materials and software which would support the online learning. It was necessary to organize the project team and online teaching training plan. Finally, the operation of the online education system had to be tested and practiced.
In summary, the project team identified these critical factors by reviewing the series of the preparation phases. Consequently, the study suggested that it was important to develop the IT, ICT and leadership of the faculties for actualizing the quality distance learning program.departmental bulletin pape
Immediate and short-term effects of leg jiggling on young women with hip labral tears
【目的】軽微な股関節関節唇損傷を呈した若年女性にジグリングを施行し,股関節周囲機能に与える即時的,短期的な影響について検討した.【方法】症例は20歳代女性.スポーツ中の接触により受傷した.外来リハビリテーションによる介入が困難であった時期に自宅で毎日ジグリングを20分間行った.1週間に1回ジグリングの施行状況を確認する際,実際にジグリングを行い,介入前後での股関節の関節可動域測定,筋力測定,疼痛を評価した.【結果】可動域は股関節中間位での外旋に最も変化がみられた.筋力は大きな変化がみられず,疼痛は介入後に増悪がみられたが,2週間後には改善方向に変化した.【考察】ジグリングにより関節包を含む軟部組織が刺激され,即時的また短期的に股関節中間位での外旋可動域に変化が生じた可能性がある.また,ジグリング治療の際には初期から1週間は治療後に疼痛が生じるが,その後は改善方向に転じる可能性があることが示唆された.[Purpose] The purpose of this study was to describe the immediate and short-term effects of the leg jiggling for a young female with the hip labral tears. [Methods] The patient was injured the hip labral tears during the sports event. The patient performed the leg jiggling for 20 minutes daily. The examiner re-evaluated the status and methods weekly. In addition, the examiner assessed the range of motion (ROM), flexor strength and pain score of the hip joint prior to and after the intervention. [Results] The ROM change was observed in the external rotation at the neutral position of the hip joint. The muscle strength showed little difference and the pain score was exacerbated after the intervention. However, this pain was decreased after two weeks. [Discussion] The leg jiggling might be involved in the soft tissue flexibility as an immediate effect. In addition, although the pain was temporarily exacerbated initially, it was suggested that the pain might be decreased immediately or after two weeks.departmental bulletin pape
Standing retention time required to assist toilet movement in stroke hemiplegic patients-Relationship between physical and mental function evaluation and standing retention time
本研究は,症例数を追加し脳卒中片麻痺患者のトイレ介助に必要な立位保持時間を明らかにすること,麻痺側下肢運動機能の関連性について,高次脳機能障害と立位保持時間の差について検討した.対象は35名(男性17名,女性18名), SIAS下肢点数(初期最終),失語症,注意障害,構成障害,半側空間無視の評価結果を先行研究に則カルテ抽出した.
結果,立位保持時間は,33.7 ± 3.3秒であった.立位保持時間と下肢SIAS点数に強い負の相関が認めた(p<0.01).また,立位高値群と全ての障害で有意な差を認めた(p<0.01).更に立位高値群と失語症,注意障害の重度群で有意な差を認めた(p<0.01).よってトイレ介助が可能となる立位保持時間は30~36秒であり,高次脳機能障害を有する場合は,その中でも長めの保持時間が必要と考えられた.Purpose: The purpose of this study was to identify the amount of assistance and time requirements for the stroke hemiplegic patients to maintain the safe standing balance for the toileting. The study was focused upon examining the paralyzed lower limb mobilities and higher brain dysfunctions for maintaining the safe and functional standing balance. Method: The study was carried out with the 35 participants, including 17 males and 18 females. The participants were evaluated for the following criteria such as the SIAS leg scores, aphasia, attention disorder, constitutive disorder and unilateral spatial neglect. The selection of the participants was also referred to the medical records to assure the appropriateness for this study. Results: The standing retention time was 33.7 ± 3.3 seconds. Discussion and Conclusion: The standing retention time for the safe toileting and the lower SIAS scores did not show the correlation (p <0.01). This was because the physical abilities to merely maintain the standing position could be different from demonstrating the safe and functional toileting (p <0.01). Moreover, there was a clear difference that the participants with the high abilities to retain standing had less severe cognitive dysfunctions. On the other hand,the participants with the severe aphasia and attention disorders showed less abilities to retain safe standing position (p <0.01) because the safe and functional toileting would require the higher cognitive abilities. In summary, the abilities to maintain the safe standing balance for the toileting was 30 to 36 seconds. The amount of assistance would be more required as the higher brain dysfunctions became more severe.departmental bulletin pape
Examining deliberate involvement by clinical nursing instructors as a means of characterizing clinical psychiatric nurse training
本研究は,2週間の精神看護学実習において,学生が行う1週目の看護場面に実習指導者が同行し,学生の援助場面のサポート,実習指導者自身の実践等を通して,そこで展開される指導内容の特徴を明らかにすることである。
研究方法は,実習指導者が学生の看護場面で行った意図的関わりの実践場面を通し,関わり後の学生とのふり返り内容を記録シートに記載してもらった。分析方法は,記録シートの内容を文脈から意味ごと抽出したものをデータとし,得られたデータを類似した内容でサブカテゴリーとし,さらにより凝集性の高い集合体としてカテゴリー化した。その結果,70のデータ数,24のサブカテゴリー,6のカテゴリーが抽出された。以上のことより,実習指導者の指導の特徴として,【看護過程を用いた看護実践の支援】,【治療的コミュニケーションを支える】,【対話の環境づくり】,【学生の体験に意味づけする】,【学生を尊重した対応・関わり】,【自己中心的な学生への対応】が明らかになった。The present study sought to characterize the nature of practical training provided to psychiatric nursing students during their 2-week clinical training course. Specifically, the study examined the content of guidance provided by on-the-job clinical nursing instructors (CNIs) as they accompanied the students during their first week of training and provided support and practical instruction.
The study used inductive content analysis based on the training review sheets completed by CNIs after a joint review with students about their experiences of the deliberate involvement in clinical nursing scenarios. The review sheet responses were analyzed to identify contextually-relevant semantic information, and these semantic units were subcategorized for similar content and then categorized into more cohesive concepts. In total, the study identified 24 subcategories and 6 categories from 70 informational units of semantic content. The study findings demonstrated that the practical guidance and instruction provided by CNIs could be characterized into the following 6 conceptual categories: (1)Supporting clinical nursing based on nursing processes; (2) Supporting therapy-related communication; (3) Creating an environment conducive to dialog; (4) Explaining the significance of student experiences; (5) Responding to and interacting with students in a respectful manner; and (6) Dealing with students who exhibit self-centered behavior.departmental bulletin pape
Adult Nursing Practice I (Chronic Nursing Practice) in Covid-19 pandemic ~ Efforts for practical training in collaboration with clinical training instructors and teachers First report ~
今般の新型コロナウイルス感染症(COVID-19)の拡大を背景に,医療提供体制の維持及び感染対策の観点から,看護学生の実習受け入れが困難となった病院がある.それにより,臨地実習*1の時間短縮や中止等となった看護師等養成機関がある.このような状況から,A看護系大学では2020年度の成人看護学実習Ⅰ(慢性期看護実習)を,臨地実習主体から学内実習主体へと変更した.本来の実習目的・目標を達成すべく方法であること,臨床実習指導者*2と教員の協働による実習指導を重点課題として,学内実習を組み立てた.具体的には,①病棟実習前の病棟オリエンテーション,②病棟実習,③入院患者の看護過程の展開,④学内における看護実践,⑤個人発表の5項目の実習指導について臨床実習指導者と協議し,実施している.本稿は,学内実習主体とする成人看護学実習Ⅰ(慢性期看護実習)における実習指導の取り組みを報告する.With the recent spread of the new coronavirus infection (COVID-19), it has become difficult for some hospitals to accept nursing students for practical training from the viewpoint of maintaining the medical care provision system and controlling infection. As a result, there are training institutions such as nurses that have shortened the training time or canceled the training. Under these circumstances, A Nursing University changed the adult nursing practice I (chronic nursing practice) in 2020 from clinical practice to on-campus practice. It should be a method to achieve the original purpose and goal of the training. On-campus training was constructed with the priority task of training guidance in collaboration with clinical training instructors and teachers. Specifically, (1) ward orientation before training, (2) ward training, (3) development of nursing process for inpatients, (4) nursing practice on campus, and (5) individual presentation of five items of training guidance are discussed and implemented with clinical training instructors are doing.
